Employee was walking to check on pit pump and tripped resulting in a shoulder strain. Accident became reportable on 11/15/2024 when employee had surgery.

Employee was cleaning the inside of a Komatsu 785 haul truck bed using a hose and ball valve nozzle opened about half way. This wasn't enough pressure to clean out the bed, so EE opened the ball valve to full open causing the water pressure to increase. This increased the force behind the nozzle causing the hose assembly to lift striking and leaving lacerations under EE's chin.

EE was using a bandsaw to cut a 350KCmil wire. While cutting outward, the saw caught and snagged the wire, and the saw turned toward the EE. This motion made a minor laceration on the EE's right index finger, resulting in three stitches.

A haul truck was being loaded in pit. The muck pile began to slough causing wall material behind the pile to shift. This material slid down onto and covered the rear of the truck enough that the operator was unable to drive away. EE safely dismounted the truck to meet the loader operator on the ground. Three days later reported had back pain and sought medical care.

An over-the-road truck driver (non-miner) with CKJ Transport LLC. of North Texas was on site at the North Bridgeport Quarry when they fell while mounting truck cab. Driver injured left foot and suffered abrasion on the back of their head.

While inflating the RRI on a 777 Haul truck, the tire manipulator was holding the tire in place on the rim. While making adjustments to rim components to allow the tire to seat, the manipulator slipped off striking the technician on the right shoulder and pushing EE into the hub where EE sustained a broken right collar bone, shoulder blade and fractured rib.

EE1 was shoveling at the primary plant. As EE1 was lifting the shovel into motion, a rock fell and struck the EE1's left 4th digit finger tip. EE was wearing impact resistant gloves at the time of the incident. EE was later seen at clinic and diagnosed with a fracture to the tip of the finger.

EE's were adding a 90 degree steel elbow to an existing poly elbow. EE#2 operated the crane pulling the elbow towards the other elbow. EE#1 positioned his hands on the elbow flange pushing down to guide the elbow to connection. The choker chain slipped from the original location catching his left thumb between the choker and the metal flange.

IW was removing a carrier roller from a 390 excavator. The track was off the machine. IW struck the roller flange with a metal hammer and this metal on metal caused a piece of metal to chip off and hit IW in stomach.

Positioning cribbing blocks under an off road water truck when technician felt a slight back strain. Diagnosis is lumbar strain, lumbar pain, and sacrificial strain.

Employee descending stairs using handrails. Last step was rotted/oxidized and broke when the employee stepped on causing him to be jarred when his weight was thrown forward. Employee maintained an upright position by holding onto the handrails but suffered neck strain.

Employee was walking down a flight of stairs (outdoors) on a catwalk when he felt a sharp pain behind his L knee. Was provided first aid and returned to work on the date of the incident. Since has had trouble and was eventually scheduled for surgery on May 14, 2010. After medical review over the last six months, the meniscus tear has been accepted as a workplace injury.

When trying to position a jack stand to balance the truck properly, the mechanic tugged on the heavy stand to get it in position. When doing this he smashed his finger between the truck frame and the metal stand. Employee took eight stitches to the left ring finger.

Employee dismounted his equipment (Haul Truck) to remove a boulder in the road. Employee says he lifted a boulder that weighed approximately 25 lbs and returned to his equipment.

Employee did not use three point contact when climbing down from his haul truck. The weather conditions were deteriorating. Employee was in a hurry to go home.

A employee was turning on the water to a wash down hose used for clean up. The valve and nozzled came off and hit the employees left hand causing a laceration.

Employee was experiencing chest pain, numbness, and tingling in his left arm at start of shift. He attempted to climb on a Caterpillar D10N bulldozer and felt a pain in his left arm. He was taken to the Doctor. Diagnosis concluded ruptured disk in the neck area.

One employee was on a loader using a lubricant. When finished, he tossed the can down to another employee. The can deflected off the employee's hand and hit him below the eye causing a laceration.

Employee was cleaning the windows while servicing his loader. A gust of wind caught him off-guard. He slipped and hit his ankle on a piece of the loader.

Employee was shoveling under tail pulley guard and tried to bend guard back in place and it popped back and hit him on the hand causing laceration. 5 stitches.

Employee was trying to remove a snake from the shop area by picking it up to throw it out. When he slung his arm it hit against the shop door causing laceration.

EE WAS DRIVING A HAUL TRUCK AND HE BACKED UP TO BE LOADED. THE LOADER OPERATOR'S HAND SLIPPED AND HE HIT THE BOOM LEVER INSTEAD OF THE BUCKET LEVER IN THE LOADER. THE FULL WEIGHT OF THE BUCKET HIT THE TOPSIDE OF THE UNIT CAUSING THE EE TO JARR HIS BACK. JARRING EFFECT SPRAINED THE EE'S LOWER BACK.

EE RECEIVED A FRACTURED TAILBONE WHILE EXITING THE BUCKET AREA CATWALK ON A BUCKET TRUCK. HE HAD BEEN REPLACING LIGHTS IN THE SHOP AND WAS USING THE BUCKET TRUCK. HE HAD TO REPOSITION THE TR UCK AND IN DOING SO, THE TRUCK WAS RAINED ON. BOTH FEET SLIPPED OUT IN FRONT OF HIM AND HE FELL ON THE TRUCK'S TOOLBOX.

