EMC employee diagnosed with an RSI by an authorized physician. Surgery will be necessary and employee was taken off of work due to seriousness of condition. An investigation has already been completed. First DAFW is 8-26-15.

Mechanic was using impact gun to remove cutting edges from dozer. The nut got hung-up on the bolt and the bolt head became unseated from the recessed hole of cutting edge. The spinning head contacted the mechanic's gloved left hand causing a laceration requiring four sutures.

This was a medical emergency due to the employee collapsing without losing consciousness. The employee was dizzy and experiencing a headache. The employee collapsed when he attempted to get up from his chair during the meeting he was chairing. Help was provided and an ambulance was called.

Employee was on break and eating when injury occurred.

Employee suffered hearing loss during his work time underground, caused EE to have a reportable hearing shift and require hearing aids. EE was diagnosed by a audiologist on 5/20/15, no days away from work.

Employee was removing excess silt from a truck tailgate skirt when a gust of wind blew through the work area causing dust to enter employee's right eye through an opening around his safety glasses.

Employee was using a magnetic drill. Power failed causing the drill to fall and smash the right index finger. Drill was not secured to the beam.

EE was tasked with removing and replacing the corner edge of the 565 loader bucket; while removing the cutting edge, using the impact to remove the bolts, the mechanic's hand was cut.

EE was assisting mechanics lift Pantograph onto Trolley Motor and caught his left pinkie finger between trolley and base of pantograph. Medical treatment only, no days away from work.

Employee was walking from the back garage door on the decline slope and step on the edge of a 2" hole and rolled his ankle resulting in a sprain. No days missed, restricted work only.

An equipment operator slipped after putting his chock block on a loader. Employee dislocated his right shoulder. No days missed, Restricted work only.

Employee stopped work and reported to supervisor, employee was taken to clinic for evaluation and treatment. Employee was given stitches; RWC opened on 8-8-13.

When employee stepped off of D-ring on jackleg, he twisted his ankle. No days away from work, restricted work only.

Employee twisted right knee upon exiting DUX concrete truck. No Days away from work, RWC only for Knee Sprain.

Pushing a bolt into a hole when control of the rock bolt was lost and the bolt slid out of the hole and stuck EE on the left side of face causing a laceration.

Employee was assisting a miner installing split-set bolts. The miner turned on the feed pressure to the pneumatic stopper leg to drive in a bolt. The bolt driver and the split-set were out of alignment and the driver caught on the edge of the bolt. The helper bumped the driver with his hand and the bolt driver went into the bolt, pinching the palm of his left hand.

Employee stepped back to observe ground conditions while assisting another miner who was bolting the roof with a jackleg drill. He tripped on the one inch jackleg air supply hose and fell backwards resulting in the back of his head striking the rib behind him. He received 2 lacerations to his scalp (treated with 17 staples) from a ground support plate installed in the lower rib.

After unloading concrete on to the maxi crete, employee was lifting up chute on concrete truck, and chute fell catching employee's thumb. No days away. Medical treatment only.

As employee was leaving the haulage trolley shop, he stopped to close the fire doors, as he shut the doors the metal latch closed and smashed employee's left thumb.

Fall of ground from left rib while bolting and scaling. A slab tipped out from upper left rib and landed on right ankle of employee while he was scaling from under supported ground. Lower right tibia was fractured and ankle was dislocated.

Employee was stepping off buggy and twisted his knee, employee felt pain immediately. Further medical evaluation determined surgery was needed. Became recordable on day of surgery, 9-9-12 DAFW.

Descending into mine on the skip hoist, there was an emergency stop of the cage. EE experienced pain, discomfort and soreness. Employee was taken off of work effective 8-13-2012 DAFW.

Employee was driving a tracker underground and eye became irritated. All PPE was worn as required. No DAFW; no RWC.

The Electrician was in the process of pushing a fish tape (tool used to pull conductors) into a raceway where it became difficult to push through the raceway and used his needle nose pliers to assist him and while pushing the fish tape the needle nose pliers slipped of the fish tape and struck himself on the lip and chipped his tooth causing an open wound on the upper lip.

Employee tried to start a generator and generator caused him to pull his arm and injure his hand. 30 minutes later he reported pain in his shoulder.

Three employees were walking on a decked walkway when the injured employee stepped and the edge of a below deck access plate pivoted allowing the left leg to fall through to the hip level. Revised to DAFW when surgery was done on 6-11-13, no restricted duty, medical treatment or no days away prior to 6-11-13.

Miner was cutting split-set bolts using a reciprocating saw, wearing goggles - got metal particles around the eyebrows. Washing face at end of shift a metal particle transferred from eyebrows into left eye but he noticed no discomfort. The next morning he thought he had an eye infection, called in sick and sought treatment where metal particle was found.

Employee was walking down a short decline about 10% grade when he stepped on loose gravel and slipped landing on his left side on top of his self rescuer that was attached to his belt. RTW on restricted duty.

Employee was using a six ton bottle jack to lift concrete form in place, when employee tried jacking up the jack, it came apart at the base and sprayed out hydraulic oil into the employees face. No RWC; No DAFW; employee was released same day with antibiotic.

Employee was releasing water from hopper for slick line wash out by pulling on plunger, when he felt a pain in his abdomen area. No days away from work, five pre-scheduled days off then RTW light duty on 2/29/12.

Employee was reconnecting the fitting on a suction hose and wrench slipped. Employee jammed his right finger. No Days away from work; xray positive for splinter fracture.

The electrician was stripping conductor in preparation of pulling the conductor and making terminations at the control panel. The electrical moved backwards while stripping the conductor and bumped into another electrician. The knife jumped out of the grove it was in and into his hand. The blade penetrated both a leather glove and a Kevlar glove.

