Employee twisted left knee while dismounting Co# 976 drill after fueling.

Foreign body lodged in right eye during grinding/welding operations.

Employee was attempting to lift and drag a cable when EE felt pain in low back.

Employee was riding in the rear of a service truck and had hand resting on the crane perch. The truck leaned to one side causing EE's hand to be caught between the crane boom and perch.

Employee was exiting off the work vehicle when EE felt a pinch in the lower left side of back that had the pain go down the left side of EE's leg into the foot.

This miner was found unresponsive in the back of a lube truck. 911 and MSHA were immediately notified. Upon further investigation it turns out this was the result of a non work related medical condition.

Employee was climbing on the machine to start shift when they slipped. The cause may have been some mud and frost on the machine. Employee did state they cleaned off their boots as best as possible.

Loading drill steel onto a truck when the drill steel moved while being lifted. Right index and middle fingers were pinched between the drill steel and the truck

A non injury accident occurred when a 785B truck came in contact with a 69KVA power line owned by a utility company. The operator was hauling rock for road maintenance. The operator dumped the load and drove away with the box still raised underestimating the distance to the power line.

While using bearing press to press master link into a chain, chain shot out of press and struck right hand

Employee was climbing ladder of a WA800 wheel loader while carrying a fire extinguisher. Employee felt a pull in right arm resulting in a strain and then banged right leg against the loader resulting in a contusion.

Carpal tunnel due to repetitive motion operating mobile equipment and hand tools. Accident occurred 1/31/2020 but EE has been working constantly up until 9/23/2020 and was told by Dr. that EE has to stop.

Employee climbed down ladder of truck to the ground. Proceeded to walk away and slipped on ice. Grabbed ladder with left hand to break fall resulting in strained left shoulder.

While walking across uneven ground, employee stepped on a rock and twisted right knee. Employee has been working all this time till 9/30/2020, employee gets surgery today.

While dismounting the truck employee climbed down the ladder to the bumper. EE did not realize that the under bumper ladder was missing. When EE stepped over the bumper from that missing ladder EE fell to the ground.

No injury - near miss accident - Employee was dumping load out of 785B truck - pulled out with the box up & made contact with overhead power lines.

Getting out of service truck. Gust of wind slammed door shut on left hand.

Employee was driving service truck thru pit on haul road. EE drove into a large water formed pothole and jarred back.

Employees were in process of removing a pump from a mine shaft. When one employee started to cut a drive shaft coupling with a torch, a gas ignition occurred causing burns to another employee.

Pulling hose off fuel truck, felt sharp pain above navel . First day off work is 8/4/17

Overhead wires knocked down by dump truck. Lines on ground were de-energized. One line on pole appeared energized and was not knocked down. De-energized line came in contact with energized line. Flash occurred causing a burn.

Employee was doing pre-shift inspection of PC 4000. Tripped in engine compartment and struck their ribs on firewall.

Unlocking gate @ jobsite entrance - slipped & fell on icy ground.

Employee stepped on bees nest & had multiple bites. Employee is allergic to bees. Injected with epi-pen & transported to hospital.

Lifting cable w/chain block hoist - hoist slipped while emp was pulling chain - Emp fell forward and struck ribs off dirt chute

Stemming drill holes - shoveling rock w/shovel. Experienced pain in low back.

Employee was installing new backs on air ride seat. Felt sharp pain in back when lifted seat up.

Cab of 785B truck caught on fire while emp was backed into rock dump. Emp jumped from platform to ground below.

Repetitive motion injury - Hand tools & power tools. 1st day of lost time 9/15/15 due to surgery.

Walking on uneven ground stepped on rock & twisted ankle.

Employee was lifting shovel cable & later exp pain & swelling in both hands.

Emp was fueling dozer. Fuel cap was frozen shut. Emp pulled on fuel cap and fell backwards to ground.

Employee was pulling 6" pump hose across uneven ground.

