Essroc Cement Corp Plant #3 Metal/Non-Metal

The employee was sucking out the dust collector at Tower 2. The employee had placed the hose inside the hopper. The hose became clogged with a large chunk of clinker. The employee brought the hose part way out of the hopper to remove the chunk. The employee tried to remove the chunk from the end of the hose, and lost control of the chunk. His hand went into the rotary valve.

Employee was tapping cement and a lump got stuck in the valve at the bottom of the silo. The lump went through the valve in to the rotary screw causing the screw to jump violently. As the screw jumped, it cause a small piece of pipe that was resting on top of the screw cover to bound down on the employees LT foot.

Employee was leaving the Change House at Plant 3 at the start of his shift. As he stepped outside the double doors of the Change House lunch room, he slipped on a patch of ice. He cut is left hand and suffered a hairline fracture of his RT hand.

The employee was standing on top of a cover to the outside incline drag conveyor in order to remove the top section of the head pulley guard to #2 drag conveyor. The employee received a call on the plant radio, and when he reached for his microphone, he lost his balance, falling to the ground. He was standing 3' 10" off the ground when he fell.

Employee was placing caution tape by #5 finish mill for a project. The employee was using a utility knife to cut the tape. While cutting the caution tape, the employee lacerated his left middle finger.

Employee was assisting with a conveyor belt replacement. He was removing an idler roll from the conveyor. As he was removing the roller, the far end of the roller dropped down and his thumb got pinched between the roller and conveyor frame.

Employee was operating the bag applicator at the pack house. Empty bags were getting stuck in the feeder. The worker estimated that empty bags got stuck in the feeder about 18 times in a short period of time. The worker had stacked approximate 30 empty bags on the table. When he picked up the stack of bags, he felt a sharp pain in his left groin. First day off work was 6/18/14.

Employee was in the basement of the Pack house emptying a trash can full of shredded bags and cement weighing approximately 50 lbs. The employee grabbed the can handle with his left hand and placed his right hand under the can. As he lifted the can to about shoulder height, he felt his left arm and shoulder go numb.

The worker was removing lumps out of silo 25 by manually operating the silo valve handles. While rocking the handles back and forth to break up a lump, the valve jammed and the employee felt a sharp pain in his shoulder.

Employee was performing maintenance on the #3 finish mill. He stated while cleaning the area on the floor below the mill, he stepped on a hard lump of material that was buried under the dust, twisting his left knee.

Employee was using a jack hammer at tower 2 near the transition between 31 and 105 belt. The jackhammer blade got stuck in some wire mesh. When employee went to pull it free, the 2" spade landed on his left foot.

Employee was working in #5 finish mill removing head liners. Four (4) liners are removed at a time. Employee had placed a liner at an angle to the side of the mill. While moving another liner, the one he previously positioned slid down striking him in the RT ankle.

After passing a jack hammer under a conveyor belt the injured felt pain in the area of his shoulder blade when he stood up. Diagnosed as thoracic muscle strain.

Employee tripped on a low mound of material on the floor and jammed his toe against the inside of his steel toe boot.

Injured suffered a strain of his left shoulder while installing a transmission oil cooler truck. He was holding the cooler in place with both hands and when he released his grip with his right hand to install the bolts on the mounting bracket he felt pain in his left shoulder.

Noise Induced Hearing Loss

Employee was trying to put the South Bulk scope back together and material fell into his right eye. He had the bracket in his hand as he was looking at it, material fell from above, most likely from the scope. The scope was cleaned down before that job was started. There was not much of a breeze at the time of the incident, but it was in a west to east direction.

Employee suffered a heart attack while at work.

Employee was working near the back packhouse door unloading a truck. The door was open as product was being moved outside. The wind was blowing at a high rate and dirt/dust blew into employee's left eye.

Employee was exiting old packhouse at plant 3 on NW corner. Descending down steps, employee slipped on ice which he thought was just water. He used his left arm to hold railing to break fall and felt sharp pain in left shoulder area.

Employee was standing on work platform removing lumps from silo using 4 ft. steel bar inserted through porthole. Large lump suddenly fell & kicked bar outward. The bar struck the employee in jaw/face areas. Employee fell backwards and was caught by safety chain. Employee was taken to local hospital.

Annual Hearing test indicates a recordable shift change (w/age correction).

Employee was walking to the mill building when he felt a sharp pain in his right ankle.

Employee was jackhammering lumps in Clinker silo #2. He was maneuvering in one hand while removing material with the other which caused strain on left wrist.

Employee was cutting out jacket of Sealtite. Improper use of utility knife (cutting toward self, instead of away) caused injury when employee slipped with knife and lacerated left wrist. 5 stitches required.

