Injured was clearing dust chamber with a shovel when hot material entered EE's boot causing a burn to EE's ankle.

While using water hose to clean under belt in stone tunnel, mud splashed onto hand.

Employee was assisting in removal of kiln 1 man door. Employee raised employee's glasses to wipe away sweat when something entered employee's eye. Employee flushed the eye and continued working. On 9/25/2024, employee sought medical treatment when the condition didn't improve.

Operator was having Kiln 5 baghouse challenges so they slowed down kiln system to assist. When kiln system was slowed down, the cooler fan kicked out which ultimately resulted in hot material being fed directly onto the belt.

Injured Person stated while picking up tools for shift end, IP felt a nick on hand and during the next shift the right hand swelled. IP reported the incident to supervisor on 5-6-2023 at 7:00 am and went to the local ER for further observation. IP was treated for a minor lime irritation to an open wound. IP was treated and released back to work without any restrictions.

Lab Technician was discarding a LECO crucible using tongs when the crucible dropped to the counter. Technician attempted to prevent crucible from falling to floor and received a burn to the thumb.

Injured was cleaning K1 coating pocket when hot material entered EE's boot.

On 5/5/2022, annual audiometric testing revealed a potential reportable shift. Employee was re-tested on 5/20/2022. On 6/1/2022, Audiologist reviewed and confirmed that retest was a reportable shift.

Injured received 2 small burns when hot dust from ID fan contacted lower leg and foot.

Injured was lifting a 40lb grease barrel into truck bed when finger was cut by one of the tabs on the barrel lid.

Injured was starting up 6 feeder in 4 baghouse. The feeder started up and the drive started jumping flinging grease which landed on hard hat and glasses. Some of the grease entered left eye.

Due to inclement weather the previous evening, injured donned ice grippers prior to going to work. While stepping up a step on coal silo, gripper caught on the stair grating which caused IE to fall, catching self with right arm causing a strain. IE was given work restrictions on 1/6/2021.

Injured was walking north along kiln 1 at 3-4 pier when they noticed something in right eye. EE went to the shop and and washed out right eye. The following morning they still felt like there was something in the eye and called to report the incident.

At 7:45am the main Innofuel storage bin doors were opened to investigate the cause of a CO2 system discharge. When doors were opened, smoke was observed and fire department was contacted. No injuries or damage occurred.

At approximately 11:32am on Saturday 08/15/2020, BU cooler belt (5 belt) ignited. Plant personnel attempted to extinguish the fire using portable extinguishers and the GFD was contacted for assistance. Fire was extinguished at approximately 12:40pm. Damage to equipment was limited and no injuries resulted from the incident.

Assistant Kiln Operator (injured) and Kiln Operator were removing the kiln gun from Kiln 1 after shooting. When attempting to remove the gun from the hood bracket and place it onto the storage rack, injured felt a pop in right shoulder.

At 2:15pm, smoke was observed coming from the area behind the coal tent. Upon further investigation, flames were observed coming from the scrap wood dumpster. The fire department was called at 2:20pm and arrived on site at 2:40pm. Fire was extinguished at 2:50pm.

Injured was drilling holes to fasten down skirt rubber on a conveyor. When exiting the conveyor hood cover, injured felt pain in left shoulder. Injury became reportable on 09/20/2019 when injured was given work restrictions and referred to an orthopedic specialist.

Injured was attempting to remove a smaller steel pipe from Kiln 4 analyzer pipe. Employee attempted to use water to loosen up lodged pipe when hot water/steam splashed back into their face.

Third party trucking company's service pickup truck caught fire while driving through the facility at approximately 12:10pm. MSHA hotline contacted at 12:22pm. Occupants exited the vehicle and cleared the area. Fire Department was contacted and responded. Fire extinguished at 12:50pm.

In an attempt to dry out refractory in Kiln 1, 3 natural gas torches were inserted into the hood at a low gas flow rate. At 7:10am, a loud noise was heard by plant personnel. Following a more thorough investigation of the noise, some damage was found to the coal mill air return ductwork at approximately 8:30am, it was determined that a gas ignition had occurred.

Injured was using an electric jackhammer to remove hard pan material from S-106 feeder in the hical tunnel. While using the jackhammer, the injured felt pain in shoulder. The injury became reportable on 6/8 when the doctor ordered physical therapy.

