JCB Telehandler tires traveled over slight drop off causing vehicle to roll onto side. Employee suffered laceration to the head requiring sutures.

Struck right hand on stationary equipment causing soft tissue contusion

Employee was using a tool to remove frozen material from a conveyor. Struck hand on framework injuring thumb.

Employee was walking along a conveyor and felt their knee pop. Knee swelling and pain. Restricted duty.

Employee was working in a low headroom area and struck head on fixed object causing dizziness and headache.

Routine travel on asphalt, employee sprained ankle

EE had changed attachment on skid steer and noticed loose pin. EE attempted to fix the pin by reaching out of the machine. EE accidentally hit the controls and in turn pinched EE's right hand between the lift arm and the frame of the machine causing multiple fractures to the ring finger and thumb of the right hand as well as soft tissue injury.

While handling a slide, EE dropped it on EE's right foot, fracturing EE's fourth and fifth toe.

EE removed a cover on a slide reached inside and had a finger struck by a rotary feeder.

EE was pulling on a slide under a silo when it broke free and caused EE to hit thumb on a piece of angle iron.

Fell when exiting a building

EE made contact with an elevator bucket

While sitting in a haul truck and being loaded with large material an EE experienced mid to lower back pain from the shake of the truck.

EE was dismounting from a skid loader at the quarry fuel tank when they slipped from the bucket and fell to the ground.

EE was moving two railcars in the shipping area and was not able to stop the railcars in time which resulted in making contact with a dump truck that was stationed on the tracks.

EE stated that EE lost consciousness while shoveling.

EE inserted an air wand into an elevator making contact with the buckets pinching a finger on the lip of the chute.

Dust fell/blew into employees eye.

EE struck a finger on the side of a rail car when attempting to open the gate.

We are reporting a change in hearing sensitivity of an average of more than 25 dB at the 2K, 3K, 4K, Hz in the right ear relative to the miner's baseline. The employee is currently enrolled in a hearing conservation program. The condition is currently being investigated for work relatedness.

EE was removing piece of jammed metal from elevator. As the piece of metal was jarred loose, the tip of EE's thumb was pinched between the piece of metal and the elevator casing. EE required stitches thus constituting MSHA reportability.

Employee punctured leg with utility knife.

EE was sitting on a platform beside the aerator using a grinder to clean up the shaft when EE lost control of the 4 1/2" angle grinder causing the spinning disc to come in contact with the inner area of EE's left thigh. Injury resulted in a laceration requiring stitches.

EE experienced pain in hands on 9/5/2019. EE had carpal tunnel surgery on 6/23/2020 thus constituting MSHA reportability.

EE was placing shims on a dryer trunnion. EE was working from a ladder and was about 6 feet off the ground. The wrenches EE was using slipped and EE lost EE's balance. EE fell from the ladder and landed on EE's feet, but received a stress fracture in right heel.

During drilling Unit #38 air compressor began to run hot so operator shut the unit down to cool. While unit was shut down, the operator attempted to loosen the radiator cap to release pressure into the overflow tank. While loosening the cap, the pressure in the tank was able to push out past the cap splashing the operator on the left forearm and on the shirt around EE's midsection.

EE was cutting flexible sealing tape into stripes to repair a leaking pipe. The utility knife slipped and lacerated EE's finger.

No SGI (Specialty Granules LLC) employee or equipment was involved in the accident. A driver from a trucking co. was out of the truck and was struck by a truck driven by an employee of another co. All horns and backup alarms worked properly on all equipment involved. Accident occurred during daylight hours in an open load out area.

EE was unplugging equipment and got fingers stuck resulting in 5 stitches.

On 5/3/2018, an EE stepped down from a ladder on a front end loader. As EE stepped down, EE twisted knee. The EE was able to conduct all job tasks after the incident. EE had surgery on 8/3/2018 due to incident and missed work starting this date, thus constituting MSHA reportability.

Employee fell and twisted knee while conducting maintenance on a piece of machinery.

