Two employees were standing on the plant structure using weed burners, there was an employee standing on each side of the conveyor leading to the hopper. One of the employees was wearing ice cleats, which caused that employee to lose balance but not fall. That employee then lost control of the torch and it passed in front of the injured employee's face.

Employee arrived at work at 6:45 and was having trouble breathing, was cold, clammy, sweaty, disorientated, and was having pains in abdomen. Prior to starting work employee took 2 prescription nitrile pills and used inhaler to try to minimize the pain. Employee has pre-existing heart issues.

Employee stated he had been experiencing some soreness in his right knee for a few days. He could not identify any one specific incident or act that may have cause an injury to his knee. This was diagnosed as a right knee strain.

Employee was pulling on electrical cable for the hot sand plant when he felt pain in his groin area. Employee did not report this incident until 11/4/2009 three weeks after injury and after going to his own doctor. Employee was taken to the clinic on 11/9/2009. Currently employee is on restricted duty on his regular job and will see surgeon in two weeks.

Started having tingling and numbness in right hand on Wednesday, June 30, 2004. Felt soreness in right hand from loading trucks, moving tilt and raise/lower levers with right hand. Notified manager, worked through shift. Tingling was bad during the night, went to clinic next day.

EMPLOYEE WAS CLIMBING INTO HIS LOADER AND FELT A POP IN HIS LEFT SHOULDER. DOCTOR DID NOT GIVE A DIAGNOSIS BUT PUT HIM ON RESTRICTED DUTY WITH NO USE OF LEFT ARM. HE HAD RETURNED TO WORK AT A DIFFERENT JOB TO ACCOMODATE HIS RESTRICTIONS.
WHILE TURNING HIS FRONT END LOADER, EE FELT SOMETHING POP IN IS LEFT SHOULDER. AFTER GOING TO THE OCCUPATIONAL HEALTH CENTER, HE WAS PUT ON RESTRICTED DUTY. HE IS NOT ALLOWED TO STEER THE LOAD ER SO HE IS TEMP. ASSISTING ANOTHER EE WITH MAINTENANCE TYPE WORK.
WHILE EMPLOYEE WAS PULLING ON A FLAT TIRE ON THE MAINTENANCE TRUCK, HE FELT A PULL IN HIS LEFT SHOULDER. WENT TO THE CLINIC ON 1/30/03 WHERE HE WAS PUT ON RESTRICTED DUTY.
EE REPORTED TO HIS SUPERVISOR THAT THE PREVIOUS AFTERNOON HE HAD BACKED HIS FRONT END LOADER INTO A BIG BOULDER. WHEN THIS HAPPENED, THE LOADER CAME TO A SUDDEN STOP. EE WAS FEELING STIFF ON 1 -15-03 SO HE WENT TO THE CLINIC WHERE THEY PUT HIN ON RESTRICTED DUTY.

THIS REPORT WAS NOT CO MPLETED AT THE TIME OF THE INCIDENT BECAUSE NO ACCESS TO CLINIC NOTES WERE AVAILABLE UNTIL 1/21/03. EMPLOYEE DID NOT VISIT CLINIC UNTIL 10/28/02.
WHILE PULLING A BLOWN HYDRAULIC HOSE TO REMOVE IT FROM THE PLANT, THE HOSE CAUGHT ON THE FRAME OF THE PLANT WHICH IN TURN CAUSED PAIN TO RIGHT HAND AND FOREARM. INJURY WAS REPORTED TO THE SUPE RVISOR ON 10/18/02 AT WHICH TIME THE EE WENT TO WIXOM OCCUPATIONAL HEALTH CLINIC WHERE THEY PUT HIM ON RESTRICTED DUTY.
EMPLOYEE WAS CLIMBING UP THE RIGHT REAR LADDER OF E1-91 LOADER AND LOST FOOTING BECAUSE OF THE DRIED MUD. THE MUD BROKE AWAY, CAUSING EMPLOYEE TO FALL BACK, HITTING HIS HEAD ON THE OPEN TOOL BOX DOOR OF HIS SERVICE TRUCK, WHICH WAS PARKED NEXT TO THE LOADER.
