MOBILE CR #3210 Metal/Non-Metal

EE using hammer to drive in track link pin. A piece of the pin hit the employee in thew right leg.

Employee was standing inside the handrails of a catwalk pulling on a wrench. The wrench slipped off and EE fell onto the catwalk and injured EE's shoulder/arm.

Employee sustained a same level fall and hit EE's knee resulting in a contusion to the knee.

Employee was swinging a hammer overhead to vibrate a bin to loosen material and strained shoulder.

Employee was unloading a metal fabricated box at the crusher wash plant when the load shifted causing him to trip/fall. His foot landed awkwardly on a rock and resulting in a sprain to his right ankle.

Employee was working on the crusher tightening bolts, when he lost his footing and fell, resulting in a chest/shoulder strain. He refused treatment on the date of injury but requested treatment later on 1/18. The doctor evaluated the employee and released him to full duty.

Employee was using box cutters to cut a belt. The knife slipped and struck the employee's forearm.

Employee was placing beams on a slab at well #3. The sling slipped off the hook and pinched the employee's finger. This resulted in an avulsion to his left index finger requiring 11 sutures.

EE ATTEMPTED TO CLEAN TAIL PULLEY WHILE RUNNING HE WAS RUNNING A PIECE OF REBAR ACROSS THE TAIL PULLEY TO CLEAN FINES OFF THE TAIL PULLEY. THE PIECE OF REBAR CAUGHT & HIT THE EE ON THE HAND.