EE came out of loading booth to load truck missed a step EE felt pain in lower back and L leg EE declined medical attention at the time EE examined on 3/12/2018 and diagnosed with a minor muscle strain in lower back and returned to work On 5/14/2018 mgmt. was informed that the employee had received an Epidural Steroid Injection on 5/3/2018 thus constituting MSHA Reporting

Lowering DTH Hammer in to service truck using a winch line. While repositioning from vertical to horizontal for transport, the hammer shifted against truck bed. This action pinched the gloved hand of the victim as ee was attempting to guide the hammer in to position. the small finger of the left hand was caught between the hammer and truck bed surfaces

Employee lost grip on the sledge hammer and handle struck another employee on the left eyebrow

Employee was working inside an elevator chute to reposition a strike plate. While the employee was working their breathing became labored and became overheated and exited the chute. The employee was taken to the breakroom to be cooled off. Was later taken to the ER where fluids were administered via an IV.

A brush fire at the reclaimed hill at 1525. 911 was called at 1530 the fire was contained at 1645. No damage to the property, equipment, or danger to employees or residents. Fire was caused by light reflecting off polished tin covers & hitting the cardboard cover, igniting it. Wind speeds at 15-20 caused the inflamed cardboard to spread to the dry grass igniting it.

EE was in the operator's shack, talking with the truck driver and another employee when they noticed that the truck was beginning to overfill. EE began to run out of the shack to the catwalks under the loading tanks. As employee was running they tripped and fell, breaking a wrist and causing several lacerations.

On 12/12/16 the EE was exiting personal vehicle prior to the start of work. As the EE exited vehicle, L. foot slipped on ice and fell to the ground. At that time the EE declined further treatment. On 1/2/17 the EE requested further treatment. On 1/12/17 the EE was referred to an MRI and on 1/24/17 the EE was diagnosed with a torn meniscus with surgery slated for 2/2/17.

Employee was standing on ground, attempting to put on insulated coveralls. Foot slipped on snow/ice and EE fell landing on back and hip. Bruised hip, sore lower back.

Employee was using a sledge hammer when it slipped striking his right thumb causing a fracture.

Employee was exiting office facility and slipped causing her to strike her head against the door frame causing a small cut to the back of her head.

Employee was cleaning off his equipment when he contacted a sharp metal edge which caused a cut to his right arm.

Slipped and fell 5 steps from ground level coming down stairs. Injured left elbow, laceration (required 8 sutures). Returned to work with no doctor restrictions.

Employee was unloading railcar when debris entered his eye.

Employee lowered his welding hood which caused debris to fall behind his safety glasses which entered his left eye.

Driller shut drill down,got out,walk around drill to check air line.Opened the side door of drill (guard was down)at same time radiator hose exploded spraying hot water on him. Employee had hard hat and safety gasses on. Employee sustained 2nd degree burns to 15% of body.

Employee was moving a piece of metal off a belt,when he picked it up he cut his left thumb-getting three stitches

Employee was holding onto a elbow fitting that attaches to the unloading valve under a rail car. The second employee attempted to strike the upper part of the fitting with a hammer when the first employee left hand was hit, which the employee received four stitches in the left hand-little finger.

Employee was lifting a piece of equipment and was struck in the face by the rigging.

Employee was rolling an 20 inches metal tire toward an truck to be loaded when a sharp metal burr caused by pressing the tire onto rim cut his left finger. Employee received three stitches in his left ring finger.

Employee was in the plant removing an bolt from a segment on the drive shift, the employee was using a hammer and chisel to remove a bolt. The employee got a piece of metal in the left eye, the employee was wearing eye glasses. The metal was removed by the ER doctor. Employee did return to work.

Employee was working in the mill plant when he got cramps in his legs and hands. The heat index was high and the temp was around 90. Employee was taking to the hospital and treated for heat exhaustion and release the same day. Employee was giving a IV and treated by a doctor.

Employee removed piece of chute with torch. Piece was rigged and controlled and lowered to the floor. Once on the floor the employee walked around the opposite side of the chute, from which he cut and stood to lower, to remove rigging. A sharp piece of metal, which he did not see, caused a laceration to his left leg.

Employee was scraping build up from the socket liner to locate the lifting holes using a putty knife. As he moved his hand around the socket liner he cut the small finger on his left hand on the edge of the liner. Three sutures were required.

Exposure to mixed dust. No evidence of pulmonary impairment by pulmonary funcion testing.

Employee was changing a bearing on a fan. Tha bearing guard became unbalaced and fell striking the employee on the right wrist. Two sutures (Staples) were required

Worker was welding a small connecting beam to a bridge beam, when hot slag fell between the leg strap of his harness and his leg. The hot slag was able to burn a hole in his pants before he could get it off. The worker received a minor burn to his leg where the slag had fallen.

