ACCORDING TO WITNESS, EMPLOYEE FELL FROM A SCAFFOLD LADDER FROM AN UNKNOWN HEIGHT.
Employee was using a pry bar to clear a plugged chute. When the rock broke free, the employee hit their hand on the side of the chute, sustaining a fractured finger.
Employee was prying on a part. The part gave way, causing the employee to step backwards into a hole, sustaining a knee sprain.
Employee knelt down in maintenance shop to pick up a 2.5 gallon jug of oil. Employee stated they felt a pop in their knee. The employee has now (Jan 2026) completed an MRI and received a steroid shot in the knee.
Employee suffering mental duress resulting from near miss.
Employee sustained a sprained finger.
Employee's hand made contact with discharge water from nozzle of pressure washer wand that the employee was operating.
Employee was using an iron worker to shear a piece of 1/4"X2" strap when they pinched their finger between the workpiece and the shear table.
Employee was installing idler pulley when the pulley slipped, cutting their index finger.
Employee received a diagnosis of a bicep tear on July 18th, related to loading an empty gas cylinder on a cart.
Employee was walking when they sprained their ankle.
Two employees were moving an electric motor in the warehouse by sliding a bar through an eyebolt in the top of the motor. As they were setting the motor on the floor, the motor slid on the bar, pinching one employee's finger, causing an injury.
Bulk truck cement driver was descending stairs on safe rack when they tripped and fell from the 4th or 5th step from the bottom of the stairs to the ground.
While manipulating a shackle on the gearbox, an Enscicon employee's finger was pinched between the shackle and the gearbox. This incident was initially reported as first aid, then escalated to a medical treatment injury after a dr visit on 5/12/2023.
An employee had a metal particle fall into the right eye. The employee was working inside the raw mill at the plant. The Employee was grinding a weld inside one of the pin pockets. When the employee finished grinding and stepped out of the pin pocket the employee lifted the face shield when an uncomfortable feeling began in the right eye.
Employee was replacing covers on air slide during a very windy day. Employee felt something hit their eye. Foreign debris was found in eye.
Access ladder slipped, causing employee to fall approximately 4 feet. The ladder was improperly secured.
Employee completed grinding a piece of steel. They removed their face shield and safety glasses. A short time later they noticed eye irritation. No foreign material was found in the eye by the Dr., but the employee was diagnosed with a scratched cornea.
Employee was walking along side of building and tripped over boot cleaning grate located near doorway. Employee fell and felt pain in their shoulder. Employee has began receiving physical therapy treatments, changing the classification from first aid.
Employee was cleaning dust material on top of clinker silo. The wind blew and employee got dust in eye.
Employee was descending 4 steps on a vertical ladder of the DM 45 HP drill when EE felt a pop in their right shoulder.
The employee was working on green tagged scaffold approximately 8-10 off of the ground around a raw mill roll pair removing the hub off of the shaft. The scaffold boards were against the hub until the housing was removed allowing for a space at which time the employee fell.
On 8/27/2021 at approximately 3:00 A.M. a maintenance technician was performing a task to reinstall a drive wheel on the stacker. While reinstalling the wheel, the wheel became off-balance from its position and tipped off the rail. When the wheel tipped it struck the employee in the chin and the palm of the right hand causing lacerations to the chin and palm.
Approximately 11:30 P.M., a maintenance technician was using a hydraulic torque wrench to torque the primary crusher bearing housing bolts. As the technician performed this task, the hydraulic torque wrench slipped off of the bolt, causing the wrench to rotate swiftly, striking the tip of the employee's right ring finger against a steel pulley.
Employee was removing a 30lb cardox tube after a shot. When EE pulled the tube out EE struck R index finger on the scaffolding. This created a bruise (palpitation)to EE's finger. On 3/11/21 EE was seen at a clinic for an exam. Nothing broken, physical therapy visit.
EE was working to unplug a clogged airline. After cleaning out debris, EE had another employee hook up the airline. EE prematurely turned on the line blowing pressurized air and cement in to EE's R eye. This caused an abrasion and EE was taken to an eye specialist.