Employee was using 3' ladder to access slides that switch from one silo to another. He was climbing ladder w/a pipe used to push the slide. Weight of ladder caused it to tip over backwards and employee fell to the ground, a distance of 3'. He landed on his left side, brusing his ribs.

Employee was on his knees cleaning clinker from under #31 belt, east side. When the employee went to stand back up, he twisted his left knee. Access to the east side of #31 belt is limited. Employee reported the incident at 14:30 pm and refused medical treatment at that time. He then requested medical treatment on Friday, March 6, 2009 at 7:00am.

1st Employee was walking out of the north doorway of the mill basement through the hanging curtains. Another employee operating the New Holland skid steer was driving into the basement at the same time. 1st employee was struck in the left leg by the bucket of the skid steer loader. Employee complained he had knee and back pain.

Employee was working to clean out hopper below #4 mill gypsum clinker feeders. He tripped over the cord on the floor, resulting in twisting his right ankle and knee also causing him to fall against a wall plate, injury his right shoulder

Employee was walking around NW corner of the storage building at the North end of the No. 3 packhouse. He slipped on a patch of ice and landed on his left wrist.

Employee was carrying a battery from the back of the truck to the track mobile. As he walked accross the railroad tracks, he felt pain on left side of his back.

Employee pushed up an empty wheel barrel. He took one step forward and caught his head on a piece of steel support for A belt line. His head was pushed backwards and down.He felt a crunching in the back of his neck. Incident occured on 12/18/2008, employee continued to do his job. However, as of February 10, 2009, he bacame restricted from his packing position.

Employee was working on #1 drag clinker silo when cover slid forward. Employee tried to catch cover and got finger caught under cover causing contusion to 5th finger of right hand.

EE was rotating a kiln using a ponymotor, 2 clutches are to be engaged. The 1st was engaged, EE was working at 2nd at the time of the accident. As the clutch was engaged, the pony began to labor and in order to avoid stalling, EE began to disengage the clutch. As he disengage it was backspinning driving the clutch plate in reverse, causing failure of the clutch.

Employee was entering the compartment #5 of #4 dust collector. Employee bumped/scraped elbow on the narrow access doorway when entering to do work.

Employee was putting freed clinker in silo through the reclaim system. Employee bent over to pick up a clinker lump and felt a pull in his back.

EE was changing a barrel of oil in the mill building, EE lost his grip on the barrel while trying to lift it. EE's fingers were caught between two barrels, about 1/8" to 1/4" space inbetween barrels. EE appears to have lost the top tips of his left index and middle fingers.

Employee was walking in front of the old plant #3 packhouse group. He slipped on ice and fell, hurting his left knee (Note: began losing time due to surgery 4/23/08).

Employee was using a hammer to straighten the edge of the plow which was on forklift. Plow slid off forks and hit employee on head and shoulder areas.

EE reached in machinery ( Palletizer) to push bag w/his right hand. The belt caught his hand and when he tried to remove it, he cut his hand by thumb area.

An employee went to throw a bucket of water at another employee. Employee raised their hand to block the bucket & the bucket struck her pinky finger on the left hand.

EE was removing the reducer from the east drag area while pulling the cart w/reducer on it he felt a strain when pushing/pulling the cart. Strain was on left knee area.

EE was using Oxygen Aceteline torch to pierce head of elevator pin for removal, when slag blew back and down collar of shirt onto chest, neck and stomach area.

After EE filled the kerosene heater along #2 stone belt, EE twisted his torso while getting down from the belt.

Employee was attempting to slide a 1/2 bag (40 lb) of used cement onto a hand truck. Employee immediately felt a sharp pain in his lower left side of back.

EE was changing light bulb in bottom of raw silos. After completing job EE turned to go out of area and slipped on wet raw material, injuring his right knee.

EMPLOYEE WAS PULLING HIMSELF UP ON TRUCK.

EE was shoveling by #4 FM top of elevator. He overworked his back. Woke up the next day and felt slight pain in his lower back. 1st day away from work 3/17/06 for sugery.

While transferring masonry from silos, dust escaped and filled some of the travel area causing employee to not see the steps clearly. Employee tripped and hit his knee on step area.

