Calvary No 81 Coal

As the middle bridge was being added in, the back carrier trammed up and came in contact with the cribs that had the bridge cribbed up and caused the bridge to slide over striking EE.

EE and the scoop man were going to get the maintenance insert and some oil to take to the miner to service it. EE got in the bucket of the scoop between the insert and the ram. The scoop man backed up the heading and pulled up into 4 right to turn. As they pulled forward the insert contacted the rib, pushing it into EE.

Injured and another employee was putting a set of scoop batteries on charge. Injured bent over to pick up the female end of the jumper cable when upon doing so, they felt a pain in their back going down right leg. We were not informed of the results from the specialist until 08/28/2025.

EE was scattering structure when EE tripped over a roller. As EE fell, the stand that EE had landed on EE's legs.

Employee was removing some curtain in 3 left cross cut. EE slung the curtain and as EE did EE's left hand struck a roof bolt plate.

a non injury roof fall occurred on 11/04/2024 due to deterioration of the top in the Right Fork Mains, Break 11.

a non-injury unplanned fall occurred on the parallel mains in the track entry beginning at the in by corner of break 36 and extending approximately 70'.

Employee was gathering parts for a head drive. EE sat down to eat lunch and took off EE's gloves. When EE stood up and was putting on EE's gloves, EE slipped, and EE's hand hit a piece of metal used for a back board on the drive. EE received three stitches in EE's right hand.

Buggy became stuck while crossing the rails. Employee got a jack and a bar to try to dislodge the buggy from off of the rails. When they pushed down on the bar that was inserted in the jack, they felt a pain in their lower back.

EE was bolting in 3 left. EE had already installed the outside bolt and swung in to install the inside bolt when a piece of draw rock fell from the top striking EE on the right hand around the pinky finger.

A roof fall occurred in the #4 entry on the Narrow Branch mains between c/cuts 65 & 66. The fall is approximately 5' high by 19.5' wide by 40' long.

While retreat mining was being performed in the #4 coal pillar, x-cut 32, on the F Panel off the Mudlick submains, a large rock from the mine roof dislodged and caused the continuous miner to become stuck.

EE picked their tool bag up out of the tool box to put in a scoop bucket and felt a pull in their shoulder as EE swung the bag into the bucket.

Weekly examiner found an unintentional roof fall in the #4 entry return roadway, break #24, in the Stratton Fork Mains parallel with #11 Belt.

Found an unintentional roof fall on the #4 Beltline at break #51.

Employee was attempting to unload a speed reducer at the track to take to the section. The speed reducer slipped while EE had a hand on it catching EE's fingers on the right hand between the shaft of the reducer and the lip of the ride. This resulted in a broken index finger and 7 stitches on the middle finger. Employee received a full release to return to work.

Found an unintentional roof fall in the belt entry of Mudlick Submains between xcuts 5 to 7.

A roof bolter operator was getting drill steel out of the hole when it fell, knocked off EE's glasses, and hit and scratched EE's left eye. A prescription medication was prescribed and used.

Employee stepped down off of belt and tailpiece and felt a pop in left leg/hip. Diagnosed with a tear in hamstring. Started missing work on 8/2/2022

Found an unintentional roof fall on the #11 Beltline (Stratton Fork Mains) at break no. 21. Roof fall was approx. 4-5' high by 20' wide by 30' long.

Employee was handling and loading belt structure stands into the scoop bucket. One on the stands was stuck. As EE pushed and another employee pulled on the stand, the stand broke free pinched EE's finger between the stand and scoop bucket. 6 sutures were required.

Employee was using a lifting jack to align the low structure to insert a pin. The jack slipped causing the pin to pinch employees' thumb between the pin and low structure. Sutures were required to repair the wound.

Employee was troubleshooting a stuck bridge conveyor chain and caught foot under the bridge conveyor as it was being let down. EE was diagnosed with a fracture to a metatarsal bone in the foot.

Found an unintentional roof fall during the pre-shift examination at break # 30 of the #4 Track entry.

Found an unintentional roof fall at break #49 of the #2 beltline.

Employee was handling a belt structure rail when a sharp edge penetrated EE's glove and cut EE's right ring finger. 3 sutures were required.

Employee knelt down on right lower leg pushed down against a roof bolt plate and it cut the employee's leg. 4 sutures were required to repair the wound.

