Employee performing a pre-shift start up and while opening inspection doors to check shoes left door bound up. While positioning himself to get a second hand and better grip on the door to apply more force to try and overcome the bind, the door fell back down on his right index finger. Employee went to emergency room and received 11 stitches but is back to work on modified duty.

On 02.22 on employee's way home from work, he rubbed his L eye, & felt like something was in it. At home he checked it & did not see anything in it. On 02.23 when he woke up, his L eye felt scratchy when he blinked. He washed his face, then felt okay & he went to work. On 02.24 when he woke, his L eye felt scratchy, it was red & irritated, & he also saw something in his eye.

While changing the anvils in the crusher, EE struck anvil to loosen it and a chip of metal broke off at a high rate of speed, embedding in his right forearm.
While lowering the lid onto crusher employee was helping rotate the lid into position. As the lid lowered employee's left arm was caught between the lid and tub, in an upright position.
