Employee was working on the crusher tightening bolts, when he lost his footing and fell, resulting in a chest/shoulder strain. He refused treatment on the date of injury but requested treatment later on 1/18. The doctor evaluated the employee and released him to full duty.

EE ATTEMPTED TO CLEAN TAIL PULLEY WHILE RUNNING HE WAS RUNNING A PIECE OF REBAR ACROSS THE TAIL PULLEY TO CLEAN FINES OFF THE TAIL PULLEY. THE PIECE OF REBAR CAUGHT & HIT THE EE ON THE HAND.