Baghouse rotary feeder was plugged. Employee was opening a slide gate. Employee had positioned a ladder for access. Employee stepped off of the ladder and on to the screw housing. Employee's left foot went through the screw housing, got caught in the screw. Their left leg was amputated below the knee.
Employee was removing a drain plug with a pipe wrench. When the plug broke loose, EE's hand struck an adjacent bolt cutting through EE's impact glove. EE received a laceration and 4 stitches to close.
Employee was accessing the Reclaimer building. EE felt something bump hard hat, then back, then back leg. When EE looked around to see, there was a bat attached to EE's leg biting EE's pants. EE stated never felt a bite or sting. Due to the high mortality rate of rabid bat bites (6-8% bats are rabid) EE decided to have the vaccine administered.
Employee was walking between electrical cabinets in the MCC room when they stepped on a elec cover ground plate and it gave way 8-10 inches. EE fell to one knee hitting hard hat on the floor. EE called a supervisor and they filled out a near miss report. Hours later EE started to feel some discomfort. EE went to the Doc on 2/26 and was given 6-7 visits with physical therapist.
Off site Truck driver for another company pulled in to get a load of cement. The driver's pneumatic lid would not open due to freezing rain conditions. When driver went to access the air switch on the trailer, the driver noticed some ice and avoided it but stepped on a thin film of wet kiln dust. Driver slipped and fell splitting lip. 4 stitches to upper lip.
Employee was throwing a rope up 1 level to lift up a bucket of tools. As the employee threw the rope, EE slipped and fell. EE got up and brushed self off, a bit embarrassed. The next day when EE came to work EE complained of shoulder pain. X rays were taken, no issues reported by doctor. EE recently went to see a physical therapist.
Employee "Tweaked" Back while attempting to remove inner pipe from outer cooling jacket on inlet analyzer probe. This incident was initially a first aid but do to the employee seeing a physical therapist on 3/15/2019 it has been moved to a Medical incident.
Maintenance employee was injured while working in the raw mill. EE was working to install a beam protector on the west beam in the raw mill. While the employee was tightening the last bolt in the beam protector, the wrench that EE was using slipped and struck EE in the right upper lip.
An employee driver for a transportation company fell while stepping out of truck. The transportation co. employee was holding onto a clipboard and the steering wheel with left hand and holding the handrail with right hand. While the transportation co. employee stepped back they lost their footing and fell from the truck to the ground onto upper left back.
A Quarry operator was driving haul truck #3 (APX00162). EE alleges they drove down the main haul road into the MA pit, hit a pot hole in the road, causing the truck to jolt leading to a sudden pain in the lower back. EE continued along with duties for the remainder of the shift, without stopping to report the injury.
EE was changing the cement valves top of Grp 2 silo. The 1st bar valve moved hard. EE used full body weight on the 2nd and it moved easier causing them to be off balance. EE rolled R ankle. EE iced it and took ibuprofen. Worked for 2 weeks then decided it was not feeling better. EE went in for an x ray on 3/1. Displaced fracture R distal fibula.
Employee was walking when the employee slipped and fell. This incident did not become a medical incident until 8/17/2018
Employee was using a poke bar to clean out a hot box when ee lost control of the bar, smashing hand between the bar and hot box plate resulting in a laceration to left pointer finger. Employee was treated at local ER and received 4 stitches to finger.
The employee was placing two 7 foot scaffold legs to be secured into place when the first leg slipped out of the right hand and hit the thumb on the left hand as they held the second leg. The impact caused a contusion injury to the tip of the left thumb.
Employee showed up at work 7/19 complaining of left shoulder pain. The previous day, the employee had been a hole watch (conf space) assisting and monitoring employees accessing the conf space. When EE left work at 5 pm, there was no complaint of injury. We took EE to the Doc, Doc put EE on light duty and gave EE Naproxen/Flexeril.
Employee was trying to remove a new pair of scissors from packaging. The scissors were attached with zip ties. In contrast with company policy the employee used a razor knife without cut resistant gloves and while attempting to cut the zip tie EE slipped and cut hand resulting in 10 stitches.
EE was rotating a gearbox with a hoist. When the gearbox was lifted it started to spin. EE reacted by putting left hand out to stop the spinning and EE caught hand between the gearbox and wall. Minor bruising. The next morning EE complained of pain in L knee. EE had x rays. Everything is fine, but placed on light duty for a few weeks.
