Quartz / Feldspar Operation Metal/Non-Metal

Employee came to work early to clean roadways and apply salt following an ice storm on 02/15/2016. After proceeding only two or three steps, the employee slipped on the ice, causing a pop sound and pain in EE's left lower leg. Employee was transported to the hospital by ambulance where X-ray revealed a fractured left tibia which required surgery.

Employee was lifting 1/4 inch flat bar, when it slipped from his hand & bent his left ring finger back. Employee stated it was a little stiff but thought he would be fine. Operator started documentation at that time. Weeks later he stated it was still stiff & we took him to be checked out. The doctor examination found palpation tenderness over 1A pulley. Diagnosis on 03/24/2015.

CSX (Railroad company) was working on the tracks using a cutting torch and a spark ignited dry foliage causing a brush fire. The fire ran onto our property and burnt estimated 15 to 20 acres. The fire was contained by the N.C. Forest service and local fire department, No employees were in danger nor was there any building damaged.

Employee injured removing tailings waste from a disc filter. In an effort to dislodge built up material. Additional material fell from the disc filter onto the cover. This falling material stuck the cover and catapulted his hand into a piece of metal which resulted in an injury to the employee's right index finger. This resulted in 5 to 7 days of modified duty.

Employee was checking belts on flotation cell. He stepped over the diffuser shaft to get to the other side when his foot slipped. He was trying to catch his balance and twisted his knee .The employee did not report the incident until 12/14/11 at 7:45 am. The employee has not missed any work but is on modified duty until further notice.

Employee was grinding welds with a grinder while wearing safety glasses and a full face shield. It appeared some of the grind must have bounced off the metal wall behind him and entered from the side.

Putting an agitator in place. Lifting with a winch and shaft of agitator moved. The employee grabbed the shaft to control it and it yanked his left arm.

Injured employee was working from a scissors lift and was pulling wire into a cable tray as part of a new building installation, when he felt a sharp pain in his lower abdomen. He was treated at an urgent care and diagnosed with a hernia. He was then referred to a surgeon. He is continuing to work until his appointment on 05/03/2011.

Employee was connecting steel on the west stairs approximately 16' above the ground while tied off 100%. He lost his balance and fell, and in the process of the lanyard arresting his fall, he reached out with his left arm to grab the steel and felt something pull in his left shoulder.

Two employees were working together taking concrete forms down and cleaning and stacking them. One employee was using a hammer to remove the concrete from the form and the injured employee thought he was done, grabbed the form and was hit on the pinky finger. The employee did not see his hand there until it was too late. It broke his pinky finger.

Employee was walking on a concrete slab and slipped & fell. Upon investigation, it appeared he had stepped on a bolt that caused him to slip & lose his balance, resulting in the fall.

EE was using a razor type utility knife to cut a piece of flexible rubber tubing. He used a technique that let the knife slip back and puncture his left forearm upon completion of the hose cut. 3 stitches were required to close the wound.

Employee was cutting expanded metal for guard and jammed his thumb into the metal causing a laceration.

Operator was attempting to straighten out 4" hose. When he turned the hose, it uncoiled and the reinforcing wire cut his small finger on his left hand.

EE had gone to 3350 level to fuel mine mobile equipment as he got out of fuel truck, wind blew dust particles into left eye.

Attempting to position drum on pallet and caught left ring finger between another drum. Metal rim ring lcaerated finger.

EE was cleaning out feed chute & dust particles got into right eye.

The accident occurred at the 6 x 10 Rod Mill. The floor condition was dry. The employee was removing rods from the rod mill. When the rods were removed the employee and a co-worker would stack the rods on a fork-lift. At the time of the accident the employee failed to move his hand and his hand was pinched between two rods.

EE was helping stack 50# bags on export pallet. Not his regular job.

Reaching over a belt conveyor housing to unplug a feed chute.

Replacing shelf in storage cabinet - cut right index finger.

REPLACING A 2 15/16" METAL SHAFT, SHAFT SLIPPED CATCHING HIS MIDDLE LEFT FINGER BETWEEN SHAFT BEARING AND MOUNTING FRAME. HE WAS CALLED BACK TO WORK AT 8:00PM.

EE WAS DISMANTLING THE CONE BOWL WORK LINER. LINER IS CEMENTED IN PLACE & MUST BE JARRED LOOSE TO REMOVE. THE LINE EDGE CANNOT BE REACHED WITH SLEDGE, THEREFORE THE SLEDGE IS IMPACTED ON THE E DGE. EE SWUNG THE 16LB SLEDGE ABOUT 5 TIMES BEFORE HE FELT SOMETHING GIVE IN LOWER BACK & HE QUIT WORKING. HE THEN USED HAMMERDRILL Y

MONDAY A.M. ROUTINE SERVICE WAS BEING COMPLETED BY EE. THE MILLS AND RELATED MACHINERY WERE SHUT DOWN. HE OPENED THE SAFETY GATE, REACHED AROUN BELOW THE MILL FEED CHUTE TO OPEN THE TRUNNION F ILL LID. HE STEPPED OFF THE DECK ONTO THE TRUNNION WITH HIS LEFT FOOT AND HELD THE SAFETY GATE OPEN WITH HIS RIGHT FOOT. HE FILLED THE 1 GAL. JUG OF 01 TO THE FILL AND FELT CATCH IN HIS BACK.

EE WAS MAKING REPAIRS TO PU-30 IN THE TUNNEL AT THE WTP. HE WAS IN THE PROCESS OF REMOVING THE PUMP FROM THE SUMP AND OUT PAST THE WIRE MESH GUARD WHEN HE FELT HIS BACK START TO HURT. HE IMMED IATELY SET THE PUMP BACK DOWN IN THE SUMP.
