Found roof fall in an untraveled area, station spad #7479, #3 entry, 1 North sub mains. 7'high X 17'wide X 40'long.
Injured was pulling a trailing cable and a piece of rock 1'L x 6" W x 1" thick fell from a potted out place in the roof. The rock hit him on the forearm.
Injured was driving mantrip when a small rock fell from top and hit him on top of rt hand.
Roof fall occurred at station spad 7833, #5 entry. It was bolted with 12'point anchors, we were setting cribs & 6" beams when it started working again. We stopped the work & let it fall. 24'long x 18'wide x 12'high.
Roof fall occurred in last intersection off belt entry between pot out and where roof had been cut down for belt head drive. 18' wide x 8' high x 20' long.
Roof fall found on weekly exam by MSHA & mine foreman 1 west mains, E panel, 10 block inby spad 5417 between #4 entry & #5 entry. 50' length, 16' wide, 6' high.

TRAMMING BOLTER FROM IN DECK AND MACHINE SLID AGAINST RIB AND SMASHED FINGER BETWEEN DECK AND RIB.
Sawing a mine post with a bow saw and cut into his finger which he was holding the post with.
EE WAS USING SCOOP TO MOVE A HEAD DRIVE, PUSHING AGAINST 2 HEADER BOARDS. THE 2 BOARDS FLIPPED OUT AND SMASHED HIS FINGER BETWEEN THE BOARDS AND THE FRAME OF THE HEAD DRIVE. (AMPUTATION: 1/2 MIDDLE FINGER ON LEFT HAND.)
Supply man was building a stopping. He cut a metal band which held a bundle of 6' solid concrete blocks together. When the band broke a 6' block fell and landed on his hand.
Dropped lid on scoop batteries on right hand and smashed his finger.
Found roof fall in old area of mines during a weekly exam, not sure when it fell. MSHA inspector present during exam. Belt entry 5 to 4 5NW G panel spad 7391(to the left)
SCOOP OPERATOR WAS UNLOADING A PIECE OF METAL FROM SCOOP BUCKET. HE STEPPED BETWEEN THE BUCKET & A MINE POST. THE SCOOP DRIFTED FORWARD & SMASHED HIS LEG. HE WORKED UNTIL 1/11/06. HIS LEG SWELLED UP & THE DR. TOLD HIM TO STAY OFF OF IT & KEEP IT ELEVATED.

CUTTING PIECE OF PVC PIPE WITH A UTILITY KNIFE, KNIFE SLIPPED & CUT HIS FINGER.
Roof fall 17 block #2 belt between spads 4393 and 4408 fall 50 ft long x 16 ft wide x 6' high.
Track bolter helper was helping bolter operator pull a stuck drill steel out of a drilled hole. A piece of bad top came down landing on helper's back forcing him down to the ground.

OPERATOR WAS CLIMBING DOWN FROM CATERPILLAR 930 END LOADER AND SLIPPED OFF THE STRP IN THE POURING RAIN. HE LANDED ON FENDER HITTING HIS RIBS. HE WORKED THE 4 DAYS, 7-12-04 TO 7-15-04 BEFORE HE WENT TO THE DOCTOR.

Towing railbus outside, ee was holding pin with his rt hand, as they took the slack out of tow chain the pin smashed his little finger against frame.
Ee standing in operator's compartment on operator's seat, lifted left leg to step on top of machine. The seat was not attached and slipped from under his weight. He fell backwards hitting his right side on edge of operator compartment.
While operating roof bolter the operator raised foot jack which allowed the front of the machine drop down. The operator had his foot under that part of the roof bolt machine.
While riding in rail mantrip, individual struck forehead on belt guard where the rail goes under #3 belt line.

HE SAID HE HURT HIS BACK ON 4-11-03 LIFTING A BOARD. HE SAID HE HURT IT AGAIN WHILE PULLING BELT BACK TOGETHER BUT NOT SURE ON DATE. HE SAID HE THOUGHT HE HAD KIDNEY STONES ON 4-21-03. HE FILE D A COMPANY ACCIDENT REPORT ON 4-30-03. HE THINKS HE HAS A LOWER BACK INJURY.