EE RUNNING HANDS ALONG CHAIN, CUT RIGHT PALM ON DEFECTIVE CHAIN LINK. LACERATION = 3 STITCHES. USING CHAIN TO SUPPORT COME ALONG.

WALKING BACKWARDS TRIPPED OVER BELT SCRAPER CAUSING FRACTURE TO LEFT SCAPHOID/WRISTBONE.

EE WAS OPERATING A LOADER. HE HIT A CHUG HOLE WHILE HIS BODY WAS TWISTED TO LOOK AT A TRUCK, CAUSING PAIN IN HIP & LOWER BACK.

EMPLOYEE ALMOST FELL, CATCHING HIMSELF AFTER A COME-ALONG CHAIN BROKE CAUSING A STRAIN TO RIGHT ELBOW.

HELPING REPLACE TOGGLE PLATE TENSION SPRING - WAS HOLDING IT FROM THE BOTTOM - MASHED MIDDLE FINGER BETWEEN SPRING & TENSION ROD.

EE WAS OPERATING A FRONT END LOADER/FEEDING PLANT. FELT PAIN IN HIS SHOULDER AND BACK. FEEDING PLANT IN CONFINED SPACE/CONGESTED AREA.

STEPPING OFF LOADER ONTO UNEVEN GROUND- TRIPPED -FELL ON LT. WRIST CAUSING SPRAIN.

HURT HIS BACK SHOVELING & HOLDING WASHDOWN HOSE USING PRESSURE HOSE TO CLEAN UP UNDER SCREENS & TAIL PULLEY & SHOVELING MATERIAL. PAIN STARTEDWHILE SHOVELING

EE WAS ATTEMPTING TO MOVE A CATWALK WHEN HE PLACED IT ON TOP OF THE CONE AND IT SLID DOWN AND CAUGHT HIS RT. THUMB BETWEEN THE CONE CAUSING CRUSH INJURY.

EE WAS STANDING ON CV-2 CLEANING CV-1 CHUTE. IT HAD BEEN RAINING ALL DAY. EE SLIPPED AND FELL INJURED (STRAINED) LT. SHOULDER ROTATER CUFF.

CAME DOWN FROM A STOCKPILE ON A DIFFERENT RAMP THAN SITE TOOK UP. THE RAMP HAD NO BURNS, WHEN SHE GOT TOO CLOSE TO THE SIDE THE RAMP GAVE AND OVERTURNED THE LOADER.

MASHED THUMB BETWEEN PIVOT POINTS OF MOTORBASE, WHEN LOADING IT ON CRANE. MASHED THUMB NAIL CUT OPEN SKIN ABOVE THUMB NAIL. WENT TO HOSPITAL DOCTOR DEADENED THUMB AND INSERTED SIX STITCHES. BU RNT TWO HOLES IN NAIL TO RELIEVE PRESSURE. BACK AT WORK - 11:00 AM.

EE WAS CLEANING WINDOWS ON LOADER AND FELL ON ARM OF LOADER.

EMPLOYEE WAS LIFTING MOTOR WITH BACKHOE, HE WAS LOOKING AT THE MAINTENANCE MAN OVER HIS SHOULDERWHEN HE FELT A STRAIN IN HIS NECK.

OPERATOR WAS LOADELARGE ROCKS INTO TRAILER OF TRUCKS. ROCKS CAUSED JARRING OF THE TRUCK. JARRING CAUSED STRAIN.

EE WAS DIGGING ROCK OUT OF HOPPER. HE OVER EXTENDED HIMSELF AND PULLED MUSCLE IN HIS CHEST.

EMPLOYEE MOVED LARGE ROCK FROM RIP RAP PLANT ON 12-15-00, WITHOUT HELP OF EQUIPMENT OR OTHER EMPLOYEES.

LOADER OP DUMPED ROCK INTO HAUL UNIT, ALLOWING IT TO FALL HEAVILY. JARRING CAUSED SEVERE LOWER BACK PAIN.

EE WAS CLIMBING LADDER TO TRUCK AND STRAINED BACK.

EE GOT OUT OF TRUCK TO MOVE LARGE ROCK OUT OF ROAD. WHEN TRYING TO MOVE ROCK, STRAINED HIS BACK.

EMPLOYEE WAS CLEANING OUT JAW WHEN A LARGE ROCK ON TOP OF THE SHAKER ROLLED OFF INTO JAW, STRIKING EMPLOYEE ON LEFT HIP AND FOREARM.

EE WAS DISMOUNTING TRUCK BACKWARDS WHEN HE GOT TO BOTTOM AND STEPPED DOWN HE HANDLED INCORRECTLY AND TWISTED ANKLE. LAST STEP WAS BROKEN.

EE GOT OUT OF TRK TO MOVE LARGE ROCK IN ROAD WHEN STEPPING DOWN KNEE TWISED.

EE ALONG WITH OTHER EE WERE LIFTING A PULLEY INTO THE BACK OF A TRUCK AND PUSHING IN IT. WHEN STRAIN OCCURRED.

LH WAS HIT BY METAL WHILE WORKING ON EQUIPMENT. TERMINATOR TRACKS WERE LOOSE AND WRENCH SLIPPED.

EE WAS LIFTING A DROP PULLEY FROM GROUND UP TO BELT.