Employee was struck with a jackleg on upper back. Employee was seen at urgent care with follow up appts. done through urgent care. Employee was referred for further evaluation and it was determined that physical therapy would be required. Changed status from first aid to restricted work case on 2/13/12.

Employee was moving a jackleg from a draw point to another draw point while lifting the jackleg he felt a slight pain to his neck/shoulder area. Employee was evaluated by doctor on 12-20-11 (MRI negative) with a second evaluation on 12-29-11 (MRI) will be done.

The hoist had been chaired to load a flatcar to be brought to the surface. When the hoist man went to un-chair, the system would not respond. Subsequent troubleshooting found the service hoist hydraulic power pack control card failed. The card was replaced and the hoist returned to service at 10:10 a.m..

Service hoist cage was chaired at shaft collar, when hoist man attempted to move cage upward, DC breaker tripped and would not reset. Initial evaluation indicated hoist would be down more than 30 min. UG employees evacuated safely. SCR failure, components were 30+ years old. Common SCR's from an out of service unit were used to restore function.

Employee was holding the lowboy hitch while loci operator positioned the loci for hook-up. The loci jumped when the motion was engaged. The loci struck the end of the hitch which then popped upward striking the employee in the chin, no days away from work.

While employee was on Chippy hoist, an E-Stop occurred somewhere around mid point causing the employee's legs to buckle and falling to the bottom of the chippy hoist floor landing on his right side. Employee was taken off of work effective 8-3-11.

Employee was using a pry bar to remove concrete forms. The chisel end between the form and concrete slipped and employee's right hand struck the rib. The point end of the bar drove through his glove and hand between the thumb and index finger. Medical treatment. No days missed.

Employee was cutting bands off bundle of wire mesh, when one of the bands sprung up and struck employee in the face. Medical treatment necessary, no days missed, no restricted work.

While holding end of pipe, employee attempted to swing his end over into reject bin pinching his pinky finger between the handrail and pipe. No days missed, no restricted duty. RTW regular duty 7-1-11.

Employee was standing on a chair in the office trying to dust a computer.

After completing a sanding job on a piece of equipment, employee removed facial PPE, and felt irritation to his right eye. No days missed, no restricted work. Final diagnosis given during follow up evaluation on 5-24-11.

The employee was spot bolting when the drill steel was approximately three feet in the hole, and it struck another bolt causing the machine to rotate jerking the employee's right arm. The employee was able to shut off the machine in a timely manner, and continued working. After final evaluation and determination by physician, surgery was scheduled and became a DAFW on 1-26-12.

Employee was using "floor dry" material to absorb oil on floor-when he felt something in his eye. He was sweating and wiped his (L) eye w/ paper towel and his eye became irritated. Employee was diagnoses with scratched cornea to left eye. No lost time, no restricted work, medical treatment only.

While employee was drilling, the jackleg sunk into the mud getting stuck. While responding to this situation, employee hurt his lower back. Employee did not experience pain immediately. No days missed, restricted work only.

During the drilling for ground support, while holding steel with left hand and the jackleg in an upright position with his right hand, employee pushed drill steel up, and immediately felt pain on his left shoulder. Employee did not miss work and incident was not reportable injury until surgery, employee's first day away was 4-11-11.

Employee was punching out a brass bushing on a drill machine when he missed the punch and smashed his left index finger between the hammer and the punch causing a blood blister and swelling to the finger.

Employee was standing on a 14" step stool to weld on a loader fender. He lost his balance. While attempting to regain balance, his right hand knocked off his welding hood and the tip of the welding rod made contact with his right eye. No lost time.

Employees were tasked to bring an electric rectifier from the surface to the haulage. During transport, the rectifier became wedged between the ribs of the drift. A 6" x 8" x 24" timber between the battery loci and rectifier was a brace for the loci to push against. When the loci contacted the timber, the timber twisted and the movement caught the employee's pinky finger.

Employee reported that he strained his neck and felt it several times before but more so on 7-2-10 after unloading rebar. He reports the strain is from 2008 injury. Employee did not report until 7-7-10; took vacation, then sought his own medical care, did not report to employer then. He was evaluated by doctor & placed on light duty. This injury may be linked to 2008.

Employee was cleaning up material spillage around a rock breaker compartment. Employee picked up a rock and tossed it into the open raise. Blowing dust and rock particles came back towards employee because the chute had just been pulled on the haulage level. The left eye was irritated from the particles and rubbing of the eye; antibiotic eye drops were prescribed.

Towards the end of the shift, Rock breaker operator was checking the windows for hang-ups in order to determine and communicate to the next shift which windows required blasting. As employee was checking window 13 West, the window started spitting rock and employee was struck in his lower right leg by a rock.

Employee was operating Loci when he jolted his back in the seat as the loci & train transitioned from rail to pocket rollers. He RTW & continued assigned duties, no day missed. No new event, inj. or work cond. aggravated injury. Became DAFW when doctor took him off work due to employee complaints of cont. pain. Appt to consult for surgery - June, 2010. First day missed 5-12-10.

Employee was in the process of troubleshooting Trolley Loci #682 in the Haulage Level Panel 14. The employee was boarding the rear electrical compartment when he felt a pulling sensation in his lower mid abdominal section. Released with no restrictions.

Two employees were leveling the rakes on #5 Thickener. They had a wrench on one of the adjusting bolts for the rake. Employee 1 was pulling and employee 2 was pushing on the wrench. Employee 1 felt a sharp pain to the lower back that traveled to the lower abdominal area. * Started losing time on 8/25/09

Haulage train, powered by Trolley #685, loading ore cars on the haulage level was struck from behind by a second train entering the same loading panel. No damage or injuries were reported at the time. Several weeks later, employee in Trolley #685 complained of back pain associated with the incident.

