Employee was performing heavy equipment maintenance, installing a roller on a bulldozer; the roller slipped and smashed his small finger on his left hand.

Employee was assisting another employee cutting metal; while handling a piece that was already cut, it shifted and when he pulled his hand away quickly he swiped his middle finger against the cut edge of the metal, causing a laceration to the underside of his right middle finger.

Employee was crossing level ground and was shuffling his feet, stubbed his toe and lost his balance causing him to fall into the adjacent conex. Employee struck his head against the outside wall. Employee sustained a laceration to his scalp. Laceration required 3 stitches with no other physical damage or impairment.

Employee was cleaning out a material chute below the 8 X 20 screen with a bar. A rock came down from the top of the screen and hit the bar, causing it to strike the employee in the face. Employee declined medical treatment until 5/8/14.

Employee was cleaning a chute in front of an 8' X 20' screen using a bar. When the material in the chute came free, the employee dropped the bar into the chute. He stated at that time that he felt a pain in his right shoulder and requested a medical evaluation.

Employee was being trained on the water truck, the trainer released him to drive on his own. He was descending a pit ramp when his brakes got hot. The truck gained speed and went across the pit floor through some rough terrain & stopped on a bench. He indicated neck pain so was taken to be medically examined by a physician. First aid only, he was given restricted duty for 1 week.

Employee was tightening bolts on 8 X 20 screen at the Crusher when a very small flake of rust from the screen filtered down behind his safety glasses into his right eye.

EE was assisting other ee in offloading 50# sacks of Anfo from a delivery truck. One employee in the line dropped a bag and this employee picked it up. He felt a pain in his lower back but finished the job. About 15 minutes later the pain increased and he was taken to town to seek medical attention. off work until recheck on 10/27

Employee was adding oil to gland water pump while kneeling down. When he pulled himself upright with the use of the handrail, he felt a pain in his lower back. He finished his graveyard shift and sought doctor's treatment later that day, which resulted in time off.

Employee was climbing equipment ladder on front end loader when he felt a pain in his abdomen.

Employee was performing maintenance on a classifier at the crushing circuit. He was turning the flywheel to reposition for the next shoe replacement when he pinched his finger between the belt and the pulley.

Employee was using a hammer to clean a spill tray at the #17 belt hopper. While striking the tray with the hammer, it slipped off and he hit his left second finger on the tray, causing a laceration. Employee was not wearing gloves.

Employee was on a maintenance assignment and was traveling on a walkway between the #55 belt and the #7 belt at the crusher, when he slipped on a patch of ice. His hard hat flew off as he came down and he struck the back of his head on the ground.

Employee was cutting a piece of pipe with a cutting torch when he lost his footing - when he grabbed for the pipe he passed the torch over the back of his left hand burning his fingers. He was wearing gloves, but not his company issued PPE pair of welding gloves.

Employee was checking crushing circuit screen area to determine source of water leak. As he was ascending ladder to access screen area, he was suddenly struck on the top of his right shoulder by a <3" rock that had fallen from the #8 conveyor belt above him.

EE was changing a tire on the #23 crew bus. When finished, he felt some discomfort in his groin area. He reported incident but initially declined medical treatment. He sought medical treatment for same on Fri, Sept 7, 2007 & was released to RTW full-duty no restrictions, by physician to have surgically repaired.

Employee was exiting crew vehicle at the parking area of the minesite (upon arrival for his shift) when he slipped and fell on icy/snowy ground onto his left side.

Employee was operating Volvo L180 loader. He was preparing to dump the 68th load into 17 belt hopper. Just before he tilted the bucket to dump into the hopper the shaft nut assembly on the dump cylinder came apart and the bucket rolled back. The material came through the front windshield into the cab. The force pushed the steering wheel down on top of both legs above knee.

EE was loading a barrel onto the oil dock with the assistance of a second employee. As they pushed and spun the barrel into place, he felt a pop in his ankle. EE did not seek medical treat- ment until 8/17/06. arter condition worsened.

Employee was unplugging the feeder chute at the crusher while standing on the 8x20 screen. The screen was wet, ee slipped, landing on his knee & bumping shoulder. When he stood up, he hit his head (was wearing his hard hat) on the end of the chute. Reported incident same day, but did not seek medical attention until 6/15/06.

EE was assisting in material handling in prep. for a well maint. job he lifted one end of an 8' section of 10' pvc piping so another EE could position a plank under it when he felt something in his back pop. Witnesses reported he was use- ing proper lifting techniques when the accident happened. EE did not seek medical treatment until 3/23/06.

EMPLOYEE WAS PERFORMING MAINTENANCE ON CRUSHER SCREEN WITH THE ASSISTANCE OF A CRANE & OPERATOR. EE WAS POSITIONING A PLATE ON THE SCREEN WHEN THE OPERATOR BEGAN LOWERING THE CRANE BOOM ON SIGNAL. THE BOOM MOVED FASTER THAN THE EE ANTICIPATED & HE STRAINED HIS BACK IN ALTERING HIS POSITION. EE FINISHED HIS SHIFT & SOUGHT DR.'S CARE ON SATURDAY, 1/14/06.

EMPLOYEE WAS USING A WATER HOSE NEAR THE CRUSHER SCREEN. WHEN HE REPLACED IT TO IT'S STORAGE HANGER AND TURNED AROUND, HE TRIPPED OVER IT AND FELL TO HIS RIGHT KNEE. HE DECLINED MEDICAL TREATMENT UNTIL THE FOLLOWING DAY.

Employee was preparing to climb up on his assigned loader when his foot slipped off of the bottom rung of the ladder and hit the ground, turning his ankle the wrong direction.