Employee stated was tossing roof bolts across the roof bolter and EE's knee started hurting. Employee did not notify mine management until 2/22/2021 and EE's immediate foreman was unaware of the injury. EE started missing work on 2/10/2021.

Employee was bending a 5' 3/4" rebar roof bolt and the bolt slipped and employee felt a pain in left arm and shoulder. Employee was diagnosed with a tear in the rotator cuff on 1/22/2021 and taken off work.

The outby examiner found an unintentional roof fall in the examiner's travel route on Stratton Fork Mains. Location of fall is in the intersection and break of #5 Entry at Break #42. Fall dimensions are approximately 30' long by 4' thick by 11.5' wide.

A piece of rock fell and struck employee on the left forearm and sutures were required to close the wound.

Employee was helping load high voltage cable into a supply car and felt a pain in the groin when EE pulled on cable. Started missing work 6/8/2020.

Found an unintentional roof fall at Break #41 of the #11 Belt Line. Fall was approximately 50' long by 20' wide and 3' to 5' thick.

Employee lifted and carried a rail motor coupler and felt a pain in shoulder. EE injured this same shoulder in January 2019 that resulted in loosing time from work. It was thought that this was a re-occurrence but diagnostic tests results received by the operator on 6/2/2020 indicated that a new tear in the bicep was found in a different location than before.

Employee was prying up on an MBC rub rail with a slate bar, the bar slipped and they felt a pain in left shoulder. Employee continued to work and was then referred to a specialist on 3/10/2020. The orthopedic specialist diagnosed employee on 4/28/2020 with a tear on rotator cuff and took employee off work.

An employee fell and struck a belt tailpiece and cut their arm. Sutures were required and they returned back to work full duty.

Found an unintentional roof fall in the #3 entry between breaks 19 and 20 on the #12 Belt line. Fall dimensions are approximately 20' wide by 60' long by 3' thick.

Found an unintentional roof fall while conducting a pre-shift examination. The fall is located #6 entry, South Mains, 3 breaks from seal #5 of Seal Set # 30.

Employee was removing an underground mandoor and the door inadvertently shut onto EE's left middle and ring fingers. A medical procedure and stitches were required on the finger tips.

Employee was using a lifting jack and the jack bar slipped out and caught finger between canopy of the MBC and the jack bar. Sutures were required.

Employee was handling and tossing steel track ties and one bounced up and struck EE on EE's left forearm. Sutures were required.

A Section Foreman was helping position a Flexco splice board when another employee inadvertently hit the foremans right thumb with a hammer. The employee was diagnosed with a fracture to EE's thumb tip.

An unintentional roof fall was found from break #29 to #30 on the #11 Beltline. Fall was 3-6' high by 20' wide by 75' long.

An unintentional roof fall was found in the travel way of a weekly exam route at break #37, #4 entry, on the Stratton Mains. The fall was dangered off and an alternate way of travel was provided.

Employee was lowering a supply car onto the track with a lifting jack and the bar sprung up and struck the employee on the chin.

Found an unintentional roof fall at cross-cut #44 on the #2 track and connecting crosscut to the belt line. Dimensions of fall were approximately 18' wide by 55' long by 4' thick.

Employee was shoveling and strained left arm, left shoulder, and neck. Employee returned back to their job as a belt man but started missing work on 7/17/2019 due to further complications.

Employee was riding a rubber tire mantrip and right knee was sticking out from the perimeter of the vehicle and struck the rib causing a dislocated femur and pelvis fracture.

Employee was using a utility knife to cut a roll of wide electrical tape into half when knife slipped and it cut employee's left thigh. 4 sutures were required to repair the cut.

Employee stepped off of a conveyor belt tailpiece and felt a pain in EE's back. EE started missing work on 4/22/2019

Employee injured (strained) left arm (bicep) while hand unloading track steel off of a supply car at the end of the track. EE was diagnosed with a tear in the bicep that will require surgery.

Employee was shoveling belt in low top and felt a pain in lower back. Employee started missing work on 1/10/2019

Employee turned around to walk and slipped and fell. Employee has continued to work since DOI of 11-7-18. They started to miss work on 6/13/2019 due to having surgery to their wrist.