Employee was refueling a truck mounted compressor engine with gasoline from a five gallon approved safety can. The fuel splashed onto the hot exhaust manifold and was ignited. The resulting flash/flame burned the employee on both arms, chest, and face. The employee was transported to the local hospital and then was transferred to the burn unit at the Univesity of New Mexico.

Employee was barring down loose ground when a chunk of concrete, approximately 40 pounds, fell on the employee's ankles, bruising both.

Employee was mucking in a line with a LHD with a cab. As the employee moved forward a rock came around the front of the bucket. The LHD wheel ran over the rock and jarred the employee resulting in a back injury.

Electrician stepped on a rock while hanging electrical cable on the back of the south boundary drift in order to supply power to the new mainenance shop. Employee did not believe the injury to be too severe and continued to complete the task. The ankle became more swollen overnight and the pain persisted, causing the employee to request medical attention the following day.

Employee was installing a rock bolt with a jackleg. While lifting up on jackleg in order to reposition leg back a few inches, the exertion caused his knee cap to pop out of place.

Employee slipped off loader while conducting pre-operational inspection. He had climbed onto loader for access to check fuel level and radiator. Employee slipped and fell to ground, landing on his right side and injuring his right wrist.

Employee was walking from the batch plant office to control room when he slipped on packed snow. The snow had been packed by the skidsteer the employee had operated to clear snow in this area. This resulted in a sprained let knee. *STARTED LOSING TIME ON 11/15/08*

Employee was entering the mine on the chippy hoist when it suddenly came to an emergency stop. Employee was jarred inside the cage and experienced pain in his neck, back, and shoulder.

Employees were working on the haulage level pulling muck. Injured was in the process of placing a 4 x 4 rock remover in the chute to remove rock that wedged in the chute. Co-worker was operating the chute door upward slowly & paying attention to the remover & chute door while injured was guiding remover into position to push rock when his finger rock or the chute.

Employee slipped and fell while drilling with jackleg. The jackleg fell towards the rib and pinched his finger against the rib. As result of incident, employee's last digit of pinky finger on left hand was amputated.

EE was using a jackleg drill on a xlift installing ground support. 2nd ee entered the work area, failing to replace the safety chain on the deck. EE stepped back lost his balance and jumped back into the articulation area of the xlift with his hand on the engine cover for balance, thumb was struck by the jackleg. Sutures and Tip of thumb fractured.

Employee attempted to unplug a cement grouting hose, bled off pressure and loosened nozzle but remainng pressure caused cement to splash into his face and eyes. RWC.

Employee was doing clean up at the 1045 conveyor belt next to and around the tail pulley area. She was using a wheel barrow to transport muck to an area where it could be dumped. When she was dumping the wheel barrow it twisted to the left and the handle of the wheel-barrow struck her on the left knee.

EE was walking down the #2 Gallery from the #3 transfer to set an e-stop that was down. He came across two lights that were out, he reached to grab a hold on the roller frame and stepped on some gravel and slipped. His foot got caught on the beam on wall side and he twisted his knee.

EE was shoveling wet muck at #3 transfer house on the third level when he felt a pop in his lower back on his right side and fell to the floor. He got up and tried to walk it off. He was helped down the stairs to the truck. Assesment was made and decided to immobilize on a back board and transport to Holy Cross Hospital. Supervisor called the ambulance.

Employee was attempting to stuff and wedge an intermediate bolt in an overhead location. While holding/pushing the bolt, he felt a sharp pain in lower back. *Did not start losing time until 1/30/09 due to surgery.

Employee in an attempt to locate hole under conveyor, ran his hand under conveyor pan. Employee's glove and right middle finger were pulled toward the screw conveyor and golve was pulled off his hand. Resulting injury to finger nail/nailbed required 3 sutures.

Employee was walking in snowpacked area alongside the tailings pipeline. The snow pack gave way underneath his right foot and employee twisted his left knee. Initially treated as first aid and released to return to work. MRI on 3/31/08 indicated a small tear to MCL tendon with doctor recommending restricted duty and physical therapy.

An employee and his coworker were loading 6"x8"x10' timbers. The employees were in the process of moving a timber a distance of 2 feet horizontally and 1 foot vertically onto the loader forks when the injured employee's foot slipped in the snow causing a twisting motion. He felt pain in his right groin and later that day diagnosed with a hernia.

The employee stopped at an intersection and did not have a good visual because of the loaded bucket. He placed his hand on the door to raise himself up to get a better look. As he did this either his elbow or his leg contacted the control causing the machine to pivot pinching his right ring finger. The finger was also fractured. The laceration took 4 sutures to close.

The employee was attempting to cut holes in his glove cuff to facilitate hanging them on a belt hook using his personal jack knife (leatherman tool). The leatherman did not have a locking blade device and the knife closed on his finger causing a laceration which took four stitches to close.

Miners were rehab bolting in P14/ vent crosscut. They were cleaning up to do additional bolting when they noticed the ground starting to move. They backed out and "flagged off" the entry. Shortly after, the roof caved in above the anchor zone. Ventilation was not interrupted. MSHA was on sight and issued no citations.

The employee was riding in a mantrip as they exited the mine at shift end. At an intersection, the mantrip hit a rib with the right front end causing employees head to snap back and forth hurting his neck. He suffered a Whiplash that was too sore to return to work.

The employee was spraying shot-crete near a ventilation raise. Some of the spray mist was blown behind the safety glasses he was wearing. His eyes became irritated. He was diagnosed with Chemical Conjuctivitis. He was given a prescription to prevent infection. He was not wearing proper PPE.

While walking through the Mill Warehouse Yard, employee stepped on some metal screens which slipped out from under him causing him to fall backward. When he threw back his arms to break the fall, a sharp corner on one of the screens made a 2 to 3 inch cut on his left wrist. He received 13 sutures at Taos Holy Cross.

