employee was patching the outside airslide; while exiting fine material fell from above the grating onto employees face. A spec was found in employees eye later that night.

EE was cutting steel in the weld shop with an oxygen acetylene torch when EE realized the oxygen hose was leaking from a hole. EE found where the leak was coming from and went to pinch the hose so EE could shut the gas tank off. As EE pinched the hose, both hoses exploded and ignited sending out a flame that contacted the EE's abdomen.

A millwright employee was installing the bracket for the pneumatic gate air actuator for Silo 1 bin bottom while using a hammer to knock the bracket into place. As the hammer was in swinging motion it hit the employee's left thumb as it was in a downward motion to knock the bracket into place.

Shift Operator tweaked EE's left shoulder as EE placed the door on the 1115 drag cover

EE was descending a fixed ladder and slipped and missed the last two rungs. EE experienced lower back pain and center abdomen pain

While winterizing the lime slurry system an EE disconnected the line to clean it out when the system was offline. When the EE disconnected the line, a loose coupling caused the line to come loose prematurely. Residual lime slurry in the hose sprayed onto the EE getting on EE's face. EE immediately flushed EE's eyes with clean water, the burning continued and was brought to ER.

Shift electrician was removing a rusted connector from a light fixture. Pump pliers slipped from fitting. EE's left thumb struck light reflector resulting in 2 inch cut. EE received stitches to close the open cut

Employee was working with another employee who subsequently tested positive for COVID-19.

EE was using metal machine when material became stuck, EE tried to loosen the material by using a hammer. EE struck EE's lt thumb with the hammer causing a contusion/laceration. EE was wearing gloves. EE received 4 sutures, antibiotics and is able to RTW same day.

Injured ee reported they were struck in the L wrist by a door when entering maintenance shop, another employee was exiting maintenance shop at the same time. Injured ee c/o of pain in L shoulder

An employee was working on removing a bearing cap to inspect the bearing on "B Elevator" when EE came into contact with a damaged metal seal on the side of the bearing head shaft. While removing a piece of the damaged metal, a sharp edge cut through EE's heavy duty glove into EE's left pinky finger resulting in 4 stitches.

The employee had finished repairs to the Preblend Stammler system immediately prior to the injury, which occurred when employee was walking down the stairs with tools/equipment. The employee stepped down from the top step with employee's right leg, and as soon as employee put pressure on employee's leg employee felt employee's knee "pop".

The employee complained of wrist and finger pain. Their main role is administrative, and is part of the sales team. It is unknown how the injury/illness occurred, as the employee believes this is a chronic condition from using a keyboard and mouse.

The employee was troubleshooting the flop gate control arm on a FK pump. The employee's finger became pinched between the pivot arm and flop gate, causing a laceration on their right hand (5th finger).

EE was working on the 1812.2 Clinker Silo Dust collector fan. EE released the button and as the motor was winding down EE went to see if it had normal air flow. On the way by, EE's fingers came in contact with the moving belts. The tip of one finger was removed and the other was injured. EE reported to the control room and was then taken to the hospital for treatment

Slid on the ice, twisting knee. Did not fall. Employee was sent to the ER. There EE was told that they thought EE may have torn ligament and instructed EE to go to an orthopedic doctor the following day.

While moving a drum of air compressor oil using a hand truck, employee felt a twinge in the left side groin area. Employee kept on working. Later in the morning pain developed and a bump appeared in the same area. Note: injury IS NOW REPORTABLE DUE TO SCHEDULED SURGERY ON 11/16/16.

The Palumbo Truck driver slipped down the last set of stairs when heading into the bulk loading office. Wet shoes appear to have contributed to slip. Note: The injured is not an employee of Lehigh Northeast Cement.

The Employee was helping to install new gudgeon bolts on the 1143 screw. While doing so it is necessary to use the drive belts and pulleys to align the screw for bolt insertion. In the process of doing this the screw backlashed and employee's right hand pinky finger was caught between the belt and pulley tearing a gash in the tip, requiring stitches.

The employee bent over and picked up a small (34lb) refrigerator and threw it into the dumpster. While throwing it in he felt a pain in the left side of his back and his leg.

The employee walked into an area that was inundated with dust to try and put a 4" cap back on to the pant leg of silo 29N. The force of the material coming through a 4" opening knocked off his dust mask and safety glasses, causing a scratch to his left eye and difficulty breathing.

The injured is not an employee of Lehigh Northeast Cement Company. On the above date and time a contractor truck driver injured his left ring finger. The driver was lifting the batching station platform using the safety rail; his finger was pinched between the safety bar and the platform beam causing fractured finger requiring stitches.

The employee was tightening bolts on a gearbox on the roller mill and in the process felt a pull sensation in his left inside forearm.

The employee was replacing a large hydraulic cylinder and relative hoses in the clinker cooler compartment 5/6. Upon completion of work employee noticed minor "tingling" and pain located in left shoulder.

K&G Contractor was walking around the front of the Roller Mill Off Spec hopper and stepped on uneven ground, which led to him slipping and bending his ankle, leading to a fracture to his right ankle.

