EE using hammer to drive in track link pin. A piece of the pin hit the employee in thew right leg.
Employee was standing inside the handrails of a catwalk pulling on a wrench. The wrench slipped off and EE fell onto the catwalk and injured EE's shoulder/arm.

Employee was unloading a metal fabricated box at the crusher wash plant when the load shifted causing him to trip/fall. His foot landed awkwardly on a rock and resulting in a sprain to his right ankle.
Employee was working on the crusher tightening bolts, when he lost his footing and fell, resulting in a chest/shoulder strain. He refused treatment on the date of injury but requested treatment later on 1/18. The doctor evaluated the employee and released him to full duty.