EE was helping to lower a 75 hp. fan from an elevated platform, approx. 7ft. The fan slid off of the cables and fell to the platform. When the fan fell, it hit him and knocked him off the platform onto the conveyor belt, hurting his back and left thigh. Fall protection was available but not used.

As employee was descending out of the cab of an excavator onto the boarding steps, employee lost his footing and slipped off of the bottom rung. Employee held onto the boarding handle with his right hand to prevent his fall but injured his shoulder in the process. Pain persisted in shoulder, ultimately required surgical remedy.

The employee stated that a small particle entered his right eye when he took off his glasses to clean them. He believes the particle came off of his glove. The particle was removed by a doctor and he was given an antibiotic to prevent infection. Upon return, the employee continued his normal work.

Employee was performing ring drilling. When inserting the drill steel and bit, steel fell from the centralizers. Employee attempted to control the falling steel. The falling steel pinched the finger between the steel and frame resulting in laceration of the right index finger requiring sutures. Treating physician did not prescribe any time away from work for injury.

The employee was tightening a 1in. ball valve with an 8 in. pipe wrench. The wrench slipped with extreme force. When this happened, he felt a sharp pain in the shoulder area. The shoulder was still sore through November. The employee had surgury on Dec. 3 for a torn ligament.

Employee was installing a self tapping screw with a hand held drill. The drill was equipped with a threaded hole for a side handle which was not installed. When the screw "grabbed", the torque twisted his arm and shoulder. He continued normal maintenance activity until the day he went for shoulder surgery.

EE had finished unloading pipe and was rolling the sections out of the roadway, approx. 6ft. While rolling a 15ft. section of pipe, he felt a sharp burning in his lower right pelvis. He was evaluated at Taos Holy Cross where it was determined he had a hernia. EE resumed his normal duties until Friday, May 18, when he had surgury on the hernia.

At 1300 incline station, employee was changing rollers from LHD bucket. LHD operator prepared to dismount and install chocks. LHD rolled several feet striking a Kubota which struck the employee. Taken to doctor initially, doctor found only bruising. After employees normal 5 days off, went to another doctor who gave restricted duty. Restricted duty started 4/20/07

Employee was performing housekeeping when he attempted to move a portable metal gate by hand. He felt a pinching sensation in his groin area which was later diagnosed as a hernia. The employee performed work with a 20lb. lifting restriction until 6/5/07, the date of the procedure to repair the hernia.

Erica was working the grizzly lines. She was on the west side of window #10, line 9 attempting to move a rock with a shovel. She was using a "prying" motion when a falling rock caused the shovel to twist, straining her right wrist.

Substandard Equipment-Cabinet was supplied with a sharp edge. Employee reached into a box six inches from the sharp edge and contacted the edge with his finger. An avulsion resulted requiring 5 stitches to close.

Inserting drill steel into jackleg, pinched finger bewteen chuck and drill steel.

Employee was bolting a heading with split sets. The split set driver momentarily stuck inside the set allowing the air to bleed off causing the Jack Leg to drop quickly. He tried to catch the falling Jack Leg. He felt a "popping" sensation in his left shoulder. Employee had surgery on his shoulder on 1/19/07.

Employee using a electric drill to bore a hole in an 6" x 8" timber. Bit bound drill spun and struck back of right hand. Fracture finger.

Contractor employee knocked a lens cover off of a sodium vapor light when removing tools from a ball mill after completing his work. Lens struck his chin causing a small laceration which received sutures to close.

The electrical power to northern Taos County was lost during a storm. Our main hoist suffered damage to a rectifier that became evident about an hour later when 51 miners were coming to the surface at the end of their shift. Electricians troubleshot the hoist to find the problem and replaced the part. Hoist returned to normal ops at 5:40 pm and all miners came to the surface.

Employee shoveling spillage from a belt. Felt sharp pain in groin. Saw doctor on 9/22/06 and was diagnosed with a slight hernia. Put on restricted duty for two weeks.

Contractor employee was walking on go-between in line 6, window 8 when he stepped on a rock and twisted his right ankle. Later determined to have hair line fracture in right ankle.

Employee using 16# doublejack to break a rock. His wrist was twisted when he struck rock & sledge bounced off.

While chucking up a six foot drill steel, he slipped. The machine rotated on its leg and pinched his right ring finger, causing laceration.

Left thumb & index finger pinched between 2 rocks. No fracture. Soft tissue injury. Restricted work case.

EE developed chest pains and was transported to a medical facility. He later had surgery due to blocked arteries in the heart. This was ruled not job related.

Stepped off of boss buggy onto a small rounded rock and twisted his left ankle. Resulted in restricted work case.

Contractor employee prying rock with bar. Rock moved, pinched finger between bar and concrete. Fractured right ring finger.

Drilling a back hole, turned the machine off, pulled it out of the hole and his shoulder popped.

While rock bolting a rock rolled down the drill steel hitting employee's left wrist cutting him.

Contractor employee was poking a material in a window to start it moving. When material broke loose, it struck his pry bar causing the bar to strike him in the face. Employee sustained a fractured jaw.

EE was drilling in face when a rock fell and struck right knee causing strain/sprain.

Employee was striking rock with double jack when a piece of the rock flew off and cut his right index finger.

Contractor employee's right foot struck by rock on 1/4/06. Employee continued to work. Incident became restricted or light duty work on 1/19/2006 due to foot hurting and slight contusion. Was not restricted until 1/19/06.

Employee was using a prybar to loosen muck in a chute. Left Thumb pinched between prybar and chute gate rail when rock started to run in the chute.

The # 1 Hoist was down due to a power failure at the Tri-State # 69 substation caused by a bad jumper. The power failure effected all of the Molycorp facility. All of the underground personnel were evacuated from the underground mine. Power was re-established at 9:11 AM. The hoist systems were checked and found OK.

Employee was washing out a pump with a 1" water hose, stepped backward and slipped on the hose. Employee strained lower back muscle. Employee missed 8 work days.