Employee cut his finger on metal requiring two stitches.

Employee had been bolting sections of duct work together for the previous couple of days. The work required awkward postures and excessive force in awkward postures. Employee has a history of back problems. On this morning he was unable to get out of his truck upon arriving at the job site

Checking threads on drill rods while head was in motion. Employee put his hands in the tower of drill accidently and was winched by a pulley, causing partial amputation of middle finger and breaking adjoining fingers.

The EE was removing an elevator bucket on a stone elevator. The bucket slipped out of his hands and hit his finger against the lower bucket. The EE did require three stitches.

Employee was turning down a roll tire at the lathe. Cut left hand while removing shavings. PPE was used.

Employee was using a bar to open the slide at the bottom of a rail car. While he was using the bar it bounced up and hit his finger on the left hand.

EE WAS CUTTING APIECE OF BELT IN THE STOCKROOM USING A UTILITY KNIFE. THE ROLL OF BELT WAS IN THE UPRIGHT POSITION, THE ROLL TIPPED OVER AS HE WAS CUTTING THE BELT & CAUSING THE ROLL TO HIT THE EE HAND. THE KNIFE STRUCK EE IN RIGHT ANTORIN THIGH

Employee was cleaning out a chute during a system clean down, the employee reached into a chute to remove material inside the chute and his glove caught the rotating equipment pulling it to a pinch point. The distance of the employee's reach was 12-18 inches. This is not a standard procedure for the clean down.

Employee was setting a new chute section on a screen tower. The chute was rigged to the crane line with a single choker hitch. As the chute was being lowered through a floor opening it got hung up on a flange. Employee tried to straighten the piece and it swung and crushed his left bicep up against the structure.

Employee was holding a piece of steel up to be bolted. When the connection could not be made, employee dopped the piece on the ground. A piece of channel attached to the dropped beam struck employee in the back of the leg. The leg was bruised and scraped. X-rays showed a hairline fracture of the tibia, left leg.

Coworker was walking holding a circular saw. Extension cord got hung up on something, so he gave it a tug. In doing so he bumped into EE & accidentally squeezed the trigger on the saw. EE was startled & jumped. EE's hand contacted the saw blade and his right middle & ring fingers were lacerated, requiring medical attention (stitches).

EE was cutting plasting banding on packing material with a pocket knife and punctured his leg right above the left knee with a pocket knife. EE did receive 4 sutures.

EE WAS CHECKING OUT THE MILL FLOOR WHEN HE NOTICED AN UPSET CONDITION ON THE SCREEN FLOOR. STONES WERE COVERING THE FLOOR AND COMING DOWN ONTO THE NEXT FLOOR. EE WAS ON TOP OF THE STAIRS FLOOR AND HE TURNED TO GO DOWN THE STEPS AND SLIPPED ON STONES HE DID NOT SEE. HE WAS CARRYING A BAR AND CAUGHT HIMSELF WITH HIS HAND AS HE FELL AND CAUGHT HIS FINGER BETWEEN BAR AND STEPS.

EMPLOYEE WAS HELPING TO PUT HE NORTH FLYWHEEL GUARD ON #1 MILL ROLL. IT WAS LIFTED UP WITH A HOIST AND THEY WERE MOVING THE GUARD TO THE SOUTH END WHEN THE BACK OF THE GUARD SWUNG INTO THE BEA M. EE HAD HIS HAND BETWEEN THE GUARD AND BEAM WHEN THE GUARD STARTED TO SWING EE TRIED TO HOLD IT BACK.

EE WAS USING A SLEDGE HAMMER TO DRIVE A KEY INTO THE FLYWHEEL OF #3 EE WAS HOLDING THE HAMMER WITH HIS LEFT HAND BACK ONT HE HANDLE AND HIS RIGHT HAND NEAR THE HEAD AT THE HAMMER AS HE SWUNG T HE HAMMER THE BACK OF THE HANDLE CAUGHT ON THE FLYWHEEL, FORCING THE HAMMER SO THAT THE HEAD MISSED THE KEY AND HE SMASHED HIS FINGER BETWEEN THE HAMMER HANDLE AND FLYWHEEL.

