Worker tripped on a set of steps falling forward. EE's right knee struck the platform grating and cut knee. The gash required 7 staples in the right knee.

Miner thought they might be having a heart attack. Transferred to the local emergency room.

Contractor was hired by Covia Corporation to provide a crane and crane operator to set a transformer on a pad next to the Covia maintenance building. After the work was completed and the crane operator was removing the rigging from the crane hook, the operator mis-stepped and fell from the crane deck to the ground, a distance of approximately five feet.

The worker was replacing a bad bearing on SR-02A. The worker had to perform lifting and hammering during the removal of the old bearing and installing the new. The front line maintenance worker's left elbow is sore. The new bearing weighs 87.5lb.

The operator said the injury came from the jerking motion of the loader when in operation.

A wheel loader caught fire and was destroyed due to fire.

The loader operator stated their back was hurting from operating the loader. The soreness and pain was located in the lower back.

The left front tire blew out on ME-02 loader. The worker jarred EE's back when the tire blew. The incident occurred 2/10/22 did not seek medical attention until 02/21/22

The employee called EE's supervisor reporting EE was not feeling well. Possible heart attack. 911 was called and the employee was transported to the hospital. The employee was examined at the hospital and released the same day. Reported back to work the next day.

The worker was striking the head of a bolt with a hammer to unplug a 1 ¼ X 4" pipe nipple. When the material inside the pipe nipple broke loose the impact of the bolt pinched the thumb of the left hand against the pipe nipple. The worker was transported to the local emergency room and received 5 stitches to the thumb.

The employee was complaining of not feeling well and chest pains. The employee notified supervisor and it was determined to transport the employee to the hospital to be examined.

A reportable hearing loss has been noted in the employees right ear. The audiometric test results were reported 03/13/2019. Calculations for this loss include age correction values. The employee is scheduled for a audiometric test April 8,2019.

The employee ascended a set of steps. All the employee remembers is picking up hardhat at the bottom of the stairs and realized head was hurting. The employee had fallen and does not remember where and how employee fell. The employee suffered a concussion.

The employee was stepping of the mine sump pad and slipped when they made contact with the ground surface. To prevent from falling they held onto a hose reel and sprained their lower back.

The worker was in the process of re positioning a 3 inch pump discharge line on PU-53. The line weighed 10lb or less. During the lifting process EE placed both hands under the pipe and locked EE's fingers in place and EE felt a sharp pain in EE's right bicep. The worker thought it was a cramp but was transported to the emergency room and was diagnosed a torn bicep.

The EE walked into the front office and said ee was not feeling well. It was determined to call 911 and not transport self to the hospital. The employee was examined by the medical staff and transported to the local hospital where ee was admitted.

The mine loader operator was returning to the high wall when right arm hit the bucket controllers causing the bucket to drop to the ground, and resulting in a sudden stop of the loader. Due to the sudden stop the employee was thrown into the windshield, causing injury to the right eye.

The employee was not feeling well and notified plant personnel to call 911. 911 was dispatched immediately to the plant site. The employee was examined by the medical staff and transported to the local hospital where they were admitted.

Two employees were unbolting a 10 inch pipe, work was stopped due to an electrical storm. In route to the plant the employee told supervisor employee had tightness in chest tingling in arms and not feeling well. When they arrived back to the plant the supervisor made the decision to take employee to the hospital.

The employee was climbing down the FVL on BE-05 to take a lunch break when ee stepped off the ladder onto the 1st. elevated platform, ee felt a strain in left knee and heard it pop. After going home that evening the knee had swollen and was in pain. The next day the employee went to an urgent care facility and was diagnosed with a sprained left knee.

The miner became agitated and punched locker. Hand began to swell and miner left work and reported to the local medical treatment facility for treatment. Miner was treated for broken bones in hand. This incident was deemed to be "Horseplay" and non-work related after consultation with a workman's compensation representative.

The Employee tripped over a jack stand and landed on another jack stand with his left thigh. The top of the jack stand punctured through the employees pants and caused a laceration to the employees thigh. The incident was caused by poor work area planning. The employee received a laceration to his left upper thigh.

EE was installing metal conduit. While installing conduit the employee had one end secured in place with a rope as he was working on the other end. The conduit came loose from the rope securing it. The employee caught the piece of conduit and felt a sharp pain in his mid-back. EE is currently out of work on doctors orders until he can be evaluated by a back specialist.

The EE was adding antifreeze to loader radiator on backside of loader. As he finished, he stepped down off platform onto top ladder rung. He was holding railing with his left, hand he stepped onto top rung with his right foot. As he then stepped down to the second rung with left foot, he had a misstep, lost his grip on railing and fell 2 feet 9 inches to the ground on his knees.

The employee started sweating and complained of chest pains and arm numbness while he was hanging a bulletin board.

The EE climbed off a belt conveyor onto a platform, he turned to go unlock the belt, & his left knee popped. He reported the incident & continued working. His knee continued to give him trouble. He went to his doctor who referred him to a specialist. The specialist scheduled him for an MRI on 4/28/14. We were informed on 4/30/14 that he will be scheduled for surgery on a date TBD.

The employee was operating a loader when the left front tire blew out. The employee was jerked forward into the seatbelt. he went to the doctor, was given medication, and returned on his next scheduled shift. **On October 14, 2013 the employee was reassessed and assigned modified duty due to issues with his leg. He is expected to be out 2 weeks.

The miner has been tentatively diagnosed with a reportable hearing loss.

Disregarding instructions and a traffic sign, truck driver drove off the main traffic road and onto a cross-over road used by Plant personnel. Trucker exited his truck without instruction and started to attach the tow pin to the front part of the truck. The Loader Operator, not realizing or expecting a truck to be there backed into the trucker and truck while back-dragging road.

