Operator was doing pre-shift on a D8N Dozer while closing engine inspection door they slipped and fell, they broke their wrist as a result of the fall.

Employee was called out to install a hose for 10 silo. The clamp failed and EE had to come back out and install it again. During the second installation the hose was found in the car, full of clay. Another Employee helped EE pull the hose out of the car and this was when the employee felt a pain in the lower stomach.
EE was crossing the grass going to the crude shed and struck right knee on a jersey barrier.

Employee was washing out a rail tanker car when he slipped and cut his foot right below the ankle. Employee was wearing brand new metatarsal boots and all his other required PPE. He got up and finished cleaning the car, but when he exited the top he realized his foot was injured.
Changing hammers in hammer mill when left hand index finger was hit with pry bar
Employee was exiting dozer had a lunch bag in hand. He started to step down from dozer and his foot slipped on the tracks. He fell to the ground.
Cleaning inside railcars with a pressure washer and the pressure washer contacted the side of right foot causing a hole in boot and a laceration to right foot.

Was walking out of rotary dryer room when the operator hollered at employee to come back and restart the dryer. When he turned to go back into the rotary dryer building, he twisted his left knee. Surgery done on 3-7-12.
Hammer mill rotor was locked down with clay. Used vacuum truck to remove clay and then tried to free up rotor by rotating it. Employee pushed on rotor and when it broke free it caught his left hand middle finger between wall and a hammer causing a laceration.

MINER FELL APPROX 17 FT ONTO CONCRETE FLOOR BELOW & SUFFERED BROKEN FEMUR.
Employee struck his head on a light fixture in a storage area of the office.
Emplyee was inside a tank car, cleaning the car. Local railroad came on site, accidently releasing rail cars above the car the employee was inside. The released cars struck the car being cleaned, knocking the employee down, bruising his lower back. No medical treatment was deemed necessary by attending physicians.

Employee was climbing off a backhoe, missed a step, fell to the ground.
Employee was washing out a rail tank car, he slipped, the pressure washer cut across his boot, cutting the top of his foot.
Employee was standing in front of the slicer on floor level. A large lump of clay fell from the top of the slicer, striking the employee on the left shoulder bruising it. The blow knocked the employee to the floor, cutting his left thumb on a tool he was holding.