PULLED A DRILL STEEL OUT OF HOLE AND PINCHED RIGHT INDEX FINGER BETWEEN STEEL AND DRILL CHUCK. FRACTURED TIP OF FINGER.

Employee was struck by a pry bar while in the process of cleaning off a grizzly. The employee recieved 10 sutures from the pry bar hitting his safety glasses, which hit his face and eye brow.

CUTTING CHAIN LINK FENCE WITH 24" BOLT CUTTER. WHEN THE WIRE GAVE WAY, THE BOLT CUTTERS STRUCK EE BETWEEN EYES AND CUT THE BRIDGE ON NOSE REQUIRING 6 SUTURES TO CLOSE

MOVING BOXES THAT CAN WEIGH UP TO 50 LBS. HAD MOVED 15-20 BOXES THROUGHOUT THE DAY. FELT PAIN IN LOWER BACK AND RIGHT SIDE OF NECK, SHOULDER. BOXES CONTAINED CORE SAMPLES

Hoist power problem due to power company signal system to the hoist sub station. The Westinghouse 4160 switchgear dropped out when it recieved the signal, the unit worked properly. The hoist was out of service for more than 30 minutes.

Driller's helper (Progressive Diamond Drilling) was removing core from a core barrel when he was struck by the core and sustained an avulsion to his right middle finger pad. Medical exam shows no fracture. The driller received 4 sutures.

BREAKING ROCK WITH 16 POUND DOUBLEJACK. SWUNG AT A ROCK AND FELT PAIN IN GROIN.

WHILE USING A PRYBAR, A ROCK FELL FROM A LOADING CHUTE AND STRUCK EE ON LEFT RING FINGER. EE SUFFERED A FRACTURED FINGER.

EE was working on a welder to adjust idle speed. As he worked the throttle linkage, his glove got caught in the fan belt and pulley. Ee left index finger was struck by fan place and cut severly. Dr amputated end of finger.

EE CLIMBED ONTO A CONVEYOR BELT & FELL BACKWARD 30" TO DECKING. SUSTAINED STRAIN/SPRAIN TO LOWER BACK ON RT. SIDE. EE LANDED ON HIS RADIO WHICH BRUISED HIS RT.SIDE.

EMPLOYEE WAS USING A BAR TO PRY A ROCK WHEN ANOTHER ROCK STRUCK THE BAR CAUSING THE BAR TO PINCH EMPLOYEE FINGERS BETWEEN BAR AND STEEL BEAM.

EE SWINGING 16 POUND DOUBLEJACK. FINGER PINCHED BETWEEN HANDLE OF DOUBLEJACK AND OBJECT HE WAS STRIKING AT.

EE CLAIMS TO HAVE HURT HIS BACK WHEN HE RAISED HIS ARM OVERHEAD. INCIDENT WAS TREATED AS PERSONAL ILLNESS. ON 5/29/03, A DR. SAID THE EE AGGRAVATED A PRE-EXISTING CONDITION WHEN HE RAISED HIS ARM & THEN SOMETIME IN APRIL EE SNEEZED & AGAIN AGGRAVATED THE PRE-EXISTING CONDITION.

EMPLOYEE WAS UNLOADING A ROLL OF 3" POLY PIPE FROM A FLATBED TRUCK WHEN THE ROLL SLIPPED AND KNOCKED EMPLOYEE FROM TRUCK BED, RESULTING IN FRACTURED LEFT ARM.

USING A PRY BAR TO MOVE A ROCK LODGED ON GRIZZLY RAILS, THE BAR SLIPPED & THE RIGHT HAND LITTLE FINGER WAS PINCHED BETWEEN THE BAR & THE RIB OF THE DRAW WINDOW.

EMPLOYEE WAS BREAKING ROCK WITH A DOUBLE JACK AND A PIECE OF FLYROCK STRUCK HIS UPPER LIP WHICH REQUIRED SUTURES TO CLOSE.

EE ATTEMPTED TO LIFT A FOAM FILLED TIRE, HE LATER REPORTED PAIN IN THE GROIN AREA. EE WAS TREATED FOR INFECTION WHICH WAS LATER DETERMINED TO BE A HERNIA. EE UNDERWENT SURGERY ON 08/05/02.

TWO EMPLOYEES WERE MOVING A 13 HP PUMP BY HAND. ONE STEPPED IN A MUD PUDDLE & TWISTED HIS ANKLE. IT WAS DETERMINED HE HAD SUSTAINED A FRACTURE TO HIS RIGHT TIBIA.

WHILE USING A PRYBAR TO MOVE ROCK, ANOTHER ROCK STRUCK BAR CAUSING IT TO STRIKE EE ON RIGHT CHEEK EE DID NOT SEEK MEDICAL ATTENTION UNTIL 4-4-02 WHEN IT WAS DISCOVERED HE HAVE A FRACTURED JAW.

WHILE USING A PRYBAR TO MOVE A ROCK, ANOTHER ROCK STRUCK BAR CAUSING IT TO STRIKE EE ON RIGHT CHEEK & NOSE. EE SUSTAINED CONTUSION & SMALL LACERATION. ON 3/28/02 A CT SCAN REVEALED FRACTURE TO RIGHT SINUS AREA.

WHILE BREAKING A ROCK AT GRIZZLY IN LINE 9, EE WAS USING 20# DOUBLE JACK, A PIECE OF FLY ROCK STRUCK EE ON CHIN. EE TREATED AT HOSPITAL, 5 SUTURES.

WHILE BARRING ROCKS AT GRIZZLY, RAISE STARTED RUNNING. EE ATTEMPTED TO STEP BACKWARD, SLIPPED & FELL INTO GRIZZLY. AS HE FELL, HE TRIED TO CATCHHIMSELF BY GRABBING CHAIN OF CONTROL BOARD. FING ER WAS JAMMED BETWEEN CHAIN AND CONTROL BOARD.