The employee placed his right foot on the top rung of the ladder to dismount the gypsum feeder platform. When doing so his foot slipped off the ladder causing him to lose his balance and fall approximately four feet to the ground, causing right ankle sprain.

The employee was working in a warm location and sweat was running down his face into his eyes. He wiped his face off to remove the sweat. He went home at end of shift but it did not seem to be bothering him. He woke up early in the morning with pain in his eye. We took him to an eye doctor the next morning. They found a tiny piece of steel and removed it.

The employee allegedly stood up to adjust the gas valve in the Control Room and says he felt and heard a popping sensation and then felt pain in the back of his right knee.

The employee was responding to a call-in, while walking through the roller mill building employee stepped on a medium sized stone, slipped and extended his left leg. Employee felt a twinge in his left knee and upper left thigh, but no pain until the next morning. Note: Employee did not seek medical attention until 11/6/13 and first lost day is due to surgery on 12/10/13.

Employee was descending stairs from 3rd floor of preheat tower. Employee stepped off the bottom stair on to a deformed catwalk section, causing his right ankle to roll laterally causing a sprain/strain.

While jackhammering the employee lacerated the tip of his right pinky finger when the jackhammer broke through the material allowing the employee's finger to strike against a piece of angle iron.

The employee was walking down a set of stairs, as he stepped out onto the main floor he stepped in wet mud and twisted his right knee.

While employee was kneeling on wooden floor assembling a partition, an apparent sliver entered his right knee, which later that night caused swelling and pain.

While disassembling a FK Compressor oil bath filter, employee caught his right wrist on the wire mesh of the filter causing a laceration to his right wrist, resulting in sutures being needed.

Employee was driving on the New York State Thruway when a turkey flew into the driver's side of the company van windshield, causing shattered glass fragments to fly into both of employee's eyes.

The employee indicates that over the last couple of years he has been noticing numbness in the fingers of both hands, with the most pronounced numbness being in the fingers of his right hand. This date of 4/25/11 is the first time the employer was made aware of this condition. We are now reporting it as a reportable, as surgery has been scheduled for July 1, 2011.

While traversing the steel storage area, the employee struck a sheet of steel and fell. This caused a laceration and bruise to the right knee. Doctor visit of 6/1/11 was the first time the employee sought medical attention for this injury.

The employee was tightening bolts on the Stone Stamper when he felt a sharp pain and burning in right shoulder.

While moving impact plates from catwalk with the hoist, other impact plates stacked for removal tipped over and struck employee in right calf and slid down the back of employee's leg pinning his foot and ankle under the plates, causing a contusion/strain injury.

While attempting to move crusher liner plates with a johnson bar, the bar slipped and employee wrenched his lower back.

While driving out liner bolts the bit of the jackhammer became wedged in the hole opening causing the hammer to pull in towards the shell of the mill. This action caused the employee's hand to become pinched between a mill rail and the trigger of the hammer. This caused severe pinching of all four fingers on left hand.

The employee was installing electrical cables into an electrical tray when he was struck by a ragged piece of limestone approx. 4"x3"x2" that fell from the 957 conveyor belt approx. 45'1. This caused an abrasion and swelling to his left side just above his waist.

The employee was sorting balls, moved the portable magnet 3'x 6" W x 2.5" thick, grasped the edge of the magnet with his right thumb being on the bottom of the magnet. Employee rotated the magnet toward himself and the magnetic force pulled it down on its bottom edge pinching employee's thumb between the magnet and the frame of the ball sorter.

The employee was shearing plate steel on metalworker. Employee attempted to catch a piece of steel from falling and pinched and lacerated his right index finger.

Employee was welding and grinding coal mill chute repairs at that time, but no symptoms were noticed until 2/5/10.

Employee alleges he was pulling on and twisting a 6" vacuum hose to straighten it out prior to loading it onto the skidsteer, at which time employee alleges feeling a pulling sensation on the right side of his back.

Employee stepped onto elevated bucket (approx. 2ft.) of skidsteer to inspect the back up alarm. His foot slipped on the skim of snow that was on the bucket, and he fell off the bucket landing on his left forearm and upper left side of back.

The employee was climbing down a small ladder at 32 airslide. The removable bottom 3 rungs had been removed to acess work in the area. Employee did not see that the rungs were removed and started down the ladder and noticed rungs were out and caught himself twisting his right knee.

The employee was entering the electric shop doorway carrying a UPS, weighing approximately 40lbs. The UPS slipped from the employee's hands and landed on his right foot.

The employee was assisting in moving the sample splitter table using a plastic pallet. It got stuck in the doorway. The employee pushed on the table legs, and his right hand got caught between the metal plate on the table leg and the door frame, causing an amputation to the distal phalanx of the ring finger and a laceration to the index finger.

The employee was pulling on a 4" vacuum hose to reach a pile of material. While pulling on the hose the employee rotated his waist from far right to left - when he did this his left foot slipped on some material twisting his left knee.