Employee was working on the raw mill. Employee was pulling on a rope attached to the RM veins. When the rope hung up, employee pulled on it hard and felt a pop in employee's right thumb. Went to the Doctor on the 16th. Doc prescribed 800 mg ibuprofen, lifting restrictions with R thumb.
Employees were removing built up material off the end of the analyzer probe. The probe was in the retract position and was stuck. As the material was dislodged, the probe moved approximately 1 inch. The injured employee was holding a flash light and while looking to see, put head next to the probe the exact minute it moved. 7 stitches to the right eyebrow.
Employee was doing ground checks, walking around on a 16' elevated platform. A section of the catwalk gave way. The employee fell forward catching self on the solid section of catwalk. Bilateral lower extremity abrasions, right shoulder sprain, back muscular sprain, right wrist sprain. The doc returned EE to work on light duty.
The employee reported the issue on 3/24. It was listed as a shoulder strain/sprain. Over the next several weeks the shoulder pain did not reside. The employee went to a specialist on 4/27. Specialist injected the shoulder/tendon with cortisone. (which is now reportable)
EE smashed right index finger between motor and shim while shifting it sideways. There is amputation of the distal soft tissues at the tip of the finger. No acute fracture or dislocation. No sutures and returned to work.
Employee had been working on removing glycol cooler grates. EE noticed no immediate injury, but back pain began to develop 1-2 am that evening. By the next morning the pain had intensified. Employee notified work on the 21st that EE was unable to return to work. Doctors visit revealed strain of muscle, fascia and tendon of lower back.
Electrician was using a razor knife to cut electrical tape off of de-energized motor leads. EE was holding the lead with left hand. The knife slipped off of the tape and it cut the fleshy part of EE's lower left thumb (palm). Mgmt took EE to the local clinic where EE received 7 sutures.
Employee was holding up a flange and fitting a pipe through the flange. The flange slipped and fell smashing fingers on the employee's left hand
Employee was trouble shooting a pan conveyor. Employee rested his hand on the rail for balance when a roller came by it pinched the end of his finger
Coal mill bag house smolder no illness or injury bag house was completely re-bagged
Employee was pulling on a cable when the cable broke causing a laceration to his left hand. employee received 3 stitches
employee was taking a cement sample from a cement tanker truck. The employee went to dump the sample into a five gallon bucket when the material splashed up and hit the employee in the left eye
Employee stepped off of a ladder stepped on a rock and sprained his ankle.
EE was found in the smoking area in front of the main office building. When he was found he was cool to the touch and no pulse. CPR was started by the plant first responders. The Florence EMS group was called and took over. The EMS group contacted St. Thomas More hospital and efforts were stopped. The Fremont county corner arrived and pronounced the pt deceased
Employee was installing an electrical motor on 1/6/2016 when the employee was taken to workers comp Dr. Dr stated strained muscle no restrictions injury did not improve MRI Completed and surgery for shoulder arthroscopic. Surgery completed on 5/29/2015. Date the injury became reportable 5/29/2015
2 EE were opening the bottom of a bin in order to vacuum it out. 1 EE hit the side of the bin to knock down the loose material. The dust came down and shot out the opening hitting EE in the chest. The dust splashed up into his eyes. EE was standing 4 to 5 feet from the opening.
Individual was climbing the ladder on a 785 haul truck missed the top grab bar and fell to the ground.
Employee states he was removing an arch for canopies and it sprung back, striking him on the left hand. This office was notified 1/16/14, when employee sought medical treatment. There is no lost time for this injury but employee is working modified duty.
Employee was removing a tool pouch weighing 20 lbs. from the back of a service truck. Employee felt shoulder move out of socket then back in. Had pain in right shoulder
Individual was traveling to get a clinker sample stepping over a chain tripped on the chain and fell striking his shoulder on the ground
EE was changing tire and was straightening the rim lock holder with a tire bar. Tire bar slipped and hit his upper front teeth. Minor chipping of front teeth. No other injuries or impairment.
Individual was preparing to clean the roof of a substation was pulling vacuum hose to the roof and experienced pain in her back.