Employee was walking beside belt line and slipped and fell on the wet and slick bottom. As EE caught self, EE struck left elbow on the bottom. EE was diagnosed with a chipped bone on the elbow.

Employee had their hand on the drill pot of the roof bolter checking the suction and a rock fell and struck their hand. Employee fractured their right middle finger.

Employee was sitting down and starting to eat lunch when a rock fell from between the rib and bolts and struck EE's right foot. Employee went to an Orthopedic Specialist on 9/11/18 and was diagnosed with a fractured bone in EE's right foot. EE started missing work on 9/12/18.

Employee was tightening bolts on the stacker speed reducer. EE socket slipped off the bolt causing right elbow to strike against a guard bracket. EE received 3 sutures to right elbow and returned back to work during the same shift.

Employee was hanging a roof bolter cable while standing in the mud and as EE turned to go back to bolter EE twisted EE's knee. Employee started missing work on July 20, 2018.

Employee changing roller on head drive. Taking bull gear off, hitting bull gear with a sledge hammer. Hammer handle hit employee's safety glasses knocking them off employee's face. The head of the hammer hit the edge of the bull gear and a piece of metal went into employee's left eyeball. Employee had surgery on eye the next day

Employee was found unconscious and unresponsive underground, employee was transported to the surface. Death Certificate was received 11/7/17 and it states the manner of death as Natural and cause of death as a Myocardial Infarction. Investigations revealed no impending circumstances at the mine contributing to employee's death.

Riding to section in a Mac 12 rubber tire mantrip. While making a turn toward section the Mac 12 hit rib catching SCSR storage box shoving it onto a fire extinguisher and the fire extinguisher hit employee's hand. The right hand palm was cut requiring 6 stitches

Employee hand pushing track rail off of flat car and when one end hit mine floor the other end bounced up hitting employee on left side of face in the cheek area cutting cheek (required stitches) also sinus fractures and a small hair line fracture in cheek bone

Looping high line cable in sled and while pushing down on high line cable felt a pop in right shoulder. Employee started missing work 09/06/2017 due to surgery.

Employee was using a jack to put a rail locomotive back onto the track and the jack bar struck on the side of EE's head resulting in EE getting 13 sutures.

Employee was installing track ties in a bad section of track and strained employee's back. Employee started missing work on 4/13/2017.

Employee was lifting a top belt conveyor roller and strained EE's lower back.

Employee caught gloved hand between the drill steel and the roof bolter drill head chuck twisting right arm around the drill steel. EE was diagnosed with 2 fractures in right forearm and surgery was required. EE started missing work on 2/22/2016.

Employee was striking a roof bolter tram chain with a hammer when a small fragment of metal came off and imbedded into left arm. A Dr. removed the metal fragment and applied 3 sutures to repair the incision on 2/15/16.

Employee was getting some nails out of a box and left thumb come into contact with a discarded utility knife blade in the box and cut EE's thumb. 5 sutures were required

A 23" by 33" by 3" thick piece of rock struck the employee on the back and bruised his back. He started missing work on 11/3/2015.

Employee was striking a belt roller with a crib block and hit his left thumb against the belt stand. X-ray indicated a fractured thumb.

Found an unintentional roof fall at Break #39 in the #2 Entry parallel with the #4 Belt line. Roof fall dimensions were approximately 20' long by 50' wide and 8" thick.

Employee was bolting top and his drill steel come out of the hole and bounced on roof bolter and struck him on the upper lip. Sutures were required, No Lost Time.

Employee was installing a panel board cover on an MBC and the wrench slipped and he hit finger against the frame of the MBC resulting in a fracture to his 5th digit fingertip.

A piece of rock fell and struck employee on his lower left leg. He went to the Dr. after his shift and returned back to work the next shift. Due to the swelling and bruising, he started missing work on 5/28/2015.

Found an unintentional roof fall 1 break inby the portal on the #2 Beltline.

Found an unintentional roof fall in the #1 entry intersection of Calvary 80 Mains and Leslie Perry Mains. Roof fall dimensions are approximately 20' wide by 40' long by 5' thick.

A surface pond overflowed and the excess water flowed into the mine portal blocking the primary and secondary escapeway with 1 1/2' to 2' of water. The blocked area was from break # 1 to # 4 from the surface portal and no persons were in the mine at the time.