THE EXAMINER WAS MAKING HIS CHECKS AND FOUND THE FALL. IT IS APPROXIMATELY 16' X 16' X 10'.
THE EMPLOYEE WAS UNLOADING SUPPLIES AND DROPPED A FEEDER FLIGHT ON HIS KNEE.
THE EE WAS BUILDING STOPPING AND INJURED HIS BACK LIFTING THE CINDER BLOCKS.
THE ROOF ON THE NO 1 MAIN BELT LINE GAVE WAY AN EXAMINER HAD JUST COMPLETED HIS EXAMINATION OF THE BELT. THE FALL WAS APPROX. 7'HIGH, 16'WIDE, AND 30'LONG. THE AREA WAS SUPPORTED WITH 6'TEMI. REBAR.
THE EE WAS MOVING A PUMP AND LIFTED PUMP CABLE.
THE EMPLOYEE WAS PICKING UP SUPPLIES IN A CROSSCUT AND A PIECE OF ROCK FELL OUT BETWEEN THE ROOFBOLTS AND STRUCK HIM IN THE HEAD AND BACK.
THE EMPLOYEE SHUT THE BELT OFF AND CROSSED THE TAIL PIECE TO GETA SHOVEL AND CAME BACK ACROSS THE TAIL PIECE AND FELL AND HIT HIS KNEE.

THE FIRE BOSS WAS MAKING HIS EXAMINATION WHEN HE DISCOVERED MORE AIR THAN USUAL ON THE BELT LINEOF THE OLD SECTION AND WHEN HE INVESTIGATED HE FOUND A ROOF FALL. THE FALL WAS 16 FT WIDE 16 FT LONG AND 7-8 FT HIGH. THE AREA WAS SUPPORTED WITH 6 FT TENSION REBAR.
THEY WERE SPLICING THE BELT AND THE EMPLOYEE WASHOLDING THE KNOCKER FOR ANOTHER EMPLOYEE WHEN HESTARTED TO REMOVE THE KNOCKER AND THE OTHER EMPLOYEE STARTED TO HIT IT AND CAUGHT HIS HAND ON TOP OF THE KNOCKER WITH AN AX.
THE ROOF BOLTER OPERATOR WAS PUTTING HIS FRILL STRUCK TOGETHER AND WENT TO BUMP THE HEAD UP ND LOST HIS BALANCE RASING THE HEAD FASTER THAN HE INTENDED TO AND CAUGHT HE FINGER BETWEEN THE DRIL L STEEL AND THE ROOF.
THE EE WAS KNOCKING A HOLE IN A STOPPING FOR VENTILATION PURPOSES IN AN AREA THAT WE HAD MOVED OUT OF. HE WAS USING A CINDER BLOCK TO KNOCK THE HOLE.
THE EE WAS INSTALLING A CONVEYOR CHAIN IN A SHUTTLE CAR. HE WAS PULLING ONT HE CONVEYOR CHAINS WHEN STOPPED TO PUT A LINK UP ON THE CHAIN GUIDES. HE GOT UP UNDER THE SHUTTLE CAR TO BEAT THE LI NK OUT STRAIGHT AND WHEN THE CHAINS CAME LOOSE IT MUST OF HAD PRESSURE ON IT AND THE FLIGHT STRUCK HIM IN THE HEAD.
THE ELECTRICIAN HAD BROUGHT THE ROOM THROUGH THE INTERSECTION ABOUT 10 MINUTES BEFORE THE ROOF GAVE WAY. HE NOTICED THE ROOF LOOKED LIKE IT SAGED. THE ROCK STARTED BREAKING OUT BETWEEN THE BOL TS THEN ALL OF IT CAME IN. THE FALL WAS APPROX 16 FEET WIDE, 25 FEET LONG AND ABOUT 7 FEET HIGH. SUPPORTED W/ 6 FOOT TENSION REBAR. FALL OCCURED2 WEST SECTION, NO. 7 ENTRY, 2 BLOCKS INBY 4025
A FEDERAL INSPECTOR AND WEEKLY EXAMINATOR WERE CHECKING THE WORKED OUT AREA AND FOUND THE FALL.T HE FALL MEASURED 8 FT HIGH, 16 FT WIDE AND 20 FT LONG. THE AREA WAS SUPPORT WITH 6 FOOT TENSION REBAR ROOF BOLTER. U
THE HIGH WALL AT THE NO. 5 PIT MOUTH FELL OUT AT THE INBY EDGE OF THE CANOPY. THE FALL WAS 15' LONG, 15' WIDE AND 7' HIGH. THE AREA WAS SUPPORTED WITH RESIN BOLTS & LT. BEAMS.
A HEADING WAS BEING TURNED IN AND THE CUT WAS ONLY ABOUT 15 FEET DEEP AND 16 FEET WIDE AND THE ROOF GAVE WAY AT ABOUT 6 FOOT HIGH. THE ROOF WAS SOFT IN HIS AREA.
HE WAS RIDING ON THE BACK OF THE 3 WHEELER ON KNEES, TOES HANGING OVER EDGE. THEY STARTED BACKING UP TO THE ROOF BOLTER TO UNLOAD AND SMASHED FOOT BETWEEN ROOFBOLTER & 3 WHEELER.
INJURED WAS CARRYING ROCK DUST TO BELT LINE WHEN HE THINKS HE HIT HIS HEAD ON A ROOF BOLT CAUSING PAIN IN HIS LEFT HIP AND LOWER BACK.

