While using a grinder, a maintenance mechanic received a laceration on the thumb when the disc caught the bolt being cut and kicked back into EE's left hand, cutting through the leather glove EE was wearing.

Contacted on 5/08/2019 by a OTR trucking company that one of their drivers had allegedly been hurt on our site. The driver did not report the injury to anyone at the time of the incident so details are limited.

EE was stacking bags on a pallet in the bag house. As EE was finishing the top row EE felt a pop in the left shoulder and felt pain. EE notified the S&H supervisor about the injury and also informed supervisor that EE had a sports injury to the same shoulder in 2005 while EE was in high school.

While hoisting an unloaded electric chain hoist to move it out of the way, the hoist hook briefly caught/bound on a piece of machinery, popped loose, and struck the worker in the lip causing a laceration. Both upper front teeth were also chipped along the inner edge.

The employee was assisting in repacking a slurry pump. After using wrenches to loosen/tighten bolts associated with the process, he began feeling pain in his back and legs.

A mill operator was using a hand-truck to move 50lb. bags of mill media, when a bag became unstable and slid off of the hand-truck onto the ground. The employee stopped to put the bag back on the hand-truck and while lifting it, felt a pain in his back.

Employee was bagging and palletizing 50 lb. bags of sand. After positioning the bag on the pallet, he felt a pain in his back when he began to stand back up.

Employee was walking up a catwalk in the sand pit carrying a screen and slipped and twisted his right foot.

The employee slipped on a tread plate ramp and fell injuring his left shoulder.

A bolt had worked itself loose from the stabilizing guide on the loadout stairs causing it to fall down and catch on the limit chains. EE reached down to free the guide and his left hand was smashed when the stairs retracted. EE received a contusion to his left hand.

Terminated employee claims he injured his elbow on the job.

Employee was squatting on a feed bearing trunnion housing to attach a cable sling to a hoist. Employee grabbed a fence guard with his left hand and a rigid pipe with his right hand to stabilize his descent. The rigid pipe moved causing employee to lose his balance and fall 52 inches striking his chest on a handrail. Employee fractured a rib.

Employee was walking down an incline to his underground hopper when he lost his footing in loose sand and fell to the ground. Employee?s head protection came off his head during the fall. When he attempted to retrieve his headgear, he bumped his head on the conveyor frame. Employee received a laceration to the head, which required 15 stitches.

Truck driver smashed his left index finger in the tailgate latch while attempting to close the tailgate. Driver sustained a fracture of the left index finger and an open wound.

Employee was bending over and using a pry bar to close the bottom gate of a covered hopper. When he stood back up he experienced pain in his back.

UNIMIN OPERATOR WAS DRIVING LOADER W/SHOVEL LOADED ADN ELEVATED OVER 6 FT. CONSEQUENTLY, THE SHOVEL IMPAIRED HIS VISION. HE DROVE DIRECTLY INTO OUR MACK DUMP TRUCK, PLOWING INTO CAB OF TRUCK A ND ABOVE THE CAB. MY DRIVER HAD RIGHT-OF-WAY ANDBLOWED HORN ETC. THE LOADER CRUSHED CAB OF TRUCK, SEVERLY INJURING MY DRIVER.
