An employee was standing on the FCT ramp for a belt move and slipped on the fines. Once the employee slipped on the fines, they felt pain in right knee. MRI was completed 1/27/2025 and was determined to be reportable on 2/7/2025.

Operator hit a pot hole and began to experience back and hip pain. The operator notified the supervisor the following shift and went to the ER. Employee has been able to perform normal job duties until last follow up appointment on 03/04/2025 where employee was given the following restrictions: Avoid painful low back movements. No lifting, pushing, pulling > 20 lbs.
Employee noticed idler that was frozen and attempted to free this idler up using a pry bar, the prybar was grabbed by the running Conveyor and pulled in between the trougher idler and the Conveyor belt, this caused the base of the prybar to smash the employee's left hand between the frame and the Prybar.

Point of contact of injury was left side.1-2-25 rec'd treatment w/no job restrictions/days off. 1-9-25 went to follow up w/no changes in instructions. 1-23-25 XRay showed fractured rib. EE hasn't missed any work & has no restrictions. But this makes the injury reportable.
Employee was crawling under rock shoot to gain access to rear panel, while climbing out stepped on rock, twisted and fell they twisted their left ankle and needs a boot and restrictions after seeing the doc.

12.29.24: Operating HT406, hit a few large dips in the roads in multiple areas. Seen by medical and released full duties. 01.27.25: Operating HT op reported back started to hurt. Medical wrote restrictions. Able to perform all normal duties at a slower pace, however.
I.E. was climbing onto dozer when I.E.'s foot slipped on an angle on the push arm and caught self on the handrail bruising left leg.

Cleaning under Rotex area. Backing up with Dingo when left wrist got pinned under structure beam and dingo controls.
Employees were hanging a 2" angle iron 107" long above the v-belts on the 7' crusher. They set the angle iron in place for welding and SE's end fell and it pinched EE's hand between the angle iron and the beam, cutting through EE's glove. EE and SE were wearing Radnor A6 cut resistent gloves and the angle iron pierced through the glove between the thumb and forefinger.
An unplanned ignition occurred on the 2 Left West section while mining the face of the #3 entry.
Employee stated that employee twisted knee while getting off of a bolt machine.
Injured was supplying the twin boom bolter with bolt supplies in 9 North 5-6 near the face of 6. When EE stepped off the bolter, EE stepped on a rock on the mine floor and rolled EE's ankle, resulting in a strain/sprain injury. No problems with equipment, compliance issues, training or PPE.
On 12-27-24 emp reported pain in lower back, didn't seek treatment until 1-14-25. EE was seen by clinic, treated for lower back pain and placed on light duty.
While pulling a 20' long 2"x2"x1/4" stick of angle iron off a rack, the employee felt pain in their left elbow. The employee worked the rest of their shift, but then went to a local clinic on their own accord the following day for an evaluation without informing the company. This evaluation resulted in lost workdays.
While roof bolter was hanging cable in Entry 10 XC in Unit 3, a cable reel from 416 RB struck the employee, causing employee to fall down.
EE felt pain in groin after lifting chain rigging to crane hook.

A recent audiologist report on 12/26/24 revealed a STS in a employees left ear. The sound level of the Rock handling is at dBA 78.4 or below. Potential recordable hearing shift; evaluation ongoing.
12.26.24 Worker was walking on the BCT tieline going past IP's haul truck when IP slipped. IP was wearing ice cleats. IP landed on backpack and right hand. Right hand and wrist became swollen, which radiated into forearm. On 01.09.24 worker was provided with restrictions since recovery was not ideal. (Submitted initially on 1.14.25. was voided. Resubmitted 1.16.25)
Employee stepped down off of our service truck onto uneven muddy ground and rolled EE's ankle
Ee was moving the vacuum to clean work area. When ee pulled on the cord, the vacuum pulled, and ee was also pulled backwards and the lid slammed on ee's finger. Initial diagnosis was "ring finger injury". Follow-up with ortho on 1/27/25 resulted in restricted duty, making this reportable.