WHILE ATTEMPTING TO BREAK A JOINT ON A 9 7/8 CASING WITH A 60" WRENCH, WRENCH SLIPPED PINCHING LEFT MIDDLE FINGER BETWEEN WRENCH AND DRILL MAST. RESULT WAS FRACTURED FINGER.

WHILE BREAKING ROCK WITH A 20LB. SLEDGE HAMMER A FRAGMENT OF ROCK STRUCK R. FOREARM CAUSING A 1" LACERATION WHICH REQUIRED SUTURES.

WHILE CONDUCTING HOUSEKEEPING IN LINE #7, EMPLOYEE GOT A WOOD SPLINTER EMBEDDED IN THE PALM OF HIS LEFT HAND. THE SPLINTER PENETRATED HIS LEATHER GLOVE.

BREAKING ROCK WITH SLEDGE HAMMER. A PIECE OF ROCK FLEW UP STRIKING SAFETY GLASSES. FRAME OF SAFETY GLASSES CUT EE JUST ABOVE RIGHT EYE.

EE STATES THAT WHILE ATTEMPTING TO TIE A SECTION OF M8" AIR LINE TO THE RIB, THE PIPE SOUPLING CAME OFF, ALLOWING THE PIPE TO STRIKE EE ON RIGHT SHOULDER. EE WAS SEEN BY DR., RETURNED TO WORK WITH NO LOST TIME.EE RETURNED TO DR ON 4-23 DUE TO STILL HAVING SOME PAIN. EE HAD MRI WHICH INDICATED A PARTIAL TEAR TO ROTATOR CUFF. SCHEDULED FOR SURGERY ON 5-23-01.

EE CLAIMS THAT WHILE ASSISTING IN ALIGNING CUTTING EDGE UNTO MOTOR GRADER BLADE, THE EDGE TIPPED UNTO ITS SIDE COMING DOWN ON PRYBAR HE WOULD USING TO ALIGN AND PINCHED HIS FINGER BETWEEN THE BAR AND THE FLOOR.

EE CLAIMS THAT WHILE PRYING ON A ROCK OUT OF GRIZZLY WINDOW, TIP OF PRYBAR BROKE CAUSING HIM TO FALL AGAINST THE CONCRETE WALL ON HIS LEFT SHOULDER. HE CONTINUED TO WORK UNTIL 3-8-01 WHEN HE R EQUESTED TO GO SEE A DOCTOR. DOCTOR REFERRED HIMTO GET AN MRI WHICH REVEALED ROTATOR CUFF DAMAGE.

WHILE ASSISTING IN LOWERING A SECTION OF 6" X 20FOOT AIR LINE, THE PIPE SLIPPED DOWN AND ROPE BEING USED TO LOWER THE PIPE SWUNG CAUSING EE'S HAND TO STRIKE LOCOMOTIVE RAIL.

EE WAS CROSSING (CLIMBING) ACROSS A CONCRETE FORM CARRIER. HE SLIPPED AND FELL, STRIKING HIS LEFT KNEE AGAINST THE RACHET JACK.

A GROUND FALL (APPROX 400 CUFT) OCCURED AT THE INTERSECITON OF LINE 15 AND THE EAST RELIAF X-CUT DRIFT IN THE 7222 UNDERCUT REVELY. THE ROCKS IN THIS AREA ARE CLAYRICH (I.E. ARGILLIZED), SOFT , WEAK, FRACTURED AND WETHER THEN OTHER AREA. LINE 15 AN 8.5'X8.5' DRIFT WAS INITIALLY SUPPORTED IWTH 4' SLLIT SET BOLTS 2" WIRE MESH AND BACK SPOTTED WITH 6' BOLTS.

EMPLOYEE CLAIMS THAT WHILE MOVING PIPE OFF OF A RACK THE PIPE STARTE ETO ROLL DOWN AND HE TRIED TO CATCH IT. THE 1 1/4" X 20' PIECE OF PIPE CAUGHT HIS LEFT THUMB AGAINST ANOTHER PIECE BELOW.

EE CLAIMS THAT WHILE DISMOUNTING BOSS BUGGY HE STEPPED ON UNEVEN GROUND AND FELT PAIN TO LOWER BAQCK. ALSO LATER WHILE RIDING THE BOSS BUGGY, ONE OF THE TIRES BOUNCED ON ROCK OR RUT ON ROAD AN D HE CLAIMS PAINGOT WORSE.

WHILE BOLTING WITH JACKLEG, THE LEG GOT STUCK INMUD AND AS HE WAS PULLING ON IT HE FELT PAIN IN THE LEFT LOWER BACK AREA.

WHILE ASCENDING THE MILL STAIRWAY, EMPLOYEE'S LEFT HEEL DID NOT CLEAR THE EDGE OF THE LAST STEP,CAUSING HIM TO TWIST HIS LEFT KNEE. EMPLOYEE DIDNOT REPORT OR SEEK TREATMENT UNTIL 9/21/00.

EMPLOYEE WAS OPERATING THE TROLLEY LOCOMOTIVE THROUGH PANEL 29 IN THE HAULAGE LEVEL. THE BOLTSHOLDING THE SEAT BROKE CAUSING HIM TO FALL AND STRIKE THE INSIDE OF THE CAB WITH HIS BACK. ON 9/21 STARTED COMPLAINING OF NECK PAIN.

EE CLAIMS THAT WHILE RINGDRILLING THE COUPLER ON DRILL ROD HIT BOTTOM OF CENTRALIZER AT AN ANGLE AND COUPLER STARTED TO CHIP OFF METAL WHILE DRILL WAS ROTATING AND METAL CHIPS HIT EE IN FACE, /FRAGMENT IN EACH CHECK.

