Avísenme cuando se presente un nuevo incidente de la MSHA en DAKOTA MAHOGANY QUARRY.
WHEN OPERATING SLOT DRILL, IT APPEARS HE GOT BETWEEN THE DRILL STEEL AND HIGHWALL AND HIS CLOTHING BECAME ENTANGLED IN THE ROTATING DRILL STEEL. THE CLOTHING WAS PULLED TIGHT AROUND HIS CHEST CAUSING HIM TO BE SUFFOCATED.
Operator twisted EE's left knee while stepping down from the 992 loader. The ground was wet-soft causing EE to slip when EE stepped off bottom rung.
Had a threshold change with hearing test.
Employee tripped and fell.
Employee was moving the hole drill and did not shut the air off first. While moving it the 3" air hose pulled off and whipped around and struck them in the ribs. Broke one rib on the left side.
EE was using a hand drill drilling into rock and the drill stuck and twisted EE's hand/wrist.
Piece of rock fell and lacerated EE's left wrist.
EE was tucking pants into boots after break and felt a pop and sting in ring right finger.
EE was holding a drill and the drill bit stuck and twisted wrist.
Loosening material with a sledge hammer and slipped to the ground and hurt their back.
Employee was bending over sliding drill on forks of our lower and strained employee's lower back.
Thawing out a frozen water pipe. EE was pushing and pulling on a smaller pipe to break loose the frozen part of the larger pipe. No immediate injury but wrist was swollen and sore a couple days later.
Heart Attack while working in our quarry.
Fixing a leak in a water line. Cutting the hose with a razor knife and sliced his right index finger. Needed 3 stitches
Was cutting a piece of metal while wearing both face shield and safety glasses when a piece of metal got into his eye.
Dropped drill steel on his right foot above the steel toe and was taken to hospital for x-ray.
Pulling wedge out of drill hole when he strained his lower back.
Was bending over to pickup a drill bit when he had pain in his lower back.
When lifting Down Hole Drill he strained his lower back.
Pulling air hose when he felt pain in lower back.
Contractor setting up equipment. stepped off ladder from drill and stepped on a stone twisting his ankle and braking a bone in his left foot.
grabbed a fan to move it an stuck the index finger of left hand in blades and broke finger and tore off finger nail.
Under usual working conditions employee bent down to pickup a tube and he felt discomfort in his lower back.
Prying the lid off an air muffler with a scraper when it slipped and hit his little finger requiring three stitches.
Rock wedge fell on left index finger, causing cut, needing 10 stitches.
Was stripping plastic coating off from diamond wire when he cut right thigh with knife.
EE was pulling a pump out of a drill when he experienced pain in his abdomin.
He jumped off a block and felt pain in lower back.
While pulling cable when installing water line, he felt pain in his lower back.
Walking on ice when he slipped and fell on his back.
Moving 3" airline when he felt pain in lower back.
Wrench slipped off bolt & hit right jaw.
Loading block in container, he was guiding block with left hand when it shifted & pressed his hand on the block & elbow against the container wall causing a fracture of left forearm.
Pulled muscle in lower back when lifting lid on fire pot.
When standing up after kneeling to work on airline he felt his right knee pop and was unable to bend it.
EMPLOYEE SLIPPED ON ICE WHILE WALKING ON QUARRY FLOOR AT WHICH TIME HE REPORTED IT TO HIS FOREMAN. ON JAN. 6TH HE SAW A DOCTOR WHO RECOMMENDED SURGERY WHICH WAS DONE ON JAN. 13TH, 2004.
WAS PULLING AIR LINE ALONG QUARRY FLOOR WHEN HE FELT PAIN IN HIS LOWER BACK.
AN ONGOING PAIN CAUSED WHEN AFTER DRIVING LOADER FOR TWO HOURS OR SO PAIN WILL START IN LOWER RIGHT BACK AND INCREASE TO A SHARP PAIN IN RIGHT LEG.
CHANGING OIL ON THE MANLIFT WHEN HE HIT HIS ELBOW ON THE IRON FRAME.
TAKING APART A 3" PIPE FITTING, USING A 3' PIPE WRENCH. PIPE WRENCH SLIPPED OFF THE PIPE AND HIT HIM ON THE LEFT KNEE.
WAS BENDING OVER TO PICK UP A 14' PIECE OF STEEL WHEN HE FELT A SHARP PAIN INHIS GROIN AREA.
WAS CUTTING AIR HOSE WITH A UTILITY KNIFE WHEN IT SLIPPED AND HE CUT THE INSIDE OF HIS INDEX FINGER ON HIS LEFT HAND REQUIRING SEVENT STITCHES.
BENT OVER TO PICKUP A WOODEN 4 X 4 WNE HE FELT PAIN IN HIS LOWER BACK.
WAS BREAKING STONE WITH HYDRAULIC SPLITTER WHEN ONE OF THE CYLINDERS BECAME STUCK. WHEN HE PULLED ON IT TO FREE IT HE STRAINED HIS LOWER BACK.
STEPPED OFF THE BACK OF HIS DRILL AND SLIPPED ON SOME ICE AND FELT HIS KNEE POP.
HOOKING UP A BLOCK WITH BIG HOOKS-WHEN HE WAS LIFTING THE HOOKS HE FELT PAIN IN HIS LOWER BACK.