The employee was changing a light bulb. The locking mechanism on the 8' step ladder buckled causing the ladder to fold up. The employee fell to the left side landing on a 3' safety railing causing a contusion to left thigh and chest.

Employee alleges that during the operation of replacing 2 screen plates (50lbs. each), he felt a twinge of pain in neck and left arm, which resulted in further treatment.

The employee was removing and replacing grizzly bars from the cooler at the clinker breaker. The employee felt pain in his left shoulder. This was not previously reported as the employee did not seek medical attention utill 12/1/08.

Employee was descending 4 steps at the end of the packhouse worm screws Alley #1. He stepped on the bottom landing and his left ankle twisted underneath his weight. After which he felt a sharp pain from the ankle to the calf area.

While removing bolts from the burner pipe, the employee felt a pain in his upper back on the left side.

The employee was using a 20 pound sledge hammer to remove finish mill 1 doors, when he felt a pain in his lower right side of back.

The employee was rapping out silo 29E with a sledge hammer when the hammer veered off the silos and the employee felt a pull in his lower back.

The employee pulled on the hopper to latch it and felt something pop in his left shoulder

The employe was pulling wire from the local disconect. The wire became stuck. In the process of moving back to untangle the wires, the employee lost his footing and grabbed the side of the dust collector jamming his left index finger.

Employee was removing the separator fan housing when he felt a sharp pain in his lower back.

The employee lifted a bucket with chains in it, felt pain in back

The employee was walking on the side of the truck scale watching a truck pull in to load and tripped on the supersucker hose coiled up on the floor.

The employee used a metal bar to loosen tight valve. Bar slipped and struck employee's forehead.

The employee was trying to unplug a 6" FastVac hose with a 5' pipe. While swinging at the hose he missed it and struck the inside of his right foot, causing a contusion.

Employee slipped on ice. Dislocated rt shoulder.

Employee slipped on icy surface. Sprain/strain r knee

The employee was walking down the steps from the parking lot to the locker room building. It was raining heavily and the employee's left foot slipped off the wet bottom step and he twisted to catch his balance.

The employee was relocating an Acetylene bottle when his left hand became pinched between the elevator door and the Acetylene bottle, causing a contusion to his left hand.

The employee was replacing Roller Mill Roll bearing cover; weld on pick eye broke and caused cover to strike employee on left forehead, causing a laceration and contusion to forehead.

The employee was removing a temporary cable from under the kiln. The employee twisted and felt a pain in his back.

The employee was dumping cement bags into a hopper. While dumping the bags the employee strained his left wrist and forearm.

The employee was grinding welds in the kiln. The grinder caught his left hand, causing a laceration.

The employee was loosening the knuckle on a line blind valve to make a bin change. While changing the bin the employee felt a twinge in his lower back.

The employee was cutting a weld using a 4" hand-held cutting wheel. The wheel broke apart and a piece of the wheel came off through the employee's glove and caused a laceration on his left hand.

The employee was opening Elevator A boot door to complete a mechanical repair, when fine dust material flushed out of the door and entered boot, causing a burn to the left foot/ankle.

The employee was working on the bucket elevator. While lifting bucket elevator with chain fall the employee allegedly felt a sharp pain in his right shoulder.

The employee was breaking open a cement bag, when his arm hit a sharp edge on the saddle that the bags sit in when loading - causing a laceration to his lower right arm.

EE was opening railcar covers for loading. As EE traveled length of the RR car, lanyard began to extend downward. Lanyard got stuck at attachment point of cable. Lanyard began to deploy. Once fully deployed, lanyard became taut, suddenly stopping EE as he traveled from one RR car to the next, causing EE to lose balance and fall (fell to pavement next to railcars).

The employee was removing a clogged 6" line from the Fast Vac (vacuum truck). While lifting on the line, the employee felt a sudden pain in the lower right side of his abdomen that traveled down to the groin area. Note: Injury became a lost time 6/1/05 due to hernia surgery on 6/1/05.

The employee was cleaning up dust in the elevator pit. While cleaning, some of the dust got into his boot (the employee was wearing half socks), causing a burn to the inside of the right ankle.

The employee was helping to remove the 1935 drag chain. Not realizing the chain was caught on a piece of angle iron, the employee looked into the opening on the west end of the Polycom to see what the chain was catching on. As the employee did this he leaned his hand on the chain and it came loose, striking his right thumb.

The employee was working with an air operated heavy impact wrench to tighten a nut. The tool came off while rotating and struck employee's right hand. Injury is now resulting in Lost Time effective 5/9/2005 due to surgery on that date.

The employee was walking down the stairwell in the Polycom Building. The employee slipped on clinker nodules and twisted his left ankle.

The employee was walking towards the east exit door of the yard break room area when he tripped on a pump sitting on the floor and twisted his right knee. Per call to company on 1-6-04, he fell against the pump, he did not fall to the floor. coder 12 This has become a reportable incident as of the doctor visit of 12/7/2004 at which time fluid was drained from employee's knee.

