Employee was servicing a Joy 14-15 continuous miner. EE was on top of the miner when EE slipped and fell to the ground (approx. 6-7 feet) and landed on EE's face. EE was taken to the hospital by ambulance and was kept one day at the hospital. EE was diagnosed with a concussion and released. EE will be kept off from work for at least 30 days.

Employee was riding to workplace in rail man trip when EE said it hit a bump injuring EE's lower back. EE completed the shift and went to the doctor the next day. EE was taken off from work for 3 days for a pulled muscle. After the 3 days EE went back to the doctor and was taken off until 04/25/2018 and EE returned to work on that day.

EE took self to the hospital to be checked. EE was taken off from work for 2 days. EE returned to work on 02/14/2018 with no restrictions.

Employee was repairing a railcar coupling, weighing about 200 to 250 lbs. EE picked up the coupling several times and as EE rotated or twisted EE's back EE strained/sprained EE's lower back. Took self to the hospital the following day. EE missed two days of work and is now back to work on 01/19/2018.

Employee pulled a rock weighing approximately 200 lbs. from the top of the continuous miner onto ee right foot. EE finished the shift and took self to the hospital where an X-Ray showed a small cracked bone in the foot. EE missed 2 days of work and returned to full duty.

IE was trying to help operator get EE's steels out of EE's hole in the roof at the roof bolt machine. IE spun the steel slightly and IE's right glove became stuck causing it to twist IE's ring and pinky fingers on IE's right hand, resulting in fractures of both fingers. IE took self to the hospital and has missed no work.

Employee was walking through an open air lock door when the door closed back on employee's left foot. Employee was taken to the hospital to be checked. Foot was only bruised and employee was taken off work for two days. Employee is now back to work.

Employee was putting a scoop tire back on a DBT scoop using a pry bar. The pry bar slipped, trapping their right thumb between the scoop tire and the wheel. This resulted in a laceration to their right thumb. The thumb required 6 stitches to repair the laceration. Employee was treated and released and missed no work.

Employee was setting up a breaker on a 995 volt miner when the cst head blew out burning both hands of the employee. Both hands received second degree burns. Employee was off from work for 20 days and returned to work on 6/14/17.

Employee was using a manual hand pumped grease gun to grease rollers on a conveyor belt line when they injured the right arm and shoulder. They took themselves to the hospital and missed no work until today when they brought in a doctors excuse stating that they will be off from work for 16 days.

Employee was replacing a panel on a shuttle car when a piece of draw rock approx. 23" X 8' long and 2 1/2 to 5" thick fell between bolts and slid down EE's back before striking EE's left foot. Foot wasn't broken but EE was hospitalized in order to receive antibiotics through an IV. EE was released on 03/26/2017. EE returned to work on 03/27/2017.

Employee and one other person were lifting a tire from the mine floor. Employee finished shift and reported ee had strained lower back but did not intend to seek medical attention. EE did go to the doctor the next day and has not returned to work.

Employee was putting a shield back on the continuous miner. The shield, weighing 50 to 75 pounds slipped and mashed the tip of the middle finger on left hand, bruising the finger. Employee continued to work that shift and until 10-03-2016. Went to the doctor on 10/04/2016.

After dinner, injured employee was riding the #1 sinking bucket back to the shaft bottom to begin mucking. Sinking bucket #2 was on the shaft bottom and not completely clear. The #1 bucket brushed against the #2 bucket as it came down causing the injured employee to lose his balance and hit his left arm against the inside of #1 sinking bucket.