Injured was opening L-400 feed screw gate with a pipe wrench to adjust material flow when EE felt pain in lower back. Incident was reported on 5/11 and medical treatment was requested on 5/17 when condition didn't improve. Injury became reportable when physical therapy was ordered on 5/31.

At 1:20pm flames and smoke were observed coming from the top of #6 concrete coal silo. The GFD was contacted at 1:25 and arrived on site at 1:35. When the GFD arrived on site, there were no visible flames however to ensure that the fire was extinguished, the GFD sprayed water into the top of the silo until no additional smoke was visible. Smoke was no longer visible at 2:15pm.

Injured was installing new bag house reducer (approximately 15 lbs.) on 1/2" studs side mounting and felt pain in back. Injured reported the incident on 4/4 and requested medical treatment on 4/16 when the condition didn't improve. Injury became reportable on 5/29 when physical therapy was ordered.

Employee was applying heat with a natural gas torch to attempt to free up a frozen flop gate at the top of #3 stone silo. In the process of heating the flop gate, employee inadvertently ignited the adjacent S-132 stone conveyor belt.

Employee noticed smoke coming from the top of #6 coal silo. Further investigation revealed a smoldering coal fire inside the silo. Site personnel installed a fire hose and began quenching with the plant water truck while the fire department was called as an additional precaution. The fire was under control and the fire department departed at 2:05pm.

At approximately midnight on 6/4/2017 several small fires were noticed in the new alternative fuels system. Personnel extinguished the fire using fire extinguishers and called GFD as a precaution. Upon further investigation, it was determined that a dust ignition/explosion had occurred inside the system's drag chains causing deflection damage to multiple covers.

Injured was assisting changing a head pulley bearing when they pulled on a pry bar and felt pain in the shoulder. Incident was reported immediately but did not become reportable until 08/24 when an MRI was performed and physical therapy was prescribed.

Employee sustained a fractured right tibia when foot caught an e-stop cable while stepping down from L-357 conveyor dust collector structure causing EE to fall awkwardly onto the walkway.

While cleaning K-121 belt in the cooler pit, a temp employee noticed a troughing roller between 4 and 5 kiln coolers that was stuck and not turning. EE starting clearing material away from center roller with a shovel. When the roller broke free, it caught the shovel causing finger to be pinched between the shovel handle and the conveyor structure fracturing pinky finger.

Injured had removed a 3' section of DCR screw conveyor cover in an attempt un-jam it. To clear material, EE ran the screw in reverse. The employee then walked to the screw to observe progress when right glove was snagged by the end of the screw flight and pulled in causing injury.

The employee was in the process of opening the Draco Inspection Door for the #5 Hopper in the #5 Bag House. Once the door was opened material came out of the hopper onto the employee who was positioned below and off to the side of the hopper. The material, Envirolime, was hot and caused injuries to the employees neck, forearms, and eyes.

The employee was cutting a metal plate with a hand grinder equipped with a cutting wheel to get it to fit. There was some pressure on the plate that caused the grinder to kick back and when it did, the rotating cutting wheel hit ee left forearm and cut it.

Injured was striking a hopper with a hammer to get material to flow when finger was pinched between hammer handle and handrail.

Employee was in the process of changing sprays for #4 kiln. After removing spray and placing on catwalk, the spray bar caught on kiln and pinched his foot between the bar and catwalk.

Rainy and warm conditions turned cold rapidly causing injured to slip and fall while walking to lab from loadout. Due to previously warm and rainy conditions, employee did not anticipate slippery conditions. Employee requested initial medical treatment on 11/30/2014.

Injured reportedly was climbing a ladder and grasped a rung awkwardly causing him to "tweak" his elbow. Injured reported incident on 8/21, requested medical attention on 9/9, and was given work restrictions on 10/8 when the condition didn't improve.

Injured was using hammer and chisel to remove a stripped out allen-head bolt on coal mill fan seal ring. Chisel slipped and finger contacted sharp edge of seal ring causing small laceration.

JLB Trucking (contract coal hauler) was on site preparing to dump a load of coal. Driver said he heard a pop and then smoke and flames entered the cab of his truck (#22.) Driver used his portable fire extinguisher but it did not extinguish the fire. The Gary Fire Department was contacted to fully extinguish the fire.

