Employee was installing a new air conditioner and alternator on a loader when EE's finger got caught in the belt, amputating the tip of EE's left middle finger up to the nail bed. Employee sought medical attention and is currently on restricted duty.
Employee was working overhead, changing a hydraulic hose on a water truck, when a socket tool they were using fell and struck their mouth, breaking three teeth.
Employee was lifting a 2.5 gallon jug of oil when EE felt pain in their back.
Employee was performing maintenance on a roll away box using a come-along when the hook came loose and struck EE in the head.

Employee was cutting a piece of rubber for maintenance on a screen when the safety utility knife slipped from employee's hand and cut right thigh. Employee received 3 sutures and was released to full duty.

The EE dropped a piece of metal on EE' left foot on 11/30/2021 and did not require medical attention. On 12/19/22 EE was having discomfort with EE' foot, sought medical treatment and released to full duty. On 10/09/23 EE sought medical attention again for continued discomfort, diagnosed with Neuroma & released to full duty. EE had surgery on 11/22/2023 and is currently losing time.
Employee was running a grader when a bin truck ran into EE. EE felt pain in EE's shoulder but did not immediately seek medical attention. On December 15th EE was diagnosed with a bicep and rotator cuff tear. EE will have surgery at the end of December and will become a lost time accident.

On 2/5/20 the employee had exited a haul truck when EE slipped and fell causing a bruise to lower left leg but didn't need any medical attention, however on 2/13/20 EE sought medical attention & was diagnosed with an infection in the lower left leg. EE began missing work on 2/17/20 due to injury until the infection is corrected. This incident is still under investigation.
Employee using high pressure water hose to wash off pivot area of conveyor, stepped back onto hose causing fall.

Employee was changing rubber punch plates. The punch plate punctured through the work boot leaving a one inch or less puncture wound that required 6 sutures.
Employee was in a Komatsu HD605 Haul Truck. Employee was headed toward the 57 pile when they over shot the turn onto the ramp striking the berm at an approximate 45 degree angle. When the right side front tires contacted the loose material along the berm, the truck continued sliding through resulting in the truck laying over to the right side.

The EE was grinding on a jaw box rebuild with an electric 4 hand grinder with dual eye protection. EE felt something get into right eye, used eye wash to flush it and finished the shift. EE came in the next shift, reported discomfort still. EE was taken to the clinic and diagnosed with an eye abrasion/given prescription eye drops/released with no work restrictions.

Employee was pushing bags of blasting agent in a drill hole with a tamping pole and as EE was removing the tamping pole EE felt discomfort in EE's left shoulder. Began RICE for a few weeks, the employee went to the doctor on 06/01/17 and was diagnosed with tendentious. Employee was given an injection of anti-inflammatory and placed on a 10 lb. work restriction.
Employees were changing out the cutting edge plates on a loader bucket. One of the bolts being removed came loose and the employee placed their hand on it and was cut when the impact wrench was activated.

EE reported a tingling feeling in both hands, unsure of the exact cause or date. EE was evaluated and determined to have existing carpal tunnel aggravated by the job. We were notified of the diagnosis on January 10, 2017.

Employee was heating up angle using a torch and started experiencing chest pains
Employee had not been feeling well and started having pains in arm & chest. Notified MSHA immediatley pending on diagnosis. Based on the diagnosis this was not an immediatley reportable injury. This 7000-1 being completed at the request of MSHA to close claim
Injured employee was walking back to the operator shack after attempting to unfreeze a flow valve on the water pump. As he was walking back he attempted to step over a cable when his right foot got hung and he fell foward landing on his right elbow. Resulting in a fracture right elbow

Injury was reported on Oct.24,2014 as pain in right arm radiating down to his elbow and hand resulting from working in the lube room over a period of time. A recent diagnosis was that his injury is work related. The injury became reportable on March 31, 2015 when he was given work restrictions that could not accommodated resulting in lost time.
Employee experienced shoulder pain while pulling on a barge cable. He sought medical attention on 8/27/14 and was released to full duty until his next appointment on 9/2/14, when he was placed on restricted duty.
Employee exited haul truck to clean his mirrors after a rain shower when he slipped and fell to the ground. He sought medical attention and was diagnosed with a hair line wrist fracture and is currently on restricted duty
Employee's haul truck was being loaded with material with a 992G loader when a rock broke the drivers side window. Employee was wearing protective eyewear but felt some grit in his left eye. He used the eye wash station at the plant but continued to feel some eye irration so he sought medical attention. He was diagnosed with a corneal abrasion and prescribed eye drops.

