AMES MINE Metal/Non-Metal

The employee was stepping off of the bus underground. Employee said they stepped in a pile of 3/8 minus and rolled the right ankle. The employee did not seek medical attention or treatment until 7-7-20. The injury became reportable 7-8-20.

Removed undercarriage to remove wheel and studs. Lug nut stuck on wheel stud, put pressure on wheel to remove lug nut. Wheel moved as lug nut came off. Pinched hand between impact gun and wheel axle. Stitches (4) laceration on palm and bruised hand.

Employee was turning to walk back to scaler upon completing area inspection, tripped and fell causing a laceration on their knee.

At approximately 8 A.M, an EE was starting conveyor 27. EE was standing to the side as ee was starting the conveyor. The panel energy discharged causing the panel door to open. The discharge of energy caused particles to come into contact with the EE. Particles from the discharge scratched the EE's right eye. The employee was wearing eye protection.

Employee alleged that as they began to climb entry ladder of a haul truck they placed their right foot on the first step. As they took the weight off the left leg the left knee popped and caused pain. EE was seen at a clinic where restrictions were placed on their work activity. No direct cause was identified.

While operating a loader the operator experienced a heart attack and lost consciousness. AED shock was administered twice while emergency response arrived. The employee was taken to the hospital, resuscitation was not successful. Based on MSHA investigator guidance we understand this to be a non-chargeable reportable since it was the result of a personal health condition.

While hauling rock to the crusher EE contacted the rock wall in front of them. This resulted in surface lacerations to their hand and face. EE received stitches and no further treatment/no restrictions.

6/26/16 ee alleged shoulder pain but couldn't attribute it to work related task. EE was seen by dr and directed with OTC meds and no work related. 7/6/16 ee reported discomfort & examined by dr who suggested ee symptoms potentially work related and imposed work restrictions. Soft tissue pain identified no indication of any sprain, or further conditions.

While working on the crusher EE fell and struck the side of the crusher. He was wearing a self-retracting fall protection harness but he swung into the side of the crusher resulting in injuries to his face and left arm. He received stitches for his injuries.

EE struck his gloved hand against the handrail while swinging a 10lb sledge in an attempt to knock loose accumulated material in one of the bins he thought had built up during the days operation. The injury was small avulsion on the outside of the knuckle on is right index finger. The injury required 2 sutures. The EE was released the same day with no restrictions.

EE was working under a haul truck. When he crawled out and tried to stand up he stepped on a rock and twisted his ankle.

Mine foreman was making routine mine checks in affected area around 12:50 a.m. When he entered Mod 12, room 8 he found that an area of the roof had fallen. The area was approx. 60' X 75' X 12' thick. No machinery or personnel were in the area at the time of the fall.

EE was attempting to remove short cable from shrive that had become wedged on the roof bolter. EE was applying pressure to a short bar when it slipped and caused the EE's hand to strike the feed rail causing a laceration to his 5th pinky finger. Laceration req. 5 stitches.

EE had gone up into the cab of the Cat 988H loader he operates approx. 15 min prior to shift. The EE caught his heel on the steps while descending from the cab & fell striking the right temporal area of his head on a 1" diameter steel rod at the rear of the machine. The rod is used to secure wheel chocks during normal operation. He received multiple injuries to his skull.

EE was taking the Fletcher drill down the ramp to the Lime Creek level when he lost control on the slope. EE complained of neck and shoulder pain. EE was taken to the emergency room and examined. EE received 1 stitch in his left index finger.

While taking bolt out of a 775 injector, the teeth on his 3/8 drive ratchet broke and he hit his elbow on an engine bracket. It felt like he hit his crazy bone and didn't think anything about it. It felt fine at that time. While working Saturday he noticed that it was starting to swell. He iced it and mentioned it Monday Morning when he arrived at work.

EE was cutting 1/4" fuel line with a utility knife. The knife sliced through the employees glove striking the end of his left index finger resulting in 2 stitches.

EE was placing a 10' long 3"x3" steel angle iron back on a rack. The EE got his hand in between the steel and other pieces on the rack, pinching the end of his right hand ring finger. Resulted in a fracture of the bone and stitches.

EE was driving an off road articulated haul truck in an underground mine. The employee was returning to the load out area down a slope when he struck a pot hole and fractured the L1 vertebrae in his back.

Employee was moving a pipe fusing machine to repair a section of pipe. He stumbled over some rock and fell striking his knee on a rock resulting in a small laceration that required 3 stitches to close.