ON 6-14-00 EE REPORTED HE WAS FEELING PAIN TO LEFT LEG BUT DID NOT KNOW WHEN IT HAD HAPPENED. HE ASSUMED IT HAD BEEN ON 6-16-00 WHILE PLACING A STEEL BEAM CAP IN PLACE. EE SENT TO DR. AND REFE RRED EE TO PHYSICAL THERAPY.

EE CLAIMS THAT WHILE LIFTING A TRASH BAG FROM A TRASH CONTAINER, FELT PAIN TO LEFT GROIN AREA. EE STATED TRASH BAG WAS PRIMARILY FILLED WITH PAPER PRODUCTS AND NOT VERY HEAVY. EE WAS INITIALLY DIAGNOSED WITH A MUSCLE STRAIN AND CONTINUED TO WORK. HERNIA OPERATION ON 3/28/00.

EE AND TWO OTHER CO-WORKERS WERE ATTEMPTING TO POSITION A SECTION OF RAISE LINER (5'X4'LONG) TO THE UPRIGHT POSITION WITH A 3.5 YD LHD AND CHAIN AS THE LOADER BUCKET LIFTED THE RAISE LINER, TH E LINER STARTED TO ROLL OFF BUCKET. INJURED EE IN ATTEMPLING TO STABLIZE IT, WAS PUSHED AND FELL WITH LINER STRIKING R.L.

EE CLAIMS THAT WHILE CO WORKER AND HIM WERE TRYING TO RERAIL AN ORE CAR, THAT HE SUFFERED LACERATION TO LEFT POINTER FINGER WHEN THE TRACK JACK HE WAS TRYING TO POSITION ON SIDE OF ORE CAR SLI PPED AND INJURED FINGER.

EE CLAIMS THAT AFTER RUNNING OUT OF ORE IN LINE 11-15N HE DISMOUNTED LOADER AND WALKED OVER TO THE WINDOW TO SEE HOW MUCH OF A HANGUP THERE WAS WHEN THE HANGUP CAME LOOSE AND HE WAS STRUCK BY A ROCK THAT ROLLED DOWN THE PILE ON LEFT KNEE.

EE CLAIMS THAT WHILE UNROLLING SOME 2" POLY PIPEOFF ROLL, THE END HE WAS HOLDING SLIPPED OUT OF HIS HAND & THE END STRUCK HIM ON THE LEFT EYEBROW CAUSING A LACERATION ABOVE EYE BROW

EE CLAIMS THAT WHEN HE WAS ASSISTING IN REMOVING THE CHUTE FROM THE #1 SHORT HEAD CRUSHER, THE CHUT HUNG UP. EE WAS USING A PRYBAR TO LOOSEN THE CORNER THAQT WAS BINDING WHEN THE BAR CAME LOOS E, IUT CAUGHT EE'S FINGER AGAINST FRAME.

EE CLAIMS THAT AS HE WAS DRILLING FOR BOLTING, RIB THE STEEL GOT PLUGGED. WHILE TRYING TO PULL THE DRILL STEEL OUT WITH JACKLEG HE TWISTED RIGHTKNEE MRI ON 9/12/99 DRS REPORT ON 9/16/99 SHOWED A TORN POSTERIER HORN & BODY OF MEDIAL MENISUS Y

EE CLAIMS THAT AFTER FINISHING DRILLING A TOP HOLE ON FACE AS HE WAS PULLING ON DRILL TO EXTRACT DRILL STEM FROM HOLE HE FELT PAIN TO THE LOWER BACK. EE REFUSED TO GO TO DR AT THAT TIME. ON 10 -6-99 HE REPORTED THAT HE HAD APPARENTLY PULLED BACK MUSCLE AGAIN AND ON 10-7-99 WAS SEEN BY DR PHYSICAL THERAPY PRESCRIBTION.

WHILE EMPLOYEE AND COWORKER WERE PREPARING TO INSTALL TROLLEY BRACKETS, A TRAIN STRUCK THE BATTERY FLATCARS THEY WERE USING, CAUSING EMPLOYEE TO FALL ON HIS BACK AND STRIKE HIS ELBOW. SUUFERED A BONE CHIP ON HIS LEFT ELBOW AND HAD COMPLAINTS THE FOLLOWING DAY OF NECK AND BACK PAIN.

EMPLOYEE WAS IN PROCESS OF CLEANING OUT OLD FILES AND BOXES OUT OF MEDICAL BUILDING WHEN HE STARTED HAVING CHEST PAINS. SUFFERED MYOCARDIAL INFARCTION.

EMPLOYEE CLAIMS THAT AS HE WAS BREAKING A ROCK WITH THE HYDRAULIC ROCK BREAKER MOUNTED ON A BOBCAT A PIECE OF FLYROCK STRUCK HIM ON THE SAFETY GLASSES AND SOME DUST/DIRT GOT INTO HIS RIGHT EYE CAUSING IRRITATION AND MINOR ABRASION TO EYE.

EE CLAIMS THAT WHEN HIM AND CO WORKER WERE LINING UP STEEL SET ON GROUND AS CO WORKER PICKED THE STEEL PIECE ON OTHER SIDE THE SET. ROLLED OUT PINCHED FINGER AGAINST ROCK ON FLOOR.

EE CLAIMS HE WAS DRILLING A UP HOLE AT LINE 10 24N WHEN MACHINE CAME OFF THE STEEL, MACHINE FELL AND LEG BOUNCED UP. STINGER CAUGHT HIM JUST BELOW RIGHT KNEE CAUSING BRUISE. EE RESTRICTED WORK DAYS WERE 12-22,23 AND 28,1998.

EE CLAIMS HE HAD A MINOR SLIP WHEN STEPPING OUT OF A LOADER, CAUSING A TWIST OF HIS LOWER BACK AND LEFT LEG.