2 employees using crane to hoist drill steel onto flat bed truck. Employee guiding drill steel got hand/wrist caught between steel & bed of truck.

Employee lifting hammer/drill from truck. Hammer and drill bit slide inside one another. Employee lost hold of equipment and hammer slid pinching his right index finger.

Using steam Jenny to wash parts sprayed right foot.

Employee was operating bucket truck to weld on boom of Demag. When finished welding and lowering bucket, employee somehow caught himself between bucket and boom of Demag.

Slipped getting off fork lift

Two employees were trying to untangle a 200' length of 1 5/8" wire cable. Employee injured back while pulling cable.

Employee was walking up an embankment & lost footing.

Hoisting bad gear into ball of another truck, caught and between gear being hoisted and box of truck

EE was removing bolts from motor cover with a wrench when the wrench slipped off of the bolt head and EE fell to floor.

EE was using dye grinder, tool slipped and struck EE's thumb causing a laceration (required 8 stitches).

This employee, along with 4 others, was hoisting a pump. The wire rope connected to the pump got wrapped around a rock on the ground. When it came loose it snapped and struck this employee across the upper chest. He was then lifted up and fell backwards down over the edge of the mine pit the pump was being lifted out of.

Deceased was a trespasser who was electrocuted while stealing copper from a sub-station.

Employee struck left knee while loading truck with 55 gallon drums.

Lifting power cable for 7800 dragline, he felt pain in left shoulder and bicep area - torn bicep ligament at elbow.

One employee shut car door on this ee's hand.

While operating dozer, building a roadway, the operator went over large rocks & bounced violently in the cab of dozer causing a neck sprain. EE worked thru 3/12/04 & his first day of lost time was 3/15/04.

WHILE CHECKING FOR OVERHEAD CONDUCTORS - THE SPRING LOADED DISCT SWITCH WAS ENGAGED - THIS ELECTRICIAN THEN ACCIDENTALLY HIT STOP BUTTON & THE SWITCH DISENGAGED HITTING MAN IN CHIN.

GRINDING BOLT FROM D9N DOZER - FB INTO LT EYE. WORKED 9/22 & 9/23 - OFF 9/24/03. SAFETY GLASSES WERE WORN DURING GRINDING.

EMPLOYEE WAS LOADING STEEL ONTO FLAT BED TRUCK WHEN HE SLIPPED AND FELL STRIKING LEFT KNEE ON GROUND. 1ST DAY OFF WORK DUE TO THIS INJURY WAS 9-3-03.

RUNNING ACROSS UNEVEN & FELL LANDING ON LT. WRIST.

EMP WAS DIAGNOSED W / THIS CONDITION APPROX 2 YEARS AGO DURING A CDL EXAM BUT WAS TOLD BY DR THAT HE COULD CONTINUE TO WORK FULL DUTY UNTIL IT BOTHERED HIM. THIS IS A CUMULATIVE INJURY OVER TH E LAST 28 PLUS YEARS.

EMP WAS GETTING INTO LOADER SLIPPED ON SILL FALLING INTO CAB STRIKING HEAD & RT SHOULDER ON DOOR FRAME.

EE WAS PERFORMING NORMAL JOB SUFFER SPRAIN/STRAIN WHILE LIFTING PARTS - NO UNSAFE ACTION OR CONDITION TO REPORT.

2 EES WERE REMOVING BUSHINGS FROM BOOM ARMS OF SAID LOADER W/SLEDGE HAMMER & BAR. THIS EE WAS HOLDING THE BAR WHEN ANOTHER EE ACCIDENTALLY STRUCK HIS HAND W/HAMMER.

WHILE OPERATING TRUCK EMP EXPERIENCED BACK & LT LEG PAIN.

6" WATER LINE BURST- EE WAS RUNNING FROM HOSE & FELL.

EE WAS CUTTING ELECTRIC CABLE W/CABLE CUTTING TOOL WHEN KNIFE SLIPPED.