This employee is not on the payroll of Glens Falls Lehigh Cement. He is employed through Keena Temp Agency. The employee was shoveling material into a 5 gallon bucket, when he felt a sharp pain in his left leg/groin area.

Note: This employee is not on the payroll of Glens Falls Lehigh Cement. He is employed and paid through a Temp Agency. The employee was cleaning up at Off-spec 2 area when he slipped on clinker and fell backwards against the guard rail, fracturing ribs on left side.

The employee was working to empty material from off-spec 2 silo, and was securing a small door on the NE side of the bin, when hot material flushed out the door onto the employee.

The employee felt a sharp pain in his right groin area while lifting an Oxygen bottle to put it into the bottle rack on the truck.

The employee was climbing into the Bobcat and slipped due to muddy conditions, which resulted in a Left Knee strain.

Employee's left knee allegedly locked up and twisted when walking on finish mill floor, carrying tools to truck. This report is just being filed as no lost time occured until surgery date of 7/16/04.

THE EMPLOYEE WAS JACKHAMMERING IN THE COOLER, WHEN DUST GOT INTO BOTH EYES.

THE EMPLOYEE WAS WORKING ON 953 BELT. AS HE PULLED ON THE BELT HE FELT PAIN IN HIS LOWER BACK.

THE EE WAS WALKING THRU THE PARKING LOT DOWN A SMALL SLOPING SIDEWALK INTO THE PALNT. THE EE ALLEGEDLY SLIPPED ON SNOW FALLING ON HIS BACK, NECK AND BACK OF HEAD.

THE EMPLOYEE WAS IN THE PROCESS OF WALKING UP THE STAIRS TO THE PACKHOUSE, CARRYING HIS TOOLBOX. THE EMPLOYEE RAISED HIS LEFT LEG TO STEP UP AND HIT HIS TOE ON THE STEP FACE, CAUSING EXTREME P RESSURE ON HIS LEFT KNEE.

The employee was using the Fast Vac to clean up piles of coal on the 2nd floor of the Coal Mill. The employee allegedly received a static shock; dropped the hose and went to backup. When the employee backed up he allegedly tripped on the Fast Vac hose and fell. He allegedly twisted his right leg and felt pain in his right knee.

THE EE BENT DOWN TO UNLOCK SWITCH GEAR: LIFTED UP ON SWITCH TO RELEASE LOCK, AND FELT SHARP PAIN IN MIDDLE LOWER BACK.

THE EE WAS LIFTING BROKEN CEMENT BAGS OUT OF THE HOPPER THAT IS USED FOR WASTE CEMENT, AND PULLED A MUSCLE IN LEFT SIDE OF NECK.

THE EE IS NOT SURE WHEN THE INJURY OCCURRED. THE EE ALLEGEDLY NOTICED SPOTS ON HIS CHEST AFTER THE JULY 4TH HOLIDAY WEEKEND. THE EE NOTIFIED US ON MONDAY, 7/7/03. PRIOR TO NOTICING THESE SPOTS , THE EE HAD BEEN WORKING IN DUSTY, HOT WEATHER CONDITIONS.

THE EE STATES THAT HE HAD COME IN FOR AN EXTRA DAY OF WORK, AND WAS CLEANING UP ON THE EAST SIDE OF THE CONVEYER BELT. THEY WERE USING THE FAST TRAC TRUCK AND A CHIPPING BAR TO PRY UP ON MATER IAL. THE EE ALLEGEDLY STOOD UP AND STRUCK HIS HEAD AGAINST A METAL CROSS BEAM, JAMMING HIS HEAD DOWN INTO HIS NECK.

THIS EMPLOYEE WAS HELPING TO REMOVE A 1/4" STEELCOVER FROM THE PALLETIZER. THE COVER SLIPPED OFF THE LEDGE AND PINCHED THE EE'S RIGHT MIDDLE ANDINDEX FINGER AGAINST THE RAILING, CAUSING A CUT TO THE MIDDLE FINGER AND A SCRAPE TO THE INDEX FINGER.

THE EE WAS SWEEPING THE BURNER FLOOR. THE EE ALLEGEDLY TURNED TO THE RIGHT WHILE SWEEPING & ALLEGEDLY FELT SOMETHING PULL ON HIS RIGHT SIDE. NOTE: THIS EE HAS A PRE-EXISTING HERNIA ON HIS RIGHT SIDE, WHICH IS NOT WORK RELATED.

THE EE WAS USING A CHAIN WRENCH TO HOLD THE #3 FAN FROM ROTATING, WHILE A SHEAVE ALIGNMENT WAS PERFORMED AND HEAT SLINGER INSTALLED. WHILE REMOVING THE WRENCH THE BACK FLOW PRESSURE FROM THE C OOLER CAUSED THE FAN TO ROTATE AND THE WRENCH PINCHED THE EE'S FINGER BETWEEN THE WRENCH HANDLE AND FAN BASE.

