The employee was operating a wheel loader to move Type III stone to the stockpile yard. While digging into the pile, the loader jolted as the bucket came to a stop, and the operator felt pain in neck and upper back.

Operator was scooping GAB from the pile when the top of the pile sloughed off and filled the bucket, causing the loader to lose balance and tip forward. The operator quickly dropped the bucket, causing the rear of the loader to fall back to the ground and jar the driver. The driver reported experiencing soreness in their back after the event.

Employee was cutting the metal shipping straps that were securing a new roll of conveyor belt. The employee placed themselves in front of the band while cutting it with a pair of needle nose pliers with side cutters. When it was sheared, the released energy caused the band to spring out and strike them in the forearm.

Injured employee was removing a wear plate from a feeder. As a weld was being cut free with a torch, a secondary weld failed, allowing the steel plate to release, striking the employee on top of the left foot. The employee sustained broken bones to the left foot.

Maintenance man was taking out bad mechanical laces on M10-2. EE had finishing the job and was cutting the outer edge of the belt to keep it from catching. During this process ee was using a standard razor box knife when cutting toward self the knife came loose and struck ee in the lower leg. Causing ee to have to get seven stitches in lower leg

Pond sand pipe clogged up. Using a pipe to unstop the pipe. the pipe was approximately 10 feet long with a 90 degree bend in it 18 inches long. When ee was done with the pipe he threw it across his body to keep pipe from getting covered up in the material. The pipe twisted and made contact with his left foreman requiring 14 stitches.

While reporting to work, an employee of a tenant suffered a fatal cardiac episode. The tenant is not associated with Hanson Aggregates and is not involved in the mining process.

This particular incident was determined to be non-work related as the victim's final cause of death was determined by the Medical Examiner' s office to be a brain aneurysm. Incident was found to be non-chargeable and should not reflect in the mine' s incident rate calculations.

Employee walking from his personal vehicle towards the time clock office when a stray dog ran under his foot, causing him to turn his ankle and fall to the pavement.

Employee clearing up under a crushing tower when a stray rock bounced down and struck him on the back of the neck.

Haul truck was backed in position to be loaded by pit loader. An upper section (20?) of the 50 feet pit wall fell and struck the right side rail of haul truck bed. This damaged the side of the haul truck bed and jarred the operator. The rock that struck the haul truck bed was approximately 3 foot in diameter. The truck was still operational after incident.

Pit Loader Operator was filling up the grease canister on his 990 loader. As EE reached up to connect the grease line from the shop, to the line that runs to the tank on the loader, his hard hat fell off, to the ground. As EE was on his way down off of the loader his head made contact with the loader fender.

The employee was using a sledge hammer to dislodge jammed rock located in the jaw feeder's chute. He was hitting the outside of the chute with the sledge hammer when his knuckle struck (caught between) the chute causing a small gash on his right index finger that required stitches to close the wound.

EMPLOYEE WAS USING THE 988G LOADER TO BACKFILL HOLES WHEN HE RAN OVER A LARGE HOLE. THE LOADER "JARRED" CAUSING HIM TO BE SHARPLY MOVED FORWARD AND BACK. DOCTOR ISSUED WORK RESTRICTIONS AND A NECK BRACE.

EMPLOYEE WAS WASHING UP SPILLAGE WITH A WATER HOSE WHEN ANOTHER EMPLOYEE TURNED UP THE WATER PRESSURE CAUSING HIM TO LOSE HIS BALANCE AND STRIKE A NEARBY CONVEYOR FRAME WITH HIS LEFT KNEE. DOCTOR ISSUED WORK RESTRICTIONS AND A KNEE BRACE WAS NEEDED.