THE MINE EXAMINER AND A FEDERAL INSPECTOR WERE CHECKING THE BLADERS AND FOUND THE FALL. IT WAS IN THE NO. 6 ENTRY OLD PANNEL. IT MEASURED 35 FT LONG 16 FT WIDE AND 10 FT HIGH.
THE EE WAS USING A UTILITY KNIFE TO CUT SCREEN THAT HE WAS PUTTING UP TO THE ROOF AND CUT HIS HAND.
I WENT UP ON THE ABANDONED PANNEL TO SEE WHAT WAS LEFT IN IT AND WAS CHECKING THE AIR ACROSS THE PANNEL AND FOUND THE FALL. THE FALL WAS 30'LX16'LWX8'H. IT OCCURRED IN THE NO. 2 ENTRY 100' I NBY SPAD 3385.
THE EE WAS WALKING AROUND THE MINER AND BUMPED HIS KNEE ON THE BOOM OF THE MINER.
THE EMPLOYEE WAS DRAGGING A PIECE OF LINE CURTAIN AND TRIPPED AND FELL AGAINST A SHUTTLE CAR.
THE EE WAS GETTING OUT OF THE ENDLOADER & STEPEDON A ROCK & TWISTED HIS ANKLE
THE EMPLOYEE WAS USING A HAMMER ON A WRENCH TO TIGHTEN BOLTS ON BUGGY. THE WRENCH HAD SLIPPED OFF ONCE OR TWICE AND HE WAS HOLDING THE WRENCH WITH ONE HAND AND SWUNG HIS HAMMER AND CUT HIS HAN D.
THE MIDNIGHT SHIFT HAD MOVED THE BELT TAIL PIECEUP THREE BLOCKS AND EE WAS WORKING IN THE AREA MOST OF THE TIME WHEN WITHOUT WARNING OR ANY THATANYONE OBSERVED THE ROOF GIVE WAY. THE FALL AREA WAS SUPPORTED WITH 6 FT TENSION REBAR. THE FALL MEASURED 30 FT LONG, 35 FT WIDE INTO CROSSCUT AND 9 FT HIGH.
A FLAT CAR WAS ON THE END OF THE SECTION TRACK AND THE CREW CAME DOWN AND GOT ON THE MANTRIP TO LEAVE THE SECTION WHEN THEY GOT TO THE CURVE THEY SLOWED DOWN TO GO AROUND THE CUREVE WHEN THEY SAW THE FLATCAR COMING TOWARD THEM & IT HIT THE MANTRIP.
THE EE STATED THAT HE WAS REPLACING ROOF BOLTS THAT HAD BEEN CUT OUT BY THE CONTINUOUS MINER ANDWAS TRYING TO CATCH HIMSELF TO KEEP FROM FALLING AND CAUGHT A ROOF BOLT PLATE.
THE EMPLOYEE WAS WORKING AROUND TAIL PIECE AND FELL INTO THE BELT CAUSING A BRUISING OF HIS LEG.