EE PLACED AN EXTENSION LADDER ON A SMOOTH CONCRETE SURFACE AND BEGAN CLIMBING THE LADDER TO TIGHTEN BOLTS, WHEN HE WAS 3' OFF THE GROUND, THE LADDER SLIPPED OUT FROM UNDER HIM AND HE FELL 3', LANDING HEAVILY ON HIS LEFT FOOT, SPRAINING HIS ANKLE. CLAY-LIKE MATERIAL UNDER THE LADDER ON THE CONCRETE CAUSED THE LADDER TO SLIP. HE WAS EXAMINED AT THE HOSPITAL RELEASED

EE WAS SORTING MILL LINERS TO BE SHIPPED OUT FOR SCRAP. HE TRIED TO MOVE ONE BY HIMSELF AND HURT HIS SHOULDER

EE was cutting notches in 1" thick spirting w/a untility knife. Knife slipped and cut ee's r thigh.

EMPLOYEE WAS WORKING ON #3 RAW MILL ELEVATOR PROBLEM. WHEN HE WENT TO DE-ENGERIZE, CONTACT STRUCK AND BLEW THE SYSTEM

Employee had worked on drag chains last couple of days and had a little pain in back. While getting dressed employee tied one shoe then went to tie the other shoe & felt pain in his back. Pain got worse in evening.

EMPLOYEE WAS PRYING ON OVERHEAD SCREW CONVEYOR, LOST FOOTING ON WET MATERIAL CAUSING EE TO FALL. PRY BAR FELL & EE FELL ON TOP OF PRY BAR FRACTURING HIS LEFT THUMB.

EE WAS TRYING TO REMOVE, WITH A BAR, A CHUNK OF COAL CAUGHT IN CHUTE. RE-INJURED LEFT SHOULDER.

EE WHILE HELPING ANOTHER EMPLOYEE LIFT SEPARATION LID OFF STRAINED HIS BACK.

EE was inspecting block grease at 2nd pier of kiln #5. He slid the lid that covers the block grease area up near the kiln trunion tire. He rested his left hand in the lid area. The lid came in contact with the trunion tire, pushing the lid onto the finger of his left hand. Left pinky finger was amputated off at the end knuckle and ring finger also severed (but no bone loss).

Employee entered 2nd gloor passenger elvator in the clinker silos. when he reached for the strap on ordered door and close it. door slammed on his l middle finger cutting tip off.

Employee was checking clinker cooler area. so was dark in this area because light blub was out. ee had a flash light tried turned and hit his head on the dust box.

EMPLOYEE WAS CLEANING ELECTRIC EYE HOLE & STRIPPER WENT IN MOTION. ROLLER CAUGHT FINGER.

EMPLOYEE WAS GOING DOWN STAIRS AT BACK END OF KILNS, HE TURNED TO CALL OTHER EMPLOYEE & HIS FOOT TWISTED, TWISTING HIS RIGHT KNEE. EE HAD SURGERY 6/7/04.

EE WAS UNCHOKING GYPSUM FROM BASEMENT AREA. EE PICKED UP A STEEL BAR WEIGHING IN EXCESS OF 80 LBS TO BEGIN POKING AREA. AFTER 5 MINUTES, HE FELT A PAIN IN HIS STOMACH.

EE WAS WORKING ON THE BRICK PLATFORM IN THE #2 KILN. PULLING BRICK EE EXPERIENCED NUMBNESS IN BOTH HAND & FOREARM & LEFT ELBOW. (PER CALL CARPAL TUNNEL)

EMPLOYEE WAS WORKING ON THE TOP OF THE FRINGE BIN. WHEN HE WAS FINISHED HE STARTED TO CLIMB DOWN TO THE DECK LEVEL. HE JUMPED THE LAST FEW FEET AND TWISTED HIS ANKLE.

EMPLOYEE WAS SLEDGING OVERHEAD ON #4 COOL FEED TO REMOVE CHOKE. HE FELT A PAIN IN HIS WRIST AREA.

EE WAS USING A BIG RATCHET WRENCH TO OPEN COAL MILL DOOR. HE WENT TO PULL UP ON WRENCH AND FITO PAIN IN HGIS L ARM BICEP AREA.

EMPLOYEE'S HEARING TEST INDICATED A THRESHOLD SHIFT. EMPLOYEE HAS BEEN RE-TESTED BY AN AUDIOLOGIST AS A FOLLOW-UP AND HAS BEEN INFORMED OF HIS RESULTS.

EE INJURED HIS RIGHT KNEE WHILE WALKING ON METALGRATE. THERE WAS OVERGROWN VEGETATION ON METAL GRATE WHICH CAUSED EE TO SLIP AND HE HURT HIS KNEE.

EMPLOYEE USED CATWALK BETWEEN FINISH MILL 3 & 4. A HOSE USED FOR WATER RETURN, EMPLOYEE TRIPPED OVER HOSE & LANDED ON LEFT KNEE.

