Employee fell while entering a skidsteer. EE lost balance and fell with arms stretched out. X-rays confirmed EE had broken EE's elbows and physical therapy was prescribed on 06/17/2024. At first, PT was prescribed for degenerative non-work-related issues with shoulder, but after investigation, the doctor stated PT was needed for elbows. Might be over reporting here.

Employee was operating a trackhoe moving boulders around when are large section of rock broke off and went through the windshield. Employee threw the rock out as soon as it happened.

Employee was attempting to park the Kawasaki Mule at end of shift. Employee was turning the Mule as the pathway, which was flat, changed from gravel to asphalt and the Mule turned over on its side. Injuries were scraped left forearm, broken finger tips to left ring finger and left middle finger. Left palm required 3 stitches and the left index finger required 6 stitches.

THE CRUSHER TOP HAD BEEN SET ON THE GROUND BECAUSE WE WERE DOING SOME MAINTENANCE INSIDE CRUSHER. EMPLOYEE WAS CLEANING OFF THE BUILD-UP ON THE CRUSHER TOP WHEN THE CRUSHER TOP SLIPPED INTO TW O PIECES. (WE DIDN'T KNOW IT WAS 2 PIECES) AND CUT HIS RIGHT THIGH REQUIRING 24 STITCHES & A HIARLINE FRACTURE IN RIGHT ANKLE.