Employee was washing concrete pad with water hose on 10/1/13. While bending, he felt pain in his knee. He went to Dr and was released to full duty. However on 11/21/13, he had surgery and began losing time from work. This incident is being reported within 10 days of it becoming a reportable accident.
While using a cable to move a barge, the cable became hung on the barge. Employee used force trying to unhook cable from the barge. After the cable was re-spooled, employee reported pain in his elbow. He did not request to see a doctor until May 10th, 2013. He was diagnosed with inflammation of the elbow. This incident is being reported with 10 days of it becoming reportable.
Employee was descending ladder to exit haul truck when he felt a sharp pain in his right knee. He sought medical attention and is currently on restricted duty.
Employee's personal physician released her to RTW on 3-4-13 w/no restrictions, following surgery on R shoulder (not work related). On 3/13 she was lifting a chock block when she felt pain in her shoulder. She sought medical attention on 3/19 and was taken off work until 3/25. She returned to work on 3/25 with no restrictions. She lost a total of 4 days as a result of her injury.
After performing routine maintenance on a shaker screen, employee had a pry bar in his hand and when he made a twisting motion to exit the deck of the shaker, the pry bar struck him on the right side of his face, causing a contusion below his safety glasses. He sought medical attention, was prescribed antibiotic eye drops and released to full duty.

Employee was removing broken glass from a motor grader and was scratching the material from the old seal when something flew into his right eye. He sought medical attention on 10/2/12 and was diagnosed with a corneal laceration and given prescription eye drops. He was released to full duty. This incident is being reported within 10 days of seeking medical attention.
Employee was spraying off a concrete pad with a water hose when something splashed underneath his safety glasses into his left eye. He sought medical attention on 9/15/12, was given an antibiotic eye ointment and released to full duty. This incident is being reported within 10 days of employee seeking medical attention.
Employee sought medical attention on 7/18 for a strained shoulder that occurred when he released a brake on a railcar. He was released to full duty. He had an MRI and an app't with an Orthopedic surgeon on 8/16 revealed a torn rotator cuff. He is currently on restricted duty while awaiting surgery. This is being reported within 10 days of being placed on work restrictions.
Employee experienced pain in his right shoulder while removing a cable from a barge July 7, 2012. On October 2, 2012 he was placed on restricted duty. This incident is being reported within 10 days of employee being placed on restrictions.

While kneeling down on both knees to weld two plates of steel together, EE felt a sharp pain in his left knee. This incident is being reported September 7, 2011 which is within 10 days of seeking medical attention.
Employee strained his right elbow when he tossed a chock block under the wheel of his haul truck after parking it at the end of his shift. He was placed on restricted duty March 11, 2011. This incident is being reported March 14, 2011, which is within 10 days of seeking medical attention.
Employee suffered an injury to his upper right ring finger while attempting to clean material from by-pass gates of screen tower. He was put on restricted duty March 9, 2011. This incident is being reported March 11, 2011 which is within 10 days of seeking medical attention.
EE was tightening the tension on a grease wheel with a ratchet, when he lost his grip and the ratchet struck his right wrist and left finger tip.
EE was walking down a flight of stairs at the primary crusher, when he felt something "give" behind his knee. He was diagnosed with a left calf strain and returned to work with restricted duty. This incident is being reported January 19, 2011 which is within 10 days of seeking medical attention.

EE attended a mtg in employee bldg before start of his shift. After mtg, he was walking from the employee bldg to his loader and suffered an apparent cardiac arrest. His co-workers performed CPR until EMS arrived and he went to hospital via ambulance where he later died. This incident was immediately reported to MSHA and investigation lead by Mike Hollis.
Employee was fueling loader with R50 fuel truck. When he climbed onto the ladder to get into the cab, he felt a pop in his right knee. This accident is being reported within 10 days of the incident.
EE was welding on the deck of a barge on 8/16/10. He reached around his body for a welding rod and when he turned he felt back pain. He went to the doctor on 8/16/10 and was diagnosed with back strain. He was put off work for 5 days, returning to full duty on 8/24/2010. This incident is being report August 26, 2010 which is within 10 days of seeking medical attention.
On August 11, 2010 employee was moving shelves from one shop to another. When he got home he realized he had a lump in his abdomen. He sought medical attention the same evening and was diagnosed with a hernia. He was put on restricted duty beginning 8/12/10. This incident is being reported August 19th, which is within 10 days of seeking medical attention.
On 4/8/10 employee was going up a flight of steps when his right knee buckled, he fell & struck his right elbow. He sought medical attention & no injury was found. He went back to doctor for a follow up on 5/12/10. Fluid was drained from his elbow and employee was put on restricted duty 5/12/10. This accident is being reported on 5/13/10, within 10 days of restricted duty.

Employee noticed irritation to his eyes a couple of hours after welding bolts and studs to a guard at the railroad clean-out. This injury occurred on July 31, 2009 but the employee did not seek medical treatment until August 2, 2009.
Employee was descending off of loader. He turned to right and heard his knee pop.

