EE was pressure washing the welding truck and stepped up on the mounted tool box to get behind the cab. When EE stepped down, EE's foot landed on a angle iron bracket, and rolled EE's ankle.
On 1/24/2025 a 777G Caterpillar haul truck caught on fire when the truck was at the crusher pad preparing to dump. The operator stated that EE felt heat and looked to EE's right and saw flames. EE engaged the fire suppression system and exited the truck. The fire department was called and contained the fire in 45 minutes. The truck was a total loss.

12-30-24 a fuel truck/lube oper was fueling excavator, slipped on grease, & fell into excavator. Point of contact of injury was left side.1-2-25 rec'd treatment w/no job restrictions/days off. 1-9-25 went to follow up w/no changes in instructions. 1-23-25 XRay showed fractured rib. EE hasn't missed any work & has no restrictions. But this makes the injury reportable.
Employee was replacing a piece of drill steel from the drill carousel, when it was placed on the drill rack below the carousel, the employee attempted to slide it off of the rack. The steel tube then fell on the employee's foot striking employee above the steel toe in the boot.
Employee was changing the cylinder on the flop wrench and the cylinder fell on EE's finger. Employee was not wearing gloves.
Contractor was replacing belt scraper, while using a torch a hot slag fell onto a Poly Screen. The fire department was called to extinguish. The initial report was that the fire was extinguished in 20 minutes. When the fire report was requested and reviewed it was stated on the report that it took 45 minutes to extinguish.

The employee was moving the toggle bearing over from the wall of jaw crusher 2. When EE pulled on the pry bar to move the bearing, EE felt a sting in lower back. It was determined to be a lower back sprain.

The employee was in the process of cutting the conveyor belt to perform a repair splice. EE lost balance and begin to fall with the open razor knife in right hand. The blade contacted middle finger left hand and required stitches.
A customer truck driver was injured after tripping over a curb in a designated parking area. Loss of consciousness was initially reported, but later determined that wasn't the case. Was released from the hospital to full duty that day. 7000-1 completed per District personnel, VMC disagrees this incident meets the definition of being reportable.

Employee was assisting in removing rail road track. After attaching chain to rail road track, the injured employee was standing too close and was struck in the lower left leg by the rail road track as it was being lowered/moved with the excavator. This resulted in a broken lower left leg.
Employee was removing the bolts from the motor base of a 500 hp motor using a wrench and 20 pound sledge hammer. As employee attempted to strike the wrench with the sledge hammer, ee missed the wrench causing left index finger to get struck between the hammer handle and wrench. This resulted in a broken left index finger.
Employee was washing rocks off of the truck scales at the bins. The employee was stepping down from the concrete around scales to a flat surface and felt a "pop" in right knee. Employee was diagnosed with a right knee sprain and placed on restricted duty.

Employee alleged wrist injury and was sent to Dr. Dr. stated employee has complaints of synovitis, probably associated with gout. Dr. opinion is the patient has existing gout which has been aggravated by work activity. Accident is being reported within 10 days of being determined to be work related.

After exiting man lift, employee stepped on uneven ground and twisted his right knee. Employee had it checked out medically on 11/17/2015.
Removing a guard to change v-belts, the guard slipped from the employees grip and struck him on the top of his right foot behind the steel toe of his rubber boots resulting in a laceration. Employee worked the rest of his shift and did not seek medical attention until 10/26/15. He was treated for a hematoma on the right foot and received 3 stitches and restricted duty.
Employee was lifting a dust collector hose to clean out and reattach to dust collector when felt a low groin pull that has been diagnosed as a hernia.
Employee was operating CAT 773D haul truck in pit. Employee lost control of the haul truck and turned the truck to the inside berm and went into the ditch, resulting in the haul truck turning over on its side. Employee alleges the brakes went out. A brake check and review of the pre-operation inspection shows that the brakes were functioning properly.

Employee was attempting to fuel a CAT 992 loader. As he raised the fuel nozzle to the fill point on the loader, he felt a "pop" in his right shoulder. Resulting in an injury to his right shoulder.

Employee strained left elbow ligament while replacing conveyor belt scraper spindle. Employee went to Dr. and was given medication and PT.
Contract employee was attempting to lower the crusher doors to begin to replace the wear plate. A 3 ton chain and come-a-long were attached to the doors to hold them as they were being lowered. The chain broke and the come-a-long sprung back causing the chain to hit the contract employee in the face. He suffered a laceration and a broken nose.

Employee was trying to remove a rock that was jammed between the conveyor frame, the belt and the return roller. Employee was using a pry bar and while he was trying to remove the rock the bar slipped. The employee lost his balance and while he was trying to catch himself he strained his left knee.
Employees were changing a feeder. The employee had two pairs of gloves on and was removing the plate while the other employee was trying to pry it out. The plate slipped and pinched the employee's pointer finger on his left hand between the plate and feeder.
Employee was making repairs to an excavator. He reported that he began to feel ill, became nauseated, then "blacked out" and bumped his head on the equipment causing a laceration which required stitches. It is believed heat exhaustion may be contributing factor and corrective actions are being put in place to prevent recurrence.
It had snowed in Georgia on 2/12. We shut down at 1700 on 2/12. Plant maintenance, pit and production came in at 0800 the morning of 2/13 to clear snow from belts, chutes and screens so that we would be able to produce that afternoon and night. Employee said he slipped a few times while hurrying around the plant. He reported a back injury on 2/16.
Employee was shoveling in tunnel. Rock came off of belt and struck employee in lower back. Employee suffered a lower back contusion.