EE WAS PERFORMING HOUSEKEEPING ACTIVITIES ON THE ROOF ABOE #4 SCRUBBER ON A SCHEDULED DOWN DAY. EE LIFTED A LARGE METAL PLATE (APPROX 135 LBS.) EXPOSING A 4' DIA. OPENEING TO THE PUMP ROOM BEL OW. AS HE LIFTED THE PLATE AND MOVE FORWARD, HE STEPPED INTO THE OPENING. EE FELL TO THE FLOOR BELOW.

EMPLOYEE TOOK A LOAD OF PALLETS IN THE DUMP TRUCK TO THE QUARRY AREA, ALONG WITH ANOTHER EMPLOYEE TO ASSIST. WHEN EMPLOYEE WALKED TO THE BACK OF THE DUMP TRUCK, HE SLIPPED ON A PATCH OF ICE, F ALLING ON HIS RIGHT SHOULDER.

EE STATES: SLIPPED & JERKED NECK, STIFF

EE & ANOTHER EE ATTEMPTED TO LOOSEN A 4" PIPE WITH A 90DEG ELBOW ON ONE END. AS EE TURNED THE PIPE WITH A 24" PIPE WRENCH, EE THUMBS WAS CAUGHT BETWEEN THE ARC ELBOW & AND ADJACENT WALL CUTTING HIS THUMB

INSTALLING A NEW DRIVE GEAR ON A ROTARY DRYER. A NEW SPRING CLIP WAS WELDED TO THE SHELL TEMPORAILY WHEN EE ATTEMPTED TO LOOSEN A MOUNTING BOLTIN THE CLIP, THE WELD BROKE & THE SPRING CLIP ST RUCK EE HAND BREAKING HIS FINGERS. THE SPRING MOVED ONLY 3/8" OF AN INCH

EE WAS HOLD THE 3' LIFTING CHAIN TO THE ROLL FEED BOX. AS HE CENTERED THE CHAIN ON THE HOIST HOOK EE INSTRUCTED OTHER EE TO RAISE THE HOOK AS THIS WAS COMPLETED THE CHAIN SHIFTED CUTTING EE'S THUMB \

EMPLOYEE MOVED THE CAR INTO POSITION, HE THEN EXITED THE CAR BY THE LADDER FROM THE BRAKE PLATFORM. HIS FOOT SLIPPED OFF BOTTOM RUNG AND HIS FOOT WENT APPROX 2 FT TO THE GROUND CAUSING A SUDDE N JOLT. HE DID NOT FALL TO THE GROUND.

EMPLOYEE WAS UNLATCHING CLEAN OUT DOOR FOR COLLECTOR DUST PIPE. WHEN CLEANOUT PIPE UNLATCHED, THE WEIGHT OF THE MATERIAL FORCED THE DOOR TO DROPPINCHING EMPLOYEE'S HAND BETWEEN THE DOOR AND ME TAL DEFLECTING CHUTE, CUTTING EMPLOYEE'S LEFT THUMB.

EE WAS MEASURING DEPTH OF RAW GRANULE STORAGE SILE.. HE TRIED TO REPOSTION MEASURING TAPE IN BETTTER LIGHTING TO GET READING. AS HE MOVED BOTH HAND TO READ HIS TAPE MEASURE, THE DOOR OVER THE ENCLOSURE BLEW SHUT STRIKING HIS RIGHT MIDDLE FINGER

EE WAS TRYING TO UNPLUG A CHUTE AT THE 51/2' CONE CRUSHER ROCK & FINE MATERIAL WAS TIGHTLY PACKED AT THE DOOR TO THE CHUTE. ANOTHER EE WAS HOLDING A BAR AGAINST MATERIAL & INJ EE WAS HITTING T HE BAR WITH A SLEDGE HAMMER. HE MISSED THE BAR WITH HAMMER & HIS FINGER HIT THE BAR CAUSING INJURY TO HIS FINGER

EE WAS ON ONE KNEE USING 18" ADJUSTABLE WRENCH, TIGHTENING A BEARING BOLT. WHILOE PULLING UP ON WRENCH, HE FELT SLIGHT PAIN IN HIS NECK. HE WAS REFERRED TO CO. DR. ON 12-08-1999 AFTER COMPLAIN ING OF PAIN IN ARM.

EMPLOYEE WAS WALKING DOWN A 10 DEGREE SLOPE AND SLIPPED, AND FELL ON HIS RIGHT KNEE. HIS KNEE WAS IMMOBILIZED FOR 2 DAYS(OVER WEEKEND) AND THE KNEE WAS ONLY BRUISED. EMPLOYEE WAS NOT SCHEDULED TO WORK WEEKEND. PHYSICIAN WAS SEEN FRIDAY.

