Employee was cranking the suction line up and the crank slipped out of EE's hand and it struck EE on the back of EE's right hand between the thumb and index finger. EE received a fracture to EE's hand.

Volunteer fireman notified Mine Management at 4:17 pm that there was a shed on fire at Red Hawk Mine. Shed was destroyed and storage trailer also received extensive damage. There was no one at the mine at this time. The fire was reported by a neighbor to 911. (There was no scheduled work and no one at the mine. System would not let me proceed without a shift start time.)

Was in the process of changing the main hyd. pump on a D-11 Dozer using a cable crane, inadvertently pushed the wrong button on the remote that operates the crane.

Individual was found lying face down by his haul truck unresponsive. CPR was performed and he was transported by ambulance to the hospital. He was pronounced dead at around 2:30PM 4/21/08. Cause of death was listed as Heart attack.

Slid Dozer off end wall to the pit floor about 10'. He was offered transportation to a hospital, he refused. Then in Dec. 07 started complaining of shoulder and neck pain, said was caused from acccident in 3/07. He started missing work and had surgery on neck and shoulder on 3/10/08. This was reported to me on 3/25/08.

Employee was using portable gas welder at plant. He reached in with right hand to pull throttle open. His hand got into fan blade resulting in fracture of index finger and lacerations requiring 18 stitches.

THE #5 ROCK CAME FROM THE SHOT IN BIG PIECES. WE WERE BUILDING A RAMP WITH THIS MATERIAL WITHOUT ANY PROBLEMS. IT STARTED TO RAIN MAKING THE BOULDERS SLIPPERY. EE CLIMBED HIS FRONT TIRE UP ON A BOULDER AND IT SLIPPED OFF THE OTHER SIDE BRINGING THE TRUCK DOWN FAST AND HARD, JARRING AND ROCKING HIM SUDDENLY. THIS MOTION IS WHAT IS SUSPECTED TO HAVE CAUSED THE CERVICAL RADICULOPATHY.
EMPLOYEE EXITED THE LEFT SIDE OF THE CAB AND AS HE WALKED AROUND THE CORNER LOST HIS BALANCE AND HAD TO SIDE STEP (SHUFFLE HIS FEET) TO THE LEFT AND GRAB FOR THE RAILING. THE RAILING DIDN'T SU PPORT HIS WEIGHT AS THE WELD WAS BROKEN. HE HUNG BY ONE ARM FOR A SECOND BUT COULDN'T HANG ON. HE FELL TO THE GROUND ON HIS RIGHT SIDE INJURING HIS RIGHT SHOULDER.
WHILE CLIMBING DOWN FROM THE R-85 EUCLID HAUL TRUCK, VICTIM STEPPED ON UNEVEN GROUND AND SEVERLY SPRAINED HIS ANKLE.
WHILE FUELING THE DOZER, EMPLOYEE STEPPED BACK FROM THE DECK TO DESCEND TO THE GROUND BUT MISSED THE TRACK (THE NEXT STEP). HE FELL, STRIKING HIS FACE ON THE RIGHT SIDE BREAKING THE ORBITAL BO NE IN TWO PLACES JUST ABOVE THE CHEEK. HE WAS ALSO COMPLAINING OF PAIN IN HIS RIGHT SHOULDER. BOTH FACE AND SHOULDER STRUCK THE LEFT PUSH ARM OF THE DOZER. HE HAD THE FULL NOZZLE IN HIS HAND.
