Heat from a defective take-up roller bearing is believed to have caught the belt on fire causing approximately 2000 ft of belt structure and the #3 head house to be damaged beyond repair.

The employee stated that he was removing a breaker box cover and he had his knife in his right hand and it was open, the cover started to slip so he let go of the knife and it fell striking him in the left thumb cutting the bottom side of his thumb.

Employee was removing a 3" pipe. Pipe was hung up and when the employee broke it loose his left hand was pinched between pipe and column. Cut left finger requiring stitches.

EMPLOYEE WAS ATTEMPTING TO UNCLOG A COAL CHUTE WITH A SHOVEL WHEN A PIECE OF COAL BROKE LOOSE AND STRUCK HIM ON HIS RIGHT KNEE CAUSING A FRACTURE.

EE WAS REMOVING A TRANSMISSION PUMP ON A 773B HAULER IN A SLIGHTLY ENCLOSED AREA. THE PUMP UNEXPECTEDLY BROKE FREE AND STRUCK HIM IN THE LEFT THIGH. INITIALLY HE REFUSED MEDICAL TREATMENT BUT THREE WEEKS LATER HIS LEG WAS SORE AND HE REQUESTED TO GO TO THE DOCTOR. DR RELEASED HIM TO REGULAR DUTY BUT PUT HIM ON LIGHT DUTY AFTER SECOND FOLLOW UP.
THE INJURY OCCURRED WHEN THE EE WAS PLACING THE BOOM BRACKET STAND BACK INTO ITS ORIGINAL SLOT. WHEN THE BRACKET SLIPPED INTO PLACE HIS HAND WAS CAUGHT BETWEEN THE BRACKET & THE BED OF THE TRU CK CAUSING THE LACERATION TO HIS RIGHT HAND REQUIRING THE STITCHES.
THE EE HAD AN INCIDENT REPORTED AS A FIRST-AID CASE ON 06-14-02 WHEN HE REPORTED TO HAVE TWISTED HIS RIGHT KNEE. EE STATED THAT HE CONTINUED TO HAVE INTERMITTENT RIGHT KNEE PAIN. HE WAS SENT T O SEE AN ORTHOPEDIC SPECIALIST WHICH SCHEDULED HIM FOR SURGERY ON 11-01-02. SURGERY WAS PERFORMED AND EE BEGAN MISSING WORK ON 11-01-02.

EE HAD LOADED A COAL SAMPLE INTO PICKUP TRK BED.WHILE SHUTTING THE TAILGATE HIS INDEX FINGER ON RIGHT HAND GOT CAUGHT BETWEEN THE TRUCK BED AND TAILGATE CAUSING A LACERATION TO TIP OF FINGER.
