Injured employee was scaling built up material off the side of feed hopper with a bar when they caught the end of their left pinky finger between the bar and hopper wall resulting in a closed fracture to the tip of the finger.

Injured employee was working on clearing a centrifugal pump that was sanded off. The discharge polyline was removed to access the inside of the pump, while pulling hand out of the discharge side of the pump after feeling for blockage IE contacted the sharp edge of the pump impeller causing a laceration to left hand, middle finger that required 23 sutures.

The injured employee was unclogging an infeed hopper when the chute gate IE was holding was contacted by a chunk of clay. IE's right ring finger was caught between the chute gate and the chunk of clay resulting in a laceration. The injured employee was escorted for further medical treatment that resulted in 11 sutures.

Employee was guiding a skid steer up to a conveyor to push it. The Injured worker advised the skid steer operator to pull back and turn to the right, but the skid steer turned to the left and struck the injured operator in the right calf with the edge of the bucket attachment.

Lack of Three Points of Contact. 2 Stitches to the Left Middle Finger. No Lost Time

Employee was attempting to clean off the outside glass of a John Deere 824K loader. EE missed a step on the loader and fell to the ground. EE injured EE's groin.

Removing pin from loader arm, when pin came out it fell and struck EE.

was on top of man basket wheel and slipped and fell on piece of steel, only a couple feet, hit lower left back above belt line.

Employee was in a man basket and got middle finger on left hand caught between the handrail of the man basket and the top of the plant.

Employee was breaking bolts loose on the pipeline and pulled a muscle in his back.

Employee had been driving around in a loader for several days with the air-ride system not functioning. By the time he reported the malfunction his back had already begun to hurt.

Employee was loosening pipe using wrench. Wrench slipped and his finger was caught between wrench and pipe.

Employee was helping change springs on a shaker. When he lifted a spring with another employee, he felt pain in his back.

EE was stepping out of a loader. He stepped onto uneven ground and sprained right ankle. EE is losing time.

EE was working on the dredge with other employees. He slipped, fell, & landed straddling a beam. He was sore but didn't think anything about it until two days later when he started hurting and urinating blood. A CT scan was done and there is no sign of internal damage. His buttocks and pelvic area were bruised. He has a full release to return to work 10/10/06.

EMPLOYEE SUFFERED STRAINED ELBOW AFTER CONTINUOUS OPERATION OF LOADER.

EE WAS SETTING UP FORMS FOR A PAVING JOB WHEN A BOLT POPPED OUT OF THE SLOT AND HIT HIS FACE.