EE injured his knee when he jumped off a barge coalman app. 2-3 feet. He did not seek medical treatment until 12-17-08 and was returned to work. He went back to the doctor on 12-22-08 was given work restrictions which made this incident reportable. It is being reported within the required 10 working day time frame since he began losing time on 12-22-08.
Employee was using degreaser provided by mine. Later that evening he had an allergic reaction and was taken to hospital emergency room
Employee was cutting a rubber hose with a utility knife and the blade went through the hose and hit his left hand between his thumb and index finger causing a deep laceration partially cutting a tendon.
Employee was positioned on top of the transmisson of a loader to remove hydraulic filters. He had loosened a filter and was turning it the rest of the way off and suddenly had a sharp pain in his back.
EE was walking out of operator building; his left foot slipped on last step of stairs and twisted his knee.Dr. originally felt he would benefit from an ace bandage while his knee mended.However, when employee returned to doctor on April 30 he was fitted with a leg brace splint to help stabalize the knee for mending.
EE twisted L knee as he walked on frozen ground to restroom while doing maintenance in plant area. Initial diagnosis was bruise and strain to knee; full duty, no restrictions. On 5/19 EE complained of occasional soreness and stiffness to knee. Sent to orthopedic surgeon; MRI determined a tear of posterior of medial meniscus. Surgery scheduled fo June 8th to repair.

The inside of an employee's left knee was hit by a rock that bounced off #3 crusher while he was cleaning with a water hose.
He was changing a cable on an automatic recoil device on a crane tagline. When the dogclutch of the recoil device was released, we believe the cable caught employee's right leg and threw him into the air. He received contusions and abrasions to his right ankle, knee and shoulder.
Employee was manually moving a piece of 100-250 lb. rip rap when another empoyee rolled a second piece of 100-250 lb. rip rap on his finger. His finger was caught between the two pieces of rip rap.
EE strained his back while welding a crack in an I-beam. He stated that he was in an awkward position to get to the beam.
Employee fell from the ladder of his haul truck while descending. He fell app. 3-4 feet when he missed a step and caught his elbow on the bumper of the truck.
Employee strained his back in the operator's booth while leaning over the controls of a hydraulic rock breaker.
Employee fell app. 15' from a 32' ext. ladder when the ladder broke. He was climbing the ladder to access loose bolts that needed to be tightened.
Employee strained his lower back while attempting to lift an air impact wrench. He was in the process of loosening cap bolts on a crusher.

Injured employee was holding a bearing tool device over his head to replace ride cylinder bearing on a Euclid fuel truck. A second employee was using a shop hammer to hit the tool to knock bearings in place. The handle on bearing tool broke and fell striking the injured employee in the mouth causing lacerations and loose teeth. The tool weighed app. 30 lbs.
Employee was using an impact wrench to loosen a nut on a cutting edge of a backhoe loader bucket. The bolt dropped down out of its square slot and turned. He pushed the bolt back up into the slot and used an impact wrench again to loosen the bolt. The bolt turned and caught his glove and cut his left index finger.
Employee received a laceration to his left ring finger while descending drill.
Employee slammed an elevator door on his left ring finger causing a fracture, laceration and loss of his finger nail.
Employee sprained his right ankle when he stepped on a peice of metal when walking down a concrete walkway.

Employee was taking rebar apart. Pulling on rebar when it gave way his forearm ran across the end of another. piece of rebar. cutting his forearm.
Employee strained his back when attempting to pull weeds from a drain ditch.
On 11-30-05 this claim was accepted as a compensable repetitive motion injury. The claim has been under investigation since 08-16-05. Employee returned to work on 12-06-05. If you have any questions, please call me.

Employee was welding brass on the mainframe seat of an omni-cone crusher. He was in the sitting position, leaned over in the crusher welding and when he rose up, he strained his back.
Employee used an air impact wrench to tighten bolts on a new liner in box. Apparently, metal was floating in the air and fell in his eye. He thought it was dust and went home at the end of his shift. His eye became irritated through the morning and he went to a physican. This is MSHA reportable because he was given prescription eye medication.
Employee was ascending a haul truck ladder when his right knee popped. He did not report the incident until 7-16-04. The employee did not seek medical treatment other than first aid until 7-29-04. This report is being filed based on the medical treatment date of 7-29-04.
AN I-BEAM ON A FLIGHT FEEDER THAT SUPPORTED ROLLERS WAS TWISTED. EE WAS STRAIGHTENING THE I-BEAM BY APPLYING HEAT, USING A COMEALONG & A HYD. JACK. HE WAS REMOVING THE JACK AFTER THE FIRST BEAM WAS STRAIGHTENED WHILE THERE WAS PRESSURE STILL ON THE JACK. THE JACK POPPED OUT & HIT THE EE'S FINGER AGAINST ANOTHER ROLLER.

Employee's right eye became irritated while taping holes in the pit. He can't give a specific event that caused a foreign object to be in his eye but it started bothering him around 10:30 am. He did not seek medical attention until the next day when it became apparent that the object could not be removed by him or the plant nurse. He saw an optometrist to remove it.