EE and three others were removing an aluminum ramp from the cooler area of the plant. Each individual was lifting and sliding the ramp. The IE attempted to slide ramp and felt his back pop.
Employee was jackhammering a vertical refractory wall in the calciner, working from his knees when he felt a shooting pain in his back.
Equip/property damage - 8:30 shut mill down suspected. Short time after shutdown temp began to rise in baghouse & co became appetent. To inert with CO2 was started. One hour later no change in temp or co. smell of smoldering coal was evident MSHA & fire dept were contacted. inerting of system continued until temp stabilized. At least 12 rows of bags were damaged
Employee was trying to roll the ring cover over when the pry bar slipped out of the bolt hole. When the pry bar came loose it allowed the plate to roll back over landing on top of the employee's right foot. Employee is on light duty.
4 ee lifting devise and began to hook up rolling pin & when attempting to hook up. They slid the pin through the hook & were attempting to center it, the pin went too far through the hook & fell out the other side, fell to the floor stringing ee on the left shin & foot. EE has broken bone in foot is on restrictions, but is working light duty.
Employee reported on 4/8/11 at 2:30pm that his eye was burning & thought it to be pink eye. He went to his optometrist for review. When the doctor looked in his eye there was a piece of metal embedded. The doctor removed the metal & had the employee wear a patch over the weekend to protect his eye. Employee returned to work with no restrictions on 4/11/11.
Employee was working on fan trying to remove a coupling. The center bolt would not come loose. Tried using a large break over bar, then used a large pipe to get more leverage. When applying pressure the adapter broke allowing the employee to go forward twisting lower back as he went down. Employee is on restricted duty.
Employee was cutting a zip tie off of a new tape measure using a knife. When the tie cut, the knife slipped stabbing his left hand between the thumb and forefinger. Employee received medical care. Received 3 stitches.
While in the process of welding, hot weld slag fell onto the boot if ICM employee. The weld slag burned through the boot, causing a burn to his foot.
Employee was cleaning up at the end of shift & picked up a hard chunk of cement & dropped it in the georgia buggy. There was dry cement in the bottom & when the chunk was dropped in the loose material splashed up into his face. Material went under his safety glasses and ended up in his left eye. 1st aid was admisnistered. Diagnosis: large corneal abrasion, restricted duty.
Employee was hammering on a wrench to break a bolt loose. When the EE swung the hammer it struck an I-beam that put the hammer in a different direction & struck his right hand between the thumb & first finger causing trauma, contusion and laceration to his right hand. Employee is on restricted duty.
EE was in reclaimer hall greasing rollers. EE bent down to grease roller when he stood back up he felt a pain in his L knee. An EE was working w/ee called for help. First responders arrived gave assistance, upon assessing the situation an ambulance was called ee was transported to hospital for further care.
EE was removing bags from main cooler bag house. The cage that the bag goes around is difficult to move so the EE was using an up/down motion to loosen the bag so it could be removed. Bags were being cut off the cage as they were being removed. While pulling up on cage the employee's ear went inside the grid of the cage & when he pulled up, the wire lacerated his ear(9 stitches)
EE was air arcing and a piece of hot slag entered his left ear. EE received burn.
Employee was woarking on removing clinker cooler roller crusher from housing. After working w/ fellow employee in setting porta-power jack, he stood up and caught the area aboce his right eye on a piece of angle iron welded perpendicular to the clinker cooler housing. The employee received a cut above right eye. Received stitches at clinic visit.
EE stated that while tightening down a bolt, the wrench slipped off the bolt head causing him to strike his elbow against the side of the hopper. He stated there was no pain, no loss of motion. He was placed on modified duty. On 3-4-09, he return to the CCOM and was diagnosed with Bursitis/tendeniti/elbow soft tissue effusion which was drained from his right elbow.
Employee was loading a rail car top (which was wet from rain) on track 2. While descending the ladder with now wet boots his foot slipped on the ladder and his right leg came in contact with a metal strapping which caused a 1/2" cut just below the knee. First aid was administered with the help of his supervisor. At end of shift he sought medical treatment and received 3 stitches.