SOMETHING BURN'T UP IN A POWER CENTER AND THE EE WAS RIGHT BESIDE IT ON THE OFF RIDE AND HE WENT BACK AND HIT THE EMERGENCY STOP BUTTON.
THE EMPLOYEE HAD PUT A WATER HOSE INTO A CAN TO FILL IT WITH WATER AND THE HOSE BLEW OUT STRIKING HIM IN THE EYE.
THE ROOF BOLTER OPERATOR SAID THAT HE WA BENDINGA ROOF BOLT WHEN HIS BACK STARTED HURTING
THE PRESHIFT EXAMINER WAS CHECKING THE INTAKE AIRWAY AND FOUND THE FALL. THE FALL WAS LOCATED INTHE #2 ENTRY INTAKE AIRWAY. THE FALL WAS IN INTERSECTION. THE FALL MEASURED 6'H X 6'W X 16'L.
EMPLOYEE HAD STARTED TO WALK THE RETURN OFF THE MAIN SECTION AND HIS MINING HAT AND LIGHT FELL OFF HIS HEAD AND WHILE TRYING TO GET HIS HAT AND LIGHT BACK HE STRUCK HIS HEAD ON A ROOF BOLT
EE WAS ROOF BOLTING DOWN ON HIS KNEES, WHEN HE STARTED TO GET UP HIS KNEE WENT OUT AND HE COULDN'T HARDLY STAND UP.
THE EE SAID HE MUST HAVE HIT HIS BACK WHILE WALKING THE BELT LINE.
INJURED WAS INSTALLING A ROOF BOLT. HIS BODY WAS ON A TWIST. HE STRAIGHTENED BOLT TO PUSH IN HOLE AND FELT PAIN IN LOWER BACK CAUSING A STRAIN.
EMPLOYEE WAS WALKING TO THE END OF THE TRACK ANDSTEPPED IN A HOLE AND TWISTED HIS KNEE.
THE EE SAID THAT HE WAS MOVING A ROCK WHEN HE HURT HIS BACK, BUT WHEN HE LEFT THE MINE HE LOOK & WALKED OK AND DIDN'T GO TO THE DR UNTIL AFTER HE HAD MISSED 3 DAYS WORK.
EMPLOYEE WAS RIDING THE MANTRIP TO THE SURFACE AT QUITTING TIME WHEN HE RAISED HIS HEAD UP AND STRUCK A CONVEYOR BELT GUARD WHERE THE TRACK CROSSES OVER THE CONVEYOR BELT.
WE HAD MINED 2 CROSSCUTS INBY THIS AREA WHEN THE ROOF GAVE WAY. WE HAD INTALLED 7 FOOT BOLTS 2 SPOTTED SOME 8 FOOT BOLTS IN THIS AREA.

