Employee experienced left hip pain while replacing a hydraulic hose on an excavator. Doctor reviewed MRI results on November 30th 2022 and placed employee on restricted duty.

Employee was walking around conveyor investigating cause of a belt failure. While looking up at the structure EE's ankle rolled, causing EE to fall to the ground. EE sought medical attention on June 25, 2019 and is currently on restricted duty.

Employee's wrist was cut while slicing a conveyor belt with a utility knife, requiring 4 sutures. There was no lost work days or restrictions as a result of this accident.

On November 23, 2009, employee strained his back while performing work activities in the plant. Employee felt pain in his lower back after shoveling, lifting a rock out of the roadway, and pulling a water hose. Employee did not request to seek medical attention until 12/9/09. This accident is being reported on 12/16/09 which is within 10 days of seeking medical attention.

Employee was removing wheel cover on haul truck. While pulling a wrench to loosen bolt in cover, the wrench slipped and he struck his wrist on lug nut.
Employee and co-worker were attempting to reline the chute underneath the primary crusher. After heating the plate injured, employee instructed co-worker to hit the plate with a hammer which caused a rock to become loose from a shelf underneath the crusher and strike employee's head above his right ear.

Employee was using an excavator to clean out the quarry sump hole. While digging, the machine became unstable and rocked back and forth jarring the employee's lower back.

Employee was straightening a head lamp gaurd. While heating and bending the guard in a vise he was pulling with a cresent wrench. The wrench slipped causing employee to fall on his left wrist.

EMPLOYEE WAS ATTEMPTING TO ATTACH A HAUL CABLE TO A BARGE FOR LOADING. THE SWIVEL WAS LAYING AGAINST THE BARGE AS IT WAS LIFTED ONLY ONE END OF THE SWIVEL RELEASED ITS ENERGY. AS THE EMPLOYEE WENT TO ATTACH THE CABLE THE BARGE MOVED CAUSING THE OTHER END TO OPEN. EMPLOYEE RECEIVED A DEEP BRUISE TO THE LEFT INDEX FINGER.
Employee became overheated while working on a dozier and suffered from heat exhaustion. Heat index was 104 degrees. Employee was not properly hydrating himself.
Employee from Ensley Central Services, MSHA ID #01-03226, was working at the Pride Quarry, MSHA ID #01-03164. He was splicing a beam together when the beam rolled onto his right index finger.
EE WAS RESETTING A CONVEYOR E-STOP. A 3" TO 4" ROCK FELL FROM A CONVEYOR FRAME OVERHEAD, STRIKING EE'S LEFT INDEX FINGER.
EE SLIPPED & FELL TO THE SAME LEVEL BECAUSE OF MUDDY BOOTS. HE HAS PAIN IN THE LEFT WRIST; X-RAYS WERE TAKEN & THERE WERE NO FRACTURES. A WRIST SPLINT WAS APPLIED TO RETRICT MOVEMENT FOR SEVERAL DAYS. THE EE CONTINUED PERFORMING HIS REGULAR JOB.
EMPLOYEE WAS CLIMBING INTO A HAUL TRUCK WHEN HE RAISED HIS LEG & FELT PAIN IN HIS RT.LEG. HE WAS CHECKED AT THE HOSPITAL THAT DAY & WAS TOLD JUST TO TAKE IT EASY & IF HE HAD ANY PROBLEMS TO SEE THE COMPANY DR. ON FRIDAY 4/2/04, HE DECIDED HE NEEDED TO SEE DR. & WAS DIAGNOSED AS LOW BACK STRAIN & PLACED ON LIMITED DUTY.
EMPLOYEE WAS PLACING THE SUCTION HOSE BACK ON THE PIT PUMP. HE SQUATTED AND LIFTED THE LINE IN PLACE WHEN HE FELT A SHARP PAIN IN HIS BACK.

WHILE CLEANING OUT CHUTE ON PRIMARY CRUSHER (PRYBAR USED TO DISLODGE ROCK), A ROCK DISLODGED ANDEMPLOYEE'S MIDDLE FINGER ON RIGHT HAND MASHED BETWEEN PRY BAR & CHUTE, REQUIRING 3-4 STITCHES.
KNUCKLE ON RAILCAR WAS DEFECTIVE AND FELL OFF HITTING EE ON TOP OF R FOOT. (ABOVE STEEL TOE.)