EE felt irritation in his right eye. The irritation increased while at lunch. EE irrigated his eye & felt better. Driving home the irritation increased & became painful.Safety Mgr took ee to clinic for treatment. RX was issued. Irritation continued, ee was sent to Optometrist. He had a piece of slag imbedded in his cornea. It has been removed & was given drops.
Wind blew concrete dust into the employee's eye.
Employee was pulling chocks another area to the grease bay to use on a vehicle he was going to service when he slipped on wet pavement & felt a pop in his left knee. Employee has been able to perform his regular duties until surgery as of Sept. 8 employee is on light duty.
EE was working in the finish mill 2. Material fell from above & some dust got into his left eye. He removed a small piece of gypsum & then contacted a first aid responder for assistance with flushing our his eye. 5/21 employees eye was still sore. Referred to specialist to be sure nothing was imbedded in eye. EE had a deep scratch. Was given RX drops to use until seen May 28th.
A sheave block previously used to hold cable in place was suspended appox. 30 ft above from where employee was working. As the cable was placed against tray the sheave slid down the cable striking employee in the head. Employee didn't black out but appeared to be confused. Employee climbed into lift basket and was treated by first responders until taken to CCOM for evaluation.
EE had a foreign object in his eye. He is unsure when and how it happened. He was taken to the clinic and had it removed
Employee finished preventive maintenance on the it loader and was walking ot the port-a-john when he stepped on a rock and twisted his right knee.
Employee was helping lower a gunite hose when it became hung up on a beam. In attempting to free it, the hose was pulled upward when the coupling came loose. The hose fell striking the employee on the hard hat and right shoulder.
Normal inspection was being done. Performed air test and observed high CO. Decided to vent & wait for another test. CO kept climbing and one employee observed smoke around 12:15 pm. Closed all gates to the baghouse; closed all doors & inerted system throughout afternoon. Fire dept. on standby during incident. Smoldering controlled with CO2 fire suppression system around 4:30 pm.
A fire was reported at approx 1:30 pm. I immediately went to investigate & found a fire located approx 1 mile east of the plant facility. Apparently a train passed through the area & a spark ignited the weeds in the railroad right away. The fire spread to the plant property. The fire department was notified and MSHA was called at 1:50. An area of 400' x 200' was burned.
EE was asked to power down a gas welder, but could not find the off switch. He decided to use the throttle cable to shut down the welder. There was a cooling fan in close proximity of the throttle cable & when the throttle was moved the fan had caught the 3rd finger of his right hand & a small but deep cut to the side by the knuckle. Stitches were needed. 1 cm in size.
Employee was on the tower shoveling and felt a snap and pop in his back Physicians review has bound a l-s strain l5 radicular pain. He is on light duty until further review.
EE was trying to drill the end of the grounding clamp for a welding lead. The clamp was held in the drill's vice. He was holding the end of the welding lead with his left hand. The drill bit hung up in the welding clamp and started twisting the clamp and cable. The cable got twisted on his glove and shirt sleeve, pulled his hand into the drill and fractured the left index finger.
The employees were pulling buckets of debris from a lower scaffolding & felt a sharp pain in the middle of his back.
Miner was cleaning the snow and ice off of his boots, slipped, striking steel plates that were stacked up against a column causing them to fall on his right ankle. The plates that landed on him were about 3/16" x 16"x 36" and about 8" or 9" high.
Putting seperator shelf into position, leaned up against wall of Mill to prep for next section. Pushed it towards wall and lowered chainfalls, it hung up on shelf and slipped off, then it came down and bounced up and landed on EE's left foot, directly behind steel toe of boot. This resulted in smashing of his left foot.
Late at night, employee was not aware of surroundings.
EE complained that over a period of time he has experienced an increase in pain and numbness in both of his hands and arms. Surgery has been performed on the left arm and once healed, surgery will be performed on the right arm.
Employee was driving a mini skid steer into the mill building & down a hallway. There was low conduit across pathway & he struck his head when going under. He felt his neck jam & his spine pop at the joints all the way down. He was taken to CCOM the next day & is on light duty for 2 weeks or until released by the doctor.
Employee was using cutting torch to install safety hand rails. Hot material fell in tire pile and ignited tires. Hot work permit was in place.
