An EE exited the area under the main primary feeder and lost footing and fell on the loose material sloping alongside the feeder sustaining a first aid only injury. On 03/23/22 the EE wanted to have the shoulder checked at a clinic. X-Rays were taken and revealed a fractured collar bone and two fractured ribs. EE was released to return to work with restrictions.

The employee went to the company doctor on 7/10/2007 and was placed on restricted duty. While chipping a weld, using a chipping hammer, a piece of hot slag bounced into the employee's left eye, resulting in a corneal burn to L-eye.

After doing pre-shift inspection, employee climbed on top of his loader to get into cab and then felt a pain in his back.

Employee was attempting to remove the hydraulic lines from the hydraulic pump setup. When he loosened a threaded line that was under pressure, the line blew and the force caused the employee to be struck with the wrench he was using to loosen the line.
Employee was rolling a cable reel and stepped on a loose decorative rock on the edge of the plant office lawn. When he stepped on the rock, he lost his balance and in correcting for this loss of balance, overcompensated with his left foot, resulting in sprain to left foot. Initially reported as first aid only.
