Fall from machine

Employee was climbing down ladder on a loader. When employee moved rt hand a gust of strong wind caused employee to fall off ladder. Causing pain to the right knee

Struck by... (Not Elsewhere Classified)

EE was cutting a conveyor belt with EE's safety knife. As EE finished EEs cut, the knife came out of the belting and jabbed into EEs wrist, causing a small puncture through the glove into EEs left wrist. The EE removed EE's glove and saw blood. We wrapped the cut with a compression bandage at the office and transported EE to the ER where they placed two stitches to close the wound.

Employee was cleaning with a shovel at the tail of PRCV04 (primary conveyer), and with force hit some hardened material with the tip of the shovel, causing a strain to the right groin area.

Contact with heat

Employee was working at PRCV03 when they became dizzy from excessive heat, Employee had been running equipment for most of the shift and then moved to working outside. The local temperature at the time was 102°F. Employee was taken to ER and treated for dehydration.

Over-exertion in lifting objects

Employee was rigging down the crane, and picked up one of the outrigger pads and rolled it to the back of the crane storage box. When employee went to lift it up into its storage place, employee felt a pull in the right bicep muscle.

Struck by rolling or sliding object

Employee was picking up the edge of a perforated metal plate which is called a punch plate; to put a chain thru the hole in the plate so employee could lift plate with the boom truck up to the top deck of screen. When employee picked up the edge of the plate to put something under it; the plate slid forward landing on right middle finger.

Employee was cutting notches in skirtboard rubber so it could be fastened on the belt. Employee had left hand on the skirtboard rubber and cutting towards that hand when the knife slipped out of the rubber cutting glove and thumb.

Struck by flying object

Employee was hitting a bearing housing with a hammer in order to break it. The housing was made out of cast iron which when it was hit, caused a chunk of the steel to break out hitting the employee in EE's eyebrow.

While being loaded with boulders in the haul truck, the employee was smoking a cigarette with the window down while not wearing safety glasses, even though being previously reminded to wear his PPE while the window was down. The employee felt something get in his right eye.

Struck by flying object

While employee was helping build truck bed for maintenance truck they stopped for lunch and his right eye was irritated and bothering him like something was in it. He came in the next day and said his right eye was still bothering him so we took him to the clinic to get eye checked and they said nothing was stuck in his eye but that it was scratched up.

Fall from machine

Climbing down off the ladder of a D465 Komatsu 60 ton Haul truck. He was on the second step coming down to the third step with three points of contact and foot slipped off third step causing the grip to loosen and him to fall to the ground.

Employee was cleaning cat walk on Primary Screen when he stepped on rock twisting his right ankle, causing ligaments on top of foot to swell.

Fall to the walkway or working surface

Employee stepped out of office for a smoke break. When returning from her car, she fell onto the asphalt parking lot breaking her right foot. Note: Employee is not sure what caused her to fall. Possible medical condition or type of footware, unknown at this time.

Employee was trying to clean some material from his loader bucket and was striking it with a hammer. A piece of the hammer broke off and embedded in his hand through his glove. Injury became reportable 2/28/2011 because employee refused treatment at the time of accident.

Injured went out to clean under #57 stacker when he noticed the belt had stopped. He turned quickly to return to his tower to shut plant off and pulled his left leg muscle.

Fall to the walkway or working surface

EE was making copies at the copy machine and started to return back to her desk when her left foot caught the edge of the copy machine causing her to trip and fall. EE was taken to the emergency room and x-rayed and no fractures were found. Testing was done later due to shoulder bothering her and MRI results showed a hairline fracture in her shoulder.

EMPLOYEE WAS OPENING DOOR TO HIS LOADER WHEN WIND BLEW DUST BUILT UP ON HIS DOOR LIP INTO HIS RIGHT EYE BEHIND HIS SAFETY GLASSES. EMPLOYEE WAS TREATED AND RELEASED TO FULL DUTY AT BARNES VISION CLINIC AND GIVEN A PRESCRIPTION FOR AN ANTIBIOTIC.

EMPLOYEE SLIPPED ON ICE INJURING HIS LEFT KNEE AFTER AN ICE STORM. THIS REPORT WAS NOT FILED DUE TO MY THINKING IT WASN'T REPORTABLE, BUT EMPLOYEE WAS PUT ON RESTRICTED DUTY.

Struck by falling object

EMPLOYEE WAS PICKING UP A PIECE OF METAL (12"" X 36"") TO LAY ON A STACK OF METAL. HE LOST HIS GRIP ON THE PIECE OF METAL AND IT FELL ON HIS FOOT BEHIND THE STEEL TOE AREA OF HIS SAFETY BOOT.

Employee was changing blade in a box blade knife. The guard for the blade was bent causing the blade to pop out of the holder resulting in a laceration to the right index finger. Employee received 3 stitches and a tetnus shot and was released to return to work.

Rubbed or abraded

The seat adjustment on the loader wouldn't work correctly & mechanic tried to repair it without success. The back of the left knee of the employee had been rubbing against the front of the seat causing soreness & pain and hurting him quite a bit. He has had surgery on his left about 3 years ago. Employee requested to go to the doctor to check out the pain.

Driller was lifting a DTH hammer in the winch line. As the hammer came through the table it caught on the table which caused it to start swinging. Hammer was swinging towards the window on the drill cab so driller reached out and grabbed it to stop it from hitting the window. When he did this the weight and momentum of the hammer pulled something in his mid-back.

EE WAS REMOVING STEEL FROM A FLAT TIRE THAT HE HAD TAKEN OFF A ROCK TRAILER. WHILE HE WAS ATTEMPTING TO REMOVE THE STEEL WITH A HAMMER, THE HAMMER CHIPPED OFF AND STUCK IN HIS LEFT HAND. DOCTO R TRIED TO REMOVE THE STEEL AND COULDN'T GET IT OUT. IT WAS THEN DECIDED TO LEAVE THE STEEL IN HIS HAND UNLESS IT GAVE HIM PROBLEMS LATER. THERE WAS NO LOSS OF TIME.
