EE was not feeling well & sat down on a picnic table within the quarry shop. Shortly after sitting down, EE experienced a medical episode, believed to have been related to diabetes, which caused EE to faint & hit head on the picnic table. Incident was not work related but due to a medical episode. EE received staples to back of head that was cut when EE fainted & hit head on table.

Miner was attempting to remove a rock that was in the collection box at the top of conveyor #3 at the primary before plant start-up. Employee was prying a rock with a bar that was about 6" that was wedged against other material. While the miner was pulling the bar, the rock became free and the bar pinched EE's pinky finger on the left hand between the top of the box and the bar.

Pulling gate closed at the end of the day and felt pain in left shoulder, was originally reported as 1st aid. Employee is now treating with physical therapy as prescribed by physician making this re-portable.

Employee was pulling on a wrench when he felt pain in his shoulder. Employee went to the Chiropractor on 10/28/14, this is the date the injury became reportable.

EE was working on a portable plant. At the end of the day he told me that his back was sore, but he did not need to go to the doctor at that time. He went the next day. It was a back sprain.
EE was cutting rubber belting when the knife (box cutter) slipped cutting his left index finger on the side.
