Employee was walking around a maintenance truck not paying attention and walked into a stick of drill steel that was laying on the closed tailgate of the truck chipping their left front tooth.

Cooling press off and fitting blew out. The steam started burning and EE couldn't see and ran away into a pipe.

Employee was holding a piece of rubber with one hand and running a rigid brand multi tool with the other hand when the tool slipped striking the employee's index finger causing laceration.

As employee was doing maintenance on the crusher, the door fell off onto the webbing of employee's finger cutting it open.

Injured employee exited haul truck to move a rock weighing approximately 160 lbs from the haul road, and while moving it felt a sting in right bicep. EE received a MRI on 09/28/2021 showing complete avulsion of the biceps brachii tendon. EE had surgery to repair tendon on 10/05/2021.

An employee was backing at the break room doors when EE turned to the left, EE tripped over a doormat that had been rolled and left on the sidewalk.

The operator was using a dozer to remove topsoil from the edge of bench. While removing the soil, they came to close to the edge of a 10' slope. When they reacted, the machine pitched and overturned down the slope. The machine rested on the right side, damaging the right and rear windows. The employee suffered a fractured right shoulder.

The employee was changing poly deck panels using a screwdriver and hammer to remove the pins. When ee removed one of the pins, it popped up making contact with safety glasses, throwing sand in right eye causing an abrasion on the cornea.

Systems Integration Specialist was utilizing part of an extension ladder to install communications cables in a shop building. The ladder shifted causing the employee to fall.

The employee was cleaning up spilled stone from the surge tunnel. When he lifted a large rock, he felt a pop in his back. He informed his supervisor, but did not seek medical care until 6/25/15, making this a reportable accident.

On 3/24/15, the employee was carrying a drill bit when he slipped and fell. He reported the incident to his supervisor but did not require medical attention. On 4/7/15, the employee went to the medical clinic and is currently losing time. This incident is being report within 10 days of the employee seeking medical attention.

While operating a Cat 980 loader to build a crane pad he hit a pothole that caused him to get shaken violently in the cab which caused a back strain.

While replacing a lift cylinder on a haul truck, employee released tension on chain jack and the employee's left middle finger was pinched between the frame and cylinder head.

Employee was removing a drill bit form the bit basket when he felt a sharp pain in his lower back

Employee was performing his area inspection of the blending tunnel. He entered the tunnel and crossed under the conveyor belt and began to ascend the steps to the upper level. He struck his hard hat against the bottom of the return roller frame resulting in a neck injury.

Employee was performing his workplace inspection. He had a flashlight in one hand and traffic cones in the other. As he was checking the bin gates he slipped on a patch of ice and fell to the ground. He struck his right elbow and jammed his right shoulder.

EMPLOYEE FELL APPROXIMATELY NINE FEET WHILE REPLACING PINION SHAFT ON PRIMARY CRUSHER.

EMPLOYEE WAS CLEANING THE 42 CONVEY0R TUNNEL. HE SQUATTED TO LIFT A 29.5 POUND ROCK. AS HE WAS LIFTING, HE FELT A POP IN HIS LOWER BACK

The employee was steam cleaning the loader around the cab. He opened an access panel in the flooring during the cleaning process. While rinsing the loader, he stepped into the opening caused by the open access door. Employee scraped his left leg. He did not seek medical attention until 05/22/2005. He was treated for infection.

EMPLOYEE WAS FOUND DECEASED IN EMPLOYEE'S LUNCH ROOM. INVESTIGATION REVEALED NO UNSAFE CONDITIONS THAT CONTRIBUTED TO HIS DEMISE. THE CORONER DETERMINED THE CAUSE OF DEATH TO BE MYOCARDIAL INFARCTION.

EE WAS SITTING IN HIS VEHICLE TALKING TO ANOTHER EE WHEN A FOREIGN OBJECT BLEW INTO HIS EYE.

EE SLIPPED WHILE CARRYING A COME ALONG DOWN A FLIGHT OF STAIRS #2 SCREEN TOWER. THE STEPS HAD ICE ON THEM. THE EE SLID APPROX. 12 FEET.

EE WAS HOLDING ONE SIDE OF THE NEW IDLER FRAME. OTHER WAS USING HIS FOOT TO PUSH THE OTHER SIDE INTO PLACE SO IT COULD BE BOLTED DOWN. AS OTHER PUSHED, THE ENTIRE FRAME SLID AND CAUGHT EE S HA ND BETWEEN THE IDLER FRAME AND THE CONVEYOR FRAME.