EE WAS STANDING ON A LADDER REMOVING PLATE AT #3 SWING MILL (RM) AREA. WHEN PLATE WAS REMOVED BUILD UP RELEASED PUSHING EE OFF LADDER CAUSING HIM TO FALL APPROX.8'TO GROUND.

EE HURT KNEE (R) WHEN HE SLIPPED ON CLINKER WALKING DOWN TO RECLAIM INCLINE AREA.

REBUILDING CLINKER BREAKER HAMMER WELDING TO BUILD UP TO SPECS, BEFORE HARD SURFACING IS APPLIED. WELDING W/MANGANESE RODS WHEN SLAG FROM ROD POPPED, PIECE OF SLAG SOMEHOW CAME BEHIND WELDING HOOD & SAFETY GLASSES & BURNED EYELID AREA.

EMPLOYEE WAS REPLACING COOLER RACK IN #2 COOLER. POSITIONING NEW RACK IN THE COOLER USING PINCH BAR TO MOVE RACK. EMPLOYEE WAS HOLDING ON TO FINISH ON RACK WHEN RACK FELL INTO POSITION & PINCH ED HIS FINGER.

EE INJURED BOTH WRISTS WHILE JACKHAMMERING A PILE OF MATERIAL BY #5 MILL FEED END.

WHILE TALKING TO SUPERVISOR, EMPLOYEE PLACED HIS LEFT HAND ON THE END OF THE PALLET DISCHARGE LINE. HAND GOT CAUGHT BETWEEN FULL PALLET & PALLET STOP. WHEN HE PULLED HIS HAND FREE THE METAL ED GE OF PALLET STOP CUT HIS HAND (LACERATION).

EE ATTEMPTING TO SECURE CUT OFF & NUT & LOCK WASH PINCHED HIS THUMB CAUSING IT TO BREAK.

EE WAS TURNING SEPARATOR VALVES WITH A SOCKET WRENCH & PULLED A STOMACH MUSCLE. LATER EE WAS SHOVELING UP RAW SPILLAGE.

EE WAS WALKING TO THE UPPER LAB WHEN TRUCK YARD PICK UP TRUCK EE MAKING HIM FALL BACKWARDS. TRUCK DRIVER PUT ON BRAKES BEFORE STRIKING EE BUT IT WAS A LITTLE LATE.

EE WAS INSTALLING SCREW IN THE 3 MM RAW PUMP. PUSHING SCREW INTO PUMP HOUSING, FELT PULL IN CHEST & SHOULDER AREAS.

EE WAS GOING DOWN STAIRWAY & SLIPPED ON CEMENT & CLINKER ON STEPS. HE FELL DOWN STEPS & INJURED LEFT HAND, RIGHT FOOT & COMPLAINED OF PAIN IN RIGHT SHOULDER.

EE WAS TIGHTENING A SEPARATOR BLADE ON #2 FINISHMILL. THE WRENCH SLIPPED & HIS THUMB MADE CONTACT WITH A VERICAL ALL THREAD ROD, HYPEREXTENDING HIS THUMB BACKWARDS.

EE WAS CLEARING A FEED CHOKE ON #1 LEVEL BOX MAIN FEED SCREW. MOVEING VALVE TO CLEAR THE BLOCK & PULLED HIS LEFT SHOULDER.

EE TRIED TO CLOSE SOUTH SLIDING DOOR IN MACHINE SHOP & PULLED SOMETHING IN RIGHT SHOULDER

EMPLOYEE WAS WALKING IN MILL BLDG. BASEMENT, SLIPPED ON A BUILD-UP OF MATERIAL AND WATER, FELT A TWINGE IN THE RIGHT KNEE.

EE LEANED OVER TO GET WRENCH A WHEN HE STRAIGHTEN UP HE HIT HIS HEAD ON DOOR WHICH WAS LOCATED ON EAST WALL OF THE MILL BUILDING. EE HAS PAIN IN NECK/UPPER BACK AREA.

EMPLOYEE WAS CLOSING THE VALVE HANDLES HE PULLEDHIS RIGHT ARM.

EMPLOYEE WAS REMOVING CHUNKS FROM #2 STONE FEEDER IN THE ROCK TUNNEL. WHILE DISLODGING CHUNK ANOTHER PIECE FELL ON EMPLOYEE'S HAND.

EE WORKING ON THE BRIDGE DRAG THAT WAS DOWN FOR SEVERAL HOURS AND EE WAS SWEATING. CEMENT DUST GOT ON RIGHT ARM CAUSING SKIN BURN.