EMPLOYEE WAS TRYING TO MOVE A 400 LB ROLL OF CONVEYER BELTING WITH A HAND TRUCK. EMPLOYEE FELL ON HIS LEFT KNEE CAP WHICH WAS FRACTURED WHEN IT HIT THE ASPHALT. HIS KNEE CAP WAS OPERATED ON.

THE PLANT HAD INCLEMENT WEATHER (FREEZING RAIN AND SLEET) FOR THE PAST 48 HOURS. HE SLIPPED ON A PATCH OF ICE, LOOSING HIS BALANCE AND FALLING TO THE GROUND. HE WAS GOING TO A LOAD HOUSE. HE F RACTURED 3 BONES IN HIS LEFT WRIST.

GROUND CONDITIONS CAUSED DRILL BAR TO TIGHTEN. AS A RESULT, THE WRENCH BROKE, FLYING OUT AND STRIKING DRILLER.

EE BARRING RAILROAD CAR UP THE RAILROAD TRACKS. THE BAR SLIPPED AND HIT EE IN THE CHIN. THE IMPACT CREATED A LACERATION WHICH REQUIRED 4 STITCHES AT THE HOSPITAL

A MAINTENANCE TEAM REPLACED A PUMP. WHILE EMPLOYEE WAS HOLDING GUARD FOR DRIVE BELTS, HE SLIPPED. AS A RESULT OF THE SLIP, CARTILEDGE WAS DAMAGED IN HIS RIGHT KNEE.

EMPLOYEE USED APORTABLE LADDER TO GAIN ACCESS TO SLIDE GATE UNDERNEATH SILO. WHEN EMPLOYEE STARTED UP LADDER, THE LADDER SHIFTED. EMPLOYEE FELL AND LACERATED HIS SCALP AND FINGER. HE RECIEVED 5 STAPLES IN HIS HEAD.

EMPLOYEE WAS DESCENDING WET STEPS WHEN HIS RIGHT FOOT SLIPPED. HE FELL WITH LEFT LEG UNDER HIS 300 LB BODY. A TENDON IN HIS LEFT KNEE WAS TORN AND WAS ATTACHED DURING SURGERY.

EE WAS USING AN UTILITY KNIFE TO CUT ELECTRICAL MATTING WHEN THE KNIFE SLIPPED OFF TO THE SIDE. THE KNIFE SLICED OFF A PIECE OF LEFT INDEX FINGER.

EMPLOYEE WAS BENDING OVER TO INSTALL FRONT SHACKLE BOLT IN SPRING ON RIGHT FRONT OF DUMP TRUCK.

ELECTRICIAN OPEN THE PRIMARY SIDE OF A 2500 KVA CLASS 0A TRANSFORMER. USING A VOLTAGE METER HE GROUND ONE OF THE PHASES TO THE TRANSFORMER TRAIN UPON CENTAIT THE ELECTRICIAN WAS THROWN SEVERAL TEST. HE HAS BURNS ON HIS LEFT HAND AND BOTH FEET IS THIS IA PRELIMARY REPORT.

OPERATOR WAS UTILIZING AN AUTOMATED BAGGER. A BAG JAMMED ON THE CONVEYOR. HE LIFTED A BAG OUT OF THE WAY. IN SO DOING, HE CAUGHT HIS HAND ON ONE OF BAGGER'S HORNS. THE LACERATION IN THE BACK O F RT HAND REQUIRED 4 STITCHES.

FRACTURED BONE IN LEFT HAND. EE FLIPPED SCREEN COVER BACK TO SCRAP SCREEN DURING THE SCRAPING PROCESS THE COVER FLIPPED OVER. THE METAL EDGING ON COVERED HIT HIS HAND.

EMPLOYEE WA SWELDIGN WHEN A ROD STUCK TO WELD. TO BRAKE ROD LOOSE, EMPLOYEE PULLED ON ROD WITH LEFT HAND. WHEN THE ROD CAME FREE IT INSERTED ITNO RIGHT ARM. EMPLOYEE REQUIRED 14 STITCHES.

EMPLOYEE WAS USING A SLEDGE HAMMER TO UNPLUG A SCREEN. THE HAMMER GLANCED OFF AND EMPLOYEE STRUCK LEFT MIDDLE FINGER AGAINST A BEAM. FINGER REQUIRED SIX STITCHES.

