THE OPERATOR PULLED MUSCLES IN LEFT LEG, AND WAS TREATED FOR SHOCK.

3/2/2026 employee with 2 yrs of exp was dismounting from scaler u/g when they stepped into hole and fell employee tweaked their right knee 3/3 employee was seen at clinic given MRI, 3/24 dr verbally released employee with no restrictions, employee refused to go back to normal job and was terminated on 3/31, on 5/21 company was notified of WC claim with PT and rigid brace.

Employee was assisting another employee in cleaning drink coolers; the employees were using a pressure washer to do so. The injured employee reached to catch the cooler as it was falling out of reaction and was struck on the top of the hand by the water stream causing laceration to their left thumb. Employee was taken to ER where they received 6 stitches.

Used razor knife to cut plastic strap off box and cut right forearm.
Employee strained EE's back while operating mobile equipment through the day. The exact cause of the strain is undetermined. EE reported discomfort on 03/29/2018 but did not request medical treatment until April 2, 2018.
Using 10 1/2' burning bar cutting loader bucket tooth out of crusher. When tooth released the pressure pushed the burn bar holder into face causing small laceration on cheek close to right eye

Employee was using a standard utility knife to cut a rubber belt side guard out of a sand chute for replacement. Employee was pulling the knife through the material towards his body. Knife slipped or cut through the rubber and stabbed their right leg.

Employee had just finished putting oil in loader. Set oil down turned to exit the machine and felt knee give out.
Employee had fueled haul truck and was climbing back up ladder to cab. Employee did not maintain 3 points of contact and fell approximately 7' to concrete fueling pad. Right foot slipped as reaching for handrail under window. Employee landed on left hip and while falling hurt left wrist grabbing for a rail.

Hydraulic hose broke breaking windshield causing bruising and minor cuts to eye.
EE's were removing an electrical motor (100 HP) from the screen tower approx 50' above ground level. A boom truck was used to lift and remove the motor. While removing, there was inadequate clearance over a permanent handrail. EE's were pushing and lifting motor over handrail, when motor cleared, it dropped forcing a seperation of the two middle fingers, splitting web, stitches.
Employee was cleaning mirror as part of pre-shift inspection. As he leaned out to clean off the bottom mirror, his left elbow came in contact with the edge of the broken mirror frame on the top causing a laceration to his left elbow that required 9 stitches to close the wound.
Employee lifting sharp edged screens. Screen slipped out of his hand causing the sharp edge to hit the cuff of his leather glove & slide down his right forearm @ wrist causing a laceration (not very deep) but about 1" long. 5 stitches administered.

Employee was pulling 1.5" diameter waterhose up the steps to an elevated platform. He jerked on the hose with a twisting motion and immediately felt a pulling sensation in his left shoulder. He was examined by a physician on 7/12/05 who administered minor physical therapy, placing employee's arm in a sling, prescribed medication and placed him on restricted duty.
Tight conditions at the tail of the reclaim conveyor. Moisture present causing slick conditions. Material built up between the back wall of the tunnel & the tail pulley. Employee shoveling, slid off material, tried to catch himself. Lacerated forearm while trying to break his fall. Employee received stitches.

EE WAS REPAIRING A DUST SUPPRESSION LINE ABOVE A CONVEYOR NEAR THE DISCHARGE AREA OF A HUMBOLT IMPACT CRUSHER. A FIST SIZED ROCK BOUNCED OUT OF THE CHUTE OPENING AND PROJECTED UPWARD INTO EE'S RT FOREARM, CAUSING A LACERATION REQUIRING 8 SUTURES TO CLOSE.
WHILE CHANGING A SCREEN EE CUT THE TOP OF HIS LEFT THUMB 6 DAYS LATER IT BECAME INFECTED.

INJURED EMPLOYEEWAS 15'-18' AWAY OBSERVING WHEN THE COUPLING SHATTERED AND A PIECE STRUCK HIM IN THE MOUTH. TWO UPPER FRONT TEETH WERE BROKE AND THE TIP OF HIS CHIN WAS CUT. BOTH EMPLOYEES WERE WEARING SAFETY GLASSES, HARD HATS, GLOVES AND STEEL TOE
EE ATTEMPTED TO MOVE A PUNCH PLATE 3' X 4' C X 3/4" WHICH WAS LODGED UNDER THE HAND RAIL. HIS FOOT HUNG IN GRATING CATWALK & PLATE SLID INTO HIMAS HE FELL BACK BREAKING 3 RIBS. HAND RAILS WERE PIPE, CATWALKS WERE GATING, IT WAS DARK BUT QUARTZ LIGHTS WERE BEING USED, ADDITIONAL EES WERE NEARBY TO HELP.