At approximately 10:00am on 12/20/2013, Kiln 4 coal weigh feeder was undergoing a belt cut which required the temporary removal of the coal feed to the mill. During this time, the coal dust inside the mill ignited causing dust to be expelled from the feeder opening and the hood of the kiln.

Injured was attaching hose to the vac truck while standing on the work platform approximately 5 1/2 feet off the ground, when he lost his footing and fell. Employee was wearing fall protection, including harness and lanyard. The equipment prevented the employee from striking the ground, however it caused him to swing back into the truck causing a rib injury.

Injured employee was assisting another maintenance employee in removing the electric motor from a fines screw. During this activity, injured received several minor lime burns to the back of his wrist. Injured first reported the incident on 9/27 at 3:15pm when he requested medical treatment after the burn became infected and swelling in his hand began to occur.

Injured and a second employee were in the process of changing L443 Weigh Belt. Injured was holding up the tail pulley take-up as employee 2 unscrewed the take-up bolts. When the bolts were removed the tail pulley dropped farther than anticipated (10"-12") to the housing below. Injured' s finger was pinched causing a laceration.

Injured was permanently mounting a 15 lb. oxy-acetylene rack in the shop truck. When attempting to relocate the rack, he felt a sharp pain in his lower back. Injured requested medical treatment on 3/27/2013.

Injured was opening manual overhead door (chain-operated) when he felt pain in his right elbow. Injured requested and was provided medical treatment on 3/23, but wasn't given work restrictions until 4/9 making the injury MSHA-reportable.

Injured was walking down a flight of stairs at the end of his shift when he felt a pop in his left knee. Injured requested medical treatment the following day.

Injured noticed swelling in his right elbow at break time. Injured reported to his supervisor that he must have bumped something during the day that caused the swelling. The following day 8/8, injured requested medical attention when the swelling didn't improve. Injured was diagnosed with bursitis.

Injured was cleaning out Kiln 5 coating pocket when he thought he smelled something burning and went down into the cooler pit. Injured walked to Kiln 2 and turned around to exit after inspecting the area. As he walked toward Kiln 5, he slipped and fell to the walkway jarring his back. Injured was treated initially on 7/6, however the injury did not become reportable until 7/13.

Employee was removing ash material from lime kiln. A large piece of ash material became lodged in the man way opening. EE tried to dislodge the material from the opening by placing his left hand between the ash material & the metal manway opening. While doing this his left hand became lodged causing him to smash his left little finger.

Kiln 5 experienced a ignition of coal dust that was contained within the mill system. No injuries or damage resulted from event. MSHA hotline was contacted immediately.

Injured was unplugging kiln 2 oxygen analyzer pipe when the air cannon cycled blowing hot dust into his eyes.

Kiln 1 coal mill experienced a minor unplanned ignition of coal dust at 4:36am following a shut-down. No damage or injury occurred as a result of the ignition. MSHA hotline was contacted at 4:40am.

Injured finished cleaning parts in the parts washer then proceeded to bench to dry them off. Injured removed his hard hat and glasses and scratched an itch on his right eye with the back of his gloved finger causing parts cleaner or unknown foreign object to enter his eye. Injured flushed eye and requested medical attention the following day.

Oil inside of Kiln 5 bull gear housing ignited at 9:39am partially due to kiln being shut down recently. The fire was contained within the gear housing and extinguished using hand-held extinguishers at 10:20am. No injury or equipment damage occurred as a result of the incident. MSHA hotline was contacted at 10:12am to inform them of the fire.

Injured was using hand-held drill to drill a hole for a bracket. The drill bit caught in the bracket causing the drill to twist. The twisting of the drill strained his left index finger. Injured did not request medical attention on the date of the incident. Injured later requested medical attention and was given a steroid injection on 3/8/2011.

Injured was striking 1A tank with a hammer to attempt to get material to flow. When injured struck tank, lime dust fell onto him from above, entering into his right eye past his goggles.

Injured was attempting to rotate the #1 turnhead to the coal silo position. As he started rotating the turnhead, it unexpectedly moved quickly causing injured to feel a "pull" in his back. Injured did not request medical treatment initially. Injured was given work restrictions on 2/14/2011 when the condition did not improve.

