Employee felt pain in lower back while lifting a battery.
Employee slipped off track/platform of dragline and landed face up on the ground.
Victim suffered heat stroke due to dehydration and extreme weather conditions.

Blaster was dropping 50 lb packages of blasting agent into boreholes and felt a sharp pain in EE's back.
While trying to remove a keeper out of a shovel tooth, employee was holding a punch and missed the punch and hit their hand with a sledge hammer.
While changing hammer on drill, with J wrench and chain wrench, continual lifting of both wrenches over several hours caused employee to suffer low back pain.
EMPLOYEE SLIPPED AND FELL TO GROUND WHILE CLIMBING DOWN LADDER OF LOADER.
Employee was operating a loader with worn pins. Excessive bouncing caused severe back pain. Employee's first day off work from this injury was 4/26/24.

Non-employee was crossing haul road on a motorcycle. Individual lost control; and crashed causing injury. EMS called to the scene. MSHA notified.

Coming off the steps of a Haul Truck, employee slipped off the last step and felt right knee pop.
Moving the head on the drill up & down for mechanic to work on and the steel hammer came back and pinned EE's left elbow between it and the drill.
SNOW - PLOWING INCIDENT. EE was plowing snow with a loader, broke through frost and hit solid ground. Machine stopped abruptly. EE was thrown into loader windshield.

An excavator operator was backing up a haul road and repairing the roadway as they traveled. The excavator traveled underneath a 69KV powerline owned by PPL. Unknown to the operator, the powerline arced to the boom of the excavator which cause a brief power interruption. The operator was only made aware of this at 11:30 am when an electrician showed up on site to investigate.
While dismounting CM785 drill, employee's hands missed grab handles & employee fell down 2 steps to the ground.
Lifting a rock from the road and felt a pain in back, same area from previous work injury.

while fueling machinery, employee slipped on a rock/hole and fell twisting lower back and right ankle
prepping to move a dragline, attached a hook to the cable on the back and when pulling started, hook broke from cable and employee fell onto right shoulder and elbow
While getting out of broom, employee caught EE's foot on the door frame and fell to the ground
Employee was walking towards hitching post to plug EE's truck in at the end of EE's shift. EE slipped on the ice and spun around striking EE's right hand against front bumper of the truck.
Employee was using a large wrench to adjust the drag cable on a 4600 drag line when employee slipped, twisted and fell behind the drum.

EE backed truck up to dump. Revved engine & pulled dump lever & immediately felt & saw flames around cab coming from engine. EE exited truck and went down ladder.
