Injured employee was scaling built up material off the side of feed hopper with a bar when they caught the end of their left pinky finger between the bar and hopper wall resulting in a closed fracture to the tip of the finger.
Injured employee was working on clearing a centrifugal pump that was sanded off. The discharge polyline was removed to access the inside of the pump, while pulling hand out of the discharge side of the pump after feeling for blockage IE contacted the sharp edge of the pump impeller causing a laceration to left hand, middle finger that required 23 sutures.
The injured employee was unclogging an infeed hopper when the chute gate IE was holding was contacted by a chunk of clay. IE's right ring finger was caught between the chute gate and the chunk of clay resulting in a laceration. The injured employee was escorted for further medical treatment that resulted in 11 sutures.

Employee was guiding a skid steer up to a conveyor to push it. The Injured worker advised the skid steer operator to pull back and turn to the right, but the skid steer turned to the left and struck the injured operator in the right calf with the edge of the bucket attachment.

Employee was attempting to clean off the outside glass of a John Deere 824K loader. EE missed a step on the loader and fell to the ground. EE injured EE's groin.

He was sore but didn't think anything about it until two days later when he started hurting and urinating blood. A CT scan was done and there is no sign of internal damage. His buttocks and pelvic area were bruised. He has a full release to return to work 10/10/06.