Kiln stopped due to disruption in raw material feed, coal mill stopped. smoldering palverized coal made its way to the baghouse. Potential ignition source was tramp metal in coal mill, CO2 inerting was effective until CO2 level in storage tank became too low to start mill system. High O2 atmosphere in baghouse due to mill off-line & lack of CO2 led to need to stop smolder w/water.
Working on Iteca Vacuum arm and while lowering plunger 2nd finger of the left hand got caught. Employee sought medical treatment at the emergency room. Stitches were needed. No work restrictions - stitches to be removed in 7 to 10 days.
EE was working to remove small pieces of metal blocking the feed chute & needed to kneel in a tight area several times to work on the blockage. EE was stiff after working and kneeling in the tight area. As the night went on there was an increase in pain & he had difficulty walking. He was taken to the hospital for evaluation. He is restricted sedentary work for 1 week.
Employee working in calciner unit operating a cordless circular saw to build wooden forms in preparation for guniting. As he was making a miter cut on 3/4" piece of plywood @ 2"x8" long, the saw blade came in contact with left index finger cutting the tip off.
Working on scaffold platform, tripped on a piece of plyboard protruding on the deck of the scaffold and struck scaffolding injuring left shoulder. He was diagnosed with cervical strain, L. shoulder strain and right eye conrtusion. Employee reached maximum medical improvement with a 6% upper extremity impairment for loss of motion.
Had Stiffness and pain in neck on the left side, numbness in arm after picking up ladder and moving it from one location to another. Carring ladder on shoulder. Employee put on restricted duty. 10 lb limit on lifting and no crawling or climbing.
Miner was filling a bottle of ammonia in lab. He pressed down on the bottle stopper and the bottle broke, causing a laceration to his right middle finger on his right hand that required sutures.
CUTTING THE SUB CONNECTOR OFF OF A PIECE OF DRILL STEM WITH OXY/ACETALINE TORCH. SUB CONNECTOR HAD MALE THREAD AT EACH END & DRILL STEM HAS AN INNER JACKET FOR DRILL OIL. APPEARS THAT SOME OF THE OIL HAD RESERVOIRED IN UPPER END OF THE MALE THREADS. AS EE CUT, THE TORCH HEATED & PRESURIZED THE DRILL OIL, CAUSING IT TO FLASH/EXPLODE. 1ST & 2ND DEGREE BURNS TO FACE & NECK.
EMPLOYEE BENT AT WAIST TO PICK UP A PIECE OF CRIBBING FOR CRANE OPERATION. EMPLOYEE HAD MUSCLE PAIN AND SORENESS IN LOWER BACK AS A RESULT OF LIFT PROCEDURE.
WALKING THROUGH RAW MILL MAIN BAG HOUSE AND WALKED INTO A LOW BEAM STRIKING THE FRONT OF HARD HAT, JARRING NECK. SENT TO CCOM FOR REVIEW AND WAS PUT ON RESTRICTIONS
EE operating a table saw cutting wedges when his glove on left hand got caught by blade and pulled his hand into blade. he sustained laceration to 1st and 2nd digits of left hand. four sutures required to close laceration
EE was working on level 6 of tower at the watertank. Plastic wrap around tank needed to be replaced so that the water lines would not freeze. EE was removing plastic when the support structure 4x8 plywood on 2x4s around outside came loose and hit EE in the right knee. EE is on light duty.
Employee slipped stepping back into empty oxyacetylene cart; it rolled out from under him causing a fall, striking his head on concrete. Fellow workers said he passed out causing fall, unsure.
Employee had felt knee pain for two weeks. He was servicing a quarry haul truck and took a step and felt extreme pain behind the knee. Cause of pain has not yet been diagnosed. Employee is on modified duty at this time.
Employee was on shift and experiencing lower back pain throughout the day. Upon examination it was determined that his hip has been out causing extreme pain to lower left side of back. He is on modified duty at this time.
EE WAS THREADING ELECTRICAL CONDUIT AND CUT FINGER. TWO STITCHES WERE REQUIRED. GLOVES WERE NOT WORN WHILE COMPLETING TASK.