Employee was cutting corrugated plastic hose for a Hi-VAC, to clean an area. Using a utility knife, the employee made a cut, causing the blade to slip and strike the employees right leg. This caused a small laceration.

Employee was wearing waterproof boot covers that were fastened to the calf with tape. The employee used a utility knife to remove the boot covers, when he accidentally cut himself on his left calf, causing a laceration that required stitches.

Employee was attempting to unclog a dust chamber when material broke loose, causing dust to enter the employees eye.

A gas lance was inside of Kiln 2 preheating, when the cooler fan was opened. This caused a draft within the kiln that extinguished the flame from the natural gas lance. Natural gas was released into the kiln, and an ignition caused the reaction. No persons were injured nor was there any exterior property damage.

At approximately 12:37 pm, on 03/7/2010, the shift foreman reported a waste hopper being used by contractors was found to be smoking and on fire. The cause of ignition is believed to be lime that reacted with the rain falling at the time.

The employee was taking a sample from the sample port gate when the cover plate struck her on the left hand. The employee originally reported the incident, but did not seek medical treatment. The following day, the employee reported swelling and soreness and wished to have it examined, it was then determined the employee's hand was fractured.

The maintance truck was used to travel to a tunnel to unclog a feeder jam. The employee left the vehicle idling, and when he returned to the truck observed smoke coming from the hood. The employee notified the shift supervisor and began spraying fire extinguishers. Local fire department was called as employees continued to suppress the fire.

The employee initially reported a back strain on 2/19/2010 after feeling a pain in his back while walking down a staircase. On 3/16/2010, the employee reported continued pain, after believing to have tweaked it while lifting a sample bucket over the weekend. The employee was taken off site for examination and was given a prescription drug.

Driver was found on customer property requesting emergency assistance for leg injury. Cause of injury is unable to determine at time due to time constraints and medical attention needed. Investigation is on going with all parties involved.

Employee was operating the Hi-Vacuum truck, maintaining the hose connections. The employee was using a utility knife to cut tape that was securing the hose connection. The employee was using one hand to cut the tape, and in doing so the knife slipped and caused a laceration to the employee's left hand.

Employee was cleaning out a dust chamber when a strong gust of wind caused dust to get in the employees eyes. The employee was wearing a face shield and safety glasses as is required for this task.

Injured was using a bar to remove a stone plug on a stone weigh feeder chute when he moved the bar and pinched his finger causing a laceration. Injured applied first aid treatment, but could not fully stop the bleeding due to personal medical condition. Injured was send to the clinic where the bleeding was stopped and work restrictions were applied.

Injured turned on a water valve connected to a hose in order to control dust near the waste pile. When the valve was turned on, hot water struck the employee's left thigh causing burn.

Injured was working under the kilns when a windblown foreign body entered his eye. Employee rinsed the eye immediately, but requested medical attention after his shift when the condition didn't improve.

Injured was kneeling in the walkway of 204 tunnel while cleaning under the conveyor with a shovel. Later in the shift, injured's knee became sore and medical attention was requested.

Injured was attempting to free a stuck feeder with a pipe wrench. Injured struck the pipe wrench with a hammer causing the wrench to bounce toward the employee and strike him below his right eye.

Injured was removing coal mill liners using a pneumatic impact wrench. The employee was holding a bolt between his thumb and index finger when he activated the wrench. The head of the bolt caused a laceration to his thumb and index finger.

Injured was washing stone fines in the boat house with a water hose. The employee shut off the ball valve on the sprayer causing pressure to build and the hose to pop free from the hose barb. When the hose popped free, one of the clamps caused a laceration to the palm of his hand requiring stitches.

Injured was hitting a storage tank with a hammer to get it to flow. When the material inside broke free, a large amount of dust was generated. The airborne dust entered the injured's left eye.

Injured was taking tank measurments on 6 tank when lime entered his eye. He initially rinsed his eye, but the condition did not improve and he requested medical attention.

Injured was raking coating the previous day when he developed pain in his left elbow. Injured requested medical attention the following day when the condition didn't improve.

Injured was walking down the stairs on top of #2 coal silo. When he reached the bottom of the stairs and turned the corner, he slipped on a patch of ice causing him to fall. When he fell, he landed awkwardly on his right wrist.