Employee was rolling out 100ft of 2in plastic pipe. The roll started falling and he changed his grip to pull it back upright and strained his arm. He originally reported the incident on 09/22/2011, but didn't go to the doctor until 10/05/2011.

The employee was opening a pump housing when the torque wrench he was using slipped off the nut. The wrench mashed the tip of his right hand ring finger against the pump housing and a few stitches were required.

On June 9th, the employee in question was attempting to remove a pump arbor from its cradle.After removing the arbor, he hear and audible pop from his elbow.He reported the incident, but did not have any serious pain until a month later.On July 7th, the employee was taken the hospital for diagnosis.After diagnosis, the employee as given a shot to relieve the pain on July 8th.

The employee allegedly slipped on a patch of ice while walking an alternate route to the maintenance ship. He was OK and no medical attention was needed. On 2/25, the employee claimed he was injured and placed on modified duty. This report is being filed even though there are serious doubts that this is work related. The case is being investigated by Workers Comp.

EE & ANOTHER WERE SANDBLASTING A 30,000 GL HORIZONTAL DIESEL TANK WHEN THE LTA OCCURRED. THE LADDER HE WAS USING WAS MOVED INSIDE THE DIKE & PROPPED UP AGAINST THE TANK. EE DID NOT WAIT FOR LA DDER TO BE SECURED. THE LADDER SLID OUT @ BOTTOM& STOPPED BY BASE. IMPACT THREW EE OF BUT LADDER STAYED UPRIGHT. HE FELL ON SIDE, BROKE ELBOW & PELVIS. HE HAD SURGERY & HAVING PHYSICAL THERPY

EE PARKED THE LOADER & PREPARED TO COME DOWN THE LADDER. AT SOME POINT HE LOST HIS BALANCE & FELL TO THE CONCRETE PAD BELOW STRIKING HIS HEAD. HE HAD A KNOT ON THE BACK OF HIS HEAD & BLEEDING OUT OF HIS RIGHT EAR. HE WAS TAKEN TO THE HOSPITAL & OBSERVED THREE DAYS THEN RELEASED. HE HAS NOT BEEN RELEASED TO RETURN TO WORK SINCE HE IS ON MEDICATION & GETS DIZZY.

EMPLOYEE STRAINED HIS BACK WHILE LEANING FORWARD AND UNBOLTING A FLANGE. THE FLANGE WAS NOT HEAVY BUT HE WAS IN AN AWKWARD POSITION. ON 10/17/02 HE REQUESTED AN X-RAY WHICH WAS NEGATIVE. HE WA S GIVEN A PRESCRIPTION FOR PAIN AND MUSCLE RELAXATION. HE CONTINUES TO PERFORM REGULAR JOB.

EMPLOYEE REPORTED ON 2/3/03 THAT HIS ELBOW STILL HURT WHEN LIFTING AND REQUESTED DOCTOR'S VISIT. HIS X-RAY WAS NEGATIVE. HE WAS GIVEN MEDICATION AND PHYSICAL THERAPY ASSESMENT. THERE WAS NO MO DIFIED DUTY OR LOST TIME.

HE WAS THROWING A RAILROAD SWITCH BEFORE THE CARHAD CLEARED. THE SWITCH SPRANG BACK AND BRUISED HIS LEG. HE REQUESTED A DOCTOR'S VISIT ON 5-14-02 BECAUSE IT WAS STILL SORE AND SWOLLEN. THE X-R AY AND DOCTOR'S REPORT SHOWED ONLY A BRUISE BUT DUE TO THE SWELLING AND SORENESS HE WAS GIVEN MODIFIED DUTY.

THE FRONT HOUSING OF A 10X8 DENVER SLURRY PUMP TIPPED OVER ONHIS HAND AND ARM. HIS ARM AND LITTLE FINGER WERE BROKEN AND HE LOST THE TIPS OF HIS MIDDLE AND RING FINGER THAT WERE CRUSHED. THE P UMP HOUSING WAS SITTING ON A CONCRETE PAD AND WAS SUPPORTED VERTICALLY BY A CHAIN HOOKED TO THE BOOM OF THE MAINTENANCE LOADER. HE WAS REPLACING A NUT ON THE BOTTOM OF THE PUMP WHEN THE CHAIN

HE WAS MOVING SAND WITH A SKID STEER LOADER. HE DROVE OUT OF THE PROCESS BUILDING OVER A 6" DROP TO THE DRIVEWAY. HE WAS HOOK IN THE PROCESS AND PULLED A BACK MUSCLE. THE X-RAY WAS NEGATIVE AND HE WAS TREATED FOR A PULLED MUSCLE IN HIS BACK.

HE CLOSED THE DISCHARGE VALVE ON A HIGH PRESSURE WATER HOSE AND THE NOZZLE BLEW OUT THE LINE CAUSING THE HOSE TO SLIDE THROUGH HIS HAND. AS THE LINE SLID THROUGH HIS HAND, THE HOSE CLAMP CUT H IS RIGHT HAND BETWEEN HIS THUMB AND INDEX FINGERTHE CUT WAS NOT TOO DEEP, BUT REQUIRED STITCHES.

HE WAS HAMMERING A BENT ELEVATOR BUCKET THROUGH THE ACCESS DOOR. HIS RIGHT RING FINGER WAS CAUGHT BETWEEN THE HAMMER HANDLE AND THE DOOR FRAME. HE WAS WEARING GLOVES BUT 4 STITCHES WERE REQUIR ED. HE RETURNED TO REGULAR DUTY.
