Employee was stacking bags and felt a pinch in lower back.

Employee was walking out of the warehouse and slipped and heard a pop in the area of EE's right foot.

A rock was stuck on the C-2 belt going into the Jaw Crusher. The employee was using a pry bar to dislodge the rock. When the rock dislodged the employee's finger was smashed between the bag and the crusher frame.

Employee was cleaning a bucket elevator.

Employee was removing the belly pan from D9 dozer when the safety chain slipped dropping the pan on the employee's left wrist breaking the bone in left wrist

Was stacking 25lb bags. Turned to pick up bag and had pain in EE's back.

There was a bridge of rock in the grizzly and the employee was using a scaling bar to remove it. Felt a pop in EE's right arm.

Employee was operating an Air-Trac Drill. They went to move the hose out of the way.

Employee was removing a scaling bar that was trapped in the apron feeder. When ee removed the scaling bar, the ladder sprang back to the feeder and trapped ee's fingers behind the top rung and apron feeder wall.

Employee started bagging belt. The head pully was turning but the belt was not moving.

Employee was shoveling under the Z-5 belt, when he collapsed & became unconscious. He was found by his supervisor. The supervisor touched the employee's shoulder & the employee regained consciousness. The employee rested for a while, then requested to be taken home. The employee has a medical condition that makes him weak & tired. This was the cause of his collapse.

Employee was walking around a pallet. The shrink wrap on the pallet had come loose & the end of the wrap was on the floor. The employee did not see it & tripped over it. The employee's knee landed on the forks of the forklift. The employee continued to work with no problems until 12/9/10. He then requested to see a doctor.

Employee was working on an axel off of a backhoe. He had taken the axel off & set it on jacks. The employee checked the axel to make sure it was stable on the jacks. When the employee stood up the axel fell off of the jacks, landing on his hand.

Employee was walking through the shop & tripped over a power cord. When he fell, he turned & twisted & hurt his back. The employee did not seek medical attention, & did not lose any time off of work until 2/7/11.

EE was working on C1 conveyor belt monitoring for debris when a large rock rolled down - he tried to stop it with his hand. EE did not report to anyone that day nor the next. On Monday, he mentioned it to foreman and was immediately sent to make report at office. EE told supervisor that he had played in a basketball tournament over weekend & wrist didn't hurt at that time.

Employee was changing shaker screen. Walking on catwalk he stepped on a spot with a hole, fell through catwalk to mid thigh (left). He knew hole was in catwalk but had not reported to maintenance dept. Tore skin on finger when he grabbed guard rail and scraped left side on guard rail. Tore jeans on catwalk.

EE WAS ATTEMPTING TO ADJUST TAILGATE LATCH ON MACK DUMP TRUCK, HAND SLIPPED CATCHING FINGER IN TAILGATE.

EMPLOYEE WAS ATTEMPTING TO UNPLUG SILO. WHEN HE REACHED TO BRACE HIMSELF HE CUT FINGER ON FORKLIFT BOOM (REQUIRED 10 STITCHES).

EE WAS GRINDING A PIECE OF METAL WHEN A PIECE OF EIGHTER GRINDING OR METAL FLEW OFF AND WENT UNDER HIS SAFETY GLASSES.

EMPLOYEE HAD MOVED VEHICLE, PARKED AND WHILE GETTING OUT HIS KNEE BUCKLED.

EMPLOYEE WAS BAGGING ZEOLITE AND STACKING BAGS ON PALLETS STATED WHEN HE STOOD UP STRAIGHT AFTER PLACING BAG ON PALLET HIS LOWER BACK HURT AND LEGS FLET NUMB.

EE WAS PULLING WEEDS, BENT TO PULL WEED WHEN HE STOOD UP HIS LOWER BACK HURT. HE STOOD FOR A FEW MINUTES AND BACK FELT BETTER. WHEN HE WENT TO GET WHEEL BARROW BACK HURT AGAIN.

EE WAS GETTING OUT OF THE BACK OF A PICKUP, WHEN HE STEPPED DOWN HE STEPPED ONTO A ROCK. THE ROCK ROLLED, CAUSING EE TO TWIST HIS RIGHT LEG & ANKLE.

EMPLOYEE HAD BEEN SHOVELING ZEALITE- AT END OF SHIFT, WHILE WALKING TO DRY, EMPLOYEE STARTED HAVING MUSCLE SPASMS IN LOWER BACK.

EE WAS LOADING CRUSHER FINES-NOTICED BELT OVER FLOWING GOT OUT OF MACK TRUCK TO STOP BELT-WEHN STEPPING OUT OF TRUCK TWISTED LEG.
