EE stabbed left forearm with a lock blade pocket knife while cutting off a belt at slurry pump tower.

Pry bar came loose while trying to remove a screen striking IW in the mouth and damaged IW's tooth

Employee dislocated finger while doing maintenance.

Employee was stepping down from shaker platform when left knee gave way and ee fell to the ground.

Employee was swinging a 20lb sledge hammer when trying to remove concaves. He had swung two times and on the third swing he contacted his right foot with the hammer.

Employee sprained ankle exiting fork lift.

Employee was leaving the office when he tripped over boot brush that was located next to the door. He tried to catch himself with both arms causing one of the bones in his arms to fracture.

Employee was replacing the floor board on the flat bed section of the truck and was welding shims underneath the floor of the bed when he got debris in his eye. Employee did not seek medical treatment until 11/27/13.

Employee picked up a quarter keg grease container and carried it 8-10 feet into a building when he felt a tingle and pull in his shoulder.

Employee picked up worn out troughing roller which resulted in a laceration to his left knuckle requiring stitches.

Employee was changing the cylinder on a loader. While taking the pin out of the bottom using a two block system the block slipped catching the employees ring finger between the block and the pin causing a laceration to the knuckle requiring stitches.

EE received two fractures in left arm at the shop after the 100 ton haul truck plate popped out and struck him while he was using an air arc to cut floor plate loose.

EE left finger was lacerated after a Kamatsu 600 loader cylinder fell and caught his finger, receiving seven stitches.

EE twisted and fractured her ankle while walking on sidewalk

A splinter entered the EE's left arm while moving railroad ties.

Employee's left finger was lacerated & fractured after it was caught between the belt press roll and the press frame, requiring hospitalization for potential grease infection since employee has diabetes and a pin emplacement.

EE strained his neck after driving his vehicle into a concrete barrier, causing the airbag to engage.

EE left index finger was lacerated while trying to cut a plug off a polydeck screen panel with a utility knife.

SBS Blaster EE was checking the depth and condition of drilled boreholes in preparation of a blast. He tripped and fell over the face of the highwall.

Small ditch approximately 36 inches wide and foot deep, employee was stepping over onto concrete, foot caught edge of concrete, foot slipped, most of weight fell on right leg causing foot to land in awkward position.

EE AND INJURED WERE CHANGING A PUNCH PLATE IN #1TOWER. INJURED BELIEVES HE STRAINED MUSCLE IN RIGHT SHOULDER AFTER LIFTING PUNCH PLATE.

4 MAN CREW IN THE PROCESS OF REPLACING A DEFECTIVE BEARING ON THE DIESTER WASH SCREEN IN TOWER #2 WHEN IT FELL FROM THE HOIST, MASHING THE VICTIM'S LEFT MIDDLE FINGER AGAINST A STEEL I-BEAM.

THE AIR IMPACT WRENCH WAS PLACED ON THE ADJUSTMENT RING OF THE CONE CRUSHER WHEN ANOTHER EMPLOYEE ACCIDENTLY HIT THE AIR HOSE CAUSING THE WRENCH TO FALL APPROX. 10' STRIKING THE EMPLOYEE ON TO P OF HIS HARD HAT THEN GLANCING OFF HIS LEFT SHOULDER THEN STRIKING HIM IN THE LOWER BACK.

EE WAS GOING DOWN THE STAIR OF THE CRUSHER STATION AT THE PLANT HE LOST HIS BALANCE & STARTED TOFALL. HE JUMPED & SKIPPED A COUPLE OF STEPS TWISING HIS LEFT KNEE

EMPLOYEE WAS TAKING GRATING OFF THE TOP OF PUG MILL #2; GRATING CAUGHT HIS GLOVE, PULLING HIS RIGHT SHOULDER AND CAUSING SHARP PAIN. HE WAS JACKHAMMERING CEMENT ALGERS AND HIS ARM GOT CAUGHT BETWEEN BOX AND SPRAYBARS WITH JACKHAMMER RUNNING; JACKHAMMER TWISTED RIGHT HAND AND HIT RIGHT ELBOW ON BOX.