THE EMPLOYEE WAS ALLEGEDLY SHOVELING MATERIAL UP IN THE FINISH MILL. THE EMPLOYEE ALLEGEDLY PICKED UP A SCOOP OF MATERIAL, TURNED AROUND TO PUT IT IN THE BOBCAT AND ALLEGEDLY FELT A PULL IN HI S LOWER LEFT BACK AND LEG. *NOTE: THE EMPLOYEE SOUGHT MEDICAL ATTENTION ON HIS OWN WITHOUT EMPLOYER'S KNOWLEDGE. 4/3/03 WAS THE FIRST TIME EMPLOYER WAS INFORMED OF MEDICAL ATTENTION AND THAT

THE EE WAS USING A 35 LB JACKHAMMER IN THE COOLER. THE JACKHAMMER WAS STUCK IN MATERIAL. AND THE EE WAS SWAYING THE JACKHAMMER BACK AND FORTH TO GET IT LOOSE. WHILE SWAYING IT BACK AND FORTH, HE JAMMED HIS LEFT PINKY FINGER BETWEEN THE JACKHAMMER AND THE WALL.

THE EE WAS REBUILDING RETURN ROLLS. THE EE'S LEFT PANT LEG CAUGHT ON FIRE DUE TO CLOSE CONTACT WITH A GAS HEATER NEARBY, CAUSIN A 2ND DEGREE BURN TO LEFT LEG BEHIND KNEE.

THE EE SLIPPED ON ICE UNDER DUST, FELL BACKWARDS AND USED LEFT ARM TO BREAK FALL.

THE EE WAS TRYING TO REMOVE BEARING HOUSING FROM THE FINISH MILL 2 FK PUMP WITH HYD. PULLER. THE PULLER WAS SUSPENDED WITH A NYLON SLING FROM THE FORKLIFT. FORCE WAS APPLIED TO THE PULLER VIA ELECTRIC PUMP. THE HOUSING FRACTURED & THE PULLER BROKE LOOSE UNDER TENSION. THE RELEASE OF PRESSURE CAUSED THE PULLER TO FLY IN AN ARCH, STRIKING THE EE IN THE RIGHT SHIN (TIBIA).

THE EMPLOYEE WAS LIFTING A HELIUM TANK ONTO THE TANK CARRIER, WHEN HE ALLEGEDLY STRAINED HIS BACK.

THE EE'S LEFT MIDDLE FINGER BECAME PINCHED BETWEEN THE HEEL OF THE FORK & THE BACK REST ON THE FORKLIFT.

THE EMPLOYEE WAS MOVING THE TENSIONING SPRING OF THE CLINKER CRANE TROLLEY BRAKE. WHILE MOVING THE TENSIONING SPRING OF THE CLINKER CRANE TROLLEY BRAKE, EE FAILED TO UNLOAD SPRING TENSION PRIO R TO REMOVING CRANE BRAKE SHOE. EMPLOYEE MOVED THE TENSION BAR, THE SPRING RELEASED AND CAUGHT EE'S RIGHT FOURTH FINGER BETWEEN THE SPRING AND THE ADJUSTING NUT, CAUSING U SHAPED LACERATION.

THE EE WAS HELPING TO CLEAR A PLUGGED FASTVAC LINE IN THE FINISH MILL BUILDING. THE EE'S PARTNER DROPPED A 3 1/2" STEEL BALL THROUGH THE PIPE TO CLEAR THE BLOCKAGE. WHEN THE STEEL BALL DISCHAR GED THE LOWER END OF THE PIPE, IT HIT EE IN THE RIGHT SIDE OF THE HEAD.

THE EMPLOYEE WAS CLOSING THE GATE ON THE CEMENT BIN. CEMENT IN THE BIN CAVED IN AND SHOT DOWN THE CHUTE WITH FORCE, ALLOWING CEMENT TO SQUIRT OUT ALL SEAMS, AND SOME DUST GOT INTO EMPLOYEE'S LEFT EYE.

THE HOPPER FROM THE DUST COLLECTOR TO THE ROTARY AIR LOCK WAS PLUGGED. THE EMPLOYEE WAS ATTEMPTING TO CLEAR THE PLUG BY PUSHING THE MATERIAL THROUGH. THE MATERIAL GAVE, LOOSENED AND WENT THROU GH, AS WELL AS THE EMPLOYEE'S FINGER, WHICH WAS STRUCK BY THE BLADE OF THE ROTARY FAN. THE DATE OF THIS REPORT IS THE FIRST EMPLOYER WAS ADVISEDOF THIS INJURY.

THE EE WAS ALLEGEDLY USING A JACKHAMMER TO CLEAN UNDER THE GRATES ONT HE COOLER ROOF. THE JACKHAMMER ALLEGEDLY GOT STUCK AND WHEN THE EE ALLEGEDLY PULLED THE JACKHAMMER BACK, HE ALLEGEDLY FELT A SHARP PAIN IN HIS BACK.

ELECTRICIAN WAS CUTTING RIGID CONDUIT W/BANDSAW,WAS WEARING GLOVES & EYE PROTECTION. AS CUT WAS ALMOST DONE THE CONDUIT BENT OVER. CONDUIT LENGTH BEING CUT OFF WAS 4" LONG. WHILE THE SAW WAS S TILL ON, ELECTRICIAN GRABBED STUBBED PIECE UNDER THE SAW. GLOVE GOT CAUGHT & CUT BACKSIDE OF LEFT HAND REQUIRING 4 STITCHES.

