METRO PORTABLE #4 Metal/Non-Metal

Office worker tripped on floor mat, falling and breaking right femur.

Miner climbed up stockpile to observe conditions of head roll on pea rock conveyor. Miner lost footing and right hand went through drive pulley pinch point, slicing three fingers through miners glove.

Prior to shift, team member fell through doorway into break room landing on left side and breaking hip. Walkway and break room are finished concrete slab which were clean and clear of identifiable obstructions. No other environmental factors were identified.

Employee was walking under a cross beam and raised his head. He hit the beam with his hard hat and received small cut to left brow and bridge of nose.

While using a utility knife, cut left hand, recieved 11 stitches

While on a concrete pad the employee stepped on the edge of a water hose. A severe sprain resulted to the left ankle.

Employee was standing next to a 6" hose when it blew off the steel pipe, pushing him in to the sand screw and cutting his ear.

WHILE EXITING FORKLIFT, TURNED ANKLE.

WHILE GRINDING A PIECE OF METAL WITH A HAND HELD AIRDISC GRINDER, DISC CAME APART WHEN IT CAME IN CONTACT WITH METAL CUTTING LEFT ARM.