THIS WELDER WAS HOLDING A PIECE OF STEEL WHILE ANOTHER WELDER WAS WELDING TO REPAIR SHOVEL BULLET AND WAS RETAILS ON LG. BOULDER. BOULDER MOVED BUCKET SLID AND CAUGHT 2 FINGERS BETWEEN BUCKET AND BOULDER.

MAN WAS GOING DOWN RAMP INTO PIT WITH WATER TRUCK, HE SAID TRUCK ENGINE SHUT OFF AND TRIED TO TURN TRUCK INTO HIGH SIDE OF ROAD TO STOP, BUT TRUCK RAN UP HIGHWALL AND FLIPPED INTO ITS RIGHT SIDE, MAN HAD SEATBELT ON.

MAN JUST PARKED LOADER #656 AND GOT OFF WHILE WALKING AWAY FROM IT, STRUCK HIS ELBOW ON BUCKET, BRUISING IT.

EMPLOYEE WAS LIFTING STEEL SAW HORSE INTO TRUCK BOX AND CUT FINGER.

EMPLOYEE WAS HEATING PIN IN DOZER TO REMOVE IT WHEN OIL RING EXPLODED AND SENT DIRT INTO BOTH EYES.

BILATERAL CARPAL TUNNEL SYNDROME

EMPLOYEE WAS REPAIRING TRANSMISSION IN A 1983 INTERNATIONAL TRUCK. WRENCH SLIPPED AND STRUCK LEFT WRIST OF OTHER ARM.

EMPLOYEE WAS INSTALLING DRIVE LINE ON LOADER. RATCHET SLIPPED AND EMPLOYEE FELL FORWARD STRIKINGFACE AGAINST LOADER.

HURT HIS BACK WHILE RUNNING DOZER.

EE WAS LIFTING MARIO 7800 DRAGLINE PARTS WHEN HE FELT A PULL IN HIS ABDOMEN.

MAN WAS USING LADDER TO CLIMB OFF LOADER, SLIPPED ON LADDER RUNG (BUT DID NOT FALL). PINCHED HAND AND FINGERS.

REPETITIVE LIFTING AND CLIMBING.

EE WAS GETTING OF SHOE OF DRAGLLINE SHOVEL SLIPPED & HIT HIS BACK ON LADDER

EE WAS GETTING OFF DOZER, STEPPED ON BLADE SIDE RAIL SLIPPED OFF HITTING HIS LOWER BACK ON SAME SIDE PUSH ARM

DEMAG OPERATOR WAS HELPING ELECTRICIAN HOOK UP TRAILING CABLE RUBING HAND OVER END OF WIRE, GOT WIRE SLIVER IN FINGER.

EMPLOYEE SLIPPED OFF LADDER STRAINIG LOWER BACK.

EMPLOYEE SLIPPED OFF OF THE LADDER ON THE SHOE OF THE 7800, HITTING HIS SHIN OFF OF THE RUNG OF THE LADDER.

EE WAS DRIVING COMPANY TRUCK INTO PIT AND HIT A BOUNCE CAUSING HIM TO HIT HIS WATER JUG AND BRUISE HIS RIBS.

WHILE SQUEEZING ELECTRIC HOLDER, FINGER BENT DOWN, STAYING THERE, NOT ABLE TO MOVE.

WELDER WORKING ON 240 DEMAG SHOVEL BUCKET WELDING CRACK AFTER HE TOOK HIS LUNCH. STARTED TO HAVE BAD CHEST PAINS LOOKED LIKE HE WAS HAVING A HEART ATTACK. CALLED 911.

SHOVEL OPERATOR WAS WALKING TO SHOVEL AND SLIPPED HURTING HIS LEFT ANKLE.

EE SLIPPED OFF DOZER, JARRING BACK CAUSING INFLAMMATION OF MUSCLES AND NERVES IN BACK.

EE WAS PULLING ON A LOADER HOSE SPRAINING LOWER BACK.