THE EE WAS WALKING DOWN THE COAL CRUSHER STAIRWAY WITH A TOOLBOX OVER HIS SHOULDER. THE EE ALLEGEDLY TURNED JUST RIGHT & STATED THAT "HIS BACK WENT OUT ON HIM", ALLEGEDLY AGGRAVATING AN ONGOIN G BACK PROBLEM.

WHILE THE EMPLOYEE WAS SHOVELING INTO THE 1104 DRAGLINE, THE TIPPING VALVES OPENED UP, BLOWING DUST OUT OF THE INSPECTION COVER, HITTING EMPLOYEE IN THE EYES, CAUSING A CORNEAL ABRASION TO THE LEFT EYE.

WHILE THE EMPLOYEE WASHOVELING INTO THE 1104 DRAGLINE, THE TIPPING VALVES OPENED UP, BLOWING DUST OUT OF THE INSPECTION COVER HITTING THE EMPLOYEE IN THE EYES, CAUSING AN ABRASION TO BOTH EYES.

THE EMPLOYEE WAS RUNNING CEMENT IN ALLEY 4. CEMENT IN THE BIN FELL FROM THE SIDES OF THE BIN AND MADE AN UNCONTROLLABLE RUSH AT THE GATES. THE EMPLOYEE CLOSED THE GATES BUT THE CEMENT SQUIRTED OUT OF THE CHUTE ALLEGEDLY HITTING THE EMPLOYEE IN THE RIGHT SIDE IF THE FACE, CAUSING CEMENT DUST TO GET BEHIND HIS SAFETY GLASSES INTO HIS RIGHT EYE.

THE EMPLOYEE WAS CUTTING BELTING WITH A UTILITY KNIFE, WHEN THE UTILITY KNIFE SLIPPED CAUSING A LACERATION TO THE EMPLOYEE'S ABDOMEN.

THE EE WAS WORKING ON THE BLENDING GATE. WHILE WORKING ON THE BLENDING GATE, CEMENT DUST FELL FROM A PIPE OVERHEAD AND WENT INTO THE EE'S LEFT EYE.

THE EMPLOYEE WAS SHOVELING IN THE MIAG BUILDING WHEN HE ALLEGEDLY FELT PAIN IN HIS BACK.

EE ALLEGEDLY PICKED UP A 40 LB BAG OF SPEEDY DRYFROM THE FLOOR WHEN HE FELT A PAIN IN HIS BACK.

NOTE: THIS IS NOT AN INJURY REPORT. THE EMPLOYEE HAS A SIGNIFICANT HEARING LOSS IN HIS LEFT EAR IN COMPARISON WITH LAST YEAR'S TESTING. THIS LOSS IS NOT RELATED TO ANY SPECIFIC INJURY DATE OR CURRENT JOB EXPOSURE. INDICATION FROM MEDICAL PERSONNEL PERFORMING THIS HEARING TEST IS THAT THE EMPLOYEE'S HEARING LOSS IS PROBABLY DUE TO A MEDICAL PROBLEM.

EE WAS CLIMBING OVER THE CEMENT LINES TO CLOSE AVALVE. HIS RIGHT LEG WAS STATIONARY ON THE BOTTOM PIPE LINE AND HE THREW HIS OTHER LEG OVER THE UPPER LINE AND TWISTED HIS RIGHT KNEE.

THE EMPLOYEE WAS TURNING THE DRIVE SHAFT ON THE 1932 ROLL BY HAND. WHILE PULLING ON THE DRIVE SHAFT, THE EMPLOYEE FELT A SHARP PAIN IN HIS RIGHT ARMPIT AND SHOULDER AREA.

THE EE WAS OPERATING THE BIG BOBCAT TO CLEAN UNDER THE FINISH MILL FEED BELTS. WHILE CLEANING IN THE AREA, DUST CREATED BY THE BELT CONVEYORS AND OPERATION OF MOBILE EQUIPMENT GOT INTO EMPLOYE E'S RIGHT EYE.

THE EE HAD POSITIONED HIS FEET ON THE GRIZZLY BARS AND THE LOWER LIP OF THE COOLER DOOR FRAME, AND HIS BODY WAS UP AGAINST THE SOUTH WALL, IN ORDER TO USE A SLEDGE HAMMER TO BEND A PIECE OF ST EEL. WHENT HE EE SWUNG THE SLEDGE HAMMER TO STRIKE THE PIECE OF STEEL, THE SLEDGE HAMMER MISSED THE TARGET AND HIT THE INSTEP OF THE EE'S RIGHT FOOT.