Employee was standing upright and tightening a bolt on the crusher. Employee states that he felt a twinge in his back while performing this duty.
Employee was removing a screen panel from the top deck of the wash screen. As he was lifting the panel, he felt his lower back pop.
Employee was not on shift waiting to clock in at the time clock, when he started started experiencing chest pains and shortness of breath. EMT's were called and employee was transported to Hospital. He suffered a massive heart attack while at the hospital during examination. Aggregates USA DOL does not believe accident injury illness citation 6598897

Tech. was torquing bolts with hyd. wrench when it became wedged. While he was trying to get the wrench head loose, the on/off switch stuck. When the wrench head came loose it cut middle finger on right hand.
7" inch rock fell from plant and landed on employee's right hand while wearing gloves.
Employee was setting the vulcanizer and the idler roller hit his leg and cut it. This was not a serious injury, only required stitches.

Employee tripped over a 7 inch rock while leaving the restroom.
Employee was jarred in cab of CAT 992 when a haul truck inadvertently backed into him.
100 lb steel conveyor clamp slipped as it was being removed. The clamp fell about 4 feet landing behind the steel toe injuring the employee's right big toe. Toe was x-rayed and no breaks were found. Employee is following doctors instructions and RICE for the swelling until he returns to the Doctor on 9/27/07.
Employee received a piece of metal, embedded in his upper thigh area, from removing teeth from a loader.

Employee cut left palm while moving cross-ties.
EE was loading AN from a tanker truck to the bins on his bulk truck. While in the process of moving the hose from one bin to another on top of his truck, he slipped on some spilled AN. This resulted in his falling and slicing his left shin about 6" below his knee. He was taken to the hospital and recieved three stitches inside the cut and twelve stitches outside.
Employee fell and slipped on loose dirt/gravel.
Employee sustained a lumbar strain to his lower back while performing maintenance in the plant.

Employee was shoveling when he strained his right wrist. This was orginally reported back on 8/31/05 as a near miss but waited till the 10/20/05 to request to go see a doctor. This is when the treating physician gave the employee light duty restrictions.
Employee injured by falling stone from overflow of surge bin while installing chute above 4 1/4 chute. (Belt was operating. Multiple stones fell, but EE was not buried.)
Employee strained back while lifting scrap metal.

EE WAS CLIMBING DOWN FROM THE CONVEYOR TO THE PILE. HE JUMPED FROM THE CATWALK WITH HIS HAND ON TOP OF AN ANGLE IRON SUPPORT FOR THE HANDRAIL. HIS WEDDING RING CAUGHT ON THE ANGLE IRON & CAUSE D THE INJURY. HE WAS NOT WEARING GLOVES AT THE TIME. DR. PRESCRIBED ANTIBIOTIC & PAIN KILLER.

AFTER HOLED DRILLED, DRILLER FORGOT TOREMOVE RUBBER DUST COVER FROM ROD; LOOKED UP TO WATCH HEAD OF DRILL & SWING ROD INTO CAROUSEL. AS HE LOOKED UP THE FALLING RUBBER COVER STRUCK HIM THE FAC E KNOCKING OFF HIS SAFETY GLASSES & HARD HAT. HEWAS TAKEN BY CO. VEHICLE FOR EMERGENCY CAREA; WAS RELEASED WITH NO BROKEN BONES; NO STITCHES; NO CONCUSSION. WAS TOLD BY ATTENDING PHYSICIAN HE

EE WAS WAITING TO BACK UNDER RIP RAP LOADER. HE WAS SETTING ABOUT 50 TO 100 FT AWAY FROM HAMMER BREAKER. EE HAD WINDOW DOWN ABOUT 6 INCHES ON PASSENGER SIDE. HE HAD PIECE(SMALL) ROCK FLY INTO SIDE WINDOW. IT HIM ON HIS RIGHT SIDE OF HEAD.
KEITH WAS CLIMBING OFF OF A 988F LOADER. WHEN HE WAS COMING DOWN HE SLIPPED AT BOTTOM STOP AND FELL. HE BRUISED LOWER BACK.
EE WAS WORKING ON A CRUSHER. HE GOT A PRYBAR STUCK TRYING TO REMOVE A CHIPPING HAMMER. HE COULD NOT GET HAMMER OUT SO HE THREW PRYBAR DOWN ON GROUND. WHEN HE THROW PRYBAR DOWN A PIECE OF METAL WENT INTO TO HIS LEFT EYE. NO SAFETY GLASSES BEING WORN AT TIME.