Trying to put 4" waterline together and while pulling on waterline, he felt a pain in his lower back. He returned back to work his next shift but had to start missing on 10/3/14 due to his back not getting any better.

A piece of rock bounced off from drill boom and struck employee on the upper lip. 3 sutures required.

A PICI contract employee was working on the No. 29 Belt and slipped in the mud and twisted his left knee.

Found an unintentional roof fall in the #4 Track Entry 3 breaks inby the #4 Track Switch. Fall was approximately 20' wide by 30' long and 4-5' thick.

Employee was standing beside the roof bolter when a piece of rock fell and struck him on the head and face. Sutures required.

Employee was installing the top rib bolt in the #3 Heading on the 004 MMU and felt something pop in his left shoulder. He started missing work on 6/23/2014 pending the results of an MRI.

While drilling / bolting, a piece of drawrock fell onto his left index finger and fractured his finger.

While moving a belt rail, the rail caught the skin between the thumb and index finger and cut him, 3 sutures required.

PICI Contract employee strained his back while shoveling the lolo structure.

An unintentional Roof Fall occurred at break #6 of the # 33 Beltline. Fall dimensions are 35' long x 35' wide x 8.5' thick

EE was drilling a hole in the top and a loose piece of scale fell and hit his left hand. Fractured the index carpal bone. He started missing work on 1-9-2014.

While bolting the ribs, EE got right 4th finger tip in the slide/hinge on boom and cut and bruised finger.

Employee was using a utility knife to trim the edges of a belt splice and slipped and cut his leg. 3 sutures were required.

Employee was doing rehab bolting work outby and a piece of drawrock fell from the top and struck his hand and fractured his left index finger. Diagnosed with a small non-displaced fracture.

Employee was loading crib blocks into the scoop bucket and a crib block rolled back and struck him on the right ankle. Diagnosed with a hairline fracture in his ankle.

Employee was installing a belt on the mine fan and caught his right index finger and middle finger between the pulley and belt and fractured his right index finger and bruised his right middle finger. No Lost time.

Found an unintentional roof fall in break #1 to # 3 on the Calvary 80 Mains in the # 7 entry. Fall dimensions are 40' long by 20' wide by 7 ' thick.

Found an unintentional roof fall at break #5 on the Calvary 80 Mains in the #2 entry. Fall dimensions are 20' long by 20' wide by 8' thick.

Found an unintentional roof fall between breaks #26 and 27 on the Leslie Perry Mains in the #6 entry. Fall dimensions are 80' long by 20' wide by 7' thick.

Found an unintentional roof fall between the # 3 and # 4 entries on the Leslie Perry Mains at break #28. Fall dimensions are 60' long by 20' wide by 7' thick.

Found an unintentional roof fall on the # 8 Belt(Right Fork Mains) between breaks # 24 and 25. Fall dimensions are 40' long by 20' wide by 7' thick.

Found an unintentional roof fall from break #6 thru 7 on the Calvary 80 Mains in the #1 entry. Fall dimensions are 80' long by 20' wide by 8' thick.

Found an unintentional fall in breaks #23 and #25 on the Leslie Perry Mains in the #7 entry. Fall dimensions are 130' long by 20' wide by 7' thick.

Employee was weighting down a ventilation curtain with a rock and another rock fell onto the tip of his left ring finger.

Employee was hitting a metal pin with a hammer and a piece of the hammer broke off and struck him on his left leg and cut his leg. (2 sutures required).

Employee was removing the drill steel for a test hole and the steel fell and struck him on the left hand and cut his hand. 3 sutures were required.

A small piece of rock (3" by 5" by 1/2" thick) fell and deflected off of the roof bolter and struck employee on the neck and scraped and cut his neck. Sutures were required.

EE was getting a rock from the belt and the rock cut his right middle finger. Sutures were required.

Employee was putting bolts in the #6 tail pieces skirt and the belt started while he was on the belt. Started missing work 7/17/2012.

EE was changing batteries on a scoop and the battery lid dropped down and caught his left thumb. Sutures were required no lost time.

Employee was getting up and out of mantrip and slipped and fell against his hand. He fractured his right little finger.

Found an unintentional roof fall during the weekly exam. Fall is in the #5 entry approximately 6 breaks inby #14 belt-drive. The fall is approximately 20'wide x 4'thick x 80'long.