PUSHING AGAINST 3/4" DRILL, HE OVER STRETCHED A MUSCLE IN LEFT KNEE. INITIAL DOCTOR VISITS WERE OBSERVATIONAL - NO WORK RESTRICTIONS. LAST DOCTOR'S VISIT DETERMINED SURGERY IS NECESSARY AND WILL BE DONE ON 12-16-04.
Employee was working in a tight area 331 wf4 dump pocket putting new lines on the discharge chute. employee went to change hand when he was welding and the torch slipped from left hand and the flame caught the right wrist causing second degree burns.
Ee was cutting radiator hose. He was using a knife that was inadequate for the job & was also cutting toward his body. EE WAS STANDING ON GROUND AND REACHED UP TO SET THE BRAKE ON A RAIL CAR. THE REACHEING ACTION CAUSED HIS LEFT SHOULDER TO DISLOCATE. NOTE EE REPORTS THAT HE HAS HAD SHOULDER DISLOCATE 2 YEARS AGO DUE TO A FIGHT AND AGAIN LAST YEAR IN ANOTHER NON-WORK RELATED EVENT. HE WAS TREATED AT THE HOSPITAL AND RELEASED TO LIGHT DUTY.

SLOWLY KNEE WENT FROM FEELING UNCOMFORTABLE TO PAIN AFTER A COURSE OF A WEEK OR SO ASSESSMENT FROM CCOM-HAMSTRING TENDONITIS & MILD KNEE STRAIN. REPETITIVE MOTION FROM CLIMBING LADDERS WHILE L OADING RAIL CARS. RESTRICTED TO NO CLIMBING, LIMITED KNEELING, LIMITED SQUATTING, LIMITED TO LIFTING, CARRYING, PUSHING & PULLING OF 20 LBS. UNSURE OF INTENSITY OF INJURY UNTIL EVALUATION BY
EMPLOYEE HAD OPENED INSPECTION PLATE AT ALKALI BY-PASS FLAPPER VALVE TO CHECK FOR BLOCKAGE. EMPLOYEE RECEIVED MINOR BURNS TO FACE AND NECK, HOWEVER BOTH FOREARMS HAD RECEIVED 1ST & SOME 2ND DEGREE BURNS. EE WAS TREATED & DID RETURN TO WORK RESTRICTED AWAY FROM HOT AREA
TWO REPAIRMEN WERE USING PRY BARS TO RAISE A TAIL PULLY SHAFT. ONE REPAIRMAN LOST HIS GRIP CAUSING THE WEIGHT TO TRANSFER ON THE BAR THE OTHER WAS HOLDING. THE WEIGHT CAUSED HIS BAR TO LIFT FO RCING HIS HAND TO STRIKE THE LEFT SIDE OF HIS FACE. THIS IMPACT CAUSED PART OF HIS #11 TOOTH TO BREAK OFF. HE IS IN THE PROCESS OF HAVING THIS TOOTH CAPPED.
EE WAS CLIMBING LADDER TO HIS HAUL TRUCK & FELL APPROXIMATELY 10' TO THE GROUND LANDING ON RIGHT SIDE. WHY ACCIDENT OCCURRED IS STILL BEING REVIEWED. EE COULD NOT REMEMBER CLEARLY WHAT HAPPENE D. INJURY OCCURRED TO SHOULDER, SIDE & HEAD. EE IS OFF WORK DUE TO SHOULDER INJURY AND OBSERVATION.

EMPLOYEE WAS ASCENDING STAIRS AND MISS-STEPPED. SHE REACHED TO GRAB HANDRAIL AND JAMMED HER HAND BETWEEN RING AND SMALL FINGER CAUSING FRACTURE IN BONE BEHIND SMALL FINGER.
EMPLOYEE WAS PULLING ON A 3" SLURRY HOSE WHEN HE FELT A PULL IN HIS LEFT CALF MUSCLE. HE WAS TRANSPORTED TO DOCTOR AND TOLD TO STAY OFF LEG FOR 1 WEEK AT WHICH TIME HE WOULD BE CHECKED AGAIN . CAN NOT PERFORM LIGHT DUTY, PER DOCTOR.
