Employee was unloading belt conveyor components from a trailer when EE stepped over an I-beam. When EE's left foot contacted the ground, EE's left knee buckled. The incident did not involve any equipment or compliance issues. It also did not involve job skill, proficiency, training, PPE or protective devices.

While working on a water line repair the employee stepped on uneven ground and fell curling EE's ankle. Employee had trouble walking and asked to go to the hospital. Employee was found to have a sprained ankle from the incident.

Employee reported that 5' X 6' 3/8 diameter screen he was handling fell onto his left foot, behind the steel toe. The screen pierced his boot and punctured his foot requiring stitches.

Employee reported that while inspecting a screen sensor, a piece of metal flatbar that he was holding came in contact with a vibrating screen causing the flatbar to hit his right shin. Injury required 2 stitches.

Injured was replacing a tire on a haul truck when tire shifted striking injured miner's hand.

While wearing gloves, employee was lifting drill steel vertically when it slipped through his hands. Employee then tightened his grip stopping the downward momentum of the slipping steel. He then felt a "twinge" in his back, released his grip, letting the steel slide through his hands to the ground. Employee then repositioned himself and lifted the steel into place.

Employee was carrying parts into a storage building when he slipped on some mud, straining his lower back.

Belly pan on loader fell, strilking and fracturing employee's right wrist.

Foreign object blew into employee's right eye while he was repairing a haul truck door. Employee was wearing safety glasses.

WORKER WAS OPENING VALVE ON WATER TRUCK WHEN HIS HAND BECAME CAUGHT IN A PINCH POINT IN THE VALVE MECHANISM. WORKER RECEIVED LACERATION TO RIGHT PINKY FINGER WHICH REQUIRED 9 STITCHES.

EMPLOYEE SLIPPED WHILE CLIMBING OUT OF TRUCK, STRIKING LEG ON STEPS.