He was changing a shuttle car tire and dropped tire on thumb. Fractured and required sutures.

Employee was moving a set of bull gears and caught his left middle finger between the gear and crib and mashed finger. Sutures and medical procedure required.

EE fell down and struck his arm on a metal strap and cut his arm. Required sutures no lost time.

EE caught his thumb between the bottom and the scoop bucket. After going to the doctor it was determined the thumb was fractured.

Slipped and fell on foot and sprained ankle - Worked 2 days restricted duty.

He was driving a supply motor through a set of air lock doors and caught his left foot against the door. Fractured ankle - started missing work 3-2-2011.

Stacking concrete block in a brattice and caught thumb between the top and a block. Fractured end of right thumb.

Drill steel fell from hole and struck hand. Fractured bone in hand.

Drill steel hung in hole and then fell and struck his left hand. Diagnosed with a fracture in his left hand.

Pulling rock out of bridge conveyor and cut right 5th digit finger. Started missing work 10/20/2010.

Mine had an unintentional roof fall on th e#9 belt line at break #41 & # 42. Fall was 30'L X 18'W X 5' H.

Employee was walking down the #3 entry, slipped on a loose rock and twisted his rt. knee. Started missing work 8-6-2010.

EE caught his foot between 2 waterlines and fell backwards. He fractured his right foot and ankle.

Had an unintentional roof fall 20X20X4 in White Oak Mains.

Had an unintentional roof fall 40'x20'x8' in the return entry.

EE was unloading crib blocks and hit finger on track rail.

EE was tramming miner and was struck by the mine tail and it knocked the EE down.

Had an unintentional roof fall on the 2nd pillar panel on west submains 40'x20'4'.

Employee was bolting top, was swinging in boom to put up inside bolt and felt something pull in right shoulder.

Employee was moving cribs, got left (4th) little finger caught between 2 crib blocks.

EE was cutting a metal band off of a bundle of roof bolts and the band flipped back and his his left eye. He had to have surgery to remove a piece of metal from his eye. Started missing work 3/5/09.

He was unloading block and felt a pain in his lower back. He started missing work 3/9/09

Stacking rock dust on supply car and hurt back. Started missing work 2/26/09.

Employee was splicing a belt and a piece of metal flew off and went into his arm, Sutures were required to close the wound.

Employee drill stell fell out of the top and struck EE's left 5th finger, finger was broke.

Drill steel fell from hole and hit EE in face. 2 sutures required.

He was riding in the scoop bucket and hit his head against the top. Strained neck. Started missing work on 8/5/08.

Employee was shoveling belts, throwing coal over shoulder and caught hand in roller.

EE was cleaning belts and a rock rolled off of the rib and struck him. Started missing work 6/23/2008.

He lowered his drill canopy and a rock from the canopy fell and struck him on the head and neck.

Employee bent over to lift piece of wood and felt/heard a popping in lower back area.

Employee bent down looking under belt, rock fell from ceiling and struck victim on the right hand.

He hurt his back while pulling on a miner cable. "He started missing work 4/11/2008".

Employee was lifting steering jack, and stumbled over propane tank, while putting tools away. He then felt pain in left side under rib cage.

Removing cables from batteries and cable grounded & flashed an arc and burned his right hand.

Employee tripped over a bolter cable and strained his left leg. He started missing work 3-3-2008.

Light cord hung in top and pulled him in between motor and top. He has a fractured rib.

Employee was loading belt structure into scoop bucket, and strained lower back.

Employee was bolting top and twisted his left knee.

Installing roof bolts and shoulder started hurting. Started missing work 12/18/2007.

Unloading overcast panels from forklift and 1 panel hit his finger. Sutures required, no lost time.

Employee was operating bolt machine, pushing bolt into hole and the bolt hit crack and bent sharply and the plate struck him on left side of his face, injuring left cheek.

He was loading a boom jack from a supply car and cut his finger. Sutures retuired - No lost time.

Employee was shoveling belt line and shovel got caught on the belt, pulling his arm into the belt and burning his right wrist.

Drawrock fell & struck EE while mining a right break EE started missing work 5/24/07.

Employee was swinging head in, he had his hand on drill pot and a piece of rock fell on his hand.

Employee has his hand on the miner as he was moving cable and a rock hit his hand. 4 sutures were required.

