Employee was in the process of changing screens. When EE tried to loosen the screen hold down clamp with a pry bar, EE mashed the end of EE's left ring finger on the frame edge, amputating the end of it.

Employee was cutting a hole in metal with a torch and the slag blew back and went under their eye protection. Employee sustained an eye injury to the ocular surface.
Employee was loading ANFO from an overhead bin into a Powder Monkey truck. EE pushed the lever to open the bin and when EE released it, it swung back hitting EE in the mouth. Employee sustained a laceration on the lip and a chip in an upper right tooth.

Employee was driving a Caterpillar 773 down a slope and said the brake retarder kicked in and caused them to slide into a pillar. Employee said they hit their head on the steering wheel. Minor damage to the trucks handrail and mirror.

Employee was driving a 2001 International truck used to haul ANFO to load the shots down a 15 degree incline. The driver lost control of the truck due to wet conditions of the roadway causing it to slide down the slope and into a rib. The truck was extensively damaged. The employee had a laceration on head and some bruising.
Employee was taking inventory of ANFO bags in a storage facility. He tripped on the plastic wrap holding the bags on the pallets striking his knee on the floor when he fell.

EE SHOVELED ROCK 11/2/99 & 11/3/99. CLAIMED HE WAS HAVING PAIN IN RIGHT WRIST AT END OF DAY 11/3/99. HE DID NOT RECEIVE ANY TYPE OF INJURY. RELEASED BY DR TO RETURN TO WORK 11/22/99. DID NOT S HOW UP.
