Employee was in the process of changing screens. When EE tried to loosen the screen hold down clamp with a pry bar, EE mashed the end of EE's left ring finger on the frame edge, amputating the end of it.

Employee was attempting to relocate a water line and when lifting, pulled or tore something in right forearm.

Employee was cutting a hole in metal with a torch and the slag blew back and went under their eye protection. Employee sustained an eye injury to the ocular surface.

Employee was loading ANFO from an overhead bin into a Powder Monkey truck. EE pushed the lever to open the bin and when EE released it, it swung back hitting EE in the mouth. Employee sustained a laceration on the lip and a chip in an upper right tooth.

Employee was driving a Caterpillar 773 down a slope and said the brake retarder kicked in and caused them to slide into a pillar. Employee said they hit their head on the steering wheel. Minor damage to the trucks handrail and mirror.

Blasters helper was loading a face with an ANFO hose and pulled something in elbow.

Employee was driving a 2001 International truck used to haul ANFO to load the shots down a 15 degree incline. The driver lost control of the truck due to wet conditions of the roadway causing it to slide down the slope and into a rib. The truck was extensively damaged. The employee had a laceration on head and some bruising.

Employee was on knees using a utility knife to cut a conveyor belt when the knife slipped out of the belt and hit right knee causing a laceration.

Employee was changing screen panels and removed his safety glasses for a moment and a piece of rust fell in his eye.

Employee was raising a counterweight on a conveyor with a chain hoist when the chain broke he jumped back landing on a tool twisting his ankle.

Employee was taking inventory of ANFO bags in a storage facility. He tripped on the plastic wrap holding the bags on the pallets striking his knee on the floor when he fell.

Employee parked the grease truck and was exiting the vehicle when his foot slipped off the step, causing him to fall to the ground, bruising his right lower back and right hip.

EMPLOYEE STEPPED ON A OIL FILTER LAYING ON THE GROUND, CAUSING HIM TO TWIST HIS ANKLE, CAUSING A SPRAIN.

Employee's foot slipped off the step of a dump truck while exiting the vehicle causing him to sprain his ankle.

EMPLOYEE WAS ATTEMPTING TO HITCH A TRAILER TO A PICKUP WHEN THE TRAILER SHIFTED MASHING THE MIDDLE FINGER ON HIS LEFT HAND.

EMPLOYEE WAS EXAMINING VEHICLE FOR DEFECTS. WHILE CLIMBING DOWN THE LADDER HE STATES HE TWISTED HIS ANKLE WHEN HE CONTACTED THE GROUND.

EE COMPLAINED OF BACK PAIN CAUSED BY LIFTING OVERHEAD WHILE HANGING A BEAM TO SUPPORT WATER PUMP.

EE TWISTED HIS KNEE EXITING A ROCK DRILL.

EE WAS USING A ROTARY HAMMER TO BREAK UP PACKED ROCK IN A BIN. HE WAS HOLDING HAMMER AGAINST HISCHEST WHILE IN USE.

EE WAS SCALING CEILING WHER A SMALL ROCK CAME THROUGH WINDOW OF SCALER AND HIT HIM ON LEFT HAND.

WHILE SHOVELING UNDER CONVEYOR BELT EE RAISED UP& HIT BACK ON CONVEYOR FRAME

EE LOST CONTROL OF TRUCK AND TURED IT OVER.

DRIVING UP RAMP - NOT GEARED LOW ENOUGH - TRUCK ROLLED BACKWARD AND OVER TURNED.

WHILE DRIVING WATER TRUCK TO WET DOWN MUCK PILES HE WAS OVERCOME BY EXHAUST FUMES.

FRONT WHEEL OF TRUCK EE WAS DRIVING HIT A ROCK CAUSING STEERING WHEEL TO SPIN. ARM CAUGHT IN STEERING WHEEL CAUSING BREAK.

HURT BACK WHILE STOOPING/BENDING TO CHANGE SCREENS IN ROCK BIN

EE WAS DISLODGING CRUSTED STONE FROM ROCK BIN WITH IRON DIGGER. CAUGHT FINGER BETWEEN DIGGER &SIDE OF BIN & FRACTURED FINGER

EE WAS BEATING SIDE OF ROCK BIN WITH SLEDGE HAMMER TO DISLODGE ROCK CAUGHT THUMB BETWEEN SIDE OFBIN & HAMMER HANDLE

EE WAS MOVING PIECE OF METAL PLATE PLATE SLIPPED& STRUCK HIM IN BACK

GETTING OUT OF TRUCK--FOOT SLIPPED AND HE FELL--STRAINING BACK.