THE EE WAS INSTALLING A 5" HOLESAW INTO A 24 VOLT CORDLESS DRILL. THE DRILL SLIPPED IN HIS HAND AND THE EE HIT THE TRIGGER AND CUT HIS LEFT PALM.
THE EE WAS TIGHTENING THE BOLTS ONT HE CORE DRILLING MACHINE AND THE MACHINE FELL OVER AND HIT HIS SHOULDER.
WHILE PULLING ON A CONSTRUCTION HOSE THE EMPLOYEE STRAINED HIS SHOULDER.

HE STATED THAT AS HE WENT TO CLIMB BACK TO LEVEL 4, HE SLIPPED AND FELL STRADDLING THE BEAM HE WAS STANDING ON HITTING HIMSELF ON THE GROIN AREA. EE WAS SEEN AND THE EMERGENCY ROOM TREATED AND RELEASED TO RESTRICTED DUTY.
EE CLAIMS HER AND ANOTHER EMPLOYEE WERE REMOVING AN ELECTRIC MOTOR FROM THE LOWEST SHELF, <> WAIST HEIGHT. THEY PICKED THE <> 12" X 12" X 12" WOODEN CRATE UP AND STANDING ON A PALLET, SHE WAS WALKING BACKWARDS, FELL AND THE CRATE FELL ON TOP OF HER RIGHT LEG. EE PICKED UP A PALLET AND STOOD IT UP. HE THEN PUSHED THE PALLET TO THE RIGHT SO IT COULD LAND ON THE FORKS OF THE LIFT TRUCK. AS HE PUSHED, HE FELT A SHARP PAIN IN HIS BACK.
AS EMPLOYEE CLIMBED DOWN THE SCAFFOLD LADDER HE MISJUDGED THE SCAFFOLD DECK. HE FELL FROM THE THIRD FROM BOTTOM RUNG HITTING HIS RIGHT SIDE ON THE SCAFFOLD MIDRAIL.

EE WAS SLOWING THE TRACK MOBILE TO STOP IT, BUT EE WAS ATTEMPTING TO PUSH HIMSELF AWAY FROM LADDER (LOST BALANCE) AND MOMENTUM OF STOPPING CAUSED EE TO SWING AROUND IN FRONT OF WHE
EMPLOYEE WAS WEARING HIS BODY HARNESS TOO TIGHTLY AND IRRITATED GROIN AREA. HE HAD VASECTOMY ON 3-10-00 AND WAS WORKING WITHOUT RESTRICTIONS. DR GAVE HIM PAIN MEDICATION AND RESTRICTED USE O F HARNESS.
WHILE MOVING A SHEET OF PLYWOOD, HE RECIEVED A SPLINTER IN RT HAND. EE ATTEMPTED TO REMOVE SPLINTER AT THAT TIME. HE DID NOT REPORT THIS TO COMPANY UNTIL IT WAS INFECTED ON 3/15/00. DR THEN TR EATED (3/16/00) INFECTION AND GAVE HIM ANTI-BIOTICS. GLOVES WERE PROVIDED BUT NOT USED.

EE WAS WORKING ON THE LUBE SYSTEM FOR #4 FINISH MILL AT THE DISCHARGE END. HE WAS WORKING OFF A LADDER AND HAD NEED TO USE HIS POCKET KNIFE. HE SET THE KNIFE ON THE MILL TRUNNION. AS HE CLIMBE D DOWN THE LADDER THE KNIFE FELL AND STRUCK HIS LEFT FOREARM.
WHILE REMOVING LINERS FROM #4 FINISH MILL, EE WAS USING A SLEDGE HAMMER TO POUND OUT A BOLT. A PIECE OF SHRAPNEL FROM THE BOLT HIT EE IN LEFT HAND. REPORTED AS MINOR CUT TO HIS SUPERVISOR ON 2 -7-00 BUT SWELLED UP AND SENT FOR TREATMENT ON 2-9-00.
EE WAS DUMPING OUT OF GEORGIA BUGGY WHEN THE HANDLE SLIPPED OUT OF HIS HAND STRIKING HIS HAND, CAUSING A HAIRLINE FRACTURE OF THE METACARPAL BONE IN THE LEFT HAND 5TH DIGIT FINGER.