Injured was attempting to clean frozen material from under a conveyor belt with a bar. Injured was holding the bar, while another employee struck it with a sledge hammer. The employee swinging the hammer missed and hit the injured's finger.

While attempting to open a manual slide gate, injured felt a pop in his lower back. Injured reported the injury initially, but did not request medical attention until 8/23/06.

Inspecting bags and strong wind blew dust in face and eyes.

Steel cable choker was tied up around bottle neck. Employee went to remove cable to change bottles, cable was frayed and cut through gloves to fingers.

Injured employee received a laceration to the back of his right hand when Kiln 1 auxiliary motor clutch came apart. A piece of the clutch struck the employee's hand, causing a laceration requiring stitches.

Injured was attempting to train a conveyor belt by adjusting a return roller by tapping on the bracket with a hammer. When attempting to strike the bracket, the injured's glove became caught in the roller causing a severe thumb avulsion/broken bone.

Injured was cleaning up around the facility with a small payloader. When the injured employee dug into a pile, the payloader bounced and jarred his back.

While performing an inspection inside the kiln #3 baghouse, a strong gust of wind blew through the doorway of a module creating a large cloud of lime dust. The dust blew into face and into eyes.

Injured was operating payloader when a bee entered his shirt and stung his left elbow.

Injured was opening flop gates when hot lime dust entered boots causing 1st degree burn to his left ankle. Injured was seen by the doctor and a dressing was applied. On 9/15 the employee returned to the doctor who determined that the burn had become infected.

Shoveling stone and thought he strained a stomach muscle. Currently being assessed for a possible hernia.

Slipped on rail track and twisted ankle while walking through center loadout bay.

Employee was attempting to open gates in #3 baghouse. The employee was preparing to use a bar for assistance. When he picked up and attempted to position the bar in his hand, he received a laceration to the palm of his right hand.

Employee was operating 515 payloader across a bumpy surface. The employee steadied himself by grabbing the side of the seat. When the shock-absorbing seat lowered under his weight, the employee's right ring finger was pinched in the seat frame.

Injured was attempting to rotate the #2 turnhead into the stone position. The turnhead would not move with normal force. Injured exerted additional force, overexerted and strained his back.

Injured was using a cleaning bar to clear material from plugged material chute when he felt pain in his lower back.

Injured was turning middle turn head with supplied turning bar. Turn head was difficult to turn so injured used his leg to provide extra force. While using leg to push on bar, injured sustained strain to knee. Injured did not seek medical treatment for the strain until 12/27/04.

Injured was attempting to clear frozen material from a chute with a hammer and bar. While clearing the material, the bar slipped and pinched the tip of his his left ring finger causing a tuft fracture.

Injured found plugged spray head. While attempting to remove hose and unplug head, hot water splashed employee and caused minor burn to left hand.

Hot lime from Kiln 5 cooler pocket dropped onto cooler conveyor causing ignition of the rubber conveyor. Fire caused damage to cooler conveyor and to misc. electrical wiring in the area.

Injured removed a plug from a lime storage tank. When the plug was removed, lime dust fell from the opening, became airborn, and entered injured's eyes.

Employee was raking coating on #1 Kiln when foreign body entered eye.

Employee was attempting to remove foreign material from #1 kiln stone silo feed chute when he strained his back.

WORKING ON THE FINES SCREW ON THE RE-SCREEN. EMPLOYEE WAS USING A HAMMER. EMPLOYEE HIT HIS LEFT PINKY FINGER WHILE SERVICING THE SCREW FEEDER WITH THE HAMMER.

WHILE UNPLUGGING THE STONE SCALE FEED MATERIAL SPOUT THE MATERIAL HANDLER HIT HIS LEFT THUMB WITH A PIPE USED TO CLEAN THE CHUTE.

EMPLOYEE (MATERIAL HANDLER) WAS MOVING A 55 GALLON DRUM. WHEN MOVING THE DRUM THE BARREL STARTED TO TIP AND THE EMPLOYEE TRIED TO STOP THE BARREL FROM FALLING. EMPLOYEE STRAINED HIMSELF IN THE LOWER STOMACH. HE NOTIFIED THE FOREMAN OF THE ACCIDENT JUST IN CASE IT WAS A PROBLEM AT A LATER TIME. THREE WEEKS LATER.