THE ROOF AT NO7 BLOCK NO1 BELT LINE GAVE WAY. THE FALL WAS APPROX 50X16X6. THE AREA WAS SUPPORTED WITH 6FT TENSION REBAR ROOF BOLTS. THE FALL HAS BEEN CLEANED UP & SUPPORTED
THE TWO EES WERE PUTTING UP BELT HANGERS FOR THE BELTLINE AND WERE LINING UP THE MACHINE FOR THE NEXT HANGER AND ONE EE TOLD THE OTHER ONE TO COME BACK AND HE THOUGHT HE SAID SET JACK AND PUT THE ROOF BOLTER FOOT JACK DOWN ON HIS LEG.
THE INTERSECTION GAVE WAY AT 23 BLOCK 0ON 2 LEFT SECTION: THE FALL WAS APPROX. 16 X 16 X 9. THE AREA WAS SUPPORTED WITH 6' TENSION REBAR ROOF BOLTS.
THE ROOF GAVE WAY IN THE NO.3 ENTRY AT 24 BLOCK ON 2 LEFT SECTION. THE FALL WAS 12X14-7. THE AREA WAS SUPPORTED WITH 6 FT TENSION REBAR ROOF BOLTS. THIS WAS ONLY A PARTIAL INTERSECTIONS FULL. THE FALL WILL NOT BE CLEANED UP.
WATER WAS DRIPPING IN THE INTERSECTION. WE WERE USING 7 & 8 FT ROOF BOLTS WITH 6X16 PLATES. THE FALL WAS 16'LX16'WX8TO9'H. THE WIDTH OF THE ENTRIES AT THE FALL WAS 15 TO 16' WIDE.
INJURED WAS PUTTING UP CROSS BARS ON CRIBS WHEN A PIECE OF ROCK FELL KNOCKING INJURED AGAINST SCOOP BRUISING LEFT HIP.
THEY WERE CHECKING THE AREA WHEN THEY FOUND THE FALL. IT DID NOT AFFECT THE VENTILATION OR ANYTHING. THE FALL WAS APPROX 20FT LONG, 16FT WIDE, AND 61/2FT TO 7FT HIGH
THE FOREMAN WENT IN AND FIREBOSSED THE SECTION AND CAME OUT AND FILLED OUT HIS BOOK AND WHEN HE WENT BACK IN HE FOUND THE ROOF FALL. THE FALL WAS ON THE 3RIGHT PANEL NO.5 ENTRY NATURAL. THE FA LL IS 20' LONG, 16' WIDE, AND 8' HIGH. THE FALL COVERS UP A POWER CENTER. WE AR MOVING OFF THIS SECTION AND ARE NOT GOING TO TRY TO RECOVE THE POWER CENTER. NO FURTHER MINING IN AREA PLANNED.
THE FOREMAN CAME DOWN TO CHECK THE AREA TO START A NEW PANNEL AND FOUND THE FALL. THE ROOF FALL OCCURED IN THE OLD 2 LEFT SECTION NO.2 ENTRY IN THE RETUME. THE FALL IS APPROX. 20 FT, 16 FT W A ND 8 TO 9 FT HIGH. WE ARE NOT PLANNING ON CLEANING UP THE FALL.
THE ROOF BOLTER OPERATOR WAS STRAIGHTENING A BENT ROOF BOLT AND IT SPRANG BACK AND CAUGHT HIM ACROSS THE BRIDGE OF THE NOSE.
A ROOF FALL 6 FT HIGH, 16 WIDE AND 20 FT LONG WAS DISCOVERED BY A FIRE BOSS MAKING HIS WEEKLY EXAMINATION AT 35 BLOCK ON THE 2LEFT SECTION, NO O ENTRY. WATER WAS DISCOVERED IN THE ROOF AFTER T HE FALL OCCURED.
AREA WAS KNOWN TO BE BAD WE HAD ALREADY HAD A ROOF FALL STRAIGHT ACROSS FROM THIS ONE. THE FALL WAS 50 FT LONG 16 FT WIDE AND 8-10 FT HIGH. WE HAVE MOVED OFF THIS PANEL AND NO FARTHER MINING I S PLANNED IN THIS AREA.
THE INTERSECTION WAS SHOWN BAD & HAD BEEN CRIBBED & CROSSBRACED AND ALSO HAD BEEN LOOKED AT BY MSHA ROOF CONTROL. WE THOUGHT WE HAD IT UNDER CONTROL. THERE HAD BEEN A FALL IN THE NO 1 ENTRY 2 RIGHT SECTTION STRAIGHT ACROSS FROM IT. THIS ONE WAS IN 0 ENTRY LEFT RETURNS.
AFTER THE PLACE HAD BEEN ROOF BOLTED AND DIRVEN INBY WATER STARTED DRIPPING FROM THE ROOF AND ATTHAT TIME WE NOT CRIBBING UP ONE SIDE OF THE PLACE
THE EE WAS HELPING THE CONTINOUS MINER CHANGE PLACER AND AS THEY REACHED THE INTERSECTIONS AT THE MOUTH OF THE NO 8 ENTRY THE MINER OPS, SHUT THE MINER OFF AND THIS EE TOOK A LOOP OF CABLE OFF THE MINER BOOM A TURNED AWAY FROM THE BOOM AND THE MINER OPERATOR STARTED TO TRAM THE MINER THINKING THE EE HAD MOVED BECAUSE HIS LIGHT WAS TURNED AWAY FROM THE BOOM CAUGHT THE EE AGAINST THE
INJURED WAS UNLOADING PIPE FROM SCOOP WHEN PIE KICKED UP CATCHING LEFT HAND BETWEEN PIPE AND ROOF INJURING LEFT RING FINGER.
TH EE WAS LOADING CINDER BLOCKS ON A SCOOP BUCKET. WHEN THE BUCKET WAS LOWERED IT WAS CAUGHT ON SIDE OF SUPPLY CAR & NOT DOWN ON THE BOTTOM AND DUE TO THE LOAD OF BLOCKS IT SLIPPED OFF THE SID E OF THE CAR AND CAUGHT EE'S FOOT UNDER IT.
THE ROOF GAVE WAS IN THE NO2 ENTRY 1 ST CROSSCUT INSIDE THE FAN. THE FALL WAS APP 206 X 15 W 10FT HIGH. THE AREA WAS BOLTED WITH 6 FT RESIN GROUTED BOLTS AND CROSS BEA INSTALLED IN THE AREA TH E MSHA AND STATE HAVE ALREADY INVESTIGATED THE FALL.

EE WAS BACKING THE CONTINOUS MINER OUT OF THE RIGHT CORSSCUT OF THE #4 ENTRY WHEN A PIECE OF ROCK FELL OUT FROM BETWEEN THE ROOF BOLTS STRIKING HM ON THE SIDE & HITTING THE BACK OF HIS LEG.
THE ROOF BOLTER OPERATOR WAS INSTALLING TWO SECTIONS OF DRILL STEEL AND NEEDED A LITTLE MORE ROOM TO GET THE STEELS TOGETHER, AND STARTED TO SLIDE THE HEAD FORWARD AND NOT LOOKING AT HIS CONTR OLS JUST REACHED IN AND HIT WRONG LEVER.