Employee was walking in low top and his back started hurting. He is working at a modified duty.

A piece of rock struck employee on back and shoulder. Diagnosed with a fractured clavicle and 2 vertebrae fractures. Started missing work 2-2-07.

Bolter head hung on roof bolt, when he lowered boom, it fell on foot.

EE was pulling a curtain from out of some muck and back started hurting. Started missing work 1/31/2007.

Taking a cover off of the miner & caught his left 5th digit finger. Required Sutures-no lost time.

EE was knocking a pin out of an ATRS arm in the shop and hammer slipped off pin and hit his hand. Received 4 sutures.

EE stood up from kneeling position and hurt lower back. Started missing work 5/25/06.

Employee was carryng a can of hydraulic oil and strained back. He started missing work on 4/13/2006.

EE WAS CUTTING A CABLE. KNIFE SLIPPED AND CUT HIS LEFT HAND. 4 SUTURES REQUIRED.

Had an unplanned inundation of water after continuous miner penetrated an old mine. No injuries and no damage to equipment.

EE WAS BOLTING IN #4 HDG, WHEN EE WAS INSTALLING A ROOF BOLT AND GLOVE CAUGHT ON BOLT TWISTING ARM AROUND THE BOLT

AS MINING MACHINE WAS CUTTING X-CUT TO PICK-UP AN ENTRY WATER BURST THRU FACE ON RIGHT RIB AND BEGAN POURING INTO MINE. ALL MEN WERE SAFELY REMOVED AND MSHA & STATE IMMEDIATELY NOTIFIED.

EE CUT THE END OF HIS MIDDLE FINGER ON LEFT HAND OFF, WHILE HE WAS OPERATING BOLT MACHINE. HE HAD HIS HAND ON THE BOOM ARM, AS HE DROPPED HIS OF MACHINE.

EE WAS UNLOADING A START BOX FROM A SUPPLY CAR AND CAUGHT MIDDLE FINGER ON LEFT HAND.

EE WAS PULLING ON A PIECE OF BELT ON SCRAPER BOARD, WHEN HE FELT BACK PAIN.

EE WAS SPLICING CABLE, PIECE OF METAL HIT HIM ON THE LEFT HAND. THIS INJURY REQUIRED STITCHED.

EMPLOYEE WAS INSTALLING BELT STRUCTURE WHEN A PIECE FELL HITTING HIM ON THE RIGHT HAND. THIS INJURY REQUIRED STITCHES.

EE WAS CAUGHT BETWEEN A SHUTTLE CAR OR SHUTTLE CAR CABLE AND THE RIB.

EE WAS CUTTING A PIECE OF BELT WITH HIS KNIFE WHEN IT SLIPPED HITTING HIM ON THE LEG. THIS INJURY REQUIRED 5 STITCHES

EE BEING PUSHED BY A SHUTTLE CAR, WHILE OPERATING THE SCOOP TO GET UNSTUCK. THE MOVEMENT CASUDE EE'S BACK PAIN TO REOCCUR. (ATTACHED IS A COPY OF A PREVIOUS PROBLEM EE HAD WITH BACK PAIN.)

EE WAS HELPING ON A BELT SETUP WHEN HE REPORTED OF HAVING BACK PAIN

EE WAS GETTING ON THE BOBCAT TO LOAD SUPPLIES WHEN HIS FOOT SLIPPED, CAUSING HIM TO TWIST HIS LEFT KNEE.

EE GOT FINGER CAUGHT BETWEEN A PIN AND A FLATCAR. THIS INJURY REQUIRED STITCHES.

EE WAS HOOKING A CHAIN WHEN IT SLIPPED, CATCHING HIS MIDDLE FINGER ON HIS LEFT HAND. THIS INJURY REQUIRED STITCHES.

EE WAS TRAMMING A SCOOP TO THE OUTSIDE AND COMPLAINED OF HAVING BACK PAIN.

EMPLOYEE WAS HELPING REMOVE BELT AND STRUCTURE FROM THE MINES AND WAS WEARING KNEE PADS AND INJURED HIS LEFT KNEE, BUT THE EMPLOYEE SAID HE DID NOT KNOW HOW THE INJURY OCCURRED.

EMPLOYEE WAS ROLLING 42" BELT UP.