EE DROPPED OFF A PALLET OF BLUE RAM AT THE FEED END OF THE KILN WITH THE FORKLIFT. AS EE WAS BACKING THE FORKLIFT OUT OF THE KILN THE TIRES HUNG UP IN A LOAD OF LOOSE MATERIAL THAT HAD BEEN CL EANED OUT OF THE FEED CHUTE EARLIER. WHILE STANDING ON LOOSE MATERIAL TRYING TO PLACE WOOD UNDERTIRES FOR TRACTION, MATERIAL SLID OUT FROM UNDERHIM ALLOWING HIS LEG TO GO STRIKE FORKLIFT FORK

THE EE WAS REMOVING THE COUPLING FROM THE CLASSIFIER REDUCER, WHEN THE COUPLING DROPPED PINCHING THE EE'S FINGER BETWEEN THE COUPLING AND THE MOTOR BASE PLATE.

THE EMPLOYEE WAS CONCENTRATING ON TAKING MEASUREMENTS BETWEEN THE TABLE AND HOUSING INSIDE THE ROLLER MILL. HE STEPPED INTO THE REJECT CHUTE ON THE WEST SIDE, CAUSING HIM TO FALL INTO THE CHUT E AND HIT HIS RIGHT SIDE ON THE CHUTE.

THE EE ALLEGEDLY CAUGHT HIS FOOT ON THE FORKLIFT WHEN HE WAS DISMOUNTING AND FELL ONTO THE FLOOR. HE ALLEGEDLY CAUGHT HIMSELF WITH HIS RIGHT HAND ON THE FLOOR ALLEGEDLY CAUSING PAIN IN HIS RIG HT WRIST AND SHOULDER. NOTE: INCIDENT ALLEGEDLY OCCURRED ON 3-16-01. HOWEVER, THE EE NEVER REPORTED IT UNTIL 3-28-01.

THE EE WAS INSERTING PINS IN THE PREBLEND FEEDER. THE EE HAD INSERTED THE PINS BY HAND, AND WHEN FELLOW EE WAS SWINGING THE 8# SLEDGEHAMMER TO DRIVE THE PINS IN THE HEAD CAME OFF THE HANDLE OF THE HAMMER, STRIKING THE EE'S LEFT THUMB.

THE EE PICKED UP ABAG OF CEMENT AND CARRIED IT INTO THE LAB. THE EE SAID HE DID NOT FEEL ANY PAIN WHEN HE PICKED UP THE BAG. ALLEGEDLY, AS THE DAY WENT ON THE EE'S BACK AND LEGS ACHED MORE.

THE EE BENT OVER TO PICK UP A 50 LB BAG OF SAND FROM FLOOR LEVEL, WHEN HE ALLEGEDLY FELT A PAIN IN HIS LWOER LEFT SIDE OF BACK.

THE EE WAS SHOVELING A PILE OF STONES OFF THE ROLLER MILL FLOOR INTO THE BOBCAT BUCKET, WHEN HE FELT A SHARP PAIN IN HIS LOWER BACK.

WHILE STACKING KILN BRICK, THE EMPLOYEE STEPPED BETWEEN 2 PALLETS TO CUT AWAY SOME PLASTIC COVERING. WHEN HE MOVED BACK, HIS LEFT FOOT GOT CAUGHT UNDER A PALLET WHICH CAUSED HIM TO LOOSE HIS BALANCE AND FALL ON HIS LEFT SIDE WITH HIS FOOT STILL UNDER THE PALLET.

THE EE WAS ALLEGEDLY BENDING OVER TO PICK UP A CEMENT SAMPLE BUCKET, WHEN HE ALLEGEDLY FELT PAININ RIGHT SIDE OF LOWER BACK

THE EMPLOYEE WAS WALKING AND SLIPPED ON SNOW COVERED GROUND, CAUSING HIM TO FALL AND LAND ON HISRIGHT SHOULD.

EE WAS REMOVING CHAIN BINDERS FROM A ROLL ON THECONTRACTOR'S TRUCK WHEN THE CHAIN BINDER DISENGAGED THROWING EXTENSION PIPE WHICH STRUCK EE ON RIGHT KNEE CAP.

THE EMPLOYEE WAS ASSISTING IN MEASURING A PIECE OF FLAT STOCK ON THE IRON CRAFTER SHEAR MACHINE.AFTER HE MEASURED THE PIECE OF METAL & WITH HIS MIDDLE LEFT FINGER STILL UNDER THE FLAT STOCK, THE OTHER EMPLOYEE ACTIVATED THE HYDRAULIC LEVERWHICH SNAPPED THE METAL DOWN ONTO HIS FINGER. HERECEIVED A LACERATION.

THE EMPLOYEE WAS CHANGING OUT THE BLUELR MILL & CAUGHT HIS LEFT INDEX FINGER ON A SHARP PIECE OFMETAL.

EE WAS OPENING A GATE IN THE CEMENT LINE WHEN PRESSURE LEFT IN THE LINE CAUSED A PUFF OF CEMENT DUST TO BLOW INTO BOTH EYES.

THE EMPLOYEE WAS WORKING IN KILN FEED SILO #2 WHEN HE SLIPPED ON THE FLOOR CAUSING HIM TO FALLONHIS LEFT KNEE.