EE dropped off a note from his personal physician on 05/19/08 indicating time off work until 05/21/08 for medical condition. On 05/21/08 EE told manager that operating haul unit aggravated his shoulder. EE was sent to company doctor on 05/22/08. Physical therapy was prescribed.

Employee was shoveling wet material inside a surge tunnel and complained of low back pain. Original diagnosis was LBP. Employee was given a epidural injection to reduce the swelling on 05/15/2008.

EE was fixing a slab shot that was drilled through causing excess Dyno Ap to fall through the holes. As he picked up a stick of explosives, several rocks fell above him, ranging from the size of 30" to 1". The EE quickly moved to avoid the falling rocks but was struck on the back of his right hand causing a small laceration requiring 4 stitches.

Ee was standing on the bottom step of the stacker cleaning rock off steps when mine phone rang. EE turned to go answer the phone and caught his left foot in the stairs. To avoid falling, the ee jumped down approx. 2 feet landing on rock, initially twisting his right foot/ankle.

EE was loading the bottom of a heading for shot from the ground. Other employees were in a basket in the air to the left of the employee. A large piece of shale/stone broke away above the EE and fell to the ground, landing on the employee knoc- king him to the ground and fracturing his ankle.

EE AND LEADPERSON WERE WORKING ON A CRUSHER REPAIR. THE LEADPERSON HAD A SLEDGEHAMMER AND THE EE WAS HOLDING A WEDGE. THE EE THOUGHT THE LEADPERSON WAS DONE & PUT HIS HAND OUT TO REMOVE THE WEDGE WHEN THE LEADPERSON SWUNG THE SLEDGEHAMMER HITTING THE EE'S THUMB THAT WAS ON TOP OF WEDGE. LACERATION TO THUMB REQUIRED STITCHES.

EE was cutting a piece of deck rubber on the top deck of screen #3 with a utility knife. As he was cutting away from his body the knife cut through the rubber and cut his left thumb requiring four stitches.

Employee was exiting powder truck in wash area of shop. Upon setting foot on uneven ground employee sprained his ankle.

EMPLOYEE WAS DISLODGING BRIDGED ROCK OVER THE TWO FEEDERS WITH A SLEDGE HAMMER. HE WAS BEATING ON THE HOPPER SIDE WALLS WHEN HIS HAND CAME BETWEEN THE HANDLE AND SOME ANGLE IRON CAUSING A PINCH POINT. THE EMPLOYEE WAS WEARING GLOVES. THE INJURY REQUIRED 5 STITCHES.

Ee was cutting a portective sleeve for a new 1" anfo line. The sleeve is hard plastic and the razor knife had a new blade. The knife slipped and cut his palm even though the ee was wearing gloves. the injury required 7 stitches to close.

Ee was operating yard loader when he drove across an intersection that had some material that created a bump in the road. The operator claims he felt a pop in his neck and later began to fell stiff and had tignling in his fingers.

EE was moving catwalk from a 769D haul truck in the mine shop. Two bolts remained on the bottom and one on the top. The Ee removed the top bolt while standing with one leg on the catwalk. The bottom bolts popped out and the ee fell to the concrete floor striking his knee cap on the floor or the catwalk.

EE was underground in shop to obtain diesel welder. The jack of the welder was broken. EE lifted up on the tongue of the welder to hook up to the pickup & twisted his back. Taken to doctor; conservative course of treatment followed until it was determined surgery was required. EE had surgery on 1/19/06 for a herniated disc & is expected to make a complete recovery.

Employee exited his truck to use the restroom. When on the ground he kicked a baseball size rock. he then went to the restroom and as he turned around to return to his truck, he felt pain on his left side which he described as feeling like a pulled muscle.

Two employees were carrying an oxygen tank with injured employee walking backwards while looking over his shoulder. He stumbled backward and fell on his rear. The oxygen tank fell between his legs onto his finger and smashed it against a rock, which split his finger open. The employee was wearing gloves.

Employee was manually operating elevation measure stick. Trackhoe bucket hit him - pushed him into a dirt wall. He ended up with dislocated shoulder and sprained ankle.

audiological testing STS - possible recordable. Retest scheduled for 9/23 -- awaiting result. (10/4/04 review of 9/23 report: retest shows hearing loss not confirmed.)

Driver of the troop carrier went too close to the edge of the road and tipped the vehicle containing employees over on driver's side into ditch.