EMP WAS DUMPING MATERIAL ONTO HAUL ROAD FROM A SINGLE AXLE DUMP TRUCK. MATERIAL WEDGED BETWEEN GATE AND BED. EMP USED HAND TO KNOCK MATERIAL OUT. FINGER SMASHED BETWEEN GATE AND BED.

EE WAS CUTTING A PIECE OF RUBBER BELT WITH A UTILITY KNIFE. THE KNIFE SLIFFED LAND CUT ACROSS HIS LEFT MIDDLE FINGER. LACEARTION REQUIRED FOUR STITCHES.

EE USING STEEL ROD TO HAMMER SLIDE GATE ON BOTTOM OF A STORAGE TANK CLOSED.

HE WAS CLOSING THE BOTTOM SLIDE DOOR TO A RAILROAD DRY TANK CAR A BAR IS USED TO TURN THE CRANK THE BAR SLIPPED OUT OF THECRANK AS HE APPLIED PRESSURE THE FORCE PINNED HIS HAND BETWEEN THE BAR & THE RAIL OF THE RAILROADS HE REQUIRED 7 STITCHES IN HIS FINGER & LIGHT DUTY

A LIFTING CLIP AND TO LIFT A 400 LB PIECE OF STEEL FAILED PROPELLING THE LIFTING CLERICE INTO THE FACE OF AN EE RESULTING IN A BRUISE CHEEK BONE AND LACERATION THE REQUIRED TWO SUTURES.

SPILLED MATERIAL FROM A CONVEYOR HEAD PULLEY HAD BUILT UP & FROZE ON A DEFLECTOR PLATE. INJURED & ANOTHER EMPLOYEE WERE REMOVING THIS MATERIAL WITH A PINCH BAR. THE BAR SLIPPED WHILE HITTING T HE MATERIAL & AND INJURED'S THUMB WAS PINCHED BETWEEN THE BAR & AN ANGLE IRON, RESULTING IN A TORN LIGAMENT & CHIPPED BONE.

EMPLOYEE WAS KNEELING/CUTTING A RUBBER CONVEYOR BELT WITH A BELT KNIFE TOWARDS HIS BODY AND PUNCTURED HIS RIGHT KNEE WITH BELT KNIFE.

EMPLOYEE WAS USING A PINCH BAR AND SLEDGE HAMEMR TO REMOVE BUILT UP MATERIAL FROM CHUTE. EMPLOYEE MISSED PINCH BAR AND HIT HAND CAUSINGA CUT ON RIGHT INDEX FIGNER THAT REQUIRED 3 STITCHES.

HE WAS ON THE ROOF OF THE WASTE SCREW CONVEYOR ATTEMPTING TO CHECK THE TANKS. HE STEPPED ON A 3' X 4' PLEXIGLASS SKYLIGHT, WHICH WAS COVERED WITH DUST, SO IT WAS NOT VISIBLE, UPON WHICH THE PL ASTIC BROKE AND HE FELL 7 FEET TO THE FLOOR BELOW. HE PLACED HIMSELF IN AN AREA NOT USUALLY TRAVELED, EXCEPT FOR MAINTENANCE PERSONNEL. TO GET TO THE LEVEL HE CLIMBED UP AND OVER THROUGH HAND

A ROLL BEARING WAS COMING OFF A SHAFT. THE ROLL BEARING CAUGHT ON THE SHAFT THE EMPLOYEE WAS LIFTING THE BEARING TO SLIDE ON A ROLL SHAFT. EMPLOYEE FELT PAIN IN HIS LOWER BACK.

I.NAME AND 3 OTHERS INSTALLING DRIVE ON #2 K KILN USING PRY BAR TO LINE UP BOLT HOLE IN DRIVER WITH HOLES IN BASE.FELT STINGING IN GROIN AREA.

EMPLOYEE WAS MOVING RAIL CAR TO LOWER END OF STORAGE TANK WHEN HE STEPPED DOWN FROM CAR TO GROUND LEVEL, HE FELT A SRAP IN HIS LOWER BACK.

EMPLOYEE WAS REMOVING HINGE PINS TO REMOVE DOOR TO CHANGE IT FROM HINGED DOOR TO A REMOVEABLE DOOR.