THE EE WAS INSTALLING THE COVER ON THE #1 FINISH MILL JOURNAL AND WAS SWINGING THE SUSPENDED PIECE INTO PLACE, WHEN HIS RIGHT MIDDLE FINGER WAS PINCHED BETWEEN THE COVER AND THE MILL END BOLTS , CAUSING A LACERATION AND FRACTURE TO THE TIP OF THE MIDDLE FINGER OF HIS RIGHT HAND.

THE EMPLOYEE WAS WORKING ONT EH BURNER PIPE, WHE HE TAPPED ON THE BURNER PIPE, DUST FELL INTO THE EMPLOYEE'S LEFT EYE.

THE EMPLOYEE WAS USING A SLEDGE HAMMER TO LOOSEN THE DOOR ON THE FINISH MILL. WHEN USING THE SLEDGE HAMMER THE EMPLOYEE ALLEGEDLY FELT A SHAR PAIN IN HIS BACK.

EMPLOYEE WAS USING A 35 LB. JACKHAMMER TO REMOVE REFRACTORY FROM 3A CYCLONE, WHEN HE ALLEGEDLY FELT HIS RIGHT ELBOW BECOMING SORE.

EMPLOYEE WAS FILLING CEMENT BAGS WHEN ONE BROKE IN FRONT OF HIM, CAUSING DUST TO GO INTO RIGHT EYE.

THE EE WAS LIFTING A BAG OF CEMENT WHEN HE FELT A POPPING IN THE LEFT SIDE OF HIS NECK.

THE EE WAS MAKING A BIN CHANGE. WHENT HE EE LOOSENED A GATE, THE GATE SPUN AROUND AND HIT HIS RIGHT KNEE.

THE EMPLOYEE WAS OPENING A GATE VALVE WHILE MAKING A BIN CHANGE, AT WHICH TIME THE BAR THE EMPLOYEE WAS USINGSLIPPED OFF THE NUT AND STRUCK HIS LEFT THUMB, CAUSING A FRACTURE AT THE TIP OF HTE LEFT THUMB.

THE EE WAS ALLEGEDLY CLIMBING THE EAST CLASSIFIER LADDER WHEN THE TOPRUNG CAME OFF. THE EE ALLEGEDLY CAUGHT HIMSELF BY GRABBING THE LADDER FRAME AND STRAINED HIS LOWER BACK.

THE EMPLOYEE WAS WALKING DOWN THE STAIRS GOING TO THE 1104 DRAG LINE, WHEN HE ALLEGEDLY SLIPPED ON APIECE OF STONE OR CLINKER. THE EMPLOYEE ALLEGEDLY FELT A PAIN IN HIS LOWER MIDDLE BACK.

THE EE WAS MANUALLY TURNING ON THE AIR CANNONS TO THE UPPER STAGE VESSELS ON THE PREHEATER TOWER. WHEN THE AIR CANNON DISCHARGED INTO THE VESSEL, HOT MATERIAL ESCAPED OUT OF THE TIPPING VALVE HITTING HIM IN THE LEFT EAR.

THE EE WAS LIFTING THE COVER ON THE HYDRAULIC JACKING UNIT WHEN HE ALLEGEDLY FELT PAIN IN HIS LOWER BACK AND LEFT LEG.

THE EE WAS WALKING FROM THE PARKING LOT, STEPPED OVER THE RAILROAD TRACK. SHE STEPPED ON AN UNSTABLE PIECE OF CONCRETE, WHICH CAUSED HER TO FALL ON BOTH KNEES. WHILE FALLING, THE EE FELT SOMET HING SNAP IN HER LEFT THIGH AREA.

THE EE RAN TO HELP ANOTHER EE WHO HAD FALLEN. WHILE RUNNING TO CALL 911 AND RETRIEVE HIS MEDICAL EQUIPMENT FROM HIS TRUCK, THE EE FELT SOMETHING SNAP IN HIS LEFT FOOT.

MAN WAS OPERATING OVERHEAD CRANE AND WAS HOLDINGON TO CRANE HOOK, WHEN THE CABLE CAUGHT ON SOMETHING, WHEN HE PULLED IT HIS THUMB WAS ON THE UP SWITCH AND THE HOOK PINCHED HIS SMALL FINGER BET WEEN THE BLOCK AND THE BALL MILL DRUM, THE CABLEWAS ATTACHED TO.

THE EE HAD JUST FINISHED CUTTING OFF BOLTS TO THE 1932 & 1931 MTR MTS. HE REACHED UP WITH HIS LEFT HAND TO BRUSH THIS MATERIAL OFF AN I BEAM & THE HOT METAL & SLAG MATERIAL FELL INSIDE OF T HE WELDING GLOVE ONTO HIS RIGHT WRIST AREA

THE EE WAS ALLEDGEDLY DOING A REPAIR JOB ON THE KILN LINING MATERIAL. A SMALL PIECE OF MATERIAL ALLEGEDLY FELL FROM OVERHEAD. ALLEGEDLY HITTING THE EMPLOYEE ON THE LEFT SHOULDER.