EE claims he felt something pop in his lower back when he bent over to mark a hole with a can of spray paint on the floor of the underground mine

EE WAS SITTING IN EMPLOYEE BUILDING. HIS MINE BELT WAS ZIP-TIED TO A CHAIR BY ANOTHER EE IN A ACT OF HORSEPLAY. EE CUT HIS HAND WITH A KNIFEWHILE TRYING TO CUT THE ZIP TIE.

EE WAS TRYING TO POSITION A SCREEN ON THE TOP DECK OF THE PORTABLE SCREEN WHEN IT GOT HUNG UP. THE EMPLOYEE GOT THE SCREEN UNSTUCK AND REACHED DOWN TO GRAB IT WHEN THE SCREEN SLID DOWN AND HIT THE EMPLOYEE'S RIGHT ARM/WRIST.

EMPLOYEE WAS PULLING ON THE DISCHARGE HOSE OF A 3" PUMP. THE HOSE CAUGHT ON SOME ROCK AND THE EE JERKED ON THE HOSE CAUSING AN APPARENT BACK STRAIN.

EMPLOYEE CLIMBED INTO PRIMARY DUMP BOX AND FEEDER TO GET SOME WELDING ROD FROM ANOTHER EE WHO WAS WORKING IN THE AREA. THE EE LOST HIS FOOTING WHEN HE TURNED AROUND. HE STEPPED DOWN APPROX 2-3 FT AND HIS FOOT LANDED ON A ROCK WHICH CAUSED HIS RIGHT ANKLE TO BREAK.

EE REPORTED ON 3/5/03 AN ALLEGED INJURY FROM 2/26/03 RESULTING FROM SHOVELING & SWING A SLEDGE HAMMER & REMOVE A LOADER PIN. EE REC'D SURGERYFOR HERNIATED HERNIA & WAS LOST TIME AS A RESULT OF THIS INCIDENT

EE WAS ALLEDGEDLY CLIMBING DOWN FROM HIS LOADER FOR LUNCH. WHEN EE PUT WEIGHT ON HIS RT.KNEE ON THE GROUND HE EXPERIENCED PAIN. EE WAS TAKEN TO LOCAL DR. WHO DIAGNOSED A KNEE STRAIN THAT WAS AN AGGRAVATION OF AN AGE RELATED CONDITION. EE WAS GIVEN A CORTAZONE INJECTION AND PRESCRIPTION FOR ANTI-INFLAMATORIES.

EMPLOYEE CLAIMS BACK STRAIN FROM OPERATING A LOADER OVER POT HOLES AND BUMPS IN STOCKPILE AREA.

EE WAS HELPING PULL BELTING ONTO A STACKER CONVEYOR WHEN THE CABLE PULLING THE BELT SLIPPED OUT OF ITS CLAMPS. THE CABLE WHIPPED BACK STRIKING THE EE IN THE LEFT LEG/ANKLE AREA. LEG SWELL UP A ND RESULTED IN LOSS OF WORK DAYS.

EE DID NOT LOWER BOX ON 40TON HAUL UNIT, CAUSING IT TO WEDGE INTO THE CEILING IN THE UNDERGROUND MINE AT APPROX. 25 MPH.

EMPLOYEE WAS LIFTING A CRUSHER BAR INTO THE PRIMARY CRUSHER. HE HAD RECENTLY HAD GASTRO SURGERY & LIFTING THE 230LB BAR APPARENTLY CAUSED A HERNIA AT THE SITE OF HIS SURGERY. HE IS SCHEDULED T O REPAIR HERNIA IN EARLY OCTOBER 2002.

EMPLOYEE ALLEDGES HE SUFFERED A BACK STRAIN FROM REPETIVE LIFTING OF EXPLOSIVES BOXES. THIS EMPLOYEE ALSO SHINGLES AND COLLECTS SCRAP IRON IN HIS OFF HOURS.

TWO EMPLOYEES LOCKED & TAGGED OUT #2 MINE CONVEYOR. THEY DID NOT CHECK TO BE CERTAIN IT WAS DEENERGIZED. THE CRUSHER OPERATOR, UNAWARE THEY WERE WORKING ON #2 TAKE-UP PULLEY, WENT THROUGH THE START-UP PROCEDURE & STARTED BELT #2 THAT HAD A DISCONNECT SWITCH THAT HAD BEEN TAMPERED WITH. ONE EMPLOYEE WAS PULLED TOWARD THE TAKE-UP BUT WAS PULLED TO SAFETY BY A CO-WORKER.