EMPLOYEE CLAIMS HE FELT LOWER NECK AFTER AN INCIDENT THAT OCCURED ON THE PREVIOUS SHIFT INVOLVING AN LHD LOADER FALLING INTO HOLE.

EMPLYEE CLAIMS HE WAS WALKING FROM THE MINE DRY TO THE H OIST HOUSE, SLIPPED AND FELL ON ICE LANDING ON HIS LEFT LEG CAUSING MINOR LACERATION.

TWO BATTERY LOCOMOTIVES WERE TRAVELING TOWARD EACH OTHER IN PANEL 27, LOC 67 OPERATED BY EE OF MCO SLOWED AND CAME TO ASTOP, LOCI 609 OPERATED BY OTHER OF MOLY CORP DID NOT SLOW DOWN AND STRUC K LOCI 617. REDARTE SAID HE APPLIED THE DYNAMICSTESTOP LOCI 609 BUT THEY FAILED. RODARTER WAS TRAVELING BY HIMSELF, WHILE EE 3 OTHER MINERS OTH.

EMPLOYEE CLAIMS WHILE IN THE PROCESS OF TRANSPORTING A TRANSFORMER AT THE #1 SHAFT STATION, WHILE TIEING THE TRANSFORMER TO BE PULLED THEY HAD TO TAKE THE SLACK OFF THE CHAINS. WHEN THE OPERAT OR LOWERED THE BUCKET, HIS FOOT GOT CAUGHT BY THE BUCKET. (RETURNED TO WORK ON LIGHT DUTY ON NEXT SCHEDULED SHIFT 10/5/98.)

EE COMPLAINED OF NUMBNESS IN BOTH ARMS AND CONSTANT PAIN RESULTING IN FATIGUE AND LACK OF SLEEP. THIS CONDITION ESCALATED OVER A PERIOD OF TIME.

EE AND OTHER CO-WORKERS WERE ASSISTING IN RE-RAILING TWO ORE CARS. A LOCI WAS BROUGHT TO THE SITE AND HOOKED TO THE LAST CAR 1029. EE REQUESTED THAT LOCI BE MOVED FORWARD TO GET ENOUGH SLACK T O UNHOOK SAFETY CHAIN HE POSITIONED HIMSELF BETWEEN CAR 1028 AND 1004 AND GOT CAUGHT BETWEEN THE CARS.

EMPLOYEE AND WITNESS STATED THAT WHILE REMOVING A BEARING OFF A PUMP SHAFT BY USING A HAMMER AND PUNCH, EMPLOYEE FELT SOMETHING FLEW AND STRUCK HIM ON THE LEFT ARM. WITNESS ADMINSTERED FIRST A ID. EMPLOYEE REFUSED TO REPORT INCIDENT TO SUPERVISOR ACCORDING TO WITNESS. REQUESTED REMOVAL OF SPLINTER ON 6-22-99. ON 7-9-99 DR REMOVED SLIVER COMMENCED LOST TIME.

EE CLAIMS WHILE WORKING ON #1 REGRIND BLANKINGS OF GRATES, HE WAS WORKING ON A CONFINED AREA CAUSING OVER EXERTION TO RIGHT SHOULDER.

EE CLAIMS WHILE INSTALLING SCREW CONVEYOR AT THEDRYER, HE WAS LOWERING SCREW CONVEYOR AND FELT PAIN IN LOWER BACK.

EE CLAIMS WHILE UNLOADING A ACETYLENE BOTTLE FROM A BOSS BUGGY, BOTTLE ROLLED AND SMASHED HIS RIGHT RING FINGER AGAINST POWDER BOX.

EE CLAIMS THAT HE FELT PAIN TO LOWER BACK WHILE ATTEMPTING TO REMOVE A COUPLER OFF THE LOCOMOTIVE. EE HAD PREVIOUSLY REPORTED ON 6-13-98 TWISTING HIS BACK WHILE ATTEMPTING TO BAR DOWN A BOULDE R STUCK BETWEEN LINER PLATES AT WHICH TIME IT WAS REPORTED AS AN IWOI.

EMPLOYEE STATES THAT WHILE LOADING ORE CARS OFF THE #6 TRANSFER RAISE IN PANEL 28, THAT AS HE STARTED TO LOAD THE 5TH CAR SOME REAL WET MUCK CAME DOWN AND FLOODED THE DRIFT BELOW AND BUILT UP ON TO PONY SET THUS TRAPPING HIM UP ON PONY SET. EMPLOYEE WAS RESCUED AT APPROX. 2:45 AM 5-21-98.

EMPLOYEE CLAIMS WHILE LOADING A 24" VENT LINE DAMPER ON 545 UTILITY TRUCK HE TRIPPED SMASHING HIS FINGER AGAINST A VENT LINE DAMPER THAT WAS ON THE GROUND.

EE WAS REMOVING STEEL OUT OF JACKLEG, WHEN STEEL PULLER WHICH IS SPRING LOADED SNAPPED BACK AND PINCHES HIS INDEX FINGER BETWEEN DRILL STEEL AND STEEL PULLER AND CUT HIS FINGER REQUIREING 2 ST ITCHES IN THE END OF HIS FINGER.

HEADING WAS BARRED DOWN AND INSPECTED - EMPLOYEE WAS HELPING TO COLLAR A LIFTER HOLE WHEN HE WAS STRUCK BY A ROCK ON HIS RIGHT SIDE CAUSING BROKEN RIBS AND SPRAINING ANKLE.

EE HAD BARRED DOWN & INSPECTED WORK AREA. AFTER BARRING DOWN & INSPECTION, THE AREA WAS CONSIDERED SAFE TO COMMENCE WORK. EE WAS INSTALLING CRIBBING WHEN ROCK STRUCK HIM ON BACK CAUSING HIM TO STUMBLE FORWARD CONTACTING FACE ON INSTALLED CRIBBING.
