The employee was using a hammer to remove the ground engagement tool (tooth) on the loader bucket. When striking the GET, a chip from the hammer face broke off and entered the top of the employee's left forearm.

Employee was passing a power drill to another employee when EE accidentally pressed the trigger. This caused the drill to rotate, striking the employee's left ring finger, resulting in a fracture to the finger.

Employee arrived at the shop to clock in for his shift and was experiencing chest pain and other symptoms typically associated with a heart attack. The employee was transported by ambulance to NE GA Medical Center. After evaluation it was determined that this incident was not a heart attack and was attributable to a personal medical issue (NOT WORK RELATED).
Employee came to office complaining of chest pain. 911 was called by a member of the office staff and the employee was transported via ambulance to Northeast Georgia Medical Center in Gainesville, GA. After evaluation and diagnostic testing it was determined that this incident was not a heart attack and was attributable to a personal medical issue (NOT WORK RELATED).
Employee was working on feeder in blending tunnel. After completing the repairs he stepped down off the conveyor and felt a pain in his right knee. He was taken for evaluation by company doctor who diagnosed him with degenerative joint disease of the knee.

Customer truck/trailing had backed up to the screenings stockpile to dump a full load of similar material. Loader operator was digging into plant area material stockpile. In doing so the rear end of the machine came off the ground. In bringing it back down to the ground it jarred his neck and strained it.

Employee (front end loader operator) who loads customer trucks was diagnosed and recieved surgical treatment for an ulnar nerve transposition (carpal tunnel surgery).
Employee (haul truck driver) was diagnosed with a repetitive strain disorder (illness) of the right knee. Employee underwent arthroscopic surgery to repair the damage.
Employee was clearing rock from the wash screen deck at the truck load out bins. While dismounting from screen deck, employee slippped and fell onto walkway. Employee sustained three fractured vertebrae as a result of the fall.

EE WAS MEASURING FOR WEAR PLATE C TWIN SAND SCREW WHEN HE WAS GETTING DOWN TO STRIKE HIS UPPER LIP INTO SAND SCREW. EE WAS WEARING ALL REQUIRED PPE PLUS SAFETY HARNESS.
