3 registradasVICTIM WAS ON A BELT CONVEYOR TO CUT OFF A LOOSE PIECE OF BELT. HE FELL APPROXIMATELY 20 FEET TO THE GROUND BELOW. HE DIED ON JANUARY 11, 2011. HARD FROST THE MORNING OF THE ACCIDENT. FAILED TO WEAR COMPANY SUPPLIED SAFETY HARNESS.
FROM PRELIMINARY: THE VICTIM WAS IN THE AREA OF AN UNGUARDED SELF-CLEANING TAIL PULLEY OF A DISCHARGE BELT FOR A PORTABLE CRUSHER WHEN HIS CLOTHING BECAME ENTANGLED IN THE SHAFT, PULLING HIM I NTO THE CONVEYOR BELT.

Night shift employee was lifting a metal plate out of the third level dump hopper. As EE was attempting to pick the plate up EE felt a sharp pain in the lower back. EE then reported the incident to the supervisor. EE was taken to a nearby medical center where EE was treated for lower back muscle strain.

Employee was climbing down the ladder of the front end loader (980K Cat) when his foot slipped off the bottom rung causing him to sprain his right thumb/hand we he grabbed the handrail to stop a fall.
Employee was using a pry bar to trip the door on a transfer chute located at level 2 crushing plant. As he was prying in a downward motion, the pry bar slipped causing him to fall injuring his neck. Note: Employee was working on the ground level near the chute.
After checking the coolant and air filter box for the pre-shift inspection of the Jumbo Drill, she slipped and fell to the ground when dismounting the drill. The employee landed on the her left side and injured her left lower back.

Employee hit a bump while driving the haul truck and felt a sharp pain in the right side of neck.
Employee was parked on a small grade. The drill was raised on jacks. He exited the drill and had his arm resting on the door frame of the drill. The door closed, striking the employee on the arm.
Employee was hand scaling the face from the bucket approx. 18' above the floor. When a "key" stone was dislodged, a piece of the face rolled over. It struck the pry bar and the bucket that the employee was standing in.
The employee bent to pick up the guard to replace on tail pulley after cleaning. When he lifted the guard from the ground he strained his back.
Employee lost footing exiting the rear of the service truck while starting the air compressor mounted on the back of the truck. The employee injured left knee.

WHILE EE WAS ROOF BOLTING WITH THE JACK-LEG-DRILL, THE DRILL VIBRATED OFF OF STAND, STRIKING RIGHT LEG. LACERATION ON BOTTOM RIGHT LEG (3-4 STITCHES). BRUISED RIGHT KNEE. (SEE ATTACHED NARRATI VE.)
ON 3-1-2000, EMPLOYEE ADVISED HIS SUPERVISOR THAT HE WAS IN PAIN IN HIS LOWER ABDOMEN, AND THAT THE 950G LOADER WAS BOUNCING HIM AROUND CAUSING THE PAIN. ON 3-2 THIS INCIDENT WAS REPORTED TO THE SAFETY DEPT. DR REPORTED HE HAD RIGHT INGUINAL STRAIN.