EE WAS PROBING A MATERIAL PILE WITH A 2" PIPE PROBE SAMPLER. HE HAD ALREADY STUCK THE PILE WITH THE PROB 10-12 TIMES WHEN HE PUSHED THE PROBE AND FELT A SHARP PAIN IN HIS NECK AND UPPER LEFT S HOULDER. HE CONTINUED TO PROBE 3-4 MORE TIMES AND FELT SOME PAIN BUT DIDN'T THINK IT WAS ANYTHING TO BE CONCERNED ABOUT. PAIN GOT WORSE AND EE GOT MEDICAL ATTENTION.

EE COMPLAINED TO MANAGEMENT OF SWELLING IN HIS LEFT WRIST DUE TO REPETATIVE USE STEERING A LOADER.

EE OPENED SIDE DOOR ON ENGINE COMPARTMENT OF CAT LOADER TO CHECK ENGINE OIL. APPARENTLY EE HAD NOT OPENED THIS DOOR COMPLETELY AND THE SHOCK ABSORBING PROP COULD NOT HOLD IT OPEN. THE DOOR FELL CLOSED, STRIKING THE EE IN THE BACK OFTHE HEAD, FORCING HIS FACE INTO ANOTHER ACCESS DOOR AND CAUSING FACIAL LACERATIONS.

EE WAS LOSOENING J BOLTS ON A SCREEN. AS ONE BROKE FREE, THE EE'S LEFT ELBOW CONTACTED HIS KNEE RESULTING IN A CONTUSION OF THE LEFT ELBOW. THIS INJURY WAS TREATED ORIGINALLY AS A FIRST AID. M EDICAL COMPLICATIONS NOW MAKE THIS INJURY A MEDICAL REPORTABLE.

EE WAS TRYING TO CUT HOLE IN CONVEYOR BELT WITH A UTILITY KNIFE. WHILE PULLING THE KNIFE TOWARDSHIS BODY, IT SLIPPED & CUT ACROSS THE TOP OF HISKNEE AT THE LOWER QUADRICEP MUSCLE (5 STITCHES).

EE WAS USING A PRYBAR TO REMOVE NEOSCREEN. THE SCREEN POPPED LOOSE CAUSING HAND TO GET PINCHED BETWEEN PRYBAR AND STEEL LEDGE. INITIALLY THIS ACCIDENT WAS TREATED AS FIRST AID. PERSISTENT PAIN CAUSED EE TO SEEK MEDICAL ATTENTION AND XRAYS REVEALED A FRACTURE TO EE'S RIGHT RING FINGER.

EMPLOYEE COMPLAINS OF STIFFNESS AND PAIN FROM EXCESSIVE ROCKING AND VIBRATION FROM OPERATING A SCALER IN AN UNDERGROUND MINE. HIS CONDITION WAS INITIALLY DISMISSED AS BEING SICKNESS. HE SEEM T O HAVE AGGRAVATED HIS PRE=EXISTING CONDITION NOWMAKING HIS CONDITION WORKER'S COMP.

EE WAS LOWERING A EUCLID HAUL UNIT WITH A HYDRALIC JACK. HE PULLED THE JACK AS SOON AS IT CAME LOOSE AND AS HE MOVED IF UNDER STEERING CYLINDER THE TRUCK CONTINUED TO SETTLE AND PINCHED HIS LE FT INDEX AND MIDDLE FINGER AGAINST THE JACK. HE HAD TO BE FREED BY CO WORKERS. THE RESULTING INJURY REQUIRED STITCHES.

EMPLOYEE WAS CLIMBING DOWN STAIRS FROM PERSONNELCARRIER. ON THE LAST STEP HE SLIPPED OR TRIPPED AND FELL 12-14" TO THE GROUND. THE FALL CAUSED THE EMPLOYEE TO SUFFER TORN TENDONS & LIGAMENTS TO BOTH KNEES.

EMPLOYEE WAS ALLEGEDLY SHOVELING MATERIAL OFF OF A STOPPED CONVERYOR AND SUFFERED A TENDEN STRAIN IN HIS RIGHT ELBOW. DUE TO THE ORIGINAL NATURE OF THIS INCIDENT IT WAS TREATED AS FIRST AID. REPEATED TREATMENTS FOR THIS CONDITION HAVE NOW MADE THIS A REPORTABLE INCIDENT.