EMPL WAS WALKING DOWN STPS TO MIXER FLOOR HE FELT MUSCLE TIGHTN IN CALF OF LEFT LEG.

EMPLOYEE WAS WALKING BY A ROLL SLIDE PLATE, WHICH WAS PROPED AGAINST A ROLLGUARD. AS HE WALKED BY, THE SIDE PLATE FELL STRICKING THE TOP OF HIS RIGHT FOOT.

SERVICING EQUIP REPLACING GREASE GUN IN STORAGE KNEELED DOWN TO PLACE GUN DOWN DOOR CLOSED R FIGNER SEVERING TIP

CLIMBING OUT OF THE EDGE OF THE WATER SUMP AND FELT HIS MUSCLE PULL IN HIS LEFT KNEE. FIVE MINUTES LATER, WHEN HE WAS CLIMBING ONTO THE DROPBALL HIS KNEE CAME OUT AND BACK INTO JOINT.

EE WAS ATTEMPTING TO EXIT FROM THE DENVER SEAT OF A 5 TON DUMP TRUCK AS HE TURNED FROM THE DRIVING POSITION AND ATTEMPTED TO STEP DOWN HE FELT SHARP PAIN IN HIS LOWER BACK.

EMPL WAS WALKING ON CATWALK OF DROP BALL IN QUARRY EMPL WAS TRYING TO REMOVE ICE BY HITTING CATWALK WITH HAMMER.

EMPL HAD EXPERIENCED PAIN ON & OFF SINCE JULY OF 1988. HE WAS LAID OFF ON 12/12/88 DUE TO MAN POWR EDUCTION. UPON RECAL ON 2/21/89 AFTER WORKING 4-6 HOURS EMPL COMPLAINED OF PAIN IN BOTHER WRI STS. EMPL WAS PLACING 80#@ BAGS OF TCM ON PALLETS TO BE SHRINK WRAPPED.

THE INJURED WAS REPLACING THE DROP BALL.THIS REQUIRED PARTS UP TO 40LBS LYING ON BACK TO WORK AND USING A 20LB HAMMER,AT THE END OF THE DAY HE HAD PAIN IN HIS LOWER LEFT SIDE.HE VISITED CO DOC TOR AND WAS TOLD HE HAD A HERNIA.

EMPL WAS IN PROCESS OF MOVING 2"X8"X10' LUMBER, HE HAD MOVED SEVERAL BOARDS. EMPL FELT TWINGE IN HIS RIGHT LOWER BACK.

EMPL CLIMBING LADDER TO PATCH HOLE IN SCRUBBER -HIT LEFT KNEE ON LADDER RUNG.

THE INJURED WAS OFF ON FOUR WEEKS MEDICAL TWICE.RETURNED TO WORK 03-21-88 WITH LIFTING INSTRUCTIONS OF 45 LBS MAX.OPERATED TCM BAGGING MACHINE(REQUIRED NO LIFTING) ON 03-22-88 DEVELOPED SWELLI NG PAIN IN WRISTS.HE BEGAN TO MISS TIME ON WEDNESDAY 03-23-88.

EMPLOYEE STATED THE FOLLOWING:ON 02-09-88 OR 02-10-88 HE WAS PLOWING SNOW AT THE REAR OF COLORIN PLANT,HE EXITED THE LOADER AND SLIPPED AND FELL AS HE STEPPED FROM LADDER.HE GOT UP AND WENT TO WORK AGAIN.HE DID NOT REPORT THIS INCIDENT UNTIL05-28-88.

EMPLOYEE WAS PU5LING A SOCKET WRENCH, THE SOCKET BROKE AND THE EMPLOYEE FELL BACKWARDS, HITTING HIS HEAD ON THE CROSS TIES HE WAS STANDING ON.

EMPL LIFTING HI3GED FLOOR PLATES ON FLOOR ABOVE #3 CRUSHER.AFTER PLATES WERE RAISED AND SECURED,I.NAME WAS PASSING BETWEEN FLOOR PLATES AND HA NDRAILS TO ASCEND DOWN STEPS.THE HANDRAIL SLIPPED FROM SUPPORT B9ACKET AS RESULT,I.NAME FELL OUT OF OPENING 30' TO GROUND BELOW,LANDING ON FEET,KNEES & HANDS.AT IMPACT HEAD HIT SMALL STONE CAUSING PUNCTURE WOUND ON FOREHEAD.