EMPLOYEE WAS ASSISTING WITH CRUSHER MAINTENANCE WHEN THE WIND PICKED UP AND BLEW SOMETHING INTO HIS LEFT EYE. THE EMPLOYEE WAS WEARING HIS SAFETY GLASSES AT THE TIME.

EE WAS STATIONED AT THE CRUSHING CIRCUIT CONTROL ROOM. HE BEGAN TO EXIT THIS AREA BY DESCENDING THE STAIRS WHEN HE LOST HIS BALANCE AND TUMBLED DOWN THE STAIRWAY, LACERATING HIS RIGHT HAND AND TWISTING OR STRIKING HIS LEFT WRIST.

TWO EMPLOYEES WERE CHANGING OUT WEAR EDGES ON A BULLDOZER. INJURED EE WAS HOLDING A BOLT IN A SOCKET WHILE THE OTHER EE WAS TIGHTENING IT WITH AN IMPACT WRENCH. THE BOLT CAME OUT OF THE SOCKET AND SPUN AGAINST THE FIRST EE'S HAND, CAUSING A LACERATION.

EE WAS USING A BAR TO CLEAN A CHUTE AT THE CRUSHER TO DISLODGE PLUGGED MATERIAL. THE BVAR BOUNCED OFF A ROCK AND BEGAN TO FALL DOWN THE CHUTE-THE EE CAUGHT THE BAR WITH HIS LEFT HAND AND FELT HIS SHOULDER PULL AS HE DID SO. EE WAIVED MEDICAL TREATMENT UNTIL THE FOLLOWING MORNING ON 9-4-02

EE WAS USING A COMALONG TO REPOSITION A RADIAL STACKER (CONVEYOR). HE SECURED BOTH ENDS OF THE COMEALONG TO WELDED EYES ON THE STACKER AND THE TRACK FRAM IT RUNS ALONG. WHEN HE TIGHTENED THE C OMEALONG, THE EYE ON THE TRAKC BROKE, FREEING THE END OF THE COMALONG AND CAUSING IT TO SPRING UP AND IT STRUCK THE EE IN THE BACK. EE DECLINED MEDICAL TREATMENT UNTIL 8-2-02 WHEN HE SOUGHT S

EE WAS OPERATING A BULLDOZER STRIPPING IN THE PIT. HE BACKED THE DOZER INTO AN UNSTABLE AREA AND THE RIGHT DOZER TRACK SLID DOWN A SHORT FREEWALL AND TURNED OVER ON IT'S RIGHT SIDE. HE WAS WEA RING HIS SEATBELT AT THE TIME. HE EXITED THE EQUIPMENT AND REPORTED THE ACCIDENT TO HIS SUPERVISOR. HE DID NOT SEEK MEDICAL TREATMENT UNTIL 4/29 FOR POSSIBLE SHOULDER STRAIN.

THE EE WAS PERFORMING MAINTENANCE ON THE HYDRAULIC SYSTEM OF THE WATER TRUCK. WHILE TIGHTENING A FITTING ON THE TANK, HIS WRENCH SLIPPED AND HE STRUCK HIS RIGHT HAND AND WRIST ON THE TANK FRAM E.

EE WAS SETTING UP A DRILL IN THE PIT WHEN HE TOOK A STEP BACKWARDS, STEPPING ON A ROCK, PARTIALLY LOSING HIS FOOTING AND STRAINING HIS LOWER BACK. EE SOUGHT MEDICAL TREATMENT AT THE END OF HIS SHIFT.

EMPLOYEE SLIPPED ON SNOWY PATH WHILE WALKING DOWN TO CHECK A STACKER BELT, STRIKING HIS RIGHT KNEE ON THE ROADWAY WHEN HE FELL. EMPLOYEE INITIALLY DECLINED MEDICAL TREATMENT UNTIL 3-7-01, AND SAW A DOCTOR ON 3-9-01. DOCTOR'S REPORT NOT RECEIVED UNTIL 3-13-01.

EE WAS USING A SHOVEL TO CLEAN UP AN AREA NEAR THE #8 CONVEYOR BELT CHUTE. WHILE SHOVELING ROCKHE BROUGHT HIS SHOVEL BACK WITH BOTH HANDS AND STRUCK HIS LEFT ELBOW ON THE CORNER OF THE SAFET Y LINE HOLDER. HE SOUGHT MEDICAL TREATMENT THAT EVENING AFTER HIS SHIFT WAS FINISHED.

WHILE EE WAS WORKING IN THE MAINTENANCE SHOP, HE WENT DOWN TO ONE KNEE, GRASPED A PIECE OF STEELAPPROX. 40 TO 50 POUNDS IN WEIGHT TO LIFT IT TO A SAW HORSE. IN DOING SO, HE FELT A PINCH IN HIS GROIN. EE WAIVED MEDICAL ATTENTION UNTIL THE END OF HIS SHIFT TWO DAYS LATER ON DECEMBER 29, 2000.

EE WAS STIKING FRAME OF CONVEYOR BELT WITH HAMMER TO DISLODGE ICE WHEN HE FELT A SHARP PAIN IN HIS LEFT ELBOW. PAIN PERSISTED, THEREFORE HE WAS TAKEN IN FOR EXAMINATION BY A PHYSICIAN.

EE WAS PERFORMING CLEANUP INSIDE THE 57" CONE CRUSHER AT THE CLOSE OF SHIFT. WHILE SCRAPING DOWN THE SIDES OF THE CONE, A CHUNK OF DISLODGE MATERIAL FELL AND STRUCK HIM ON THE RIGHT WRIST AREA . EE DID NOT SEEK MEDICAL ATTENTION UNTIL THE FOLLOWING DAY.
