WAS INSTRUCTED TO HOW OUT OF ANOTHER PIT. THE EE MADE THE TRONSITION WITH NO PROBLEM. ON THE EE'S LAST LOAD BEFORE THE END OF THE SHIFT, FOOT DEEP POOL OF WATER. AFTER EVENING THE WATER THE EE RAISED HIS TRUCK BED & TURNED HIS WINDSHIELD WIPERS ON. THE EE THEN EMPTED TO EXIT FROM VEHCILE ONTO A DIRT ENBANKMENT THAT WAS ABOUT 2 FT AWAY. Employee received hand laceration while changing bolts on cutting edge of loader bucket. Laceration required stitches.
While repositioning the keeper on the middle pin of the 980H loader bucket linkage, the operator used a 12-lb sledgehammer to tap its edge. The hammer glanced off the keeper and continued to move forward. The operator's left hand struck the boom of the loader, injuring the left index finger.
EE was changing a Wear Plate on the 988H. EE was using Impact Wrench to take nuts off of the bolts. EE stopped to check if the bolt had dropped down and at the same time, he was holding the impact tool with his right hand accidentally pulling the trigger, causing the bolt to turn, cutting through the glove on his left hand and cutting his left index finger.
Employee suffered from pre-existing back injuries, degenerative bone/joint condition. A chiropractor examined employee and told him pain was a result of bouncing in the seat while operating truck. Injury is still under investigation but is being reported as a result of doctor's statement received on 07/02/2007. Employee did not report injury until 06/04/2007.
Employee was climbing out from between the swing jaw and the pittman, When he reached the ground he felt the pain in his right side. Employee was working in that space for about thirty minutes with a one inch air impact wrench, which was used to tighten up the swing jaw die bolt.
The employee was offloading screens, and his finger was caught under the screens. The finger is not broken. The employee received no stitches, but a tetanus shot and antibiotic.
A contractor and lafarge employee were installing a new belt scraper on the head section of a belt, and a piece of slag fell through one screen to get to another and landed on the bottom deck and ignited the polydeck screens. Minimal damage
Employee was walking out of the wearhouse to the mens room. Upon exiting the wearhouse door he turned right and bumped the metal tree stand with his right leg. Employee did not seek any immediate first aid. Employee requested medical aid 5 days after receiving injury due to increase pain in the injured area.
Employee was changing out the bucket wear parts on his loader. He was attempting to knock off the bottom wear plate on the corner adapter of the bucket, striking it from corner edge, when a small piece of metal came off the wear plate striking him in the right shin and left knee.
Employee was replacing tool - slipped and fell onto a 5-gallon bucket. Employee suffered contusion to left arm and right side of torso. On Oct 14, employee opted for off-site medical treatment.
Two employees were gathering screen panels to transport to plant; while separating the panels the employee's left hand was caught between when pry bar slipped.
EE was changing a worn return roller. When he put the roller in the last bracket it slipped out, causing the roller to fall and smash his middle finger.
Employee was walking down the steps to the lower level of the RC primary crusher when his feet slipped out from under him causing the employee to fall landing on his lower back and tail bone. The employee was not using three points of contact while going down the stairs. The employee did not seek medical attention until 3-11-05.
EE WAS REPORTING TO HIS LOADER. HE STEPPED ON A PALLET & THERE WAS A SMALL ROCK THAT WAS ON THE PALLET & HIS FOOT SLID ON THE ROCK.
EMPLOYEE WAS ASSISTING ANOTHER LAFARGE EMPLOYEE WITH REPAIR - ATTEMPTING TO REMOVE COIL SPRING. EMPLOYEE'S FINGER BECAME TRAPPED BETWEEN SPRING AND METAL CATWALK. EMPLOYEE SUFFERED LACERATION RESULTING IN 8 STITCHES AND A FRACTURE TO RIGHT INDEX FINGERTIP.
EE WAS WORKING ON REASSEMBLYING 7 FOOT CRUSHER. HE GOT IN A SQUAT POSITION, TOOK HOLD OF THE HYDRAULIC CYLINDER WITH HIS LEFT HAND AND SHOULDER TO PUSH AGAINST CYLINDER TO TILT ITS POSITION TO CENTER A SPACER UNDERNEATH IT. AT THIS POINT HE EXPERIENCED PAIN IN HIS LOWER BACK.
EMPLOYEE WAS OPERATING THE LOADER WHEN BOULDER/DEBRIS FELL, STRIKING AND JARRING HIM. EMPLOYEE BEGAN EXPERIENCING BACK PAIN.
EMPLOYEE WAS SWINGING A SLEDGE HAMMER ATTEMPTING TO REPAIR A CRUSHER. WHILE DOING SO THE EMPLOYEE TWISTED HIS KNEE, THIS CAUSED THE EMPLOYEE TO FEEL A PAIN IN HIS KNEE.
EE WAS WORKING ON A BUSTED HYDRAULIC LINE. WHILE ATTEMPTING TO LOOSEN THE HOSE FITTING THE EE SLIPPED & FELL. THE EE LANDED ON HIS THUMB, THUS CAUSING THE INJURY.
EE WAS ATTEMPTING TO INSTALL A NEW LOADER SHANK. UPON DOING SO, THE EE STRAINED HIS LOWER BACK.
EE PICKED UP A SMALL BOX ATTEMPTING TO PLACE IT IN THE BACK OF HIS SERVICE TRUCK. WHILE DOING SOHE FELT A SHARP PAIN IN HIS LOWER BACK.
EE WAS ON TOP OF HOOD TO THE RIGHT OF THE CAB, MAKING HIS WAY TO THE BATTERY BOX. AS HE STEPPEDDOWN TO THE LOWER LEVEL HIS FOOT SLIPPED. HE LOST HIS FOOTING & LANDED ON HIS BACK & WRIST.
EMPLOYEE WENT DOWN INTO THE #2 TUNNEL TO CLEAR A HANG UP. THE EMPLOYEE PROCEEDED TO CLEAN OUT THE SAND THAT BUILT UP WITH A LONG METAL ROD. WHILE ATTEMPTING THIS THE LOADER OPERATOR DUMPED A LOAD OF SURGE STONE INTO THE CHUTE, THIS CAUSED THE ROD TO STRIKE THE EMPLOYEE IN THE CHEST.
EMPLOYEE WAS OPERATING DROPBALL MACHINE BREAKING OVER SIZE ROCK. DURING NORMAL OPERATION ROUTINE, THE HOIST BRAKE PEADL KICKED OUT HITTING THE EMPLOYEE'S LEFT FOOT AND ANKLE.
OPERATOR HAD ATTEMPTED TO POSITION DRILL OVER ROCK WHERE HOLE WAS LAID OUT; HE TRIED A DIFFERENT APPROACH THAT INVOLVED SLOPED TERRAIN. AT THIS TIME, THE DRILL TIPPED OVER TO CURB (RT. HAND) SIDE WITH OPERATOR IN CAB. IMPACT OF CRASH THREW OPERATOR AGAINST INSIDE OF CAB.