OPERATING FLOOR EE WAS INVESTIGATING A MOTOR THAT FAILED ON THE MATERIAL FEED TO THE #2 KILN 18"BELT. THE EE ENTERED A ELECTRICAL ROOM AND DID NOT FOLLOW THE PROPER PROCEDURE TO RESET THE STA RTER. EE ENTERED THE STARTER CABNET PROCEDURE TORESET STHE STARTER. EE ENTERED THE STARTER CABNET AND TRIED TO OVERLAOD A STICK WHEN THE STARTER FAULTED.

EMPLOYEE WAS WORKING AT THE #1 TURNHEAD LOCATION. WHEN STEPPING OFF THE S132 BELT HE HEARD HIS KNEE POP. NOTIFIED THE FOREMAN ON DUTY OF THE PROBLEM AND DID NOT GO TO THE CLINIC.

ASH BUILDUP INSIDE THE COAL BIN CAUSED SMOKE AND FLAMES. THE BINS WAS EMPTIED AND THE ASH BUILDUP WAS REMOVED. NO PROBLEMS ARE NOTICED SINCE THE DATE ON 1-6-03 ON THE COAL BIN INVOLVED.

SLIPPED ON STAIRCASE IN LIME LOADOUT WORK AREA. HE WAS GOING FROM ONE LOCATION OF LOADING TO ANOTHER. WHEN HE SLIPPED HE HURT HIS BACK.

EE WAS USING PORTABLE GRINDER TO COMPLETE REPAIR TASKS ON TRUCK SCALE. EE WAS WEARING ALL REQUIRED EYE PROTECTION(S.GLASSES/FULL FACE SHIELD). WIND APPARENTLY WAS BLOWING AT EE'S BACK & AIRBOR NE MATERIAL WAS BLOWN INTO THE FACE SHIELD CAUSING A PIECE OF MATERIAL TO FLY INTO HIS EYE.

EE SLIPPED WHILE WORKING ON KILN PIER BASE & HIT RIGHT SHOULDER. HE INITIALLY DECLINED TREATMENT BUT AFTER ONE WEEK WAS FEELING PAIN & WAS SENT TO THE CLINIC. HE WAS THEN PUT ON LIGHT DUTY WIT H BRUISED SHOULDER. HE WAS RETURNED TO WORK WITH RESTRICTIONS OF LIFTING NO MORE THAN 5 LBS AND NOT RAISING SHOULDERS ABOVE HEAD.

SLIPPED & HIT RIGHT SHOULDER ON PIER BASE AT STAIRCASE LOCATED BETWEEN #1 & #2 KILN DRIVER PIER.

WORKER USING RIVET BUSTER CHIPPING OUT BRICK INSIDE KILN AREA WHEN A BRICK FELL STRIKING HIM ON THUMB ON RIGHT HAND.

MOVING MATERIALS ON OPERATING FLOOR HE TRIPPED &FELL ON SHOULDER.

WORKING IN #1 BAGHOUSE, EE HIT HIS KNEE ON THE DOOR WHEN EXITING THE ROOM.

WHILE RAKING COATING EE SPRAIN UPPER BACK.

EE WAS LIFTING A PIECE OF GUARDING OVER A HANDRAIL FOR REPAIRS AND STRAINED A MUSCLE IN HIS RIGHT SHOULDER. HE WAS USING A PIECE OF ROPE TO LIFT THE PIECE OUT OF A TRANSFER POINT APPROX. 30".

WHILE LOADING DUMP TRUCK, LIME DUST FELL FROM ABOVE LOADING BAY INTO EMPLOYEE'S LEFT EYE.

USING A MANLIFT ELEVATOR, EE WAS TRANSPORTING A FIBERGLASS POLE FROM ONE LOCATION TO ANOTHER. THE POLE WAS CAUGHT WHEN THE ELEVATOR WAS MOVING. THE POLE HIT THE EE IN THE HEAD CAUSING INJURY.

WHILE DUMPING LOAD MOVED TRUCK FORWARD GRADE OF THE GROUND BECAME UNLEVEL AND TRUCK TIPPED OVER.
