THERE WERE NO WITNESS ES & IT HAS NOT BEEN DETERMINED:[A] THE EXACT MACHINE INVOLVED, [B] WHETHER OR NOT THE EMPLOYEE WAS WALKING OR IF HE HAD COLLASPED DUE TO A PHYSICAL PROBLEM JUST PRIOR TO BEING STRUCK.
EE WAS OPERATING LOCOMOTIVE LOOKING IN OPPOSITE DIRECTION OF TRAVEL WHEN HIS HEAD WAS CAUGHT BETWEEN LOCOMOTIVE AND MINE ROOF.
WHILE TROUBLE SHOOTING ELECTRICAL CIRCUIT ON CONTINUOUS MINING MACHINE EMPL'S FOREARM CONTACTED ENERGIZED 950 VOLT A.C. CIRCUIT. SUBJECT ELECTRICAL CIRCUIT WITH WHICH PERSONAL CONTACT WAS MADE WOULD NOT NORMALLY BE ENERGIZED HOWEVER ELECTRICAL CONTACTOR WAS STUCK IN MACHINES'S CONTROL PANEL. EMPL COULD NOT HAVE KNOWN THIS CONDITION EXISTED.
EMPL OPERATING MOBILE BRIDGE CARRIER IN BELT ENTRY STRUCK IN THE HEAD WITH 48" LONG PIECE OF ROOF BOLT DRILL STEEL.