EE WAS MAKING CUTS ON #4 & #5 KILNS FOR ALIGNMENT. REPETITIVE SLEDGING ON WRENCHES TO MAKE ADJUSTMENT WITH A 20 LB. SLEDGE CAUSED PAIN IN HIS ELBOW.

EMPLOYEE WAS CHECKING MILL DISCHARGE, EE WENT TO THE CONTROL PANEL AND ON THE WAY TO THE PANEL HE DUCKED UNDER A SCREW CONVEYER AND HIT HEAD JARRING NECK ON THE CONVEYER SCREW AREA.

EE WAS CUTTING RUBBER SEAL FOR HATCH SEAL FOR FINISH MILL. UTILITY KNIFE SLIPPED AND CUT HIS RIGHT LEG.

EE WAS PUTTING COVERS BACK IN PLACE WHEN DUST BLEW UP INTO HIS FACE. NOT SURE WHERE THE DUST CAME FROM.

EE WAS OPERATING 980 LOADER AT PLT. II. BEGAN FEELING PAIN & DISCOMFORT IN RIGHT LOWER BACK.

EE HAD THE SADDLE IN THE VICE RIGHT SIDE UP & WAS USING A HAMMER & CHISEL TO CUT THE BOLTS OFF. WHEN THE BOLT CUT HIS HAND CAME IN CONTACT WITH THE SHARP (WORN) EDGE OF THE SADDLE & CUT HIS HA ND.

EE WAS WALKING DOWN INCLINE TO RECLAIM HOPPER BOX TO CLEAN A CHOKE AT FEEDER GATE. EE SLIPPED & FELL HUTING HIS BACK.

EMPLOYEE WAS USING 950E LOADER TO SCRAPE AWAY MUD FROM THE ROADWAY WHERE CEMENT TRUCKS BLOW OFF CEMENT INTO PLANT SILOS. EMPLOYEE HIT SOMETHING HARD IN THE GROUND WITH THE BLADE OF LOADER BRAK E CAUSING AN ABRUPT STOP AND EMPLOYEE STRUCK HEAD ON STEERING WHEEL, CAUSED CUT ON LEFT SIDE OF FOREHEAD.

EMPLOYEE WAS CLEANING UP RAW MATERIAL AT #5 KILN FEED. FEEDER WITH A KILCH HIT SIDE OF ELEVATOR WITH HANDLE AND FINGER.

EE WAS CHECKING GYPSUM SETTING ON WALL. CONTRACTORS HAD STEEL CABLE COME INTO BUILDING FROM OUTSIDE TO #3 FINISH MILL ELEVATOR. EE TRIPPED OVER CABLE AND FELL HITTING LEFT ELBOW WHILE TRYING T O BREAK HIS FALL.

EE WAS LEAVING SUPERVISOR'S OFFICE3 WALKING TOWARDS SERVICE ROOM EE SLIPPED ON ICY AREA & FELL

EE WAS WALKING W/ANOTHER SHIFT SUPERVISOR ACROSS THE YARD TOWARD KILNS IN FRONT OF MAINTENANCE SHOP. EE ALL OF A SUDDEN WAS FALLING. EE WAS NOT SURE IF IT WAS UNEVEN GROUND OR SLIPPERY SURFACE . EE FELL ON HIS LEFT KNEE.

EE WAS WORKING ON COAL LUMP STUCK IN FEEDER. HISFOOT SLIPPED OFF OF THE STEP DOWN TO LOWER STEP.EE FELT SOMETHING PULL IN HIS BACK AND UPPER ARMAREA RIGHT SIDE.

EE WAS ATTEMPTING TO CHANGE #6 WATER SPRAY ON #5 KILN. EE DISCONNECTED HOSE FROM WATER SPRAY AND A BUILD UP OF STEAM BURNED HIM IN THE UPPER ARM THROUGH OF HIS SHIRT.

ONE EE ACCIDENTLY HIT NAOTHER EE IN THE MOUTH CUTTING TAKE LOWER LIPS AND CHIPPING THE FRONT TOOTH. WRENCH SLIPPED OFF THE BOLT HOLDING FATE IN PLACE WHILE LIFTING THE HEIGHT OF THE GATA ON TH E GYPSUM FREQUENCY CONVEYOR.

EE WAS FINISHING A CHECK ON BRIDGE DRAG. HE WAS WALKING DOWN ALONG SIDE OF DRAG WHEN HE SLIPPED ON CLINKER COVERED BY LAYER OF DUST. EE TWISTED LEFT KNEE.

EMPLOYEE WAS POKING #4 STACK; STACK FLUSED OUT HOT MATERIAL, SOME OF IT FALLING INTO HIS BOOT AND FOOT AREA AND BURNING RIGHT ANKLE AREA.
