Upon arrival at the mine site a brush fire was located on the back side of the property between the pit and rail line. This area is away from the plant and consequently did not expose miners to the hazard. 800 number called. Fire extinguished and monitored by local fire department.

EE was washing out in the surge tunnel. While pulling the hose to reposition, it got caught around a pile of dirt and pulled EE's left arm. Injury date occurred on 10/16/2024 but injury did not become reportable until 1/13/2025.

Employee was pushing and guiding a toggle bearing in place to be bolted and heard a pop and felt pain in their back. This became reportable when they received restrictions the following day at a doctor's appointment.

Employee was lifting a rock that was stuck between the idler frame and the belt. As the rock reached the top of the idler frame, the weight shifted and the rock slid, smashing the employee's left thumb.
Employee was replacing a spray bar on the bottom deck of the wash plant and bumped knee on an existing J bolt that was installed on one of the bottom pads.

Employee was clearing a walkway area and tripped striking head against a grease tank filter
Operator was measuring hole depth and as operator was shutting the cab door. Operator moved the tape measure out of the way with right hand. Operator closed the door and right index finger was closed in the door.

3 employees were changing a bearing on a screen. The injured employee was pushing the pry bar against a drive shaft and it slipped, causing EE to fall into the drive shaft and resulting in approximately 1.5" laceration on left cheek.

Employee was working on Drill driving out a bent pin with a hammer. When he struck the pin, he hit his thumb, employee refused treatment. * Employee did in fact seek medical attention after work on the day of the injury and received five stitches but did not inform employer that stitches were applied until now. * Five Stitches medical reportable, no lost time, no restricted duty.
Employee was lifting a plate of steel to block off a chute. As he stood up with plate in hand, he felt a pop in the bicep of his left arm. Doctor diagnosis is not complete but he suspects a possible bicep rupture. * Regular duty as foreman with No lost workdays.

The loader moved to fast causing the metal to contact his left leg below the knee causing him to fall backwards resulting in back pain. ** He continued to work until doctor wrote Him out one September 28 making October 1 his first lost time day.**

Employee was assisting repairs on a cone crusher. As he removed a part of the machine it moved unexpectedly. A tool attached to the machine part struck the employee, causing a laceration on the back of his leg.

EMPLOYEE WAS TALKING WITH A VENDOR IN THE HALLWAY OF THE OFFICE. SHE TURNED TO RETURN TO HER DESK & TRIPPED OVER SOME BOXES THAT HAD BEEN STACKED IN THE HALL.
WHILE FIXING A SCREEN, HE STEPPED OVER SOME SCREENING MATERIAL AND BENT HIS KNEE THE WRONG WAY WHEN HIS FOOT SLIPPED.

EE AND ANOTHER EE WERE CLEANING RAIL CARS. THE OTHER EE WAS PULLING ON A PRY BAR, TRYING TO GET THE DOOR TO CLOSE TIGHTLY. INJURED EE LEANED IN TO STRIKE THE LATCH MECHANISM. WHEN HE HIT THE LATCH, THE PRYBAR SLIPPED AND STRUCK INJURED IN THE SIDE OF THE FACE.
EE WAS HELPING LIFT A HYDRAULIC RAM ON A OMNICONE CRUSHER. THE RAM TILTED AND CAUGHT EE'S FINGER AGAINST THE CRUSHER FRAME. EE WAS WEARING GLOVES WHICH PREVENTED MORE SERIOUS INJURY.

EMPLOYEE WAS ATTEMPTING TO ADJUST TWISTED V-BELTS ON THE WASH SCREEN BY ROCKING THE DRIVE SHEAVE. AS HE DID THIS, HIS FINGER WAS PINCHED BETWEEN THE V-BELT AND THE SHEAVE, RESULTING IN INJURY.
CONTRACT MECHANIC WAS ADJUSTING DRIVE CHAINS ON QUARRY DRILL WHILE STANDING ON THE TOOLBOX OF HIS TRUCK. AS HE ATTEMPTED TO STEP DOWN OFF THE TOOLBOX, HE MISJUDGED THE STEP (DUE TO THE BIFOCAL LENSES IN HIS GLASSES.) AND FELL INTO THE BED OF THE TRUCK.

EMPLOYEE WAS ASSISTING AT JAW CRUSHER. HE RESTED HIS HAND ON A HANDRAIL NEXT TO THE JAW FEED BOX. A LARGE ROCK ENTERED THE FEED BOX AND MOVED IT AGAINST THE HANDRAIL, PINCHING THE EMPLOYEE'S HAND. THE ACCIDENT RESULTED IN A LACERATION TO THE RIGHT HAND.
EMPLOYEE WAS UNLOADING SOME MACHINE PARTS FROM THE SERVICE TRUCK, WHILE HANDLING SOME HEAVY PARTS, HE STRAINED A GROIN MUSCLE.
EMPLOYEE WAS USING A 8 LB. HAMMER TO ENGAGE LOCKON RAIL CAR DOORS. WHEN HE STRUCK THE LOCK, THE HAMMER GLANCED OFF AND STRUCK HIS LOWER LEG, CAUSING A BRUISE.
